Objective:
To investigate the objective characteristics of tongue manifestation in different stages of damp-heat syndrome in diabetic kidney disease (DKD).
Methods:
A cross-sectional study enrolled 134 patients with DKD G3-5 stages who met the diagnostic criteria for damp-heat syndrome in DKD. The patients were treated at Dongzhimen Hospital, Beijing University of Chinese Medicine, from May 2023 to January 2024. The patients were divided into three groups: DKD G3, DKD G4, and DKD G5 stage, with 53, 33, and 48 patients in each group, respectively. Clinical general data (gender, age, and body mass index) and damp-heat syndrome scores were collected from the patients. The YZAI-02 traditional Chinese medicine (TCM) AI Tongue Image Acquisition Device was used to capture tongue images from these patients. The accompanying AI Open Platform for TCM Tongue Diagnosis of the device was used to analyze and extract tongue manifestation features, including objective data on tongue color, tongue quality, coating color, and coating texture. Clinical data and objective tongue manifestation characteristics were compared among patients with DKD G3-5 based on their DKD damp-heat syndrome status.
Results:
No statistically significant difference in gender or body mass index was observed among the three patient groups. The DKD G3 stage group had the highest age (P<0.05). The DKD G3 stage group had a lower score for symptoms of poor appetite and anorexia(P<0.05) than the DKD G5 group. No statistically significant difference was observed in damp-heat syndrome scores among the three groups. Compared with the DKD G5 stage group, the DKD G3 stage group showed a decreased proportion of pale color at the tip and edges of the tongue (P<0.05). The DKD G4 stage group exhibited an increased proportion of crimson at the root of the tongue, a decreased proportion of thick white tongue coating at the root, a decreased proportion of pale color at the tip and edges of the tongue, an increased hue value (indicating color tone) of the tongue color in the middle, an increased brightness value (indicating color lightness) of the tongue coating color in the middle, and an increased thickness of the tongue coating (P<0.05). No statistically significant difference was observed in other tongue color proportions, color chroma values, body characteristics, coating color proportions, coating color chroma values, and coating texture characteristics among the three groups.
Conclusion
Tongue features differ in different stages of DKD damp-heat syndrome in multiple dimensions, enabling the inference that during the DKD G5 stage, the degree of qi and blood deficiency in the kidneys, heart, lungs, liver, gallbladder, spleen, and stomach is prominent. Dampness is more likely to accumulate in the lower jiao, particularly in the kidneys, whereas heat evil in the spleen and stomach is the most severe. These insights provide novel ideas for the clinical treatment of DKD.

Ferroptosis is a novel iron-dependent mode of programmed cell death characterized by iron deposition and accumulation of lipid peroxidation. More and more studies have found that ferroptosis is closely related to the pathogenesis of type 2 diabetes mellitus (T2DM). The yin-fire theory is an important part of LI Gao's spleen-stomach theory, and it is believed that qi-fire imblance and yin-fire internal generation is the main pathogenesis of T2DM. Abnormal iron metabolism may be an important prerequisite for T2DM yin-fire internal generation, while oxidative stress is the specific manifestation of T2DM qi-fire imbalance. Reactive oxygen species (ROS) is the end product of qi-fire imbalance, and lipid peroxide is the pathological products of T2DM yin-fire internal generation. This study intends to explore the pathological mechanism of qi-fire imbalance and yin-fire internal generation from the perspectives of iron metabolism, oxidative stress and lipid peroxidation, enriching the modern connotation of yin-fire theory, and benefiting traditional Chinese medicine to target against ferroptosis, and prevent and treat T2DM precisely.

ObjectiveTo observe the effects of Yuejuwan in the treatment of psychological and heart diseases (PHD) and explore its mechanism. MethodThirty 6-week-old healthy male SPF AopE-/- mice and 10 homologous C57BL/6J mice were selected for the experiment. The 30 AopE-/- mice were divided into a model group, low-dose (7.58 g·kg-1·d-1) and high-dose (30.32 g·kg-1·d-1) Yuejuwan groups, with 10 mice in each group, and 10 C57BL/6J mice were assigned to the blank control group. Intragastrical administration lasted 12 weeks. During feeding, the PHD model was induced by chronic unpredictable mild stress (CUMS) combined with high-fat diet in mice. After intragastric administration, the behavioral results [open field test (OFT) and sucrose preference test (SPT)] of mice in each group, the content of aspartic transaminase (AST), alanine aminotransferase (ALT), 5-hydroxytryptamine (5-HT), noradrenaline (NE), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), total cholesterol (TC), and triglyceride (TG) in serum of mice detected by the automatic biochemical analyzer, the oil red O staining and HE staining of aorta and liver and Masson staining of myocardial tissues were used for model evaluation. Finally, liver TMT-labeled quantitative proteomics was used to explore the mechanism of action. ResultThe model mice showed obvious manifestations of depression, anxiety, loss of interest, and despair, manifest lipid deposition in the aorta and liver by pathological observation, and increased myocardial fibrosis in myocardial tissues. After intragastric administration of Yuejuwan, the above symptoms and indexes of the PHD model mice were improved. Compared with the blank control group, the model group showed decreased standing times, cumulative time in the central area, total moving distance, moving speed, and sucrose preference at week 12 (P<0.01). Compared with the model group, the Yuejuwan groups showed decreased indexes mentioned above (P<0.01). After sample collection, AST, ALT, and TG levels in the model group were higher (P<0.01) and the levels of 5-HT, NE, and HDL-C were lower than those in the blank control group (P<0.01). The results of liver TMT labeled quantitative proteomics suggested that the PHD model mainly caused the changes in protein expression levels such as ApoE, UGT1A5, and FASN in mice, involving acetyl CoA metabolism, response to bacteria, cellular amino acid catabolism, and other processes, which were related to the abnormal metabolic function of the liver. The efficacy of Yuejuwan against PHD was achieved mainly through the regulation of high mobility group nucleosomal-binding domain 2 (HMGN2), CALD1, and Mup7 protein expression levels and correcting the biological processes and abnormal pathways related to the pathogenesis of PHD,including muscle contraction, tight junction pathway, myocardial contraction pathway, and focal adhesion pathway. ConclusionCUMS combined with high-fat diet is reasonable in the induction of the PHD model in AopE-/- mice. Yuejuwan can correct the depression and anxiety conditions of PHD model mice, reduce the aortic plaque, and recover the abnormal blood lipid and liver function levels. Furthermore, Yuejuwan can correct abnormal biological processes and pathways of PHD model mice. The differential proteins screened throughout the experiment and the involved physiological and pathological changes are the focus of the next experiment.

