1.Adolescent anxiety and non-suicidal self-injury behavior: the mediating role of depression and the moderating role of social support

BackgroundNon-suicidal self-injury (NSSI) behavior among adolescents has become a global public health concern. Anxiety and depression are considered key factors influencing NSSI behavior, while social support may play a protective role in alleviating emotional and behavioral issues. However, existing research has primarily focused on the direct impact of individual factors on NSSI behavior, with insufficient exploration of the combined effects of anxiety, depression and social support. ObjectiveTo investigate the direct effect of anxiety on NSSI, the mediating role of depression and the moderating role of social support in relationship between anxiety and NSSI behavior, thus to provide references for the prevention and intervention of NSSI behavior among adolescents. MethodsIn February 2022, a total of 40 820 students in grades 7 to 12 across 10 middle schools in a district of Chengdu were selected as participants, and they were assessed using Generalized Anxiety Disorder Scale-7 item (GAD-7), Patient's Health Questionnaire Depression Scale-9 item (PHQ-9), Social Support Scale for Urban Students (SSSUS) and Adolescent Self-Harm Scale (ASHS). Pearson correlation analysis was conducted to examine the correlations between scale scores among adolescents with NSSI behaviors. Mediation and moderation analyses were performed using Process 3.5 in SPSS, and the significance was tested with bootstrapping. The interaction was visualized by using simple slope analysis. ResultsAmong 34 534 (84.60%) valid respondents, 542 adolescents (1.57%) reported engaging in NSSI behavior. Significant differences in gender, GAD-7 scores, PHQ-9 scores, and SSSUS scores were observed between NSSI behavior group and non-NSSI group (χ²/t=62.889, 71.120, 94.365, -41.464, P<0.01).Adolesents with NSSI showed positive correlations between GAD-7 scores and both ASHS and PHQ-9 scores (r=0.158, 0.166, P<0.01). PHQ-9 scores were positively correlated with ASHS scores (r=0.364, P<0.01), but negatively correlated with SSSUS scores (r=-0.290, P<0.01). SSSUS scores were negatively correlated with ASHS scores (r=-0.247, P<0.01). Depression partially mediated the relationship between anxiety and NSSI behavior, with an effect size of 0.544 (95% CI: 0.162~0.944), accounting for 35.79% of the total effect. Social support moderated the relationship between depression and NSSI bahavior, with an effect value of -0.082 (95% CI: -0.135~-0.029). ConclusionAnxiety not only directly influences NSSI bahavior among adolescents, also indirectly exacerbates it through depression, while social support mitigates the impact of depression on NSSI behavior. 2.Influence of deviation of the bolt in femoral neck system on the short-term clinical outcomes

Objective:To explore the influence of deviation of the bolt in femoral neck system (FNS) on the short-term outcomes in young and middle-aged patients with displaced femoral neck fracture.Methods:A retrospective analysis was conducted of the 114 young and middle-aged patients with displaced femoral neck fracture who had been treated with FNS at Department of Orthopaedics, Suzhou Municipal Hospital from December 2019 to January 2023. Based on the postoperative measurements of the deviation of the bolt tip to the central axis of the femoral head and neck (W), the patients were divided into a central group (W≤20%) and a deviation group (W>20%). In the central group of 63 cases, there were 27 males and 36 females with a mean age of (46.4±8.0) years. In the deviation group of 51 cases, there were 20 males and 31 females with a mean age of (45.1±9.8) years. The 2 groups were compared in terms of weight-bearing time, fracture healing time, tip-apex distance, degree of femoral neck shortening, Harris Hip Score and EuroQol five-dimensional questionnaire-5L (EQ-5D-5L) utility value at the last follow-up, as well as complications and revision surgeries.Results:There was no statistically significant difference in the preoperative general information, auxiliary reduction or quality of fracture reduction between the 2 groups, showing comparability between groups ( P>0.05). There was no significant difference in the partial weight-bearing time between the 2 groups ( P>0.05). In the central group, the full weight-bearing time [15.0 (14.0, 16.0) weeks] and fracture healing time [14.0 (12.0, 15.0) weeks] were significantly shorter than those in the deviation group [16.0 (15.0, 19.0) weeks; 15.0 (13.0, 17.0) weeks], the tip-apex distance [(21.4±3.4) mm] was significantly shorter than that in the deviation group [(23.5±2.7) mm], the Harris Hip Score [(90.6±6.1) points] and EQ-5D-5L utility value [0.9 (0.8, 0.9)] at the last follow-up were significantly higher than those in the deviation group [(87.7±6.2) points; 0.9 (0.8, 0.9)], and the incidences of moderate and severe femoral neck shortening [25.4% (16/63)], avascular necrosis of the femoral head [0 (0/63)] and revision surgery [0 (0/63)] were significantly lower than those in the deviation group [66.7% (34/51), 7.8% (4/51), 9.8% (5/51)] (all P< 0.05). Conclusion:A closer positioning of the FNS bolt to the central axis of the femoral head and neck favors satisfactory short-term outcomes and a lower revision surgery rate in young and middle-aged patients with displaced femoral neck fracture.
3.GATA4 Forms a Positive Feedback Loop with CDX2to Transactivate MUC2 in Bile Acids-Induced Gastric Intestinal Metaplasia

