1.Research progress on quality control methods of traditional Chinese medicine based on artificial intelligence sense technology

In order to further promote and achieve the modernization of traditional Chinese medicine,it is particularly important to establish a quality control system of traditional Chinese medicine which not only respects the theory of traditional Chinese medicine but also conforms to the development of the times.Since ancient times,the appearance character of traditional Chinese medicine,such as color,gas and taste,is one of the important criteria to distinguish its authenticity and quality.At present,the research on the appearance of traditional Chinese medicine has gradually shifted from subjective"evaluation of quality from appearance traits"to artificial intelligence sensory technology which can provide objective data support.According to the different simulated senses,intelligent sensory technology can be divided into electronic eyes,electronic nose,electronic tongue,electronic ear and electronic skin.This paper combs the principles of five kinds of artificial intelligence sensory technology and their application in the quality evaluation of traditional Chinese medicine,introduces the research status and future development trend of the quality control system of traditional Chinese medicine based on intelligent sense,in order to provide a reference for the upgrade and modern development of the quality control system of traditional Chinese medicine.
2.The diagnostic value of deep medullary vein score for cognitive impairment in cerebral small vessel disease

Objective To explore the diagnostic value of deep medullary vein (DMV) score for cognitive impairment in patients with cerebral small vessel disease (CSVD).Methods Collect clinical and imaging data of 108 patients who visited Lishui Hospital of Traditional Chinese Medicine Affiliated to Zhejiang Chinese Medical University from May 2022 to December 2023.According to the mini-mental state examination,patients were divided into a cognitive impairment group (36 cases) and a cognitive function normal group (72 cases).On magnetic sensitive images,the DMV scores of the six regions on both sides of the frontal lobe,parietal lobe,occipital lobe are rated from 0 to 3 points based on their visual conditions,and the total scores of the six regions is the DMV score (0 to 18 points).Applying international CSVD guidelines to assess CSVD burden in patients.Analyze the correlation between DMV score and cognitive impairment,and compare the diagnostic value of DMV scores and CSVD score for cognitive impairment.Results The median CSVD burden score of 108 patients was 1 (0,2) points,and the median DMV score was 4 (1,9) points.The DMV score was positively correlated with the presence of cognitive impairment (r=0.525,P<0.001).There were statistically significant differences in age,cerebral microbleeds,perivascular space enlargement,white matter hyperintensities,CSVD burden,and DMV score between the cognitive impairment group and the cognitive function normal group (P<0.05).The area under the receiver operating characteristic curve of diagnosed with cognitive impairment using DMV score was 0.820 (95%CI:0.741-0.898,P<0.001).Conclusion DMV score is positively correlated with CSVD cognitive impairment,and it has certain predictive value for cognitive impairment.
3.Clinical characteristics and follow-up status of patients aged 75 years and above undergoing coronary stenting and the rate of achieving low-density lipoprotein cholesterol

【Objective】 To investigate the clinical characteristics, long-term follow-up rate, level and control rate of low-density lipoprotein cholesterol (LDL-C) in patients with atherosclerotic cardiovascular disease (ASCVD) aged ≥75 years who underwent percutaneous coronary intervention (PCI) during hospitalization. 【Methods】 We selected ASCVD patients aged ≥75 years with PCI from January 2016 to December 2020 in The First Affiliated Hospital of Xi’an Jiaotong University, collected the baseline data of the patients and the follow-up of 1 month, 3 months, 6 months and 12 months after discharge by HIS system, and analyzed their LDL-C and control rate at each follow-up. 【Results】 A total of 1 129 patients were enrolled in this study, aged 78 (ranging from 75 to 89) years. Among them 72.1% were male; myocardial infarction was the main type of ASCVD (71.5% ); hypertension was the most common risk factor, accounting for 85.2% (717/842), followed by diabetes, 58.6% (493/842); 74.6% met the ultra-high risk criteria of the 2020 Chinese Expert Consensus on Lipid Management in Ultra-High Risk ASCVD Patients, and the LDL-C control rate was only 8.1% . The four routine follow-up rates of 1 129 elderly ASCVD patients were 49.5%, 24.1%, 17.1%, and 24.6%, respectively. The detection rates of LDL-C during follow-up were 26.3%, 5.3%, 10.4%, and 13.8%, respectively. LDL-C control rates in ultra-high risk ASCVD were 59.4%, 45.1%, 37.1%, and 17.6%, respectively, while LDL-C control rates in non-ultra-high risk ASCVD patients were 67.3%, 55.6%, 47.4%, and 44.0%, respectively. 【Conclusion】 The elderly patients with ASCVD-PCI were mainly ultra-high risk patients. The routine follow-up rate and the LDL-C compliance rate during follow-up were low and showed a downward trend.

