China has classified the Corona Virus Disease 2019(COVID-19) as a statutory category B infectious disease and managed it according to Category B since January 8, 2023.In view that Omicron variant is currently the main epidemic strain in China, in order to guide the treatment of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2) infection in children with the times, refer to the Diagnosis and Treatment Protocol for Novel Coronavirus Infection (Trial 10 th Edition), Expert Consensus on Diagnosis, Treatment and Prevention of Novel Coronavirus Infection in Children (Fourth Edition) and the Diagnosis and Treatment Strategy for Pediatric Related Viral Infections.The Expert Consensus on the Diagnosis, Treatment and Prevention of Novel Coronavirus Infection in Children (Fifth Edition) has been formulated and updated accordingly on related etiology, epidemiology, pathogenic mechanism, clinical manifestations, auxiliary examination, diagnosis and treatment, and added key points for the treatment of COVID-19 related encephalopathy, fulminating myocarditis and other serious complications for clinical reference.

3.Mechanism of action of oxidative stress in severe traumatic brain injury and its complications: recent progress

Severe traumatic brain injury (sTBI) is characterized by critical condition, high lethality and poor prognosis. Its development and progression will lead to the damage and death of a large number of nerve cells, eventually causing a series of serious complications. The current treatments of sTBI and its complications are not optimistic due to problems such as unclear mechanism of action, challenges in treatment, and lack of effective prevention strategies. In recent years, more research evidences have shown that oxidative stress plays an important role in the development and progression of sTBI and its related complications. Therefore, it is of great significance to clarify the relationship of oxidative stress with sTBI and its complications and to understand the way of oxidative stress participating in the development and progression of sTBI. However, relevant researches are scattered and there lacks comprehensive and systematic summaries of oxidative stress participating in sTBI and its related complications. To this end, the authors reviewed the progress of the mechanism by which oxidative stress involves in sTBI and its complications, hoping to provide references for the research, treatment and prevention of sTBI.
4.Efficacy and safety of bendamustine-rituximab combination therapy for newly diagnosed indolent B-cell non-Hodgkin's lymphoma and elderly mantle cell lymphoma: a multi-center prospective phase II clinical trial in China

Objectives:This study aimed to assess the efficacy and safety of bendamustine in combination with rituximab (BR regimen) for the treatment of newly diagnosed indolent B-cell non-Hodgkin's lymphoma (B-iNHL) and elderly mantle cell lymphoma (eMCL) .Methods:From December 1, 2020 to September 10, 2022, a multi-center prospective study was conducted across ten Grade A tertiary hospitals in Shandong Province, China. The BR regimen was administered to evaluate its efficacy and safety in newly diagnosed B-iNHL and eMCL patients, and all completed at least four cycles of induction therapy.Results:The 72 enrolled patients with B-iNHL or MCL were aged 24-74 years, with a median age of 55 years. Eastern Cooperative Oncology Group (ECOG) performance status scores of 0-1 were observed in 76.4% of patients, while 23.6% had scores of 2. Disease distribution included follicular lymphoma (FL) (51.4% ), marginal zone lymphoma (MZL) (33.3% ), eMCL (11.1% ), and the unknown subtype (4.2% ). According to the Ann Arbor staging system, 16.7% and 65.3% of patients were diagnosed with stage Ⅲ and stage Ⅳ lymphomas, respectively. Following four cycles of BR induction therapy, the overall response rate was 98.6%, with a complete response (CR) rate of 83.3% and a partial response (PR) rate of 15.3%. Only one eMCL patient experienced disease progression during treatment, and only one FL patient experienced a relapse. Even when evaluated using CT alone, the CR rate was 63.9%, considering the differences between PET/CT and CT assessments. The median follow-up duration was 11 months (range: 4-22), with a PFS rate of 96.8% and an OS rate of 100.0%. The main hematologic adverse reactions included grade 3-4 leukopenia (27.8%, with febrile neutropenia observed in 8.3% of patients), grade 3-4 lymphopenia (23.6% ), grade 3-4 anemia (5.6% ), and grade 3-4 thrombocytopenia (4.2% ). The main non-hematologic adverse reactions such as fatigue, nausea/vomiting, rash, and infections occurred in less than 20.0% of patients.Conclusion:Within the scope of this clinical trial conducted in China, the BR regimen demonstrated efficacy and safety in treating newly diagnosed B-iNHL and eMCL patients.
5.Practice and exploration of improving the rational use of drugs in primary medical institutions by the general pharmacist system of the medical consortium

