1.Effect and mechanism of Sanqi danshen tablets in the treatment of non-alcoholic fatty liver disease

OBJECTIVE To investigate the potential mechanism of Sanqi danshen tablets in the treatment of non-alcoholic fatty liver disease (NAFLD). METHODS Core targets of Sanqi danshen tablets in the treatment of NAFLD were explored by network pharmacological methods. Gene ontology (GO) functional enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis were also performed. Based on the results obtained from network pharmacological studies, using SD rats as subjects, the NAFLD model was induced by feeding them high-fat diet. The effects of Sanqi danshen tablets on pathological changes such as lipid droplet vacuoles and lipid accumulation in the liver tissue of NAFLD rats, as well as its impact on relative indicators of lipid metabolism, inflammatory responses and oxidative stress, were investigated. RESULTS A total of 20 core targets for the treatment of NAFLD with Sanqi danshen tablets were screened, primarily involved in GO functions such as biological regulation, cellular membrane and binding, and enriched in signaling pathways related to inflammatory responses, oxidative stress and lipid metabolism. Compared with the model group, lipid droplet vacuoles were reduced significantly in low-dose, medium-dose, high-dose groups of Sanqi danshen tablets and positive control (simvastatin) group, the number of lipid droplets decreased significantly and the color became lighter. The contents of total cholesterol, triglyceride (except for medium- dose group of Sanqi danshen tablets), aspartate transaminase, alanine transaminase, tumor necrosis factor-α (except for low-dose group of Sanqi danshen tablets), interleukin-17 (except for Sanqi danshen tablets groups) and malondialdehyde (except for low- dose group of Sanqi danshen tablets) in liver tissue were significantly decreased, while the content of superoxide dismutase was significantly increased (P<0.01 or P<0.05). CONCLUSIONS Sanqi danshen tablets exert anti-inflammatory, antioxidant and lipid metabolism regulating effects by influencing the levels of inflammation, oxidative stress and lipids metabolism-related indicators, thereby improving NAFLD in rats.
2.Detoxification Strategies of Triptolide: A Review

Tripterygium wilfordii is a traditional Chinese medicinal herb belonging to the genus Tripterygium in the Celastraceae family, which has the effects of clearing heat and detoxifying, dispelling wind and dampness, and invigorating blood circulation to relieve pain, and is used to treat diseases such as rheumatoid arthritis, glomerulonephritis, nephrotic syndrome, lupus erythematosus, scabies, and stubborn tinea. Its chemical composition is diverse. Among them, triptolide(TP) is one of the main active and toxic components of T. wilfordii. It has significant biological activities such as anti-inflammation, anti-tumor, and immunosuppression. However, it causes serious adverse reactions such as liver and kidney function damage and reproductive system disorders. At the same time, TP has poor water solubility and low bioavailability, and the enhancement of bioavailability by increasing the dosage undoubtedly improves the exposure of the drug in non-target organs, leading to the occurrence of adverse reactions, and these largely limit the clinical application of TP. Based on this, this article extracted relevant data from the Web of Science, PubMed, and China National Knowledge Infrastructure(CNKI) databases, summarized the research on the adverse reactions of TP in recent years, and reviewed the progress of toxicity reduction research from the perspectives of structural modification, novel drug delivery systems, and compatibility. Structural modification can precisely alter the chemical structure of TP, reduce the activity of its toxic groups, and retain its biological activity while fundamentally reducing the occurrence of adverse reactions. New drug delivery systems can achieve targeted delivery of TP, increase its concentration in target organs, and reduce its exposure in non-target organs, thereby enhancing therapeutic efficacy and reducing adverse effects. In addition, the combination of TP with Chinese medicine compound, single-flavored Chinese medicine or monomer can reduce the adverse effects of TP and enhance the efficacy to different degrees, which is of clinical value. This paper systematically explains attenuation research from the above three perspectives, aiming to provide a theoretical basis for the full utilization of biological activity and drug development of TP.
3.Comparison of intraoperative balanced salt solution injection or postoperative air filling for superior bullous retinal detachment