Background/Aims:
Gastric intestinal metaplasia (GIM), a common precancerous lesion of gastric cancer, can be caused by bile acid reflux. GATA binding protein 4 (GATA4) is an intestinal transcription factor involved in the progression of gastric cancer. However, the expression and regulation of GATA4 in GIM has not been clarified.
Methods:
The expression of GATA4 in bile acid-induced cell models and human specimens was examined. The transcriptional regulation of GATA4 was investigated by chromatin immunoprecipitation and luciferase reporter gene analysis. An animal model of duodenogastric reflux was used to confirm the regulation of GATA4 and its target genes by bile acids.
Results:
GATA4 expression was elevated in bile acid-induced GIM and human specimens.GATA4 bound to the promoter of mucin 2 (MUC2) and stimulate its transcription. GATA4 and MUC2 expression was positively correlated in GIM tissues. Nuclear transcription factor-κB activation was required for the upregulation of GATA4 and MUC2 in bile acid-induced GIM cell models. GATA4 and caudal-related homeobox 2 (CDX2) reciprocally transactivated each other to drive the transcription of MUC2. In chenodeoxycholic acid-treated mice, MUC2, CDX2, GATA4, p50, and p65 expression levels were increased in the gastric mucosa.
Conclusions
GATA4 is upregulated and can form a positive feedback loop with CDX2 to transactivate MUC2 in GIM. NF-κB signaling is involved in the upregulation of GATA4 by chenodeoxycholic acid.
4.Progress of lateral unicompartmental knee arthroplasty for lateral compartment osteoarthritis of the knee

With the development of the knee-preservation concept and surgical techniques, lateral unicompartmental knee arthroplasty (UKA) has become an effective method for the lateral compartment osteoarthritis of the knee.The incidence of knee osteoarthritis is low in the lateral compartment, which is different from the medial compartment in anatomical and kinematic characteristics. These two factors increase the challenge of lateral UKA, so the clinical efficacy of lateral UKA has been controversial. With the improvement and renewal of treatment concepts, surgical techniques and prosthesis design, the clinical effect of lateral UKA and the survival rate of the prosthesis have been continuously optimized. This article aims to summarize the characteristics of lateral compartment knee osteoarthritis, clinical efficacy, prosthesis selection, reasons for revision, future application and prospects, combined with relevant literature in recent years, to review the current progress of lateral UKA.
5.Willingness to receive influenza vaccines and its influencingfactors among residents in Ezhou City

