1.Practice and thinking of designated hospital medical emergency support for the 19th Hangzhou Asian Games

Objective:To analyze the construction and operation of the 19th Hangzhou Asian Games designated hospitals, and to discuss the medical emergency security work of large-scale sports events, so as to provide references for the planning of designated hospitals in future large-scale sports events.Methods:Retrospective analysis was made on the establishment principles, requirements, selection of medical support personnel, and training exercises of the designated hospitals, focusing on the key links such as organizational system, staffing, designated areas, and drug management.Results:Total of 40 designated hospitals have successfully completed the task of medical security by rebuilding the medical security area of the Asian Games, elevating the process, equipping facilities, and strengthening staff training. During the Asian Games, 349 people were transferred to designated hospitals by ambulance, 54 people were hospitalized, 19 people underwent surgery, and 1022 people went to designated hospitals by themselves.Conclusion:The construction of the designated hospitals during the 19th Hangzhou Asian Games was of high quality, efficient and smooth operation. It is suggested that efforts should be made in the reconstruction of the medical security area for the Asian Games to be "relatively independent". The treatment process of self-visiting patients should be fully considered and the flat urgent emergency response mechanism needs to be established.
2.Factors influencing hemolysis rate of leukocyte-reduced red blood cells at the end of preservation period in blood donors

【Objective】 To compare the current standards and explore the influencing factors for hemolysis rate of leukocyte-reduced red blood cells at the end of the preservation period, in order to formulate reasonable internal control indicators. 【Methods】 A retrospective analysis was performed on hemolysis rate of 427 samples of leukocyte-reduced red blood cells at the end of the preservation period in Nanning Blood Center from 2015 to 2022. Compared with the current standard for hemolysis rate at the end of the preservation period (GB 18469-2012 Quality Requirements for Whole Blood and Component Blood), the differences were analyzed, and the factors influncing the hemolysis rate were analyzed in terms of different blood donor groups. 【Results】 1) Among the 427 samples, the hemolysis rate of 418 (97.89%) did not exceed 0.4%, all lower than 0.8%; 2)the hemolysis rate of the male group was higher than that of the female group; 3) the hemolysis rate of the 18-29 years old group was lower than that of the 30-39 year old group and the 40-60 year old group, with statistically significant difference; 4) in terms of occupation, the hemolysis rate of students was the lowest, and the differences between groups were statistically significant; 5) no statistical significance was found in ethnicity and blood type. 【Conclusion】 Statistics indicated that gender, age, blood donation volume and occupation of blood donors were the influencing factors of hemolysis rate. The current standard is obviously higher in the qualified range of blood quality control in Nanning. It is advisable to formulate a reasonable quality control strategy with internal control index of hemolysis rate set <0.4%, which is conducive to making accurate evaluation of internal quality control and ensuring blood safety.
3.Current situation of professional quality learning needs among nursing students in colleges and universities in China under the background of five educations

Objective:To explore the professional quality learning needs among nursing students in colleges and universities in China under the background of the five educations.Methods:Convenience sampling was used to select nursing students who interned at Chinese Academy of Medical Sciences & Peking Union Medical College Hospital in 2021 as the research subject. The survey was conducted using a self-made "Professional Quality Learning Needs among Nursing Students in Colleges and Universities ". Multiple linear regression was used to explore the influencing factors of nursing students' professional quality learning needs. A total of 349 questionnaires were distributed, and 348 valid questionnaires were collected, with an effective recovery rate of 99.7%.Results:Among 348 nursing students, the five items with the highest need for professional quality learning were basic nursing skills, correct implementation of nursing procedures, communicating with patients and their families in appropriate ways, understanding of patient safety related concepts and nursing professional knowledge. Academic degree was a factor that affected the professional quality learning needs among nursing students ( P<0.05) . Conclusions:The training of nursing students' professional quality should be need oriented and establish a new era of "five education" concept. It should not only meet job requirements, but also combine students' own characteristics and growth patterns to cultivate nursing talents that meet clinical needs.
4.Construction of newly recruited nurses' core competence indicator system based on the concept of safety and quality improvement