AIM: To observe the outcome of intravitreal balanced salt solution(BSS)injection to increase intraocular pressure(IOP)after extrascleral subretinal fluid drainage, then scleral buckling(SB)to treat superior bullous retinal detachment(SBRD), and compare it with the effect of conventional surgery(without any intravitreal filling)and postoperative air filling.METHODS: Retrospective case-control study. A total of 72 patients(73 eyes)who underwent SB for SBRD from January 2018 to December 2022 in ophthalmology department of Xijing Hospital were included. The extrascleral subretinal fluid drainage was performed in all eyes. According to whether intravitreal injection was performed and different injections, patients were divided into three groups: with 24 cases(24 eyes)in the conventional group(no intravitreal injection), 23 cases(23 eyes)in the air group(sterile air was injected after surgery), and 25 cases(26 eyes)in the BSS group(BSS was injected during extrascleral subretinal fluid drainage). All patients were followed up until subretinal fluid was absorbed completely. The average surgery time, postoperative IOP, retinal reattachment rate, subretinal fluid absorption, visual acuity(LogMAR)and major complications were compared.RESULTS: All surgeries were completed successfully. The average surgery time of the conventional group, air group and BSS group were 63.17±13.22, 61.65±15.55 and 57.30±11.70 min, respectively. There had no significant difference among these groups(F=0.825, P=0.443). On the first post-operative day, the average IOP of the conventional group, air group and BSS group were 13.69±2.69, 16.40±2.86 and 18.35±2.88 mmHg, respectively. The average IOP of the air group and the BSS group were significant higher than that of the conventional group(F=17.18, P<0.001). Primary reattachment rates were 88%, 96%, and 100%, respectively. The postoperative BCVA was 0.71±0.42, 0.59±0.44, and 0.91±0.50, respectively, which were significantly higher than those before operation(all P<0.05), but there was no significant difference among groups(F=3.046, P>0.05). The main complications included subretinal hemorrhage in 1 eye from the conventional group and 1 eye from the air group, and a new retinal tear in 1 eye from the air group, resulting in localized retinal detachment.CONCLUSIONS: For SBRD patients with hypotony during SB surgery, intravitreal injection of BSS to properly increase the IOP and then complete the surgery can improve the reattachment rate and reduce postoperative complications. This method is safe and effective for selected SBRD patients.
4.Advances in the multi-omics research on nonalcoholic fatty liver disease

The prevalence rate of nonalcoholic fatty liver disease(NAFLD)reaches up to 30%around the world,and the disease has a serious impact on human health and constitutes a public health burden.Due to difficulties in the diagnosis and monitoring of NAFLD,it is important to identify potential drug targets and biomarkers,and multi-omics techniques hold great promise in the search for early diagnostic markers,therapeutic targets,and outcome and prognostic assessment of NAFLD.This article reviews the research advances in multi-omics techniques in the field of NAFLD in recent years,in order to provide a richer theoretical basis and new strategies for the prevention and treatment of NAFLD.
5.Research progress on health-related quality of life in sepsis patients after discharge

This article summarizes the current research status of health-related quality of life (HRQOL) in sepsis patients after discharge, including the current states of short-term and long-term HRQOL, influencing factors, and interventions. Recent studies have shown that both short-term and long-term HRQOL of sepsis patients after discharge are suboptimal. Given the numerous and long-lasting factors that affect HRQOL, it is recommended that individualized nursing interventions targeting these factors be implemented in clinical practice. Further research should explore effective methods to improve the HRQOL of sepsis patients.
6.Discussion of Application Status and Key Attentions on Registration for Ossicular Replacement Prosthesis.

The ossicular replacement prosthesis should have good biocompatibility, stability, easy to install, and excellent sound transmission capacity. In this study, the characteristics of ideal materials for the ossicular replacement prosthesis were analyzed by searching the types of materials used in clinical practice and comparing the advantages and disadvantages of various materials and structures. At the same time, in combination with the current evaluation requirements and evaluation experience, the focus of the performance research project of ossicular replacement prosthesis in the process of registration is discussed to clarify the performance evaluation requirements of these products, so as to provide reference for the future work of manufacturers and regulators. The performance evaluation of ossicular replacement prosthesis focuses on its mechanical properties, fixation stability, sound transmission characteristics, biological characteristics, and magnetic resonance compatibility.

7.Construction of training program system for hospice care volunteers

Objective:To construct the training program system for hospice care volunteers and provide reference for the training of hospice care volunteers in China.Methods:The training program system for hospice care volunteers was initially determined by using the method of literature analysis and investigation, and 16 experts were consulted by two rounds of letters using the method of expert inquiry from May to July 2022, and finally the training program system was established.Results:The effective recovery rate of the two rounds of expert consultation questionnaire was 100%, the expert authority coefficient was 0.88, and the Kendall coordination coefficient was 0.141, 0.131 (both P<0.05). The final training program system for hospice care volunteers contained 7 first-class indicators including training objectives, training objects, training contents, training methods, training hours, training resources and training evaluation, 27 second-class indicators and 92 third-class indicators. Conclusions:The training program system for hospice care volunteers constructed in this study has high reliability and scientificity, and has a good guiding role and reference value for the training of hospice care volunteers.