OBJECTIVE To explore and establis h a general pharmacist system suitable for China ’s national conditions ,and to improve the rational use of drugs in primary medical institutions . METHODS Under the leadership of Tianhe District Health Bureau of Guangzhou ,relying on the regional pharmaceutical specialty alliance ,general pharmacist system of medical consortium was established ,and the general pharmacist was responsible for the overall planning of pharmaceutical care in the medical consortium. The joint management office of pharmaceutical care was established ,and the training of the pharmacists in the medical consortium was organized. A regional audit center was established to realize the prescription review of 13 community health service centers in the medical consortium. “Internet plus ”home pharmaceutical care was carried out ,and science popularization education was provided for communities ,schools,enterprises and institutions. RESULTS After systematic training and assessment ,three pharmacist teams had been successfully established in the medical consortium to provide prescription review ,science popularization and education and family pharmacist services for community residents ;the regional audit center successfully intercepted 17.17% of unreasonable prescriptions ,reducing the amount of unreasonable drug use by a total of 6.56 million yuan. After the intervention of prescription review system ,the qualified rate of outpatient prescriptions in community health service centers was ≥95%,and the qualified rate increased by an average of 6%. The department of pharmaceutical science popularization and education held 35 science popularization and free clinic activities ,of which 71.20% of the residents believed that the activities had improved their understanding of drugs. In addition ,111 cases patients serviced by home pharmaceutical care were carried out successfully by pharmacist team ,and the patients ’acceptance of pharmacist intervention was 91.89% . CONCLUSIONS Under the new medical reform ,it is feasible to implement a regional general pharmacist system within the medical consortium , which improves the pharmaceutical administration and pharmaceuticalcare capabilities of m edical institutions in the medical consortium,as well as the level of rational drug use ,and reduces the me dical burden.

Monkeypox is a zoonotic disease.Previous studies have shown that children are vulnerable to monkeypox and are also at high risk for severe disease or complications.In order to improve pediatricians′ understanding of monkeypox and achieve early detection, early diagnosis, early treatment and early disposal, the committee composed of more than 40 experts in the related fields of infectious diseases, pediatrics, infection control and public health formulate this expert consensus, on the basis of the latest clinical management and infection prevention and control for monkeypox released by the World Health Organization (WHO), the guidelines for diagnosis and treatment of monkeypox (version 2022) issued by National Health Commission of the People′s Republic of China and other relevant documents.During the development of this consensus, multidisciplinary experts have repeatedly demonstrated the etiology, epidemiology, transmission, clinical manifestations, laboratory examinations, diagnosis and differential diagnosis, treatment, discharge criteria, prevention, case management process and key points of prevention and control about monkeypox.

Since December 2019, severe acute respiratory syndrome coronavirus 2(SARS-CoV-2) infections have raged globally for more than 2 years.China has always adopted scientific and effective prevention and control measures to achieved some success.However, with the continuous variation of SARS-CoV-2 cases and imported cases from abroad, the prevention and control work has become more difficult and complex.With the variation of the mutant strain, the number of cases in children changed, and some new special symptoms and complications were found, which proposed a new topic for the prevention and treatment of SARS-CoV-2 infection in children in China.Based on the third edition, the present consensus according to the characteristics of the new strain, expounded the etiology, pathology, pathogenesis, and according to the clinical characteristics and experience of children′s cases, and puts forward recommendations on the diagnostic criteria, laboratory examination, treatment, prevention and control of children′s cases for providing reference for further guidance of effective prevention and treatment of SARS-CoV-2 infection in children in China.
8.Clinical characteristics of the SARS-CoV-2 Omicron variant infection in children with different basic diseases

