1.Role of Iron-lipid Disorder in Nonalcoholic Fatty Liver Disease and Research Progress of Traditional Chinese Medicine Intervention: A Review

Nonalcoholic fatty liver disease (NAFLD) is one of the most common complications of type 2 diabetes mellitus (T2DM). Cell death caused by iron-lipid disorder is a new mode of regulating programmed cell death, which is characterized by lipid peroxidation induced by the accumulation of lipid reactive oxygen species and excessive accumulation of iron ions induced by iron metabolism disorders. Among them, iron homeostasis disorder and metabolic pathway disorder are the main causes of iron-lipid disorder, which play an important role in a variety of pathological processes related to cell death. Because the liver is an important organ for iron storage and lipid metabolism, iron-lipid disorder is an ideal target for liver disease, and inhibition of iron-lipid disorder may become a new strategy for the treatment of NAFLD. However, the pathogenic relationship and mechanism between NAFLD and iron-lipid disorder have not been fully elucidated. Based on the complex molecular regulation mechanism of iron-lipid disorder, by expounding the role of iron-lipid disorder in NAFLD and its related mechanism, this paper summarizes the research status of traditional Chinese medicine on the target treatment of NAFLD in recent years, so as to provide a new perspective and point out a new direction for the treatment of NAFLD in the future.
2.Exosomal miRNA-222 Alerts shRNA-PCSK9 Induction Brain Tau Hyperphosphorylation

OBJECTIVE
To investigate whether the microRNA-222(miRNA-222) carried by plasma exosomes can serve as an early warning marker for cognitive impairment induced by shRNA-PCSK9.
METHODS
The high-fat diet(HFD) was used to prepare a hypercholesterolemic mouse model group. The model group mice were divided into HFD-shRNA control group and HFD-shRNA-PCSK9 group. The shRNA-PCSK9 was constructed, injected intravenously into the body, and the expression of PCSK9 mRNA was detected by real-time PCR(RT-PCR). Tau protein and phosphorylation in brain tissue were observed by immunohistochemistry (IHC). Western blotting was used to detect Tau protein and P-Tau protein. Serum amyloid Aβ1-42Ab levels were determined by ELISA. The kits extracted plasma exosomes step by step, identify the exosome morphology by negative staining electron microscopy, and determined the size of exosomes by NTA technology. RT-PCR technique was used to detect the expression level of miRNA-222 carried in plasma exosomes.
RESULTS
The model mouse were prepared by feeding HFD for 13 weeks, whose total cholesterol(TC) and low-density lipoprotein(LDL-C) contents in serum were significantly increased. At the same time, the expression of PCSK9 mRNA in the brain tissue of model group was significantly increased. After shRNA-PCSK9 lentivirus interference, PCSK9 mRNA expression was inhibited, and IHC observed that shRNA-PCSK9 induced abnormal expression and hyperphosphorylation of Tau protein in brain tissue, indicating that the pathological changes of neurofibrillary tangles had occurred. However, at this time, serum Aβ1-42Ab had not been significantly increased, and it had not yet been of significance for the diagnosis of cognitive impairment. The miRNA in plasma exosomes was extracted, and RT-PCR results showed that the expression of miRNA-222 carried in the exosomes of the HFD-shRNA-PCSK9 group was significantly lower than that of the HFD-shRNA control group.
CONCLUSION
Plasma exosomes carried miRNA-222 provides an early warning marker for shRNA-PCSK9- induced cognitive impairment.
3.Thoughts and suggestions on digital services to enhance the level of vaccination management

With the development of information technology and the increasing demand for vaccination services among the people, it is a definite trend to enhance the quality of vaccination services through digitization. This article starts with a clear concept of digital services for vaccination, introduces the current development status in China and abroad, analyzes the advantages and disadvantages of existing models in leading regions, takes a glean from the summation, and proposes targeted solutions. This study suggests establishing a departmental coordination mechanism for data interconnection and sharing, formulating data standards and functional specifications, enhancing the functionalities of the immunization planning information system, strengthening data collection and analytical usage, and intensifying appointment management and science and health education to provide expert guidance for the construction of digital vaccination services across the country in the future.

