1.Mechanism Study on Chaihuang Qingyi Huoxue Granules Reduce Inflammatory Damage in Severe Acute Pancreatitis Rats by Suppressing NLRP3 Inflammasome Activation

Objective To investigate the protective effect and mechanism of Chaihuang Qingyi Huoxue Granules on pancreatic tissue of rats with severe acute pancreatitis,and to observe its regulation on NLRP3 inflammasome activation.Methods Sixty-four SD rats were randomly divided into sham-surgery(SO)group,severe acute pancreatitis model(SAP)group,Chaihuang Qingyi Huoxue Granules(CH)group,and MCC950(NLRP3 inhibitor)group.Each group was further divided into 12-hour and 24-hour subgroups,with rats in each group.The SAP group,CH group,and MCC950 group were retrogradely injected with 3.5%sodium taurocholate(2 mL·kg-1)into the pancreatic ducts to establish SAP model.The MCC950 group was immediately intraperitoneally injected with MCC950(1 mg·mL-1)after model preparation.After awakening from anesthesia,the CH group was administrated by gavage with Chaihuang Qingyi Huoxue Granules solution(0.35 g·mL-1)once every 6 hours.Ascites,abdominal aortic blood,and pancreatic tissue were collected at 12 hours and 24 hours after SAP model construction.The serum amylase and lipase activities were detected using an automated biochemical analyzer.HE staining was used to observe pancreatic injury.Serum levels of IL-18 and IL-1β were detected by ELISA.The expressions of gene and proteins related to the activation of NLRP3 inflammasome were analyzed by IHC,qRT-PCR and Western Blot.Results Compared with the SAP group,the pathological damage of pancreatic tissues in the CH and MCC950 groups was significantly reduced,and the pathological score was significantly reduced(P<0.05).The levels of serum lipase,amylase,IL-18,and IL-1β were also significantly decreased(P<0.05).After treatment with Chaihuang Qingyi Huoxue Granules or intraperitoneal injection of NLRP3 inhibitor,the positive expressions of NLRP3,ASC and Caspase-1 in pancreatic tissues,as well as the mRNA levels of NLRP3,ASC and Caspase-1,the protein levels of NLRP3,ASC,Pro-Caspase-1 and Caspase-1 were significantly reduced compared to the SAP group(P<0.05).Conclusion Chaihuang Qingyi Huoxue Granules can inhibit the activation of NLRP3 inflammasome,reduce the mRNA and protein expressions of NLRP3,ASC and Caspase-1 in pancreatic tissues,and suppress the release of the downstream inflammatory factors IL-18 and IL-1β and alleviate pancreatitis damage in SAP model rats.
2.Epidemiological characteristics of hemorrhoids in a healthy physical examination population in China

Objective:To describe the epidemiological distribution of hemorrhoids in a physical exami-nation population in China,which could provide evidence for precision prevention and early intervention of hemorrhoids.Methods:Chinese subjects over 18 years of age who underwent a physical examination in a nationwide chain of physical examination centers in 2018 were studied in a cross-sectional design,which collected information by a questionnaire and physical examination results from each subject.The epidemiological distribution of hemorrhoids was described using Logistic models.The gender-,age-,and region-detection rates of hemorrhoids were standardized to the Sixth National Population Census of the People's Republic of China(2010).Results:A total of 2 940 295 adult subjects were included in the study,of whom the average age was(41.7±14.0)years,and 52.6%were females.The standardized detection rate of hemorrhoids was higher for females(43.7%)than that for males(17.7%;P<0.001)in this study.In the females,the age distribution of hemorrhoids was inverted U-shaped,with the highest standardized detection rate of hemorrhoids in the age group of 30-39 years(63.5%).In the males,the standardized detection rate of hemorrhoids increased along with age,with the highest percentage of 17.2%in the age group of 50-59 years,and the standardized detection rate of hemorrhoids in the age group of 60 and above decreased slightly(P<0.001 for trend test).The participants with hypertension had a higher standardized detection rate of hemorrhoids than those with normal blood pressure in both males and females(P<0.001).The standardized detection rate of hemorrhoids showed a positive corre-lation with body mass index(P<0.001 for trend test in males).Conclusion:The detection rate of hemorrhoids varied to gender,age,obesity,and hypertension status,which could help to identify the risk factors and the high-risk sub-groups,and hence to strengthen health education and early detection accordingly,which could eventually reduce the incidence of hemorrhoids and improve the quality of life and health in the Chinese population.This study was conducted in a physical examination population,and the conclusions of this study should be extrapolated with caution.
3.Individualized estimation of cumulated activity distribution in patients undergoing 18F-FDG PET scans based on the biokinetic model