The condition of patients with severe traumatic brain injury (sTBI) complicated by corona virus 2019 disease (COVID-19) is complex. sTBI can significantly increase the probability of COVID-19 developing into severe or critical stage, while COVID-19 can also increase the surgical risk of sTBI and the severity of postoperative lung lesions. There are many contradictions in the treatment process, which brings difficulties to the clinical treatment of such patients. Up to now, there are few clinical studies and therapeutic norms relevant to sTBI complicated by COVID-19. In order to standardize the clinical treatment of such patients, Critical Care Medicine Branch of China International Exchange and Promotive Association for Medical and Healthcare and Editorial Board of Chinese Journal of Trauma organized relevant experts to formulate the Chinese expert consensus on clinical treatment of adult patients with severe traumatic brain injury complicated by corona virus infection 2019 ( version 2023) based on the joint prevention and control mechanism scheme of the State Council and domestic and foreign literatures on sTBI and COVID-19 in the past 3 years of the international epidemic. Fifteen recommendations focused on emergency treatment, emergency surgery and comprehensive management were put forward to provide a guidance for the diagnosis and treatment of sTBI complicated by COVID-19.
5.Rapid Characterization of Chemical Constituents in Dayuanyin by UPLC-Q-Exactive Orbitrap MS

ObjectiveTo characterize the chemical constituents of Dayuanyin based on ultra-performance liquid chromatography-quadrupole/electrostatic field orbitrap mass spectrometry(UPLC-Q-Exactive Orbitrap MS). MethodThe detection was performed on a Thermo Acclaim™ RSLC 120 C18 column(2.1 mm×100 mm, 2.2 μm), the mobile phase was acetonitrile(A)-0.1% formic acid aqueous solution(B) for gradient elution (0-7.5 min, 10%-19%A; 7.5-12 min, 19%-22.5%A; 12-23 min, 22.5%-27%A; 23-27 min, 27%-56%A; 27-35 min, 56%-84%A; 35-36 min, 84%-90%A), the flow rate was 0.3 mL·min-1, and the column temperature was 30 ℃. The data were collected in the positive and negative ion modes by heated electrospray ionization(HESI), and the detection range was m/z 80-1 200. Combining the retention time of the reference substance, fragment ions, databases such as PubChem and related literature, Xcalibur 3.0 was used to identify the chemical constituents of Dayuanyin. ResultA total of 161 compounds were identified, including 14 alkaloids, 60 flavonoids, 16 terpenoids, 26 saponins, 18 phenylpropanoids, 16 organic acids and 11 others. ConclusionThe established method can effectively and quickly identify the chemical components in Dayuanyin, and clarify its chemical composition, which can provide a basis for the development of compound preparations of this famous classical formula.
6.Clinical features of patients with epilepsy in Neurosurgery Outpatient and influencing factors for their seizure control