Objective To understand the changing distribution and antimicrobial resistance profiles of Proteus,Morganella and Providencia in hospitals across China from January 1,2015 to December 31,2021 in the CHINET Antimicrobial Resistance Surveillance Program.Methods Antimicrobial susceptibility testing was carried out following the unified CHINET protocol.The results were interpreted in accordance with the breakpoints in the 2021 Clinical & Laboratory Standards Institute(CLSI)M100(31 st Edition).Results A total of 32 433 Enterobacterales strains were isolated during the 7-year period,including 24 160 strains of Proteus,6 704 strains of Morganella,and 1 569 strains of Providencia.The overall number of these Enterobacterales isolates increased significantly over the 7-year period.The top 3 specimen source of these strains were urine,lower respiratory tract specimens,and wound secretions.Proteus,Morganella,and Providencia isolates showed lower resistance rates to amikacin,meropenem,cefoxitin,cefepime,cefoperazone-sulbactam,and piperacillin-tazobactam.For most of the antibiotics tested,less than 10%of the Proteus and Morganella strains were resistant,while less than 20%of the Providencia strains were resistant.The prevalence of carbapenem-resistant Enterobacterales(CRE)was 1.4%in Proteus isolates,1.9%in Morganella isolates,and 15.6%in Providencia isolates.Conclusions The overall number of clinical isolates of Proteus,Morganella and Providencia increased significantly in the 7-year period from 2015 to 2021.The prevalence of CRE strains also increased.More attention should be paid to antimicrobial resistance surveillance and rational antibiotic use so as to prevent the emergence and increase of antimicrobial resistance.
5.Icariin improves hypertensive renal fibrosis and injury through Cx32-Nox4 signaling pathway

AIM:To investigate the effect of icari-in on renal fibrosis and injury in hypertension through Cx32-Nox4 signaling pathway.METHODS:Models of hypertensive nephropathy(HN)were es-tablished in spontaneously hypertensive rats(SHRs).The experiment was divided into 4 groups:normal control group(WKY rats),model group(SHR),icariin 10 mg·kg-1·d-1 group(icariin once dai-ly),icariin 30 mg·kg-1·d-1 group(icariin once daily),n=10.The expression of fibrosis-related proteins was detected in vivo.NRK-52E cells exposed to An-gⅡ were selected to observe the effects of icariin on kidney injury.Extracellular matrix(ECM)levels,including α-smooth muscle actin(α-SMA),collagenⅠ(Col-Ⅰ)and fibronectin(FN)expression were mea-sured by Western blot and immunohistochemistry.The expressions of oxidative stress markers includ-ing superoxide dismutase(SOD)and malondialde-hyde(MDA)were determined by the test kit.RE-SULTS:Icariin reduced renal fibrosis in SHR rats in vivo.Icariin down-regulated the expression of α-SMA,FN,and Col-Ⅰ and protected hypertension-damaged kidney tissue from progressive fibrosis(P<0.05).Icariin increased the total SOD activity and decrease the MDA level in kidney and serum of SHR rats(P<0.05).In addition,icariin increased the expression of Cx32 and decreased the expression of Nox4 in the kidneys of SHR rats(P<0.05).Icariin had a protective effect on AngⅡ-mediated NRK-52E cell damage and fibrosis.CONCLUSION:Icariin can improve renal tubulointerstitial fibrosis and delay the progression of HN.Renal protection may be at-tributed to the regulation of oxidative stress medi-ated by the Cx32-Nox4 signaling pathway.

7.Predictive model of endocrine drug resistance in hormone receptor-positive breast cancer based on ultrasound radiomics

Objective:To establish an ultrasound radiomics model by integrating clinical, pathological, and conventional ultrasound features with radiomics characteristics, and to explore its clinical value in predicting endocrine resistance in hormone receptor(HR)-positive breast cancer.Methods:A retrospective analysis was performed on 478 patients with HR-positive breast cancer from January 2017 to December 2021 in the Second Affiliated Hospital of Harbin Medical University, of which 430 were resistant and 48 were sensitive. The clinical, pathological and immunohistochemical data and ultrasound images were collected.Firstly, the propensity score was used to process and match the data. Secondly, Logistic regression was used to screen clinical, pathological, and conventional ultrasound features associated with endocrine resistance. Then, PyRadiomics was used to extract the radiomic features of grayscale ultrasound images, and a series of methods such as Lasso regression were used to screen the radiomic features related to endocrine resistance. Seven machine learning methods such as random forest were used to build a radiomics model. Finally, clinical, pathological and ultrasound features were added to establish a clinical pathological model, a clinical pathological ultrasound model, a clinical pathological radiomics model and a combined model of the four features, and the model effectiveness was evaluated.Results:①Propensity score matching: 96 patients were matched, including 48 patients in the drug-resistant group and 48 patients in the sensitive group. ②Screening clinical pathological conventional ultrasound features related to endocrine resistance: lymph node metastasis, tumor diameter, posterior echo attenuation, and growth orientation were independent predictors of endocrine resistance (all P<0.05). ③Screening radiomics features related to endocrine resistance: 18 features such as Dependence Entropy. ④Establishing radiomics model: the machine learning model of random forest method (AUC=0.80) performed best. ⑤Radiomics model integrating clinical, pathological and conventional ultrasound features: the AUC of the clinical pathological model was 0.70, the AUC of the clinical pathological ultrasound model was 0.78, the AUC of the clinical pathological radiomics model was 0.82, and the AUC of the combined model was 0.86. Conclusions:The radiomics model established by the random forest method performs best in predicting endocrine resistance in HR-positive breast cancer. The model that integrates multiple features performs best in assessing endocrine resistance.which is expected to provide an objective basis for clinicians to predict endocrine resistance in HR-positive breast cancer.
8.Predictive value of plasma atherosclerosis index on metabolic associated fatty liver disease in physical examination population

