1.Effect of different energy of extracorporeal shock waves on diabetic neuralgia in rats

Objective To observe the effects of different energy of extracorporeal shock waves (ECSWs) on diabetic neuralgia in rats.Methods Fifty clean-grade healthy male Sprague-Dawley rats of both sexes,aged 8 weeks,weighing 180-200 g,were divided into 5 groups (n=10 each) using a random number table method:control group (group C),diabetic neuralgia group (group DN),low-energy ECSW group (group L + DN),medium-energy ECSW group (group M + DN),and high-energy ECSW group (group H+DN).Diabetic neuralgia models were established by intraperitoneally injecting streptozotocin (60 mg/kg) in DN,L+DN,M+DN and H+DN groups.ECSWs at 1,2 and 3 bar were applied during 4 consecutive weeks after successful establishment of the model once a week (T1-T4) in L+DN,M+DN and H+ DN groups,respectively.The mechanical paw withdrawal threshold (MWT),thermal paw withdrawal latency (TWL) and motor nerve conduction velocity (MNCV) were measured at T1-T4.Animals were sacrificed after the last measurement,and the sciatic nerve samples were obtained for determination of the expression of tumor necrosis factor-alpha (TNF-α) and interluekin-6 (IL-6) (by Western blot) and expression of TNF-α and IL-6 mRNA (by real-time polymerase chain reaction).Results Compared with group C,MWT,TWL and MNCV were significantly decreased at T1-T4,and the expression of TNF-α and IL-6 protein and mRNA was up-regulated in the other groups (P<0.05).Compared with group DN,MWT at T2-4 and TWL and MNCV at T3,4 were significantly increased,and the expression of TNF-α and IL-6 protein and mRNA was down-regulated in L+DN,M+DN and H+DN groups (P<0.05).Compared with group H+ DN,MWT at T2-4 and TWL and MNCV at T3,4 were significantly increased,and the expression of TNF-α and IL-6 protein and mRNA was down-regulated in L+DN and M+DN groups,and the expression of IL-6 mRNA was significantly down-regulated in group L+DN (P<0.05).Conclusion ECSWs can mitigate diabetic neuralgia in rats,and the low-and medium-energy ECSWs produce better efficacy,and the mechanism is related to inhibiting inflammatory responses.
2.Combined measurement of H-FABP and MPO in the prognosis of non-ST elevation acute cornary syndromes

Objective To evaluate the prognosis value of heart‐type fatty‐acid binding protein(H‐FABP)and myeloperoxidase (MPO) in non‐ST elevation acute coronary syndromes(NSTE ACS) .Methods 181 NSTE ACS patients were divided into 4 groups according to the level of H‐FABP and MPO ,and the baselines of 4 groups such as gender ,age ,hypertension ,smoking ,body mass in‐dex ,diabetes mellitus ,hyperlipidemia were compared .The incidences of adverse cardiac events in 4 groups were compared after a 2 years′follow‐up .Results By multivariate COX regression adjustment for other risk factors ,the relative risk(RR) of H‐FABP for adverse cardiac events was 2 .023(95% CI:1 .029 -3 .987 ,P=0 .002) ,and MPO was (95% CI:2 .196 -5 .325 ,P< 0 .05) .The 2 years′follow‐up showed the incidence of adverse cardiac events in NSTE ACS patients with higher H‐FABP and MPO levels was higher than the NSTE ACS patients with one or two indicators of normal levels .Conclusion The combined application of H‐FABP and MPO could has clinical significance for the prognosis of patients with NSTE ACS .
3.Clinical Application of Myeloperoxidase in Diagnosis and Prognosis of Non-ST Elevation Acute Cornary Syndromes