Objective:To investigate the clinical features of patients with epilepsy in Neurosurgery Outpatient and influencing factors for their seizure control.Methods:Six hundred and seventy-three epilepsy patients admitted to Neurosurgery Outpatient of 6 hospitals including Fifth Affiliated Hospital of Zhengzhou University from September 2017 to December 2022 were chosen. Clinical data (including general demographic data, education level, onset age, onset cycle and duration, course of onset, family annual income and seizure control) were collected using a questionnaire prepared by He'nan Epilepsy Systematic Diagnosis and Treatment Center to summarize the clinical features. Univariate and multivariate Logistic regressions were used to analyze the influencing factors for their seizure control.Results:(1) In these 673 epilepsy patients, 50 (7.4%), 78 (11.6%), 192 (28.5%), 100 (14.9%), 68 (10.1%), 72 (10.7%) and 113 (16.8%), respectively, were <1 year old (infant stage), 1-2 years old (children stage), 3-5 years old (preschool stage), 6-16 years old (juvenile stage), 17-39 years old (young stage), 40-64 years old (middle-aged stage) and ≥65 years old (elderly stage). In the past medical treatment history, 23.0% (155/673) patients did not receive intervention, 72.4% (487/673) received medication, and 4.6% (31/673) received surgical treatment; 55.9% (376/673) had good seizure control and 44.1% (297/673) had poor seizure control. (2) Secondary education ( OR=2.199, 95% CI: 1.037-15.221, P=0.033), primary education or below ( OR=3.544, 95% CI: 2.101-21.343, P=0.012), daily seizures ( OR=4.788, 95% CI: 1.369-33.103, P=0.011), each seizure lasted ≥3 min ( OR=4.179, 95% CI: 3.338-18.550, P=0.003), course of disease≥3 years ( OR=0.199, 95% CI: 0.077-0.602, P=0.001), course of disease for 1-3 years ( OR=0.379, 95% CI: 0.108-0.882, P=0.031), and currently taken antiepileptic drugs for 3 or more ( OR=6.237, 95% CI: 2.195-17.837, P=0.001) were independent risk factors for poor seizure control in epilepsy patients. Conclusion:In Neurosurgery Outpatient, children with diseases before childhood enjoy the largest proportion; drug therapy remains the main treatment; low education level, short seizure cycle, long duration of attack, long course of disease, and multiple drugs used in these patients imply poor anti-epileptic effecacy.
7.Epidemiological characteristics of severe fever with thrombocytopenia syndrome and association with tick density

ObjectiveTo understand the epidemiological characteristics of severe fever with thrombocytopenia syndrome (SFTS), and the association with seasonal fluctuation in tick density, so as to provide scientific evidence for better control and prevention measures. MethodsDescriptive epidemiological methods were used to analyze the SFTS epidemic situation in Chongyang County from 2013 to 2021. The cloth flag method was used to investigate the seasonal fluctuation in tick density and determine the association with the SFTS. ResultsA total of 71 SFTS cases were reported in Chongyang County from 2013 to 2021 including 8 deaths, with the annual morbidity rate of 1.88/10-5. The majority were mainly from June to August, accounting for 61.97% of the total reported cases. The cases were geographically distributed in 41 administrative villages and 4 urban areas in 11 towns. The highest incidence was in Lukou town (n=26) and Baini town (n=12), which accounted for 53.52% of the total reported cases. Furthermore, the cases were mainly 50‒69 years old, accounting for 57.75% of the total reported cases. The ratio of male to female was 1.84∶1, and most were farmers. Blood samples during acute phase in 41 cases were collected for examination of SFTS virus, of which 25 cases tested positive (60.98%). There was a positive association between tick density and the number of reported SFTS cases (r=0.78, P=0.003). ConclusionSeasonal density of free adult ticks and nymphs is associated with the number of reported SFTS cases in Chongyang County.
8.Research progress of tertiary lymphoid structure on hepatocellular carcinoma

Tertiary lymphoid structure (TLS) is the ectopic lymphoid tissue around the chronic inflammatory site of tumor, infection diseases, autoimmunity diseases, organ transplantation and so on. TLS is regarded as the vital niche of antitumor immune response in tumor microenvironment for abundant immune cells, and is correlated with better clinical outcomes and immunotherapy response in most solid tumors. As a typical inflammation-driven cancer, hepatocellular carcinoma (HCC) is controversial with the influence of TLS on patients prognosis. In this paper, the composition, formation mechanism, recognition and clinical value of TLS in HCC were briefly reviewed.
9.Immunogenicity and immune persistence of purified Vero cell rabies vaccine with Zagreb and Essen regimens

