1.Imaging features of severe unilateral transverse sinus and sigmoid sinus thromboses and efficacy and safety of intravascular interventional therapy in them

Objective:To summarize the imaging features of severe unilateral transverse sinus and sigmoid sinus thromboses, and evaluate the efficacy and safety of intravascular interventional therapy in them.Methods:Thirty-seven patients with severe unilateral transverse sinus and sigmoid sinus thromboses clinically mainly manifested as intracranial hypertension and accepted endovascular intervention in Department of Interventional Radiology, First Affiliated Hospital of Zhengzhou University from June 2012 to September 2022 were chosen; their clinical data were retrospectively analyzed and imaging features were summarized. Short-term efficacy was evaluated according to blood flow restoration degrees and pressure gradient reduction in the occlusive sinus and modified neurological symptoms before and after endovascular intervention. Hospitalized complications were observed; safety and long-term efficacy were evaluated according to postoperative clinical follow-up and imaging results 6-12 months after endovascular intervention.Results:(1) Preoperative brain MRI and (or) CT showed different degrees of swelling of the brain tissues, with the affected side as the target; mixed signals/density shadow could be seen in the blocked transverse sinus and sigmoid sinus; venous cerebral infarction or post-infarction cerebral hemorrhage could be combined in some patients. MRV, CTV and DSA showed poor or completely occluded transverse sinus and sigmoid sinus while normal in the contralateral side; obvious thrombus filling-defect was observed in the occluded venous sinus after mechanical thrombolysis. (2) Occlusive sinus blood flow was restored in all patients after endovascular intervention, and pressure gradient of the occlusive segment decreased from (16.6±3.3) mmHg before to (2.8±0.8) mmHg after endovascular intervention. Before discharge, clinical symptoms of all patients were significantly improved (modified Rankin scale [mRS] scores of 0 in 30 patients, 1 in 5 patients, 2 in 1 patient and 3 in 1 patient), and 2 patients had unilateral limb movement disorder (muscle strength grading III and IV, respectively). All patients received clinical follow-up for (9.6±3.0) months. At the last follow-up, neurological function obviously improved compared with that before endovascular intervention, without new neurosystem-related symptoms (mRS scores of 0 in 30 patients, 1 in 6, and 2 in 1 patient). In 34 patients received MRV or DSA follow-up, 28 had complete recanalization of occlusive sinus and 6 had partial recanalization, without obvious stenosis or recurrent occlusion.Conclusions:Severe unilateral transverse sinus and sigmoid sinus thrombosis can cause local intracranial venous blood stasis, and then cause "increased regional venous sinus pressure", which is manifested as unilateral brain tissue swelling and even venous cerebral infarction or post-infarction cerebral hemorrhage. Early diagnosis and endovascular intervention can obviously improve the prognosis of these patients, enjoying good safety.
2.OCT and IVUS evaluating stent apposition and endothelialization after FD implantation in aneurysm animal models

Objective:To investigate the application value of optical coherence tomography (OCT) and intravascular ultrasound (IVUS) in evaluating flow diverter (FD) apposition and endothelialization in aneurysm animal models, and analyze the effect of incomplete stent apposition (ISA) on aneurysm lumen healing and stent endothelialization.Methods:Lateral common carotid artery aneurysm models in swines were established by surgical method and then FD was implanted. Immediately after surgery, OCT and IVUS were used to evaluate the locations and degrees of ISA, and difference between these 2 methods in evaluating FD apposition was compared. DSA was performed at 12 weeks after surgery to evaluate the aneurysm occlusion (Kamran grading) and stent patency. OCT and IVUS were used again to observe the stent endothelial situation; by comparing with histopathologic results, effect of ISA on aneurysm healing and stent endothelialization was analyzed.Results:Lateral common carotid artery aneurysm models in 6 swines were established, and 6 Tubridge FDs were successfully implanted. Compared with IVUS (3 stents, 4 locus), OCT could detect more ISA (6 stents, 14 locus); and the vascular diameter change area (7 locus), aneurysm neck area (4 locus) and the head and tail of FD (3 locus) were the main sites of FD malapposition; average distance between stent wire and vessel wall was (560.14±101.48) μm. At 12 weeks after surgery, DSA showed that 1 patient had a little residual contrast agent at the aneurysm neck (Kamran grading 3), and the remaining 5 had complete aneurysm occlusion (Kamran grading 4). One FD had moderate lumen stenosis, and the other 5 FDs had lumen patency. OCT indicated mostly disappeared acute ISA; ISA proportion decreased to 21.4 % (3/14), including 2 in the aneurysm neck and 1 in the partial stent. Histopathological results showed bare stent woven silk, without obvious endothelial coverage; in one FD with luminal stenosis, intimal hyperplasia was mainly composed of vascular smooth muscle cells.Conclusion:In carotid artery aneurysm model with FD implantation, OCT can detect more ISA than IVUS; most acute ISA have good outcome at 12 th week of follow-up, while severe ISA can cause delayed FD endothelialization and delayed aneurysm occlusion.
3.Correlation of poorly differentiated cluster grade with clinicopathological features and prognosis of patients with colon cancer