Objective:To confirm the efficacy and safety of cinepazide maleate injection in acute ischemic stroke patients with obvious motor function deficit. Methods:This study is a subgroup analysis of multi-center, randomized, double-blind, placebo-controlled phase Ⅳ clinical trial. A total 812 patients of acute ischemic stroke with obvious limb motor deficit [motor function of limbs score in National Institutes of Health Stroke Scale (NIHSS) ≥4] were enrolled in this subgroup analysis. Patients received either cinepazide maleate injection or placebo. The treatment period was 14 days and follow-up was 90 days. The efficacy endpoints included the proportions of patients with a modified Rankin Scale (mRS) score ≤2, mRS score ≤1 and Barthel Index <95 on day 90. Safety was evaluated by recording all adverse events, monitoring vital signs, laboratory parameters and electrocardiogram. Results:A total of 732 patients were involved in the final efficacy analysis (361 in cinepazide maleate group and 371 in control group). The baseline limb motor function score of NIHSS was 5.23±1.43 in the cinepazide maleate group whereas 5.20±1.36 in the control group. Logistic regression analysis showed that following treatment for 90 days, the proportion of patients with a mRS score ≤2 was significantly higher in the cinepazide maleate group than in the control group [56.0% (202/361) vs 44.2% (164/371), OR=0.60, 95% CI 0.44-0.82, P=0.002]. The proportion of patients with a mRS score ≤1 was higher in the cinepazide maleate group than in the control group [43.3% (139/361) vs 35.2% (118/371), OR=0.69, 95% CI 0.50-0.97, P=0.031]. The proportion of patients with a Barthel Index <95 on day 90 was significantly lower in the cinepazide maleate group than in the control group [45.2% (145/361) vs 55.2% (185/371), OR=0.64, 95% CI 0.46-0.88, P=0.007]. During the treatment and follow-up period, the incidence of the most common adverse events in the cinepazide maleate group was 50.4% (199/395). Constipation and abnormal liver function were more common, but there were no statistically significant differences between the two groups. Conclusion:Cinepazide maleate injection is superior to placebo in improving neurological function and activities of daily living, reducing disability, and promoting functional recovery and safe in patients with acute ischemic stroke with obvious limb motor deficit.

Objective:To investigate the value of 18F-fluorodeoxyglucose (FDG) PET imaging in evaluating the 18F-FDG influx rate of lungs and its relationship with parameters of pulmonary hemodynamics in patients with pulmonary arterial hypertension related to congenital heart disease (PAH-CHD). Methods:From January 2018 to June 2019, a total of 16 PAH-CHD patients (6 males, 10 females, age (29.2±10.6) years) and 22 health controls who received physical examinations (8 males, 14 females, age (45.4±3.8) years) in Fuwai Hospital were respectively enrolled. All cases underwent dynamic 18F-FDG PET imaging for whole lung 18F-FDG influx rate (presented as Ki). Right heart catheterization was performed to evaluate pulmonary hemodynamic parameters such as pulmonary vascular resistance (PVR), mean pulmonary vascular pressure (mPAP) in PAH-CHD patients after imaging within one week. Independent-sample t test was used to compare Ki of 2 groups, and Pearson correlation analysis was used to analyze the relationship between Ki and PVR, mPAP in PAH-CHD patients. Results:Ki of the lungs was significantly higher in PAH-CHD patients than that in controls ((0.000 6±0.000 3) vs (0.000 4±0.000 3) ml·g -1·min -1; t=2.15, P=0.038). Ki was not correlated with PVR ((10.86±4.45) Wood units) or mPAP ((69.75±18.93) mmHg; 1 mmHg=0.133 kPa) in PAH-CHD patients ( r values: 0.202 and 0.006, both P>0.05). Conclusions:Pulmonary vascular remodeling can lead the increasing lung 18F-FDG uptake in patients with PAH-CHD. 18F-FDG PET may have the ability in monitoring and evaluating pulmonary vascular remodeling in PAH-CHD.
