1.Effects of Tianma Gouteng Yin combined with butylphthalide on symptom improvement and vascular elasticity in patients with acute cerebral infarction

Objective To explore the effect of Tianma Gouteng Yin combined with butylphthalein(NBP)on symptom improvement and vascular elasticity in patients with acute cerebral infarction(ACI).Methods Eighty-two ACI patients admitted to Lu'an Hospital of Traditional Chinese Medicine were included in this study,and they were randomly separated into the single group and the combination group,with 41 cases in each group.The single group was given intravenous infusion of NBP sodium chloride injection,and the combination group was given treatment with Tianma Gouteng Yin on the basis of the single group.Cerebrovascular reserve function,endothelial function,neurological function,independent living ability,hemodynamic indicators,vascular elasticity,clinical efficacy and incidence of adverse reactions were compared between the two groups of patients at 1 day after admission(T0)and 14 days after treatment(T1).Results There were no significant differences in cerebrovascular reserve function,endothelial function,National Institutes of Health Stroke Scale(NIHSS)scores,independent living ability scores,hemodynamic indicators and vascular elasticity indicators at T0 between the two groups(P>0.05).At T1,levels of cerebrovascular reactivity(CVR),nitric oxide(NO),nitric oxide synthase(NOS),large artery elasticity index(C1)and small artery elasticity index(C2)were significantly increased in both groups,and levels were higher in the combined group than those of the single group(P<0.05).At T1 moment,levels of ET-1,vascular endothelial growth factor(VEGF),NIHSS score,MRS score,red blood cell count,whole blood viscosity,platelet adhesion rate and arterial pressure were reduced in both groups,and levels were lower in the combined group than those of the single group(P<0.05).The total effective rate was higher in the combination group(80.5%)than that of the single group(46.3%).The overall incidence of adverse reactions was lower in the combined group than that in the single group(P<0.05).Conclusion Tianma Gouteng Yin combined with NBP can effectively improve the cerebral vascular reserve function,endothelial function and neurological damage in ACI patients,increase vascular elasticity and improve hemodynamic levels.

Abstract
E cigarette has become more and more popular among adolescents, and the awareness and use of e cigarettes among middle and high school students have shown a significant upward trend. E cigarette use is not only harmful to adolescent health, but also may become a gateway to other addictive substances. Individual characteristics, products features, environment and other factors were associated with e cigarette use among adolescents. The purpose of the review is to understand the current status and associated factors of adolescent e cigarette use, so as to provide scientific suggestions for the prevention of e cigarette use among adolescents.
3.Herbalogical study on Liangmianzhen (Zanthoxyli Radix).

Liangmianzhen(Zanthoxyli Radix) has long been used as medicine. The current medicinal parts are different from those in the ancient. As recorded in the Chinese Pharmacopeia, the medicinal part is root. However, in ancient works, the medicinal parts include root, stem, leaf, and fruit. In an attempt to find the historical basis that stem is a reasonable medicinal part, the herbalogical study was carried out on this medicinal based on the formal names, synonyms, original plant, medicinal parts, habitat of the medicinal plant, producing area, processing and preparation methods, efficacy, and indications recorded in ancient Chinese materia medica and local gazetteers. The results showed that Liangmianzhen was firstly recorded as a medicinal in Shennong's Classic of Materia Medica with the formal name of "Manjiao". "Manjiao" was adopted from the Han Dynasty to the Qing Dynasty when it was changed to "Rudijinniu", the name originating from the folk in the south of the Five Ridges. Now, the formal name is "Liangmianzhen", which was firstly recorded in Wuxuan County Gazetteer in 1914 and then as a synonym in the Updated Records of Picking Herbs in the South of the Five Ridges. According to the formal names, synonyms, and the descriptions of the original plant, the medicinal plants of Liang-mianzhen have the characteristics of shrub-like young seedlings, vine adult seedlings, corymbiform thyrsus, stems with thorns, amphitropous golden-yellow roots with horn-like branches, and thorns on both sides of the leaves. Thus, "Manjiao", "Rudijinniu", and "Liangmianzhen" were from the same species of Zanthoxylum nitidum(Rutaceae), which was also verified based on the growth environment, habitat, processing and preparation methods, efficacy, and indications. In ancient times, the stem and root were the main medicinal parts and leaves and fruits were also used. However, in the Chinese Pharmacopeia, root is recorded as the only medicinal part, which is obviously inconsistent with the records in the ancient classics. In light of the limited medicinal resources for Liang-mianzhen, other medicinal parts of Z. nitidum is recommended. This study clarified the medicinal parts of Z. nitidum in history. It is recommended that the stem be added to the medicinal parts of Z. nitidum in the next edition of Chinese Pharmacopeia.