Objective:To construct the core competence indicator system of newly recruited nurses based on the concept of safety and quality improvement, so as to provide a reference for the construction of the training system of the core competence and the formulation of evaluation standards of newly recruited nurses.Methods:Through literature retrieval and analysis, a framework of core competency indicator system of newly recruited nurses guided by the concept of safety and quality improvement was initially constructed. From November 2020 to March 2021, 15 nursing education and nursing practice experts across the country were consulted for 3 rounds through the Delphi expert consultation, and the expert opinions were collected to finally determine the indicator system.Results:Among 3 rounds of expert consultation, the response rates of the questionnaires were all 100%, and the expert authority coefficient was 0.90, and the Kendall coordination coefficient of the three-level indicators was from 0.14 to 0.20 ( P<0.05) . The finalized core competence indicator system of newly recruited nurses included 7 first-level indicators (professional knowledge and ability, patient-centered nursing, cooperative communication, quality promotion, safety, nursing information ability, professional development) , 18 second-level indicators and 103 third-level indicators. Conclusions:The indicators of the core competence indicator system of newly recruited nurses are good, which can provide a reference for optimizing the training courses for newly recruited nurses and improving the evaluation standards.
5.The application of Kirkpatrick Model in the evaluation of standardized training effect of new nurses

Objective:To evaluate the effect of standardized training for new nurses in the first year based on Kirkpatrick model as a theoretical framework.Methods:A cross-sectional study was conducted. New nurses' satisfaction with training, theoretical and operational assessment, job performance, core competence, turnover rate and other indicators were used, to evaluate the effect of standardized training for 62 new nurses in the first year.Results:At the reaction level, the overall satisfaction score of new nurses was 4.66±0.47; at the learning level, the theoretical assessment score was 95.32±2.38, the operational assessment score was 97.53±1.07, and the performance score was 97.12±1.64; at the behavioral level 88.45±9.75; and at the result level, there were no complaints and errors among new nurses, and the turnover rate was 1.6% (1/62). The theoretical and operational assessment scores were positively correlated with work performance, with r value of 0.424 and 0.399, respectively, and P = 0.001. The theoretical assessment scores were negatively correlated with the behavioral level, with the r value of -0.264 and P= 0.038. There were no correlation with the operational assessment scores and the behavioral level, with the r value of 0.147 and P value of 0.253. Conclusions:Kirkpatrick model validates the standardized training program for new nurses effectively, and provides scientific basis for improving the training program for new nurses. The results level of the training needs to be evaluated in the long term.
6.An empirical study of objective structured clinical examination in N4 nurse promotion assessment

Objective To explore the application effect of objective structured clinical examination in N4 nurse promotion assessment. Methods In the 2017-2018 promotion assessment of 58 N4 nurses in a ClassⅢ Grade A hospital in Beijing, the objective structured clinical examination was applied. Based on the responsibility requirements of N4 level, the stations and standards were set, and the core competence was evaluated. The total score≥ 80 was taken as the assessment passing standard. Results Finally, 51 nurses passed the examination and were promoted to N4 nurses. In 2017, 38 of them applied to participate in N4 assessment, with a comprehensive score of 82-100, all of them passed the assessment; in the next year, qualification review was carried out for those who passed the assessment, and 7 of them failed to meet the requirements of scientific research output, so N4 qualification was cancelled. In 2018, 20 applicants applied for N4 assessment, with a comprehensive score of 94-100, all of them passed the assessment. Conclusions The combination of objective structured clinical examination and N4 nurse promotion examination is helpful to evaluate the ability of nurses comprehensively and objectively, which is worthy of further application.
7.Prognostic value of PD-L1 expression in non-small cell lung carcinoma and its associa-tion with SUVmax

Objective:Previous studies have shown an association between programmed death-ligand 1 expression(PD-L1)in non-small cell lung cancer(NSCLC)and clinical factors and that PD-L1 is positively correlated with TNM staging.This study aimed to explore the prognostic significance of PD-L1 and its correlation with the maximum standardized uptake value(SUVmax).Methods:Clinicopath-ological data and the follow-up information of the 122 de novo primary NSCLC patients were analyzed.PD-L1 expression was detected by immunohistochemistry in this 122 surgically resected non-small cell lung carcinoma tissues.Survival outcomes were analyzed using the Kaplan-Meier method and multivariate Cox proportional hazards model.Correlation between SUVmax and PD-L1 expression was analyzed using Spearman's rank correlation analysis.Results:Multivariate analysis revealed that PD-L1 expression(HR=4.518,95% CI:1.176-17.352,P=0.028)and tumor size(HR=1.404,95%CI:1.020-1.933,P=0.037)were independent risk factors for overall survival(OS) in early NSCLC patients.Sex,age,pathological type,CEA level,and SUVmax group had no obvious effect on OS(P 0.05)in early NSCLC patients.In univariate analyses,sex,pathological type,tumor size,and SUVmax group affected OS in stageⅢ-ⅣNSCLC patients.How-ever,age,CEA level,and PD-L1 expression had no effect on OS.PD-L1 expression was not an independent risk factor for OS in stageⅢ-ⅣNSCLC patients.The SUVmax group had no association with PD-L1 in all patients.Conclusions:PD-L1 expression is an independent risk factor for OS in early NSCLC patients but not in stageⅢ-Ⅳpatients.
8.The Effect of Contusion and Exhaustive Exercise on Satellite Cells Activation and Pax7/CBF1/DAPT Contents in Skeletal Muscles of Rats