Objective:To evaluate the immunogenicity and immune persistence of purified Vero cells rabies vaccine (PVRV) with Zagreb and Essen regimen.Methods:Prospective study: Patients with first Class II exposure to rabies were recruited from the Dog Injury Cinic of Guangxi Center for Disease Control and Prevention (Guangxi CDC) and randomly divided into the Zagreb (2-1-1) and Essen (1-1-1-1-1) regimen group. All patients were inoculated with the vaccines from the same manufacturer and batch, and 3 ml serum was collected at the 45th day and in 1-year, 2-year and 3-year after immunization. Rapid fluorescent inhibition test (RFFIT) was used to detect rabies virus neutralizing antibody (RVNA). The attenuation of RVNA positive rate and geometric mean titer (GMT) with time was analyzed. Retrospective study: The informed consents for rabies vaccine in the Dog Injury Clinic of Guangxi CDC were checked out. The patients who were injected with PVRV (same manufacturer but unlimited batch) but without passive immune agents for the first time within 3 years were selected and divided into 1-year, 2-year and 3-year group. Each group was further divided into Zagreb and Essen regimen group. The serum (3 ml) was collected at 1 year, 2 years and 3 years after immunization and detected the RVNA by RFFIT.Results:Prospective study: The RVNA positive rates on the 45th day and in 1-year, 2-year and 3-year after immunizationin in the Zagreb and Essen regimen group were 100%, 95%, 85%, 80% and 98.25%, 89.47%, 89.47%, 85.96%, respectively. There was no statistically significant difference in the RVNA positive rates at the same time point between the two regimen groups ( P>0.05). The RVNA GMT on the 45th day and in 1-year, 2-year, and 3-year in the Zagreb and Essen regimen group were 11.32 IU/ml, 1.69 IU/ml, 1.30 IU/ml, 1.30 IU/m and 13.18 IU/ml, 2.13 IU/ml, 1.87 IU/ml, 1.84 IU/m, respectively. There was no significant difference in the RVNA GMT levels at the same time point between the two regimen groups ( F=1.971, P=0.164). The RVNA GMT levels in the two regimen groups had the same trend of attenuation over time (time*group F=0.702, P=0.435). Retrospective study: The RVNA positive rates in 1-year, 2-year and 3-year after immunization in the Zagreb and Essen regimen group were 100%, 95%, 91.43%和94.73%, 86.21%, 87.5%, respectively. There was no statistically significant difference in the RVNA positive rates at the same time point between the two regimen groups ( P>0.05). The RVNA GMT in 1-year, 2-year, and 3-year groups after immunization in the Zagreb and Essen regimen group were 2.65 IU/ml, 2.03 IU/ml, 1.57 IU/ml和3.2 IU/ml, 2.58 IU/ml, 2.45 IU/ml, respectively. There was no significant difference in the RVNA GMT levels at the same time point between the two regimen groups ( P>0.05). Conclusions:The PVRV showed the same excellent immunogenicity and immune persistence after the vaccination with the Zagreb and Essen regimens.
10.Preventive medicine curriculum system in training program of clinical medicine in the era of Healthy China

Objective:To understand the provision of preventive medicine curriculum system in the training programs of clinical medicine in the era of Healthy China.Methods:A total of 36 training programs of clinical medicine were selected from different areas of China for a statistical analysis on their basic information, involvement of concept of preventive medicine in program objectives, and provision of preventive medicine curriculum system.Results:Of all the 36 training programs of clinical medicine, 22(61%) have no mentions of prevention medicine in their program objectives; only one university’s training program states preventive medicine together with basic medicine and clinical medicine as one of the three main disciplines. The total class hours for the core courses of preventive medicine (hygiene, medical statistics, epidemiology, evidence-based medicine, and social medicine) range from 80 to 252, with an average of (156.7±43.2) hours. The average percentage of class hours for preventive medicine courses among the total class hours is 4.3%±1.1% (range: 2.5%-7.5%), and obvious differences exist among universities.Conclusions:In current training programs of clinical medicine, the proportion of prevention medicine curriculum is insufficient, the percentage of hours for preventive medicine course is very low, and the differences among various universities are obvious. It is urgently needed to strengthen preventive medicine curriculum in training programs for clinical medical students in new era. It is suggested to further promote the concept of putting prevention first, improve the curriculum system of clinical medicine, intensify the integrated development preventive medicine and clinical medicine and pay attention to clinical research ability enhancement for the further improvement of training program of clinical medicine.
