1.Association of stress hyperglycemia with occurrence and prognosis of sepsis-associated encephalopathy

Objective:To investigate whether stress hyperglycemia (SH) is an independent risk factor for the occurrence and mortality of sepsis-associated encephalopathy (SAE).Methods:From August 2016 to October 2021, sepsis patients admitted to the ICU of Guangdong Provincial People's Hospital were selected as the study subjects. According to whether they developed to SH (RBG>11.1 mmol/L) within 7 days of enrollment, the pat ients were divided into the SH group and the non-SH group for analysis. Logistic regression was used to analyze whether SH was an independent risk factor for SAE occurrence, and ROC curve was used to analyze the predictive value of SH to SAE. Kaplan-Meier curve was used to compare the 90-day survival of SAE patients with or without SH. Cox regression analysis was used to analyze the risk factors of 28-day and 90-day death in SAE patients.Results:A total of 183 sepsis patients were included, including 62 patients in the SH group and 121 in the non-SH group. Logistic regression analysis demonstrated that SH was an independent risk factor for SAE ( OR=4.452, 95% CI: 2.021-9.808, P <0.001). ROC curve demonstrated that SH could accurately predict SAE (AUC=0.831; Sensitivity=78.4%; Specificity=76.8%; and Yoden index=0.553). Kaplan-Meier curve demonstrated that the 90-day survival of SAE patients with SH significantly declined (log-rank test: P<0.01). Cox regression analysis suggested that SH was a risk factor for death at day 28 and day 90 in SAE patients (28 d, HR=2.272, 95% CI: 1.212-4.260, P=0.010; 90 d, HR=2.456, 95% CI: 1.400-4.306, P<0.01). Conclusions:SH is an independent risk factor for SAE and can predict SAE occurrence. SH significantly reduces 90-day survival and increase mortality at 28 and 90 days in SAE patients.

This article summarizes and analyzes the reported synthetic routes of brivudine in patent and literature.2′-Deoxyuridine was employed as starting material, affording brivudine through iodization, heck coupling, hydrolysis, decarboxylation, bromination and recrystallization.After optimization of reaction conditions of each step, a synthetic route suitable for industrial production was achieved with simple synthetic process, high yield and excellent purity.
3. Analysis of the characteristics of second primary malignancy affecting the survival of lymphoma patients

Objective:
To analyze the characteristics of the second primary tumor affecting the survival of patients with lymphoma, and to explore the risk factors of death from the second primary tumor.
Methods:
The medical records and related death information of 1 173 lymphoma patients who had already died with known causes were collected. The basic causes of death and the characteristics of patients who died of the second primary tumor were analyzed. Cox regression model was used to analyze the risk factors of lymphoma patients who died of the second primary tumor.
Results:
Among the 1 173 patients who had died, 94 (8.0%) died of the second primary tumor, 935 (79.7%) died of the primary lymphoma and 144 (12.3%) died of other diseases. The second primary tumor accounted for 17.5% (38/217) of all causes of death in patients with the survival period of more than 5 years, and the second primary tumor accounted for 28.3% (17/60) of all causes of death in patients with the survival period of more than 10 years. Among 94 cases who died of second primary tumors, 31 died of lung cancer, 15 died of gastric cancer, 13 died of liver cancer, 9 died of pancreatic cancer, 6 died of colorectal cancer, 6 died of second primary lymphoma and 14 died of other types of tumors. Objective:To understand and explore the risk factors of the death of lymphoma patients from cardiovascular disease.Methods:The medical records and death information of 1 173 patients with lymphoma were collected, cases that died from cardiovascular disease were screened. A binary logistic regression model was used to analyze the independent risk factors of patients with lymphoma died from cardiovascular disease.Results:Among 1 173 patients with lymphoma, 75 (6.4%) died of cardiovascular disease, including 27 cases of coronary heart disease, 25 cases of stroke, 7 cases of hypertension, 5 cases of sudden cardiac death, 4 cases of pulmonary embolism, 3 cases of heart failure, 4 cases of others. Among the patients who survived for more than 5 years, 16.1% (35/217) died of cardiovascular disease. Among those who survived for more than 10 years, 11.7% (7/60) died of cardiovascular disease. Multivariate Logistic regression analysis showed that the primary site of lymphoma ( OR=0.521, P=0.039), stage (stage Ⅱ: OR=2.487, P=0.016; stage Ⅲ: OR=3.233, P=0.002) and cardiovascular toxicity in the course of diagnosis and treatment ( OR=3.019, P=0.001) are independent influencing factors for the death of cardiovascular disease in patients with lymphoma. Conclusions:Cardiovascular disease is an important factor affecting the survival of patients with lymphoma. With the extension of survival time, the risk of dying from cardiovascular disease increases significantly. The primary site, tumor stage, and cardiovascular toxicity that occur during the diagnosis and treatment may be the independent influencing factors for patients with lymphoma that die from cardiovascular disease.