9.Establishment of a prognostic nomogram model for predicting acute renal injury in patients with moderate and severe burns.

OBJECTIVE:
To establish a prediction model of acute kidney injury (AKI) in moderate and severe burn patients, so as to provide basic research evidence for early identification of burn-related AKI.
METHODS:
Patients who were admitted to the department of plastic burn surgery of the Affiliated Hospital of Southwest Medical University from November 2018 to January 2021 were selected, and their clinical characteristics, laboratory examinations and other indicators were recorded. Multivariate Logistic regression analysis was used to screen out the risk factors of AKI related to moderate and severe burns, and R software was used to establish the nomogram of moderate and severe burn patients complicated with AKI. The Bootstrap method model was used for internal verification by repeating sample for 1 000 times. Consistency index and calibration curve were used to evaluate the accuracy of the model, and the receiver operator characteristic curve (ROC curve) and the area under the curve (AUC) were used to evaluate the prediction efficiency, decision curve analysis (DCA) was used to evaluate the clinical utility of the model.
RESULTS:
A total of 186 patients with moderate and severe burn were included, among which 54 patients suffered from AKI, and the incidence rate was 29.03%. Multivariate Logistic regression analysis showed that the total burn surface area [TBSA; odds ratio (OR) = 1.072, 95% confidence interval (95%CI) was 1.031-1.115, P = 0.001], estimated glomerular filtration rate (eGFR; OR = 0.960, 95%CI was 0.931-0.990, P = 0.010), neutrophil (NEU; OR = 1.190, 95%CI was 1.021-1.386, P = 0.026), neutrophil/lymphocyte ratio (NLR; OR = 0.867, 95%CI was 0.770-0.977, P = 0.019), D-dimer (OR = 4.603, 95%CI was 1.792-11.822, P = 0.002) were the risk factors for patients with moderate and severe burn complicated with AKI. Taking the above indexes as predictive factors, a nomogram prediction model was established, the ROC curve was plotted with AUC of 0.998 (95%CI was 0.988-1.000). Optimum threshold of ROC curve was -0.862, the sensitivity was 98.0% and the specificity was 98.2%, and the consistency index was 0.998 (95%CI was 0.988-1.000). The calibration curve showed that the prognostic nomogram model was accurate, DCA showed that most patients can benefit from this model.
CONCLUSIONS
The burned patients with higher TBSA, NEU, NLR, D-dimer and lower eGFR tend to suffer from AKI. The nomogram based on the above five risk factors has high accuracy and clinical value, which can be used as a predictive tool to evaluate the risk of AKI in moderate and severe burn patients.

10.Application value of humidifying high-flow nasal cannula oxygen therapy after weaning of pediatric intensive care unit patients

Objective:To explore the application value of humidifying high-flow nasal cannula oxygen therapy (HHFNC) in children with pediatric intensive care unit (PICU) after weaning.Methods:From January 2018 to October 2021, 42 children with endotracheal intubation admitted to PICU of Tai′an city Central Hospital were prospectively selected and randomly divided into HHFNC group and nasal continuous positive airway pressure (NCPAP) group, with 21 patients in each group. The blood gas analysis [arterial partial pressure of oxygen (PaO 2), partial pressure of carbon dioxide in artery (PaCO 2), PaO 2/oxygen concentration (FiO 2)], blood oxygen saturation (SaO 2), comfort, non-invasive ventilation time, and total hospital stay of the two groups of children 1 hour after using HHFNC and NCPAP were compared, and the rate of reintubation of trachea within 48 hours, gastroesophageal reflux, nasal injury, facial skin indentation, abdominal distension, and pulmonary air leakage were recorded. Results:There was no significant difference between the two groups in terms of blood gas analysis (PaO 2, PaCO 2, PaO 2/FiO 2), SaO 2, pulmonary air leakage, non-invasive ventilation time, hospital stay, and reintubation rate within 48 h after weaning (all P>0.05). Compared with NCPAP group, HHFNC group had higher comfort, lower incidence of facial skin indentation, gastroesophageal reflux, nasal injury and abdominal distension, and the difference was statistically significant (all P<0.05). Conclusions:HHFNC and NCPAP can both be used as the transitional respiratory support mode after weaning, and the clinical treatment effect are similar. The HHFNC group has higher comfort, which is more conducive to improving the tolerance of children, reducing adverse reactions, and has higher safety.