Objective To achieve personalized assessment of cumulated activity distribution in patients undergoing 18F-FDG PET scans using the biokinetic model based on the single-time-point PET images.Methods A biokinetic compartmental model for the organs of interest was established,and the variation of activities in each compartment was described as a set of differential equations.The PET image data at a specific time point of each patient was used to obtain the kinetic model parameters which were then optimized by a simulated annealing algorithm to calculate the cumulated activity of each organ.Additionally,the effects of different voiding patterns on the cumulated activity of the bladder were analyzed.Considering the heterogeneity of radioactive nuclide distribution in organs,a whole-body cumulated activity distribution map was computed,and the accuracy of the proposed method was evaluated by comparison with the existing methods.Results For the 11 cases analyzed in the study,the calculation results of the biokinetic model generally showed a good agreement with experimental data,with an average deviation of less than 15%.Meanwhile,under different urinary excretion conditions,the average cumulated activity in the bladder differed by up to 2.4 times,indicating the positive significance of enforced urination in reducing radiation exposure.The comparison with the biokinetic data obtained from ICRP Publication 128 revealed relatively large deviations for certain organs such as the brain and heart,with standard deviations of 61.0%and 46.3%,respectively.Conclusion The constructed biokinetic model can effectively estimate the individualized cumulated activity distribution in patients undergoing PET scans,providing valuable insights for the internal dosimetry assessment.
4.Emphasize and standardize the implement of the multidisciplinary team model in the diagnosis and treatment of acute mesenteric ischemic disease

Acute mesenteric ischemic disease(AMI) is a critical surgical emergency. Due to its insidious and atypical initial symptoms, a delayed diagnosis significantly increases the mortality rate of AMI patients. The diagnosis and treatment process of AMI often involve close collaboration and communication among multiple disciplines and departments.Our team deeply recognizes the importance of a multidisciplinary team(MDT) model in the standardized management of the AMI disease. It can significantly improve the efficiency of disease diagnosis, delay disease progression, provide timely treatment, and improve the prognosis of patients, thereby reducing the overall mortality rate. Therefore, combining our center′s experience in treating AMI patients with the implementation of the MDT model, we elaborated on the key steps and quality control measures for the standardized application of the MDT model in AMI disease, with the aim of providing a theoretical foundation and experiential reference for colleagues and professionals.

Objective:To summarize the research progress of medical emotional labor in China, and provide data reference for medical workers in the field of emotional labor.Methods:In March 2020, we searched the research content of emotional labor of medical workers through CNKI system, and analyzed it by using CiteSpace software. We visualized the topic of emotional labor of medical workers by using the knowledge map formed by keyword co-occurrence and keyword clustering, as well as the emergent words and time zone map. The analysis contents include the basic information characteristics of the selected literature, research hotspots and topics, research trends and frontiers.Results:The first related literature was published in 2008, and 126 related literatures were retrieved. Research hotspots include 'emotional labor', 'nurse', 'work burnout', 'medical staff' and 'work satisfaction', etc. Topics can be divided into three categories: research objects, variables affecting factors and medical workplace. There are 24 emergent words. In 2008, 2011, 2013, and 2015, important turning points for research changes exist.Conclusion:In the future, researchers in China can try to broaden the scope of medical subjects and influencing factors, etc., and be closely linked to the results of existing studies.

7.Research Progress of Circular RNA in Lung Cancer.

Lung cancer is the most common malignant tumor with the highest morbidity and mortality worldwide, and its imposes an insupportable burden on patients due to its poor prognosis. The diagnosis and treatment of lung cancer is under great pressure. Therefore, it is urgent to explore effective therapeutic targets and molecular markers. Circular RNA (circRNA) is a kind of covalently closed non-coding RNAs, which has attracted much attention due to its conservation, stability and tissue specificity. Many studies have found that circRNA participates in the regulation of lung cancer through various mechanisms such as sponging miRNA and plays a part vital role in the early diagnosis, treatment and prognosis evaluation. In recent years, there have been numerous studies on circRNA in lung cancer. This paper summarizes the current progress of circRNA in the diagnosis, treatment and prognosis of lung cancer.