Objective:
To investigate the willingness to receive influenza vaccines and its influencing factors among residents in Ezhou City, Hubei Province, so as to provide the evidence for improving the coverage of influenza vaccination.
Methods:
Residents at ages of 15 years and older were sampled from 12 communities in Ezhou City using a multi-stage random sampling method in May 2022. Participants' demographic features, influenza vaccination, awareness of influenza vaccine knowledge, attitudes towards influenza vaccination and willingness to receive influenza vaccine were collected using questionnaire surveys. Factors affecting the willingness to receive influenza vaccine were identified using a multivariable logistic regression model.
Results:
Totally 840 questionnaires were allocated, and 820 valid questionnaires were recovered, with an effective recovery rate of 97.62%. The participants included 385 men (46.95%) and 435 women (53.05%) and had a mean age of (42.04±14.82) years. Among all participants, 47.93% were willing to receive influenza vaccine. Multivariable logistic regression analysis identified age (15-24 years, OR=6.010, 95%CI: 3.342-10.809; 25-39 years, OR=4.555, 95%CI: 2.815-7.369; 40-59 years, OR=1.697, 95%CI: 1.057-2.724), living in Echeng District (OR=4.246, 95%CI: 2.968-6.075), urban household registration (OR=4.424, 95%CI: 3.210-6.096), educational level (junior high school, OR=1.712, 95%CI: 1.149-2.551; high school/technical secondary school, OR=3.883, 95%CI: 2.645-5.702; diploma and above, OR=6.830, 95%CI: 4.033-11.568), occupation (self-employed person, OR=3.195, 95%CI: 1.747-5.843; retiree, OR=6.600, 95%CI: 3.240-13.446; worker, OR=8.250, 95%CI: 4.163-16.349; student, OR=9.562, 95%CI: 4.560-20.051; civil servant/public institution personnel, OR=11.756, 95%CI: 5.571-24.809), monthly income (≥1 500 Yuan, OR=2.177, 95%CI: 1.427-3.321; >4 500 Yuan, OR=6.359, 95%CI: 3.391-10.289), times of influenza vaccination in the past two years (once, OR=6.552, 95%CI: 4.086-10.504; twice, OR=9.601, 95%CI: 3.995-23.073), awareness of influenza vaccine knowledge (OR=14.641, 95%CI: 10.426-20.558), and positive attitudes towards influenza vaccines (OR=28.706, 95%CI: 19.660-41.915) as factors affecting the willingness to receive influenza vaccines.
Conclusions
The willingness to receive influenza vaccine was 47.93% among residents in Ezhou City, and age, residence, household registration, educational level, occupation, income level, history of influenza vaccination, awareness of influenza vaccine-related knowledge and attitudes towards influenza vaccines correlate with the willingness to receive influenza vaccine.
6.Progress of drug-loaded β-tricalcium phosphate in the treatment of bone defects

Bone defects caused by trauma, infection, tumor and other factors is a thorny problem in orthopedic clinic, and promoting bone repair and regeneration is the key and difficult point of treatment. In addition to autologous bone grafting, artificial bone materials are often used for large bone defects. β-tricalcium phosphate has good biocompatibility, bone conduction and bone induction properties, and has been studied deeply because of its excellent drug delivery performance and has shown broad application prospects. In this paper, the author will summarize the research progress of β-tricalcium phosphate composites loaded with different drugs in the treatment of bone defects caused by trauma, infection and tumor.
7.Identification of a COL2A1 mutation in a Chinese family with Stickler syndrome type 1 via whole exome sequencing