Objective To evaluate the diagnostic value of myeloperoxidase (MPO)in diagnosis of non-ST elevation acute cor-onary syndromes (NSTE ACS).Methods The MPO was determined in 61 unstable angina pectoris (UAP)patients and 54 non-ST-elevation myocardial infarction (NSTE MI)patients,then analyzed the data with receiver operationg characteristic curve (ROC curve)statistical software.The major adverse cardiac events (MACE)were obseved in 106 NSTE ACS patients through 1 year of follow-up.Results The MPO values of NSTE MI group and UAP group were 672±418 ng/ml and 459± 328 ng/ml,the difference was statistically significant (t=3.928,P <0.001).The cTnI values were 558±261 pg/ml and 405 ±278 pg/ml of respectively.The difference was statistically significant (t = 3.089,P = 0.003).The area under the ROC curve of MPO was 0.726 when the optimal cutoff value was 382 ng/ml in the diagnosis of NSTE MI.The sensitivity and specificity was 92.6% and 39.8%.The area under the ROC curve of cTnI was 0.799 when the optimal cutoff value was 484 ng/ml in the diagnosis of NSTE MI.The sensitivity and specificity was 65.5% and 92.3%.High MPO maintained a strong association with the risk of major adverse cardiac events through 1 year of follow-up.Conclussion MPO is ahigh sensitivity marker of the NSTE MI and of clinical value for prognosis of NSTE ACS.
4.eIF3a gene polymorphism and chemo-sensitivity to platinum-based drugs in ovarian cancer.

OBJECTIVE:
To investigate the relationship between the eukaryotic initiation factor 3a (eIF3a)polymorphisms and chemo-sensitivity to platinum-based drug in ovarian cancer.
METHODS:
Matrix-assisted laser desorption ionization time-of-flight mass spectrometry (MALDI-TOF MS) analysis was performed to detect 57 cases of eIF3a polymorphic genotypes (rs3824830, rs77382849, rs10787899 and rs3740556) after platinum-based chemotherapy drugs up to 6 cycles in primary ovarian cancer. The association between these gene sites was analyzed.
RESULTS:
There were 3 genotypes for eIF3a rs3824830, named AA, GA and GG. The frequency distribution for them was 43.86%, 36.84% and 15.79% (2 cases did not detect the genotype, 3.51%), respectively. There were 2 genotypes for eIF3a rs77382849, named CC and TC. The frequency distribution for them was 85.96% and 12.28%(1 case did not detect the genotype, 1.76%), respectively. There were 3 genotypes for eIF3a rs10787899, named GG, GA and AA, respectively. The frequency distribution for them was 26.32%, 47.36% and 26.32%, respectively. There were significant difference in different genotypes between age group and FIGO stage (P<0.05). The genotype of eIF3a rs10787899 GA was easier to resist platinum drug compared with the GG genotype and the odds ratio could be increased by 2.676 (95%CI: 0.544-13.159). The genotype of eIF3a rs10787899 AA was easier to resist platinum drug compared with the GG genotype and the odds ratio could be increased by 5.419(95%CI: 0.964-30.471). Rebalanced by age and FIGO stage, there was no significant difference (P>0.05) among these genotype groups. In all blood samples, there was only one genotype for eIF3a rs3740556, named GG.
CONCLUSION
There is no mutation genotype in eIF3a rs3740556 loci. Polymorphism in the eIF3a rs3824830, rs77382849 and rs10787899 doesn't affect the response of ovarian cancer to platinum-based chemotherapy.

BACKGROUNDInsulin resistance (IR) plays an important pathophysiological role in the development of diabetes, dyslipidemia, hypertension, and cardiovascular disease. Moreover, IR can occur even in non-obese people without diabetes. However, direct detection of IR is complicated. In order to find a simple surrogate marker of IR early in non-obese people, we investigate the association of commonly-used biochemical markers (liver enzymes and lipid profiles) with IR in urban middle-aged and older non-obese Chinese without diabetes.
METHODSThis cross-sectional study included 1 987 subjects (1 473 women). Fasting blood samples were collected for measurement of glucose, insulin, liver enzymes, lipid profiles and creatinine. Subjects whose homeostasis model of assessment-IR (HOMA-IR) index values exceeded the 75th percentile (2.67 for women and 2.48 for men) of the population were considered to have IR. The area under the receiver operating characteristic curve (ROC) was used to compare the power of potential markers in identifying IR.
RESULTSTriglycerides (TG) and ratio of TG to high-density lipoprotein cholesterol (TG/HDL-C) discriminated IR better than other indexes for both sexes; areas under the receiver operating characteristic (ROC) curves (AUC) values were 0.770 (95% confidence interval 0.733-0.807) and 0.772 (0.736-0.809), respectively, for women and 0.754 (0.664-0.844) and 0.756 (0.672-0.840), respectively, for men. To identify IR, the optimal cut-offs for TG and TG/HDL-C ratio were 1.315 mmol/L (sensitivity 74.3%, specificity 71.0%) and 0.873 (sensitivity 70.1%, specificity 73.4%), respectively, for women, and 1.275 mmol/L (sensitivity 66.7%, specificity 74.4%) and 0.812 (sensitivity 75.8%, specificity 69.2%), respectively, for men.
CONCLUSIONTG and TG/HDL-C ratio could be used to identify IR in urban middle-aged and older non-obese Chinese without diabetes.

