1.Comparative analysis of tumor staging versus clinical staging in reimbursement scope restrictions under medical insurance for antineoplastic agents

OBJECTIVE To comparatively analyze tumor staging versus clinical staging in reimbursement scope restrictions under medical insurance for antineoplastic agents in order to better implement the medicare drug payment policy. METHODS Antineoplastic agents included in the National Basic Medical Insurance, Workers’ Compensation Insurance and Maternity Insurance Drug Catalogue (2024) (hereinafter referred to as the “Medical Insurance Catalog”) were used as research subject to compile and analyze reimbursement scope restrictions regarding tumor staging. By consulting clinical diagnosis and treatment guidelines and relevant literature, the tumor staging in reimbursement scope restrictions of the Medical Insurance Catalog was mapped and compared with clinical staging. RESULTS & CONCLUSIONS A total of 89 antineoplastic agents’ medical insurance payments had tumor staging. Among these, there were 86 western drugs (including 17 ordinary western drugs, 68 negotiated drugs, and 1 competitive drug) and 3 Chinese patent medicines (including 1 ordinary Chinese patent medicine and 2 negotiated drugs). Non-small cell lung cancer involved the most restricted payment drugs, with 36 drugs. The tumor staging in reimbursement scope restrictions was mostly “metastatic” and “locally advanced”, involving 67 and 48 drugs respectively. Tumor staging in most reimbursement scope restrictions could correspond to the clinical staging of the tumor. However, mid-advanced esophageal cancer, unresectable gastrointestinal stromal tumors, unresectable locally advanced neuroendocrine tumors, locally advanced basal cell carcinoma, and unresectable neurofibromatosis type Ⅰ did not have a corresponding clinical staging mentioned in authoritative guidelines or high-quality clinical studies and need to be determined by the clinic according to the actual situation of the patient. Therefore, it is recommended that the interpretation of tumor staging in reimbursement scope restrictions should be accurately defined and standardized, so as to improve the accuracy of the drug payment policy in the actual implementation process.

To explore the role of the "Clinical Practice Guidelines" column and others in the We collected papers published by the Lanzhou University Evidence-Based Medicine Center team in the "Clinical Practice Guidelines" column and others from 2018 to 2025. These publications were analyzed across multiple dimensions, including authorship and institutional affiliations, citation metrics, and research themes and content. A total of 59 papers were included in the analysis, with authors representing 70 domestie and international research institutions. The cumulative citation count was 639, with the highest single-paper citation frequency reaching 101. The average citation per paper was 10.8, and total downloads exceeded 30 000. The content focused on key themes such as guideline terminology, development methodology, guideline evaluation, and dissemination and implementation. The evolution of research topics progressed from critiques of common misconceptions and hot topies in the field to multidimensional evaluations of thecurrent state of Chinese guidelines, culminating in the fommulation of industry standards for guidelines. 3.Impact of cancer-associated fibroblasts on immunotherapy and liver metastasis in colorectal cancer

Objective To investigate the impact of cancer-associated fibroblasts(CAFs)on immunotherapy and liver metastasis in colorectal cancer(CRC).Methods The single-cell sequencing data(GSE205506)of CRC patients with mismatch repair deficiency(MMRd)were downloaded from the gene expression omnibus database,and R software was used to preprocess the original sequencing data and establish the umap of fibroblast subpopulations,with each subpopulation named based on signature genes.GraphPad was used for the statistical analysis of the proportion of each fibroblast subpopulation,and the key subpopulations with significant differences were analyzed among CRC patients before and after PD-1 immunotherapy,as well as between the patients with pathological complete response(pCR)and those without pCR(non-pCR)after treatment.The analysis of differentially expressed genes and the gene pathway enrichment analysis were performed for the key subpopulations.The TCGA database was used to perform a prognostic and survival analysis of the signature genes of key CAF subpopulations,and RNA sequencing data were used to score and calculate the proportion of key CAF subpopulations in the primary lesions of CRC patients with liver metastasis.The independent-samples t test was used for comparison of normally distributed continuous data between two groups;the Kaplan-Meier method was used to plot survival curves,and the log-rank test was used to calculate survival rates.CellPhoneDB software was used to analyze the receptor-ligand interaction between fibroblast subpopulations and tumor cells,and in vitro cell experiments were used to validate the effect of NRG1,a key ligand molecule,on the migration and invasion abilities of CRC cells.Results After PD-1 immunotherapy for CRC patients,there was a significant reduction in the proportion of F6_MMP1+CAFs(P<0.001),which was only observed in patients achieving complete remission after immunotherapy.F6_MMP1+CAFs were upregulated,as well as the genes and signaling pathways associated with tumor migration and invasion,and in addition,there was a significant increase in F6_MMP1+CAFs in the tumor tissue of CRC patients with liver metastasis(P<0.000 1).As a ligand,NRG1 expressed by F6_MMP1+CAFs interacted with ERBB3 receptor expressed by tumor cells,and the in vitro experiments confirmed that NRG1 promoted the migration and invasion abilities of tumor cells by activating the ERBB signaling pathway(P<0.05).Conclusion F6_MMP1+CAFs may affect the efficacy of PD-1 immunotherapy in CRC patients and play an important role in promoting liver metastasis in CRC.F6_MMP1+CAFs,along with NRG1 that is produced by them and can promote tumor metastasis,can be used as potential therapeutic targets and prognostic markers for CRC.