8.The study on the resistance of autophagy of fibroblast-like synovial cells to apoptosis induced by methotrexate in rheumatoid arthritis

Objective To clarify the mechanisms that the response of fibroblast-like synovial (FLS) cellsto methotrexate (MTX) in rheumatoid arthritis (RA) and to provide theory basis for the drug treatment of RA.Methods Synovial fibroblasts were isolated from synovial tissue specimens obtained from patients with RA andexposed to MTX.Cell viability was measured using a MTT assay and cell apoptosis was valued by flow cytometry.Western blotting analysis of LC3 and immunocytochemistry were used to analyze the induction of autophagy in RA-FLS after treating with MTX.Transfection of siRNA was used to interfere the expression of Beclin1 to down-regulate the autophagy,cell apoptosis was valued by flow cytometry and western blot analysis was used to test the PARPp85 with or without the presence of MTX.Statistical product and service solutions (SPSS) 18.0 statistical software was used for statistical analysis of all experimental data.Independent sample t test was used according to data distribution status,homogeneity of variance,and normal distribution.GraphPad Prism 5.0 was used to draw statistical graphs.Results MTX induced apoptosis was increased in RA-FLS.MTX stimulated the autophagy response in RA-FLS by inducing autophagosome formation.In RA-FLS,transfection with Beclin1 siRNA inhibited autophagy and increased the susceptibility to MTX,which induced cell death.Conclusion Autophagy of RA-FLS contributes to the resistance to apoptosis induced by methotrexate.
9.Study on Improvement Effect and Mechanism of Shuangshen Yin on Liver Fibrosis Model Rats

OBJECTIVE: To study improvement effect and mechanism of Shuangshen yin on liver function and the degree of liver fibrosis in liver fibrosis model rats. METHODS: Healthy SD rats were collected and given 40% CCl4 olive oil solution intraperitoneally for consecutive 7 weeks to induce liver fibrosis model. 47 model rats were randomly divided into model group, positive control group and Shuangshen yin low-dose, medium-dose and high-dose groups, with 9, 9, 9, 10, 10 rats respectively; another 10 healthy SD rats were included in normal group. From the next week after model induction, model group and normal group were given water intragastrically; positive control group [colchicines 0.2 mg/(kg·d)] and Shuangshenyin groups [Shuangshen yin water decoction 3, 6, 12 g/(kg·d), by crude drug] were given corresponding medicines respectively for consecutive 28 d. The serum levels of liver function indicators (ALT, AST, TBIL, ALB, GLB) were determined by Rye’s analysis method. The serum levels of liver fibrosis indexes (HA, LN, Ⅲ-PC, Ⅳ-C) were determined by ELISA. The pathological changes of liver tissues were observed by HE staining and VG staining. The levels of SOD, MDA and HYP in liver tissues were determined by spectrophotometry. The expression of α-SMA was detected by immunohistochemistry. RESULTS: Compared with normal group, the serum levels of ALT, AST, TBIL, HA, LN, PC-Ⅲ and Ⅳ-C in model group were increased significantly, and the level of ALB was decreased significantly (P<0.05). There were some pathological phenomena such as vacuolar degeneration and fibrous tissue proliferation in rat liver. The levels of SOD in liver tissue were decreased significantly, while the levels of MDA and HYP were increased significantly (P<0.05). A large number of positive staining granules of α-SMA protein were observed in liver tissue, and the expression level of α-SMA protein was significantly increased (P<0.05). Comparison of model group, the serum levels of ALT, AST and liver fibrosis indexes were significantly decreased and ALB level was significantly increased in Shuangshen yin groups, while the TBIL levels in Shuangshen yin medium-dose, high-dose groups were significantly decreased (P<0.05); the phenomena of hepatocyte degeneration and fibrosis tissue proliferation were significantly improved; the levels of SOD in liver tissue were significantly increased, and the levels of MDA and HYP were significantly decreased (P<0.05); the positive staining of α-SMA protein decreased significantly, and the expression level of α-SMA protein was decreased significantly (P<0.05). CONCLUSIONS: Shuangshen yin has protective and anti-fibrotic effects on liver in rats with hepatic fibrosis, the mechanism of which may be associated with enhancing antioxidant capacity, down-regulating the expression of α-SMA, inhibiting the activation of hepatic stellate cells, reducing extracellular matrix synthesis so as to reverse liver fibrosis.
10.Advances in the Influence of EGFR Mutation on the PD-L1 Expression in Non-small Cell Lung Cancer.

Researches on the inhibitors of programmed death-1 (PD-1) and programmed death-1 ligand (PD-L1) enjoy considerable breakthroughs in recent years, which has brought relative changes in the therapeutic model of non-small cell lung cancer (NSCLC) at an unexpected speed. However, it seems that PD-1/PD-L1 inhibitors are less effective in patients with epidermal growth factor receptor (EGFR) mutation than those without. Previous studies have shown that the expression rate of PD-L1 on tumor cells is correlated with the therapeutic effect of PD-1/PD-L1 inhibitors. Yet, there is no complete agreement on the effect of EGFR mutation on PD-L1 expression. In this review, relevant studies will be summarized with an expectation of making some contributions to basic researches and to the clinical treatment.