6.Impact of sirolimus and tacrolimus on the tumor recurrence after liver transplantation due to HCC beyond milan criteria: randomized controlled clinical trial

Objective To compare the impact of the sirolimus and tacrolimus on the tumor recurrence after liver transplantation due to HCC beyond Milan criteria.Method Sixty-one liver transplantation recipients due to HCC beyond Milan criteria,between Jan.2008 and Apri.2012,were randomized,with the informed consent,into two different immunosuppression groups: sirolimus group (n=30) and tacrolimus group (n=31).In tacrolimus group,tacrolimus was used as the basic immunosuppressant,methylprednisolone was discontinued within one month postoperatively,and mycophenolate mofetil was used within the dosage of 1.5 g/d accordingly.In sirolimus group,the immunosuppresive scheme was the same as that of the tacrolimus group within postoperative one month,and from that,tacrolimus was transferred to sirolimus.No antineoplastic agents were given before tumor recurrence.The tumor recurrence rate and the survival rate of the recipients were compared between the two groups.Result The median follow-up duration was 35.2 months (10.3~ 60.2).The tumor recurrence rate at postoperative year 1,2,3 and 4 in the sirolimus group (13.3%,36.7%,43.3% and 53.3%) was significantly lower than that in the tacrolimus group (38.7%,67.7%,74.2% and 77.4%),P < 0.05 for all.The one-year survival rate in the recipients postoperation had no significant difference between sirolimus group and tacrolimus group (90.0% vs.87.1%,P=0.438).The 2-,3-and 4-year survival rate in the recipients was significantly higher in the sirolimus group (53.3%,33.3% and 20.0%) than that in the tacrolimus group (41.9%,22.6% and 9.7%),P < 0.05 for all.The liver function and renal function of the recipients at the postoperative year 1,2,3 and 4 showed no significant difference between the two groups,P>0.05.Conclusion In comparison with tacrolimus,sirolimus could significantly reduce the tumor recurrence rate and increase the survival rate for the liver transplant recipients due to HCC beyond Milan criteria.
7.Clinical application of splitting liver transplantation

Objective To investigate the safety and efficacy of splitting liver transplantation (SLT). Methods Clinical data of 10 donors and 19 recipients undergoing SLT in Institute of Organ Transplantation, General Hospital of Chinese People's Armed Police Forces from January 2008 to August 2013 were analyzed retrospectively. Of these 19 recipients, 9 were male, 10 were female and 9 were children,10 were adults. The age ranged from 7 months to 62 years with a median age of 31 years. The graft recipient weigh ratio (GRWR) was 1.0%to 4.3%. The informed consents of all patients were obtained and the ethical committee approval was received. The 10 cadaveric donor livers were split in vitro, in which 9 pair grafts were transplanted in 9 children and 8 adults respectively. The left lateral lobes (segmentⅡ,Ⅲ) were transplanted in children and the extended right lobes (segmentⅠ,Ⅳ-Ⅷ) in adults. The other donor liver was split in left and right half lobes and were transplanted in 2 adults. Piggyback liver transplantation was performed in the 9 children receiving left lateral lobes and 1 adult receiving right half lobe. Classic orthotopic liver transplantation was performed in the 8 adults receiving extended right lobes and 1 adult receiving left half lobe. Peri-operative death and complications of the patients were observed. The patients were followed up regularly for observing the survival situation. Results All the operations of 19 recipients were completed successfully. Seventeen cases were cured and discharged from hospital and 2 cases died during the peri-operative period. The 2 dead cases were both children. One died of septic shock 2 weeks after operation and the other died of primary liver graft nonfunction 2 d after liver retransplantation. Biliary leak of liver section occurred in 2 pediatric recipients and was cured by puncture drainage. One adult recipient with intrahepatic cholangiocarcinoma died of lung and bone multiple metastases 15 months after operation. Conclusions SLT is a safe and feasible transplant operation and it can help to alleviate the shortage of donor livers to some extent.
8.The effect of mild hypothermia on inflammation after cerebral ischemia-reperfusion in rats