Objective:To analyze the correlation of poorly differentiated cluster(PDC)grade with clinicopathological features and prognosis of patients with colon cancer.Methods:Hematoxylin-eosin-stained specimens and data from 102 patients with colon cancer who under-went surgery at the Affiliated Hospital of Anhui West Health Vocational College between December 2016 and July 2021 were analyzed in this study.The PDC grades of tumor tissues were determined in all colon cancer cases.Additionally,associations between the PDC grade and clinicopathological features of patients were analyzed via Chi-square test and Logistic regression analysis,whereas the association between the PDC grade and patient prognosis was evaluated using Cox regression analysis.Results:In total,52 cases were identified as PDC G1,14 as PDC G2,and 36 as PDC G3.Tumor budding(TB)was independently and positively associated with PDC,whereas tumor infiltrating lympho-cytes(TIL)were independently and negatively associated with PDC(all P<0.05).Lymph node metastasis or presence of cancerous nodules(hazard ratio[HR]=2.377;95%confidence interval[CI]=1.237-4.568;P=0.009),distant metastasis(HR=7.455;95%CI=2.597-21.399;P<0.001),and the PDC grade(HR=2.095;95%CI=1.099-3.994;P=0.025)were independent risk factors for colon cancer progression.Conclu-sions:Our findings suggest that both TIL and TB affect the colon cancer PDC grade.Therefore,evaluating the PDC grade in cancer tissues can facilitate the prognostic stratification of patients with colon cancer.
4.Association between carotid artery perivascular fat density and carotid artery steno-sis,prognosis

Aim To investigate the relationship between carotid artery perivascular fat density and carotid artery stenosis,prognosis.Methods A total of 209 consecutive patients with extracranial internal carotid artery stenosis in Taizhou Central Hospital(Taizhou University Affiliated Hospital)were retrospectively included from January 2017 to Janu-ary 2021.The carotid artery perivascular fat density in the narrowest axial layer and in the same contralateral axial layer was evaluated by computed tomography angiography.Clinical data of patients were collected.The stenosis was graded according to the guidelines.Symptomatic carotid stenosis was determined according to the medical history.The patients were followed up for one year.Results Carotid artery perivascular fat density was 4.2%higher on the stenosis side than those on the opposite side(P<0.001).The fat density increased with the stenosis severity.On the stenosis side,carotid artery perivascular fat density was 6.25%higher in symptomatic patients than that in asymptomatic patients(P=0.015).In asymptomatic patients,perivascular fat density in patients with stenosis related cerebrovascular events oc-curred within 1 year was 12.4%higher than that in patients without related cerebrovascular events(P=O.017),and the difference remained after adjusting the clinical parameters by Logistic regression analysis(OR=1.060,95%CI:1.006~1.117,P=0.028).Conclusions Carotid artery perivascular fat density is positively correlated with the degree of ca-rotid stenosis.Patients with symptomatic carotid artery stenosis or recurrent related ischemic cerebrovascular outcome e-vents have higher carotid artery perivascular fat density.
5.High STING expression exacerbates renal ischemia-reperfusion injury in mice by regulating the TLR4/NF-κB/NLRP3 pathway and promoting inflammation and apoptosis