Objective:To understand the epidemiological characteristics of human respiratory syncytial virus (HRSV) among acute respiratory infection (ARI) cases in 16 provinces of China from 2009 to 2023.Methods:The data of this study were collected from the ARI surveillance data from 16 provinces in China from 2009 to 2023, with a total of 28 278 ARI cases included in the study. The clinical specimens from ARI cases were screened for HRSV nucleic acid from 2009 to 2023, and differences in virus detection rates among cases of different age groups, regions, and months were analyzed.Results:A total of 28 278 ARI cases were enrolled from January 2009 to September 2023. The age of the cases ranged from<1 month to 112 years, and the age M ( Q1, Q3) was 3 years (1 year, 9 years). Among them, 3 062 cases were positive for HRSV nucleic acid, with a total detection rate of 10.83%. From 2009 to 2019, the detection rate of HRSV was 9.33%, and the virus was mainly prevalent in winter and spring. During the Corona Virus Disease 2019 (COVID-19) pandemic, the detection rate of HRSV fluctuated between 6.32% and 18.67%. There was no traditional winter epidemic peak of HRSV from the end of 2022 to the beginning of 2023, and an anti-seasonal epidemic of HRSV occurred from April to May 2023. About 87.95% (2 693/3 062) of positive cases were children under 5 years old, and the difference in the detection rate of HRSV among different age groups was statistically significant ( P<0.001), showing a decreasing trend of HRSV detection rate with the increase of age ( P<0.001). Among them, the HRSV detection rate (25.69%) was highest in children under 6 months. Compared with 2009-2019, the ranking of HRSV detection rates in different age groups changed from high to low between 2020 and 2023, with the age M (Q1, Q3) of HRSV positive cases increasing from 1 year (6 months, 3 years) to 2 years (11 months, 3 years). Conclusion:Through 15 years of continuous HRSV surveillance analysis, children under 5 years old, especially infants under 6 months old, are the main high-risk population for HRSV infection. During the COVID-19 pandemic, the prevalence and patterns of HRSV in China have changed.

6.Analysis of laboratory indexes and pathological features of crescent formation in children with allergic purpura nephritis

Objective To investigate changes of laboratory indicators and pathological features of Henoch-Schonlein purpura nephritis(HSPN)children after crescent formation,and to explore non-invasive biomarkers for predicting crescent formation.Methods A total of 278 children with HSPN who were hospitalized from January 2018 to July 2023 were selected and divided into the crescent formation group(196 cases)and the non crescent formation group(82 cases)based on their crescent formation status.Patients in the crescent formation group were sub-divided into the cellular crescent formation group(52 cases)and the cellular fibrous crescent formation group(144 cases)based on the type of crescent formation.Laboratory indicators and pathological characteristics were compared between different groups.The correlation between each indicator and the proportion of crescent formation was analyzed.The influencing factors of crescent formation were analyzed by Logistic regression.Receiver operating characteristic(ROC)curves were plotted,and the effectiveness of laboratory indicators in predicting crescent formation was evaluated.Results Compared with the non crescent formation group,24-hour urine protein quantification(24 hUP),urine immunoglobulin G/creatinine(UGCR),urine red blood cell count(URBC),neutrophil/lymphocyte ratio(NLR),blood urea nitrogen(BUN)and triglycerides(TG)were significantly increased in the crescent formation group.There were increased proportion of diffuse mesangial hyperplasia(Mb),renal tubular atrophy or interstitial fibrosis(T1)(P<0.05).Compared with the cellular crescent group,the proportion of glomerular segmental sclerosis or adhesion(S1)and T1 were increased,and the proportion of crescent formation was higher in the cellular fibrous crescent group.The proportion of capillary endothelial cell proliferation(E1)was decreased in the cellular crescent group(P<0.05).Spearman correlation analysis showed that 24 hUP,UGCR,URBC,NLR,BUN,TG,Mb and T1 were positively correlated with the proportion of crescent formation(all P<0.05).The results of multivariate Logistic regression analysis showed that elevated UGCR and T1 were risk factors for crescent formation.The area under the curve(AUC)predicted by UGCR for crescent formation was 0.731(95%CI:0.667-0.795,P<0.05),with an optimal cutoff value of 5.00 mg/mmol,sensitivity of 0.744 and specificity of 0.610.Conclusion UGCR can be used as a non-invasive biomarker to assist in evaluating crescent formation in children with HSPN.
7.Analysis on the difference between the payment limitations of anti-cancer drugs and the application scope of drug indications