Objective:To investigate the predictive value of plasma atherosclerosis index (AIP) on metabolic associated fatty liver disease (MAFLD) in physical examination population.Methods:It was a cross-sectional study. Total of 97 076 people who completed physical examination in the Health Management Center of the First Affiliated Hospital of Anhui Medical University from January to December 2021 and met the integrity of the study were selected as study subjects. Of the subjects, 31 176 people who met the diagnostic criteria of MAFLD were set as the MAFLD group, and the other 65 900 people were set as the non-MAFLD group. Laboratory indexes, height, weight, blood pressure, liver ultrasound and other indicators in the two groups were collected, and the AIP was calculated. The t-test was used for measurement data and chi-square test was used for counting data to compare the differences between the two groups. Univariate and multivariate logistic regression were used to analyze the influencing factors of MAFLD. The two groups were grouped further according to gender and age, and the difference of AIP prediction efficiency in different groups was evaluated by receiver operating characteristic (ROC) curve. Results:The average age (47.3 years, Z=-31.734), male proportion (76.9%, χ2=7 837.54) and the average value of AIP (0.23, Z=-155.089) in MAFLD group were all higher than those in non-MAFLD group (all P<0.001). After stratified by age, gender, body mass index (BMI), hypertensive or not, hemoglobin A1c (HbA 1c), triglyceride (TG), fasting blood glucose (FBG), the difference of AIP between the two groups was still statistically significant (all P<0.001). Multifactorial regression analysis showed that alanine aminotransferase (ALT) ( OR=1.024), aspartate aminotransferase (AST) ( OR=0.974), serum creatinine (sCr) ( OR=0.975), serum uric acid ( OR=1.004), HbA 1c ( OR=1.231), hemoglobin (HB) ( OR=1.011), platelet(PLT) ( OR=1.002), FBG ( OR=1.131), BMI ( OR=1.419), AIP ( OR=11.318), systolic blood pressure ( OR=1.002), and diastolic blood pressure ( OR=1.012) were independent risk factors for MAFLD (all P<0.001). In the overall population, AIP had an area under the ROC curve (AUC) of 0.808, a cut-off value of 2.045, a sensitivity of 74.5% and a specificity of 72.4%; in the gender subgroup, the AUC was greater in women than in men (0.815 vs 0.764), and the cut-off values, sensitivities and specificities in the two groups were -0.044 vs 0.091, 75.6% vs 72.2%, 73.3% vs 67.6%, respectively; in the age sub-group, the largest AUC (0.848), cut-off value (0.034), sensitivity (79.1%) and specificity (75.3%) were found in the 18-44 years group; the differences were statistically significantin the ROC curve analysis of each group ( P<0.001). Conclusion:AIP is an independent risk factor for MAFLD, which has good predictive value for the occurrence of the disease, and has better predictive effect in women and young groups (18-44 years old).
9.The influence of pulmonary artery diameter on the image quality of dual-layer spectral detector CT pulmonary angiography with low contrast agent dosage