Objective To investigate renal expression level of STING in mice with renal ischemia-reperfusion injury(IRI)and its regulatory role in IRI.Methods C57BL/6 mice were divided into sham operation group,IRI(induced by clamping the renal artery)model group,IRI+DMSO treatment group,and IRI+SN-011 treatment group.Serum creatinine and blood urea nitrogen of the mice were analyzed,and pathological changes in the renal tissue were assessed with PAS staining.RT-qPCR,ELISA,Western blotting,and immunohistochemistry were used to detect the expression levels of STING,KIM-1,Bcl-2,Bax,caspase-3,TLR4,P65,NLRP3,caspase-1,CD68,MPO,IL-1β,IL-6,and TNF-α in the renal tissues.In the cell experiment,HK-2 cells exposed to hypoxia-reoxygenation(H/R)were treated with DMSO or SN-011,and cellular STING expression levels and cell apoptosis were analyzed using RT-qPCR,Western blotting or flow cytometry.Results In C57BL/6 mice,renal IRI induced obvious renal tissue damage,elevation of serum creatinine and blood urea nitrogen levels and renal expression levels of KIM-1,STING,TLR4,P65,NLRP3,caspase-1,caspase-3,Bax,CD68,MPO,IL-1β,IL-6,and TNF-α,and reduction of Bcl-2 expression level.Treatment of the mouse models with SN-011 for inhibiting STING expression significantly alleviated these changes.In HK-2 cells,H/R exposure caused significant elevation of cellular STING expression and obviously increased cell apoptosis rate,which was significantly lowered by treatment with SN-011.Conclusion Renal STING expression is elevated in mice with renal IRI to exacerbate renal injury by regulating the TLR4/NF-κB/NLRP3 pathway and promoting inflammation and apoptosis in the renal tissues.

7.Refined operational management practices in public hospitals based on disease analysis under the back-ground of disease type-based payment

Objective In response to the new landscape of the DIP payment reform,this study uses a public tertiary comprehensive hospital in Jingzhou as a case study.By focusing on disease analysis,it explores refined operational management strategies to promote the hospital's transformation,upgrade,and operational efficiency.Methods Starting from July 2022,a problem-oriented approach was employed to analyze the characteristics of disease structure using methods such as the Boston Ma-trix,Pareto Analysis,and the Onion Method.A multidisciplinary operational management model was adopted to provide targeted supervision to departments for improving diagnosis and treatment processes,optimizing disease types and cost structures,and im-plementing refined management.Results The sample departments showed continuous growth in medical service capabilities,with the number of disease types increasing by 5.76%year-on-year and the number of discharges rising by 20.9%.The CMI val-ue for DIP-related drug consumption increased by 0.22.The economic structure improved significantly,with average drug costs per inpatient decreasing by 14.19%and average material costs dropping by 30.62%.Medical insurance settlements saw substan-tial improvement,with losses reduced by over 80%.Operational efficiency was notably enhanced,with the average length of stay decreasing by 1.0 days year-on-year.Patient experience improved,with satisfaction rates exceeding 97%.Conclusion The re-fined operational management model based on disease analysis under the DIP framework can facilitate efficient hospital operations,promote rational use of medical insurance funds,and effectively reduce patient burdens,leading to a win-win situation for all stakeholders.
8.Effect of astragalus polysaccharide synbiotics onfecal scores and serum indices in calf diarrhea induced by Escherichia coli

This study examined the impact of astragalus polysaccharide synbiotics on fecal scores and blood indices in calves suffering from E.coli-induced diarrhea.Twenty Holstein calves,aged 6-9 days and weighing approximately(44.56±5)kg,were randomly divided into a control group and an experimental group.The experiment lasted 10 days.Both control and experimental group calves received daily intramuscular injections of borlococci(1 mg/kg)and enrofloxacin(20 mg/kg)dur-ing experimental period.In addition to the standard treatment regimen,calves in the experimental group were given 20 g of Astragali polysaccharide synbiotics in their milk each day.Fecal scoring was conducted daily at 9:00 a.m.,and jugular blood was collected at the end of experiment to eval-uate relevant blood parameters.Results indicated a 17%reduction in diarrhea rates in the experi-mental group compared to the control group,although there was no statistically significant differ-ence in fecal scores.Compared to the control group,the experimental group showed a significant in-crease in the levels of immunoglobulin A(IgA)and immunoglobulin M(IgM)by 54.81%and 60.40%,respectively(P<0.01).Immunoglobulin G(IgG)and interleukin-10(IL-10)levels increased by 50.89%and 46.80%,respectively(P<0.05).while interleukin-6(IL-6),interleukin-1β(IL-1 β),and tumor necrosis factor-α(TNF-α)decreased by 28.99%,20.88%,and 37.10%,re-spectively(P<0.01).The experimental group showed a significant reduction in the level of D-lac-tic acid(D-LA)and endotoxin(LPS)by 47.46%and 65.31%compared to the control group,re-spectively(P<0.01),and diamine oxidase(DAO)activity was 30%lower(P<0.01).Total su-peroxide dismutase(T-SOD)content increased by 12.78%(P<0.05),and catalase(CAT)and glutathione peroxidase(GSH-Px)contents increased by 53.09%,and 60.57%,respectively(P<0.01).Furthermore,malondialdehyde(MDA)content decreased by 33.29%(P<0.01)in the ex-perimental group.In conclusion,administering astragalus polysaccharide synbiotics could reduce in-testinal mucosal damage in calves with E.coli diarrhea,enhance their immunity and antioxidative stress response,and mitigate the impact of pathogenic bacteria and oxidative stress on intestinal mucosal permeability.
9.Preparation and preliminary of astragalus polysaccharide synbiotics and its effect on growth performance of calves