Objective:To investigate the clinical characteristics of SARS-CoV-2 Omicron variant BA.2.2 infection in children with different basic diseases.Methods:A total of 76 children with different basic diseases admitted to Shanghai Children′s Medical Center, Shanghai Jiao Tong University School of Medicine from March 2 to June 10, 2022 were retrospectively analyzed in terms of clinical manifestations, supplementary examination, treatment and prognosis.These children were divided into hematologic tumor group and non-hematologic tumor group which included cardiopulmonary disease group, neurological disease group and congenital malformation group.The clinical characteristics and prognosis of SARS-CoV-2 Omicron variant infection in children with basic diseases were analyzed.Results:Among 76 children with basic diseases, 61 patients were newly diagnosed with fever and 15 patients were asymptomatic.There were 26, 22, 20, and 8 cases with hematologic tumor diseases, cardiopulmonary diseases, neurological diseases and congenital malformation, respectively.The average fever time was 3-7 days.There was no significant difference in the clinical manifestations among each group ( P>0.05). However, children with hematological tumors were more prone to single organ or multiple organ dysfunction.The level of C-reactive protein was higher than that in the non-hematologic tumor group, while the white blood cell count, neutrophil count, lymphocyte count and hemoglobin were lower than those in the non-hematologic tumor group.The time of virus negative transformation and the duration of fever were higher than those in the non-hematologic tumor group, with statistical significance ( P<0.05). Among the children in the hematologic tumor group, the blood routine and inflammation indexes of children infected with SARS-CoV-2 after chemotherapy within 2 weeks were worse than those in the non-chemotherapy group, and the duration of fever and the time of virus negative transformation were significantly longer ( P<0.05). Three children with hematologic tumor showed virus positive recovery after chemotherapy and surgical treatment after virus negative transformation. Conclusion:Omicron variant BA.2.2 of SARS-CoV-2 has no serious impact on children with cardiopulmonary diseases, nervous system diseases and congenital malformations.However, the duration of fever and virus positive time in hematologic tumor group extended, especially after chemotherapy.It has an important impact on the formulation of epidemic prevention and timing selection of chemotherapy for children with hematological tumors.
9.Advances in the mechanism of plant adaptation to acid aluminum stress.

The aluminum stress in acidic soil areas of China is an important abiotic stress factor that hampers the normal growth and development of plants and seriously affects the agricultural yield. The forms of plant resistance to aluminum stress are complex and diverse, which include secretion of organic acids, increase of rhizosphere pH, secretion of mucus, cell wall fixation of Al3+, organic acid chelation of Al3+ in cell solute, and vacuolar area isolation. Most of studies focus on analyzing conventional physiological characteristics, but in-depth molecular biological analyses are lacking. This review summarizes the mechanisms how plants adapt to acidic aluminum stress. This includes the effect of acid aluminum stress on plant growth and physiological metabolism, the two main physiological mechanisms of plant adaptation to acid aluminum stress (aluminum exclusion mechanism, aluminum tolerance mechanism), and the aluminum resistance related genes. Finally, this paper puts forward some prospects for further revealing the mechanism of plant adaptation to acid aluminum stress and excavating high-quality crops suitable for cultivation in acidic soils.

10.Evaluation of multiplex PCR for viral central nervous system infection in children

Objective:To evaluate the clinical significance of molecular detection testing multiple pathogens in children with viral central nervous system infections.Methods:We retrospectively included 176 children who were suspected with central nervous system infection at Shanghai Children′s Medical Center from January 2017 to May 2021.Film Array Meningitis/Encephalitis Panel(FA-M/E) was used to test cerebrospinal fluid samples of these children.The results were analyzed compared with clinical symptoms and cerebrospinal fluid indices.Results:There were 34 samples with positive FA-M/E virus detection(19.32%, 34/176). Among the 34 samples, enterovirus was the most common pathogen(27 cases, 79.41%). In different combinations, the sensitivity and positive predictive value were all less than 90%.The median time for antiviral drugs used in FA-M/E virus-positive and negative children was 4.5(0, 8.5)d and 2.6(0, 2.0)d, respectively.The difference was statistically significant( P<0.05). Conclusion:Molecular tests of multiple pathogens can quickly and sensitively detect pathogens.It can improve the efficacy of clinical diagnosis of viral central nervous system infection.