Objective:To identify the disease-causing mutation in a Chinese family with Stickler syndrome type 1.Methods:The pedigree investigation was conducted.A Chinese family with Stickler syndrome type 1 was enrolled in the Shantou International Eye Center in June 2012.Medical history collection and clinical examinations, such as vision, intraocular pressure, slit lamp microscopy and fundus, were carried out in all the included family members and the diagnosis was made by clinical experts.Total genomic DNAs were extracted from the peripheral blood samples (5 ml) obtained from 5 patients and 4 healthy members.The potential variant of the proband's father Ⅲ-5 were screened by whole exome sequencing (WES) and stepwise bioinformatic analysis.The segregation and mutation conformation of the variant was verified by Sanger sequencing.The pathogenicity of the variant was predicted by SIFT, Polyphen2, and MutationTaster.Conservation and three-dimensional structure of amino acid mutation were analyzed by multiple sequence alignment and UniProt.This study adhered to the Declaration of Helsinki.The study protocol was approved by the Ethics Committee of Joint Shantou International Eye Center (No.EC20110310[2]-P02).Written informed consent was obtained from each subject or the guardian.Results:An autosomal dominant inherence in 39 members of 4 generations including 15 patients and 24 phenotypically normal members was found in the family.The proband (Ⅳ-4) showed high myopia, retinal detachment and strabismus in the right eye, and the left eye was blind.A patient (Ⅲ-5) showed high myopia and cataract in the right eye, atrophy in the left eye.A patient (Ⅳ-9) showed binocular high myopia.A heterozygous variation, c.1693C>T: p.Arg565Cys, within the exon 26 of COL2A1 gene was revealed in patient Ⅲ-5, which was only found in the patients and not in phenotypically normal members, indiacating co-separation in this family.The variant was predicted to be a severe damage by SIFT, Polyphen2 and MutationTaster.The amino acid mutation at position 565 was highly conservative among human, mouse, rat, bovine and Xenopus laevis, which caused the arginine to cysteine substitution at the X position in triple helix repeat region Gly-X-Y, affecting the function of fibrous protein and becoming pathogenic. Conclusions:Variant c.1693C>T: p.Arg565Cys in COL2A1 gene is disease-causing in this family and this is the first report about the variant in China.
8.Effects of different negative pressure suction devices on collagen, MMP-1 and TGF-β secretion of skin

Objective:To compare the effects of circular arch device and dressing device on the secretion of collagen, matrix metalloproteinase (MMP-1), transforming growth factor (TGF-β) of fibroblasts in the treated skin, and to explore the changes and mechanisms.Methods:The animals were purchased from the Southern Medical University Laboratory Animal Center. Using the established animal model, 54 mice were randomly divided into three groups: 18 mice each group, including control group, dressing device group and and circular arch device group. At the first, third and fifth day after negative pressure suction treatment, dermal thickness and density of collagen levels were and eosin staining, collagen level in local skin was determined using spectrophotometry, and MMP-1 and TGF-β levels were determined using enzyme linked immunosorbent assay.Results:Dermal thickness level was increased significantly in negative suction treated groups [after 5 days of treatment, control group: (71±8) μm; circular arch device group: (351±9) μm; dressing device group: (267±12) μm, P< 0.05]. Compared with the control group, collagen level in the dermis was significantly higher in circular arch device groups [after 5 days of treatment, control group: (30.9±4.3) mg/g; circular arch device group: (72.7±3.6) mg/g, P< 0.05]. TGF-β level in circular arch device treated group was increasing gradually, and markedly higher than that in the dressing device group and the control group [after 5 days of treatment, control group: (0.24±0.1) ng/ml; circular arch device group: (0.78±0.08) ng/ml; dressing device group: (0.39±0.18) ng/ml, P< 0.05], while MMP-1 level was decreasing. In addition, TGF-β and MMP-1 levels in the dressing device treated group remained stable during the entire experiment. Conclusions:The interface of the circular arch negative pressure suction device can effectively activate the fibroblasts, promote the secretion of TGF-β and collagen, and thicken the skin tissue.
9.Enzalutamide for metastatic prostate cancer: a case report and review of the literature

To investigate the efficacy and adverse reactions of enzalutamide in the treatment of metastatic prostate cancer(mPCa). Two male patients were reported, Patient 1 was hospitalized due to elevated PSA for 6 months and fatigue for 2 weeks. Tests on the admission showed that tPSA > 1 000 ng/ml, pathological of prostate biopsy: prostate acinar carcinoma, Gleason score 4+ 4=8, imaging examination revealed multiple metastases of prostate cancer throughout the body. Patient 2 was admitted to hospital due to cough, sputum, chest tightness and difficulty in defecation for more than 20 days, and the patient had radical surgery for prostate cancer five years ago. Tests on admission showed that tPSA > 100 ng/ml, fPSA>50 ng/ml. The patient was diagnosed as postoperative local recurrence of prostate cancer with bilateral pleural effusion, and cancer lymphangitis was considered. Both patients were treated with enzalumide combined with goseraline. Both patients were followed up for 6 months, PSA decreased significantly within half a year, general condition continued to improve, and they were well tolerated, no obvious adverse reactions occurred.
10.Sex differences of upper tract urothelial carcinoma: results from the Surveillance, Epidemiology, and End Results database and a single-center experience