In this study,freeze-drying methods was used to prepare astragalus polysaccharide synbi-otic powder.The optimized freeze-dried protective agent formulation using response surface meth-odology in this study was 17.99%skimmed milk powder,7.26%sucrose,8.04%galactooligosac-charides,and 9.64%astragalus polysaccharide,and the remaining amount was distilled water.The survival rate of lactic acid bacteria in the prepared bacterial powder was 88.06%.A calf feeding ex-periment was conducted over 28 days with thirty healthy newborn calves with similar age,the calves were randomly assigned to either control group and the astragalus polysaccharide synbiotic group,each comprising 15 calves.The astragalus polysaccharide synbiotic group received a supple-ment of astragalus polysaccharide in addition to their regular feed.Compared to the control group,the results showed that:(1)Astragalus polysaccharide synbiotics significantly increased the aver-age daily gain(ADG)of calves and decreased the fecal score and diarrhea rate in calves.(2)By tes-ting the immunological indexes,we found that astragalus polysaccharide synbiotics significantly in-creased the serum level of IgA,IgG,IgM,IL-2 and IL-10,and decreased the level of IL-6 and TNF-α.(3)Astragalus polysaccharide synbiotics notably enhanced the Chao1,Sobs,and Shannon indi-ces,while reducing the Simpson index.At the phylum level,astragalus polysaccharide synbiotics group showed a significant increase in the relative abundance of Cyanobacterium and Firmicutes,and a decrease in Proteobacteria.At the genus level,the relative abundance of Esche-richia-Shigella in astragalus polysaccharide synbiotic group significantly decreased.Whereas norank_f__norank_o__Clostridia_UCG-014 and Butyricicoccus significantly increased.In conclu-sion,this study successfully formulated an astragalus polysaccharide synbiotic powder.The powder can significantly promote the growth of calves,enhance immune function,improve the structure of gut microbiota,and reduce the incidence of calf diarrhea.
10.Dual-source CT virtual monoenergetic imaging for differentiating prostate cancer and benign prostatic hyperplasia

Objective To observe the value of dual-source CT(DECT)virtual monoenergetic imaging(VMI)for differentiating prostate cancer(PC)and benign prostatic hyperplasia(BPH).Methods Thirty-three patients with PC(PC group)and 44 patients with BPH(BPH group)were retrospectively enrolled,and 40-100 keV(with 10 keV interval)VMI were reconstructed based on arterial phase DECT images,respectively.Clinical data,focal CT value and contrast-to-noise ratio(CNR)of VMI with different energy levels and conventional linear fusion images of arterial phase were compared between groups.Binary logistic regression models were constructed based on clinical data being significantly different between groups,focal CT value of VMI with energy level with the highest CNR,also focal CT value of conventional linear fusion images of arterial phase,respectively.The area under the receiver operating characteristic curve(AUC)was calculated to evaluate the efficacy of each model for differentiating PC and BPH.Results Free prostate-specific antigen(f-PSA),total prostate-specific antigen(t-PSA),focal CT value of 40-100 keV VMI and conventional linear fusion images of arterial phase in PC group were all higher,while short diameter in PC group was smaller than those in BPH group(all P<0.05).Logistic regression models were constructed based on f-PSA,t-PSA,short diameter,CT value at 40 keV VMI and CT value at conventional linear fusion images of arterial phase,respectively,with AUC for differentiating PC and BPH of 0.879,0.902,0.701,0.911 and 0.857,respectively.Conclusion DECT VMI could be used as a supplementary examination for prostate diseases,and 40 keV VMI had the best efficacy for differentiating PC and BPH.