4.The relationship between the onset of cerebral infarction in anterior and posterior circulation and the"three high" risk factors in Guangzhou communities

Objective To investigate the relationship between the onset of cerebral infarction in anterior and posterior circulation and the "three high" risk factors in Guangzhou communities. Methods From Jan. 2014 to Jan.2016,367 patients from Guangzhou communities were diagnosed with new cerebral infarction by head diffu-sion weighted imaging(DWI).The data were divided into two groups of anterior and posterior circulation,and uni-variate and multivariate methods were used to analyze the relationship between the "three high" and other risk factors and the onset of cerebral infarction in anterior and posterior circulation.The data of anterior circulation cere-bral infarction were further divided into two subgroups of lacunar and non-lacunar infarction,and the same statisti-cal methods were employed to analyze differences of risk factors between the two subgroups. Results The frequen-cies of hypertension(P = 0.040)and large atherosclerotic infarction(P = 0.012)were significantly higher,and the serum high-density lipoprotein(HDL)level(P = 0.045)was significantly lower in posterior circulation than those in anterior circulation,respectively;and the onset of posterior circulation cerebral infarction was more associ-ated with the incidence of hypertension(OR = 1.767,P = 0.035)and the decrease of HDL(OR = 0.380,P =0.021). In anterior circulation,the levels of systolic blood pressure(SBP)(P = 0.011)and diastolic blood pres-sure(DBP)(P=0.000),as well as the frequency of large atherosclerotic infarction(P=0.000)in non-lacunar infarction subgroup were significantly higher than those in lacunar infarction subgroup respectively,and the onset of non-lacunar infarction was more closely related to increased SBP levels(OR=1.045,P=0.001). Conclusions Among the"three high"risk factors,the onset of posterior circulation cerebral infarction is more closely related to the incidence of hypertension and the decrease of HDL.In anterior circulation,and the onset of non-lacunar infarc-tion is more closely related to the increased levels of DBP and SBP,especially to the elevated levels of SBP.
5.Effect of lateral malleolus perforator flap for the repair of soft tissue defect of foot and ankle

Objective To study the effect of lateral supramalleolar perforator flap for skin and soft tissue defect of lateral malleolus.Methods From January 2012 to December 2015,36 patients with soft tissue burn of ankle in the Traditional Chinese Medicine Hospital of Cixi were selected as study objects.The average wound area was 4 ~ 10cm.The average flap size was 5 ~ 13cm.There were 17 cases received skin flap of behind perforator of the lateral malleolus.There were 19 cases received skin flap of front perforator of the lateral malleolus.The effect of repair was observed.Results All flaps survived in 36 cases after operation.There were 7 cases of 2.6cm of blood stasis,and necrosis occurred in 2 cases which healed after dressing change.Transplantation of skin flap and the original skin had good adhesion,skin color and texture was similar to the surrounding skin during the follow-up period of 6 to 12 months.The survival time and walk time of the repair of anterior perforator flap were (2.20 ± 0.70) weeks,(4.70 ± 1.10) weeks,respectively,which were longer than those of the lateral retromalleolar perforator flap [(2.10 ± 0.80) weeks,(4.20 ± 1.10) weeks],but the differences were not statistically significant (all P > 0.05).The American orthopaedic foot and ankle society(AOFAS) score of the anterior perforator flap was significantly lower than that of the lateral retromalleolar perforator flap,the difference was statistically significant[(77.47 ± 3.41) points vs.(80.12 ± 4.12) points,t =2.110,P < 0.05].Conclusion Lateral supramalleolar perforator flap has significant effect for skin and soft tissue defect of lateral malleolus.The operation is simple,the success rate is high,it is worth to be popularized in clinical.
6.Effects of Danqi Granules combined with Xianling Gubao Capsules on bone metabolism and bone mineral density in patients with postmenopausal osteoporotic vertebral compression fractures after surgery

Objective To observe the effects of Danqi Granules combined with Xianling Gubao Capsules on bone metabolism and bone mineral density (BMD) in patients with postmenopausal osteoporotic vertebral compression fractures (OVCF) after surgery. Methods 90 cases of postmenopausal women with OVCF were randomly divided into the control group and the observation group, 45 cases in each group. The two groups were treated by vertebroplasty (PVP), and after the surgery, the control group was treated with Xianling Gubao Capsules while the observation group was additionally given Danqi Granules. The serum markers of bone metabolism [(25 (OH) D, PINP, β-CTX] and BMD of L2-4 and left femoral neck were detected before and after treatment. Results The level of serum 25 (OH) D in the observation group after treatment was significantly higher while the levels of PINP and β-CTX were significantly lower than those in the control group (P<0.05), and the BMD of L2-4 and left femoral neck of the observation group were significantly higher than those of the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups. Conclusion Danqi Granules combined with Xianling Gubao Capsules can significantly accelerate fracture recovery, and improve bone mineral density in the treatment of postmenopausal women with OVCF.
7.Comparison of curative effects of volar and radial column of open reduction and internal fixation in the treatment of radius far end fracture