6.Causes of death in 1,173 patients with lymphoma

Objective: To understand the causes of death and long-term prognosis of lymphoma patients. Methods: Data from 6 200 patients with lymphoma admitted to the Department of Lymphoma, Peking University Cancer Hospital, from January 1995 to Decem-ber 2017, were collected. Those who had died and whose causes of death were known were selected. Clinical records and information on death were collected. Results: A total of 1,173 patients were selected, 742 of whom were male (63.3% ), and 431 were female (36.7%). The median age was 56 (8-92) years. There were 77 cases (6.6%) of Hodgkin's lymphoma, 1,095 cases (93.4%) of non-Hodg-kin's lymphoma, and 1 case of unclear pathological classification. Overall population survival was 0-253 months, with a median surviv-al rate of 20 months. The direct causes of death included lymphoma in 688 (58.7%), various infectious diseases in 119 (10.1%), cardio-vascular diseases in 96 (8.2%), secondary primary tumors in 68 (5.8%), and other diseases in 202 cases (17.2%). The underlying causes of death included lymphoma in 936 (79.8%), secondary primary tumors in 94 (8.0%), cardiovascular diseases in 75 (6.4%), respiratory diseases in 32 (2.7%) and other diseases in 36 cases (3.1%). The underlying causes of death in cases wherein survival time exceeded 5 years included lymphoma in 129 (59.4%), secondary primary tumors in 38 (17.5%), cardiovascular diseases in 35 (16.1%), and other dis-eases in 15 cases (6.9%). The underlying causes of death in cases wherein survival time exceeded 10 years included lymphoma in 28 (46.7%), secondary primary tumors in 17 (28.3%), cardiovascular diseases in 7 (11.7%), and other diseases in 8 cases (13.3%). Conclu-sions: Primary tumors remain the main cause of death in patients with lymphoma. After primary tumors, secondary primary tumors and cardiovascular diseases are the most common causes of death, and with the prolongation of survival, the risk of death caused by these factors increases significantly.

Objective To investigate the quality of life and psychological status of donors and recipients of living-related donor kidney transplantation. Methods Selected the treatment of living donors in the hospital (n = 80)and recipients(n = 80)from January 2014 to January 2017 as the research objects,and 80 cases of hemodialysis patients at the same period as the control group. Using the SF-36,Zung Self-rating Anxiety Scale (SAS),and the Self-rating Depression Scale(SDS)to evaluate the psychological status of the two groups and com-pared.Results Scores of donors′physiological function,and general health compared with the norms were not sig-nificant(P>0.05),while the somatic pain scores were significantly lower than the norms(P<0.05).There was no difference between the donors group and the norms in scores of vitality,social function and emotional function (P>0.05).The scores of physical health and mental health of recipients were significantly higher than those of the control group(P < 0.05). The donors′ SAS and SDS anxiety and depression scores were significantly higher than the norms(P<0.05).Besides,SAS and SDS anxiety and depression scores of the recipients were also significantly higher than the norms(P < 0.05),but significantly lower than the control group(P < 0.05). Conclusion The living-related donor kidney transplantation does not affect the quality of life and psychological state of the donor, but can improve the quality of life and reduce the anxiety and depression of the recipient.

In order to accelerate the comprehensive reform of clinical education,the Ministry of Education and the Ministry of Health decided to implement the program of excellent doctor education training plan in 2012.On the basis of the plan,taking a certain medical college as an example,on the problems such as lack of teachers,unreasonable age structure,the lack of advanced experimental technology and comprehensive talents and the lack of innovation and enterprising spirit and so on,the author puts forward some proposals from the development of experimental teaching,the teachers' personal and professional growth and the institutional and policy level,such as strengthening the foreign exchange,making learning training plan and personal development planning,establishing the evaluation system of the experiment teaching quality and opening up a variety of learning methods to improve the teachers' comprehensive ability.
9.Evidence-based evaluation of Chinese students' social practice and guarantee mechanism

Objective To investigate and analyze the researches and guarantee mechanisms of Chinese undergraduate students' social practice, to understand the current situation of college students' social practice and research quality and to provide the reference for the development of college students' social practice guarantee mechanism in China. Methods With the “University” “college students”“social practice” as key words , we retrieved published documents about long-term mechanism of CUSSP dating to March 2014, from Chinese journal full text database(CNKI), China outstanding master's degree thesis database, Chinese Excellent Doctor Degree Dissertation Full-text Database, Wanfang, CMFD, CDFD,VIP, Google and Baidu, etc. We classified the including documents based on evidence-based scientific evidence grading standards, analyzed Grade C and above documents only. Results A total of 22 relevant studies were included; 4 of them are policy studies with Grade B, 18 of them are cross-sectional studies with grade C. The studies showed: ①The policy studies give important sugges-tions on the content, form, effect, construction and evaluation system of long-term mechanism of CUSSP.②18 cross-sectional studies surveyed over 14 504 students' social practice conditions show the method-ology of most of the studies is described unclearly or non-standard however. ③12 studies have inves-tigated the Chinese college students' attitude towards social practice and actual participation, with 5 studies about the time of participation and 5 about the condition of combination with specialty. The undergraduate students can recognize the importance of social practice, however, the range, duration and times of their actual participation are limited, and the combination with their major is not close enough. ④7 studies have investigated the main problems of college students' social practice and 5 studies have investigated the students' satisfaction. At present, there exist many problems in CUSSP, of which the most difficult things are: hard-to-contacting practice units, lack of funds, lack of teacher guidance, hard-to-determining the practical projects etc. The students' overall satisfaction is“general”. Conclusions ①There is lack of high-quality studies about CUSSP, and the methodology of studies should be improved and standardized. ②Strengthening the long-term mechanism and evaluation system construction is urgently needed to ensure that students can conduct long-term, effective and stable social practice.
10.Design, synthesis and activities of 4-(2-acetoxybenzoylamino) butyramide derivatives.

To explore new agents of gamma-aminobutyric acid (GABA) derivatives with more potent antiepileptic activity, a series of 4-(2-acetoxybenzoylamino) butyramide derivatives were designed and synthesized. All of the novel compounds (5a-51) were synthesized from GABA as starting material, and their structures were confirmed with IR, 1H NMR, EI-MS and elemental analysis. Preliminary pharmacological test in vitro showed that all target compounds displayed strong antiepileptic activities and were worth for further study. The structure-activity relationship of 4-(2-acetoxybenzoylamino) butyramide derivatives was also discussed preliminarily.