Objective To study the effect of contusion and exhaustive exercise on satellite cells' activation and Pax7/CBF1/DAPT contents in skeletal muscles of rats,and reveal the repair mechanism of the skeletal muscle injury.Methods Twenty-four seven-week-old male Sprague-Dawley(SD)rats were randomly divided into a control(C)group,an immediately after exhaustive exercise(E0)group,a 24 hours after exhaustive exercise(E24)group and a 48 hours after exhaustive exercise(E48)group,each of 6.Other 18 SD rats were randomly divided into 3 groups,6 rats in each group:an immediately after contusion group(D0),a 24h post-contusion group(D24)and a 48h post-contusion group(D48).All groups were killed at different time points after exhaustive exercise and the contusion respectively while the control group was at resting state,and their serum was extracted.The right gastrocnemius muscles were resected and divided into 2 parts:one was used immediately for culturing satellite cells,while the other was stored in the bridge at-80℃.All the serum and gastrocnemius muscles were tested for Pax7,CBF1 and DAPT contents using the enzyme-linked immunosorbent assay(ELISA).Moreover,three-day-old newborn rats were also slaughtered for the above experiment.Results The results of culture in vitro showed that one day after the culture began a small amount of spindle satellite cells could be seen in the newborn rats,but the satellite cells of other groups were growing slowly.However,on the third day and the fifth day,the spindle satellite cells of each group began to proliferate in quantity and reached the peak.Then all the satellite cells had a good proliferation until the seventh day after serial passage,and some patial fusions to microtubules occurred.The biggest cell number and proliferation rate was in newborn rats,but no skeletal muscle satellite cell was found in the control group.At the same time,the number and the proliferation rate of skeletal muscle satellite cells in contusion groups were significantly higher than the exhaustive exercise groups.The results of Elisa showed that the contents of CBF1 and Pax7 in skeletal muscles in exhaustive groups and contusion groups,were significantly upregulated,compared to the control group(P<0.05).Amomg them,the CBF1 content in group D0,together with the Pax7 content in group D24 and D48,was significantly higher than that of the other groups(P<0.05).On the other hand,the content of DAPT in skeletal muscles of exhaustive groups and contusion groups,compared to the control group,was significantly downregulated (P<0.05).To be more specific,the value of group D0 was significantly lower than that of the other groups (P<0.05).However,there was no significant difference in it among group E0,E24 and E48.Conclusions Contusion and exhaustive exercise can activate the skeletal muscle satellite cells to proliferate and differentiate.The number of activated skeletal muscle satellite cells in the contusion groups is significantly larger than that of exhaustive groups maybe due to the stronger stimulating by contusion.Both contusion and exhaustive exercise increase the contents of Pax7 and CBF1 in skeletal muscles,but decrease that of DAPT.The Pax7 and CBF1 may play a positive regulating role in terms of activating skeletal satellite cells and repairing skeletal muscle injury,while DAPT may play a negative regulating role.

10.Effect and mechanism of melatonin in anti-oxidative stress in N2A cells under hypoxia conditions

Objective To investigate the effects of melatonin on the changes of superoxide dismutase ( SOD ) , reactive oxygen species (ROS),and malondialdehyde(MDA) in N2A cells under hypoxia conditions.Methods Randomly divided the primary cultured neuroblasto-ma cells of mouse into the control group ,hypoxia group ,and MT treatment group .The MT treatment group were given melatonin 5μg/mL for 24 h to set up the treatment model .Rsbiotech of the hypoxia group and MT treatment group were given gaseous mixture of 95%N2 and 5%CO2 ,The ischemia hypoxia model of N 2A cells was set up with cells in the oxidative stress state and cultured for 24 hours at low concentra-tions of serum .The content of SOD , ROS, and MDA was measured respectively by xanthine oxidase , fluorogenic quantitative detection and thiobarbituric acid chromatometry .Results The expression of SOD in hypoxia group and MT treatment group were significantly decreased compared with that in control group(P<0.05).The level of SOD in MT treatment group significantly recovered compared with hypoxia group with singnificant difference (P<0.05).The expression of MDA and ROS in hypoxia group and MT treatment group were significantly in-creased compared with that in control group (P<0.05).The level of MDA and ROS in MT treatment group significantly recovered compared with hypoxia group with singnificant difference (P<0.05).Conclsion Melatonin provides a protective effect on the secondary damage of nerve cells with hypoxia ischemia .The possible mechanism is melatonin could play the role of free radical scavenging and up -regulate the ex-pression level of antioxidants .