OBJECTIVE To analyze the difference between the payment limitations of anti-cancer drugs and application scope of drug instructions, so as to better implement the payment policy of medical insurance drugs. METHODS The differences between the payment limitations of anti-cancer drugs and application scope of drug instructions in the National Catalogue of Drugs for Basic Medical Insurance, Industrial Injury Insurance and Maternity Insurance (2022) were compared and analyzed; the evidence-based basis of the difference was discussed, and the scope of limited payment was interpreted. RESULTS Totally 118 drugs had payment limitations; limitations scope mainly included limited evidence of gene detection results, limited indications, limited second-line and above treatment, limited payment duration, limited specialist prescription, limited medical institution grade, etc. Among them, 43 drugs had differences between the payment limitations and drug instructions, and the indications of 31 drugs were greater than payment limitations; for seven drugs, the drug indications beyond the payment limitations were recommended by the guidelines. The payment limitations of 75 drugs were consistent with drug instructions. The second-line and multi-line treatment was ineffective or intolerable with first-line drugs. There was a certain relationship between locally advanced, advanced or metastatic tumor and tumor stage, but different tumors had different criteria. Systemic treatment mainly referred to systemic treatment with drug. The results of limited genetic test required that the result was positive or negative. In addition, six kinds of TCM injections were limited to the level of medical institutions; the payment of two drugs did not exceed 12 months; when lenalidomide was combined with isazomide citrate, the medical insurance only paid for one of the drugs. CONCLUSIONS The payment limitations of some anti- cancer drugs are inconsistent with the drug indications. The drug payment limitations should be expanded according to the actual situation of clinical medication and the recommendations of guidelines. At the same time, the payment limitations should be formulated accurately and in detail, thus clinical and medical insurance staff can understand it and fully protect the interests of patients.
8.Relationship between preoperative widespread pain and chronic post-surgical pain following total knee arthroplasty in patients with knee osteoarthritis

Objective:To evaluate the relationship between preoperative widespread pain and chronic post-surgical pain (CPSP) following total knee arthroplasty (TKA) in the patients with knee osteoarthritis.Methods:Two hundred American Society of Anesthesiologists physical status Ⅰ-Ⅲ patients with knee osteoarthritis, aged 40-70 yr, undergoing elective the first unilateral primary TKA under general anesthesia, were enrolled.The widespread pain index, visual analogue scale score, Hospital Anxiety and Depression Scale and Central Sensitization Inventory scores were recorded at 1 day before surgery.The patients were divided into CPSP-positive group and CPSP-negative group according to visual analogue scale score at 6 months after surgery.Risk factors for CPSP were analyzed by logistic regression.Results:The results of logistic regression analysis showed that increased preoperative widespread pain index score, Central Sensitization Inventory score and Hospital Anxiety and Depression Scale score and female were risk factors for CPSP after TKA.Conclusions:Preoperative widespread pain is a risk factor for CPSP following TKA in the patients with knee osteoarthritis.
9. Comparison of disinfection effects of different disinfection methods for caps of three-porous polypropylene infusion bottles

Objective:
To compare the disinfection effect of three-hole polypropylene infusion bottle stopper by central sterilization method and groove central sterilization method, and to explore the best method of three-hole polypropylene infusion bottle cap disinfection.
Methods:
A total of 1 088 bottles of three-pore polypropylene infusion bottles from 10 hospitals in Panzhihua City and the surrounding areas of Chengdu were selected for disinfection, and the effects were compared by fluorescent powder tracing method and bacterial culture method respectively. The two methods were divided into two groups: bottle stopper Central Sterilization group and groove Central Sterilization group. Each nurse sterilized two groups of liquid.

10.Meta-analysis of the Effectiveness of Trolamine for Preventing and Treating Radiation Dermatitis and Quality Evaluation of GRADE Evidence

OBJECTIVE: To evaluate the effectiveness of trolamine for preventing and treating radiation dermatitis (RD) and evidence quality, and to provide reference for clinical use. METHODS: Retrieved from PubMed, Cochrane library, Embase, CNKI, Wanfang and VIP database, randomized controlled trials (RCTs) about trolamine (trial group) versus usual care (control group) for preventing and treating RD were collected. After data extraction, Cochrane bias risk assessment tool 5.0.2 was used to assess the bias risk, and Rev Man 5.3 statistical software was used to perform the Meta-analysis. GRADE evidence quality grading system was used to evaluate the evidence quality of outcome indexes. RESULTS: Seven RCTs were included, involving 782 patients. Results of Meta-analysis showed that there was no statistical significance in total incidence of RD [OR=0.50, 95%CI (0.23, 1.11), P=0.09], and the incidence of grade Ⅰ RD [OR=1.32, 95%CI(0.96,1.81), P=0.09], grade Ⅱ RD [OR=1.07, 95%CI(0.80,1.42), P=0.66], grade Ⅲ RD [OR=0.69, 95%CI(0.45,1.04), P=0.07] or grade Ⅳ RD [OR=0.43, 95%CI(0.17,1.05), P=0.07] between 2 groups. Results of Grade evidence quality evaluation showed that total incidence of RD, and the incidence of grade Ⅱ RD and grade Ⅳ RD were recommended by moderate-level evidence in 2 groups, while the incidence of grade Ⅰ and grade Ⅲ RD were recommended by low-level evidence. CONCLUSIONS: Trolamine is not effective in preventing and treating RD, and can not reduce the incidence of RD.