Objective To compare the curative effect of the different palm dorsal approach of open reduction and internal fixation for the treatment of distal radius fractures.Methods According to the stochastic indicator method,84 patients with distal radius fractures were divided into control group and observation group,42 cases in each group.The control group was treated with dorsal approach open reduction and internal fixation,while the observation group was treated by volar approach open reduction and internal fixation.The clinical curative effect was assessed,and indicators related to the operation and the fracture healing was recorded in detail.Postoperative complications of patients were analyzed during postoperative follow-up.Results The total effective rate of the observation group was 88.10%(37/42),which was significantly higher than 66.67%(30/42) of the control group(x2=5.509,P=0.019).Compared with the control group,the operation and hospital stay time were significantly shorter,intraoperative and postoperative blood loss of the observation group decreased obviously,and the differences were statistically significant(t=2.092,2.137,2.256,2.199;P=0.040,0.036,0.027,0.031).The Gartland-Weriey score and ulnar inclination of the observation group were (9.18±2.96)points and (10.52±2.64)°,which were significantly lower than those of the control group[(12.56±3.24)points,(16.22±2.87)°],while radial height[(11.61±2.87)mm]was higher than the control group[(10.92±2.65)mm],the differences were statistically significant(t=2.206,2.316,2.185;P=0.030,0.023,0.032).The incidence rate of complications of control group was 26.19%(11/42),which was higher than 9.52%(4/42) of the observation group(x2=3.977,P=0.046).Conclusion The curative effect of volar approach of open reduction and internal fixation in the treatment of distal radius fractures was better compared with dorsal approach.At the same time,it can effectively improve the operation indicators and fracture healing effect,in addition,it can effectively reduce the incidence of postoperative complications.
8.Research on Herbal Medicine Application Regulation of Professor Wu Liangcun in Gastric Cancer Treatment

Objective]To summarize herbal medicine application regulation of Professor Wu Liangcun in gastric cancer treatment. [Methods]Collect 116 gastric cancer patients from Chinese medicine clinic system during January 1,2014 to December 31, 2014 and establish clinical prescription underlying database. Apply descriptive analysis to drug cumulative frequency, drug-induced taste and nature and apply cluster analysis to 42 drugs using more than 10 times. [Results] In the 116 gastric cancer patients,159 kinds of herbs were used ,the cumulative frequency of herbal medicine was 1816.GanCao was the herb with highest usage frequency, and then was FuLing, ShanYao, TaiZishen;According to accumulated frequency, herbs with tonifying function were at the top position(herbs with tonifying qi function were most), and then was the herbs with function of herbs with the function of clearing excessive heat, herbs with function of herbs with the function of relieving dyspepsia, herbs with function of herbs with the function of regulating qi. In terms of herbs' nature and taste, herbs with sweet taste, plain nature were used most. Cluster analysis divided all used herbs into 5 sections, involving prescriptions Sijunzi decoction, Erchen decoction, and couplet medicines DaoYa with GuYa, JiNeijin with MiHoutao, SheShecao with XianHecao and so on. [Conclusions] Professor Wu Liangcun treats gastric cancer with the law of tonifying qi, the main prescription is the same as Sijunzi decoction, emphasizing nourishing Yin and clearing away the heat-evil, protecting gastric, accompanied with promoting digestion and regulating qi.
9.Serial CT findings of cryptogenic organizing pneumonia and changes after corticosteroid therapy

Objective To review the serial CT findings of cryptogenic organizing pneumonia(COP)and semiquantitatively analyze the changes after corticosteroid therapy.Methods The clinical and radiological features of 13 patients with COP confirmed by pathology were retrospectively collected and analyzed.The lung lesions extent on CT scans was estimated using semi-quantitative method.Changes of overall disease extent were evaluated by comparing the initial CT images and the follow-up CT scans after corticosteroid therapy.Results CT images of all cases revealed diffuse and bilateral lesions,mainly located in lower zone,distributed as predominantly subpleural and(or)along the bronchovascular bundle.The two most common manifestations of lung abnormality on initial scans were ground-glass opacity(GGO)(13 cases)and consolidation(12 cases),the average areas of which were 13% and 10% respectively.The treatment with antibiotics was ineffective.After treatment with corticosteroid,the lesions resolved completely in 2 patients ; the disease was decreased in extent in 8 patients ; and the lesion showed no change in extent in one patient,while the disease progressed in extent in 2 patients on follow-up CT.The most common CT findings were GGO(10 cases)and reticulation(5 cases),followed by consolidation(4 cases),the average areas of which were 19%,2% and 1% respectively.Conclusion The CT features of the patients with COP are characteristic.The most lesions resolved or improved after corticosteroid therapy on follow-up CT scans.The estimation of disease extent with semi-quantitative method is helpful for the clinicians to evaluate the therapeutic effect.
10.Examination of exercise stress echocardiography in pacemaker-dependent patients with absolute chronotropic incompetence

Objective To evaluate the diagnositic value of exercise stress echocardiography (ESE) in patients with pacemaker-dependnet.Methods Forty-six patients remained pacemaker dependent with a fixed heart rate throughout ESE and coronary angiographic studies.Results Thirty-four of 46 patients completed the ESE. Coronary artery disease was confirmed in 28 of 34 who had coronary angiograms. ESE had a sensitivity of 75% and specificity of 83% in pacemaker-dependent patients.Conclusions ESE has preserved diagnostic utility in pacemaker dependent patients with absolute chronotropic incompetence.