Objective:Evaluate the image quality of dual-layer spectral detector CT pulmonary angiography with low-dose contrast agents, and explore the influence of pulmonary artery diameter on the image quality.Methods:A total of 91 spectral CT pulmonary angiography from March 2022 to August 2022 were retrospectively analyzed. The cases were divided into Group 1 ( n=34, main pulmonary artery diameter ≥ 30 mm) and Group 2 ( n=57, main pulmonary artery diameter<30 mm). The dosage of contrast agent was 30 ml. The CT attenuation values(CT values), standard deviation(SD), signal-to-noise ratio(SNR), and contrast-to-noise ratio(CNR) values of pulmonary artery from the main trunk to the subsegmental pulmonary artery between two groups were compared. The CT dose index volume(CTDI vol) and dose-length production (DLP) were recorded. Two readers evaluated the image quality using three-point method. The inter-reader agreement was performed by Kappa test. Results:The CT values of the pulmonary trunk and left pulmonary artery between two groups was not significantly different ( P>0.05). The CT values of the left upper lobe artery, segmental artery, and subsegmental artery in Group 1 were lower than those in Group 2 ( t=-2.13, -2.17, Z=-2.33, P<0.05). The SD values of pulmonary trunk and segmental artery in Group 1 were higher than those in Group 2 ( t=2.27, Z=-2.23, P<0.05). The SD values of left pulmonary artery, left upper lobe artery, and subsegmental artery between two groups were not significantly different ( P>0.05). The SNR and CNR values of main pulmonary trunk, left pulmonary artery, left superior lobar artery, and segmental artery in Group 2 were higher than those in Group 1 ( Z=-2.45, -2.57, -2.09, -3.58, P<0.05; Z =-2.33, -2.42, -2.07, -3.45, P<0.05), while these values of the subsegmental artery between two groups were not significantly different ( P>0.05). The two readers had good consistency in evaluating image quality (Kappa value>0.75, P<0.05). Conclusions:Spectral CT pulmonary angiography with 30 ml contrast agent would generate good quality images. However, the distal pulmonary artery would be poorly revealed when the diameter of main pulmonary artery more than 30 mm, especially in patients with suspected pulmonary hypertension.
10.Effects of Couplet Medicine of Rheum palmatum -Salvia miltiorrhiza on Enterogenous Urotoxins Contents and Intestinal Barrier Function in Rats with Chronic Renal Failure

OBJECTIVE:To study the effects of couplet medicine of Rheum p almatum-Salvia miltiorrhiza on the contents of enterogenous urotoxin and intestinal barrier function in chronic renal failure (CRF)model rats. METHODS :Totally 55 male Wistar rats were randomly divided into sham operation group (10 rats)and modeling group (45 rats). In sham operation group ,the kidneys were isolated but not removed ;CRF model was reproduced by 5/6 nephrectomy in modeling group. After modeling (excluding 5 dead and non-modeling rats ),modeling rats were divided into model group (water),Niaoduqing granules group (2.5 g/kg),couplet medicine of R. palmatum -S. miltiorrhiza groups(6,3 g/kg,by crude drug ),with 10 rats in each group. Sham operation group and model group were given constant volume of water intragastrically. Administration groups were given relevant medicine intragastrically ,once a day ,for consecutive 12 weeks. After last administration ,the contents of creatinine (Scr)and urea nitrogen(BUN)in serum ,the content of urinary creatinine (Ucr) in urine were determined by automatic biochemical analyzer;creatinine clearance rate (Ccr)was calculated. The contents of enterogenous urotoxin [trimethylamine-N-oxide (TMAO),indoxyl sulfate (IS)and p-cresyl sulphate (PCS)] were determined by UPLC-ESI-MS/MS. Real-time RT-PCR and immunofluorescence assay were used to detect the mRNA and protein expression of Occludin and ZO-1 in the ileum tissue. HE staining and Masson staining were used to observe the pathologi cal changes of renal tissue. The ultrastructural changes of rat colon were observed by transmission electron microscope. RESULTS :Compared with sham operation group ,serum contents of Scr,BUN,TMAO,PCS and IS were increased significantly in model group (P<0.01),while urine content of Ucr ,Ccr,mRNA and protein expression of Occludin and ZO- 1 in ileum tissue were decreased significantly (P<0.01);renal glomerulosclerosis , renal tubules dilation and inflammatory invasion and fibrosisin the interstitium were all found ;the intestinal epithelial barrier structure of colon tissue was severely damaged. Compared with model group ,serum contents of Scr ,BUN,TMAO,PCS and IS were decreased significantly in administration groups (P<0.05 or P<0.01);the mRNA and protein expression of Occludin and ZO-1 in the ileum tissue were increased significantly (except for mRNA expression of ZO- 1 in R. palmatum -S. miltiorrhiza low-dose group (P<0.05 or P<0.01);the infiltration of inflammatory cells in renal interstitium ,the degree of fibrosis and the damage of intestinal epithelial barrier structure in colon tissue were reduced. CONCLUSIONS :Couplet medicine of R. palmatum -S. miltiorrhiza can effectively protect the residual renal function of CRF model rats ,the mechanism of which may be associated with reducing the serum contents of enterogenous urotoxin ,up-regulating mRNA and protein expresssion of Occludin and ZO- 1 in the ileum tissue so as to improve intestinal barrier function.