Objective To evaluate the protective effect of mild hypothermia against inflammatory cascade reaction in rats during cerebral ischemia and reperfusion.Methods After middle cerebral artery occlusion(MCAO)for 3 h in rats,the expression levels of ICAM-1,TNF-? and IL-1 ? in the ischemic regions were determined at different reperfusion time (6 h,12 h,24 h,48 h and 72 h).At 24 h,the cerebral infarction volume and neurologic function were evaluated.In the control,these were assessed at 24 hours reperfusion.Results (1)Mild hypothermia could ameliorate neurological deficit score and decrease infarct volume induced by cerebral ischemia and reperfusion.(2)The expression of ICAM-1,TNF-? and IL-1? rose obviously at 6 h after cerebral ischemia-reperfusion,and peaked each at 48 h,24 h and 6 h.There was significant difference between the various groups and the sham-operative group(P
9.Alteration of serous inflammatory cell factors associated with the neural functional defect and its related factors in patients with ischemic stroke

BACKGROUND: With the deep investigations of pathophysiological mechanism of acute cerebral infarction, it is discovered that inflammation occupies an important stance in the ischemic injuries of central nervous system ( CNS ), in which tumor necrosis factor-αt (TNF-α), interleukin- 1β(IL-1β), and soluble intercellular adhesion molecule(sICAM-1) become hotspots in the researches.OBJECTIVE: To investigate the relationship between the levels of serous inflammatory cell factors and the course of the disease, the severity of the situation in patients with ischemic stroke.DESIGN: A case-control study based on patients and healthy individuals.SETTING: Department of neurology in a university hospital.PARTICIPANTS: Fifty ischemic stroke patients including 23 males and 27 females with an average age of(60.26 ± 8.77) years old were selected from the outpatient and inpatient Departments of Neurology of the Renmin Hospital of Wuhan University between January 2001 and December 2003. Forty healthy controls including 18 males and 22 females with an average age of (61.05 ± 8.09) years old were selected from the subjects who had physical check up at outpatient department during corresponding period.INTERVENTIONS: Serous TNF-α, IL-1 β and sICAM-1 levels were detected by double-antibody-ELISA.MAIN OUTCOME MEASURES: Serous levels of TNF-α, IL-1β and sICAM-1 in patients with ischemic stroke of different stage, with different infarction volume and different neural functional defects.RESULTS: Serous TNF-α, IL-1β and sICAM-1 levels of patients with cerebral infarction during acute phase and convalescence were significant higher than that of control group( P < 0.01 ), and the levels was significantly higher in acute phase than convalescence ( P < 0.05 ) . The elevation was closely correlated with the degree of neural functional defect and the size of infarction volume, and furthermore, the serous content of TNF-α was also correlated with IL-1β and sICAM-1 levels.CONCLUSION: TNF-α, IL-1β and sICAM-1 interact and participate in the inflammation and reperfusion injury of acute cerebral infarction. Surveillance on them can provide experimental indicators for early clinical therapy and rehabilitative intervention, which is good for the control of the development and recurrence of stroke.
10.Observation on effect of combined Sanjin Tablets and antibiotic on female urinary tract infection after sex life

Objective:To observe the effect of combined Sanjin Tablets and antibiotic on women with urinary tract infection after sex life. Methods: 306 patients were randomly divided into two groups. The treatment group, taking oral SanJin Tablets and intravenous drip of ciprofloxacin, and the control group only ciprofloxacin given by intravenous drip, treatment course lasted for half a month. Results: In the treatment group, the total effective rate reached 96.77% and the rate of symptomatic improvement was 100.00%, and those indexes in the control group can only reached 80.00% and 82.35% respectively.