Objective:To validate gender-related differences in patients with upper tract urothelial carcinoma (UTUC).Methods:We used a method of combination of SEER database analysis and validation in our center. We selected 2 125 men (57.8%) and 1 552 (42.2%) women who underwent radical nephroureterectomy (RNU) for UTUC between 1995 and 2015 within the surveillance, Epidemiology, and End Results registries (SEER). In male cases, the median age was 71(22-99)years old, median follow-up time was 53.0 (0-227) months, 902 (89.5%) cases were Caucasian, 1 384 cases (65.1%) were located in the renal pelvis, and 810(38.1%) cases were T 3-T 4. For female cases, the median age was 73.6(25-99)years old, median follow-up time was 53.1(0-226) months, 1 417(91.3%) cases were Caucasian, 1 043 cases (67.2%) were located in the renal pelvis, and 508 (32.7%) cases were T 3-T 4. The analysis of SEER database showed that female patients were older compared to male patients ( P=0.002), the proportion of T 3-T 4 was even lower ( P=0.028). There were no statistically significant differences in race, tumor location, and follow-up time between males and females (all P>0.05). We also enrolled 131 men (55.3%) and 106 women (44.7%) who underwent RNU for non-muscle invasive (T a/T is/T 1) UTUC between January 2000 and January 2015. These patients had no history of bladder cancer, UTUC and kidney transplantation. Preoperative cystoscopy was performed to confirm the absence of bladder tumor. The male group was (65.8±12.4)years old, with history of aristolochic acid medications in 7(5.3%) cases, 98 smoking cases (74.8%), and estimated glomerular filtration rate (eGFR) of (64.2±29.4)ml/(min·1.73 m 2). In the female group, mean age was (66.7±11.9)years, 14 (13.2%) cases had history of aristolochic acid medications, 16(15.1%) had history of smoking, eGFR (56.3±27.9) ml/(min·1.73m 2). Compared with female patients, male patients tended to had less aristolochic acids exposure (5.3% vs.13.2%, P<0.001), frequent smoking (74.8% vs. 15.1%, P<0.001) and better renal function ( P=0.036). The Kaplan-Meier test was used for time-to-event analysis. Univariate and multivariate logistic regression models were adopted to examine the effect of gender on the development of T 3-T 4 tumor. Univariate and multivariate Cox regression analyses were used to assess the roles of factors on overall survival (OS) in both SEER and cases in our center, and competing-risks regression model was used to assess the roles of factors on cancer-specific survival (CSS) in both SEER and cases in our center. Results:Multivariate logistic analysis showed that gendere represented an independent risk factor of T 3-T 4 UTUC development( OR=0.86, P=0.02). Kaplan-Meier analysis showed that better OS and CSS for females only existed in the non-muscle invasive stages(5-year OS rates were 80.4% vs.87.3%, χ 2=31.0, P<0.001; 5-year CSS rates were 82.6% vs.89.2%, χ 2=31.2, P<0.001). In multivariate competing-risks regression models, no statistically significant differences in survival were observed between males and females ( HR=0.83, P=0.115). For the patients in our center, there were also no statistically significant differences existed in the non-muscle invasive stage between two genders on OS and CSS ( HR=0.93, P=0.071; HR=0.87, P=0.064). Conclusions:Females were less likely to have advanced pathological T stage. The differences on OS and CSS between males and females only existed in non-muscle invasive stage. However, after accounting for gender related factors, gender no longer had effects on UTUC prognosis.
