1.Is intravenous thrombolysis necessary before mechanical thrombectomy for acute ischemic stroke?

Acute ischemic stroke with large vessel occlusion (AIS-LVO) refers to ischemic stroke caused by large vessel occlusion of internal carotid artery, middle cerebral artery and vertebrobasilar artery, which has the characteristics of large infarct, relatively poor outcome, higher disability and mortality. Therefore, early vascular recanalization and rescue of ischemic penumbra are the key to improve the outcome of patients with AIS-LVO. Mechanical thrombectomy (MT) has a definite effect on AIS-LVO. The current guidelines recommend that MT should be performed on the basis of IVT for AIS-LVO patients without contraindications of intravenous thrombolysis (IVT), the so-called bridging therapy. IVT can increase the risk of bleeding to a certain extent, delay MT and increase the cost of hospitalization. However, there are still many controversies about whether the patients with AIS-LVO can directly perform MT. This article reviews the comparative study of direct MT and bridging therapy in patients with anterior circulation AIS-LVO, hoping to provide reference for clinicians in the treatment of AIS-LVO.
2.Research on core competency and influencing factors of standardized training period for newly recruited nurses in Traditional Chinese medicine hospital

Objective:To investigate the status of core competency of newly recruited nurses in Traditional Chinese Medicine (TCM) hospitals during the standardized training period and explore its influencing factors.Methods:From March to April 2018, new recruits of nurses from various levels of TCM hospitals in 11 provinces or municipalities directly under the central government were selected to conduct a questionnaire survey, and the results of the survey were statistically analyzed.Results:728 questionnaires were actually recovered, including 723 valid questionnaires. The total score of the core competency of newly recruited nurses in the standardized training period of the TCM hospital is (160.42±37.25) . The results of single factor analysis showed that the differences in core competency scores of nurses with different educational backgrounds, graduated colleges and working stages were statistically significant ( P<0.05) . Correlation analysis showed that the scores of occupational identity of newly recruited nurses in TCM hospitals were positively correlated with the scores of core competency ( P<0.05) . Multiple linear regression analysis showed that the types of colleges, academic qualifications, working stages, and the sense of selfdetermination, self-efficacy, Sense of organizational influence, consistency in the occupational identity were the influencing factors of nurses' core competency ( P<0.05) . Conclusions:The core competency of newly recruited nurses in TCM hospitals is affected by the types of colleges, academic qualifications, working stages, and the sense of self determination, self-efficacy, sense of organizational influence, consistency in the occupational identity. Targeted measures should be taken to improve the core competency and professional identity of newly recruited nurses, so that can enhance the cohesion of the nursing team and improve the overall level of nursing.
3. The Development of Services System of Integrated Home Care for the elderly based on Omaha System

Objective:
To develop a comprehensive services system of integrated home care for the elderly based on Omaha System.
Methods:
The Omaha system was used to determine relevant indicators of the content of integrated home care services for the elderly, and identified through the Delphi expert consultation.
Results:
After two rounds of expert correspondence consultation, the effective recovery rate of the questionnaire was 100.00%, the authoritative coefficient of the experts was 0.872 and 0.851, the variation coefficient of the first and second indexes in the second round of expert correspondence consultation was 0.00 to 0.28, The mean importance ratings ranged from 3.80 to 5.00 on a 5-point scale, and the full score was 33.33% to 100.00%. Finally, four first-level indicators including physiological field, social-psychological field, health-related behavior field, environmental field and other fields were established, as well as 43 second-level indicators.
Conclusions
The indicators related to the services system of integrated home care services for the elderly based on Omaha system are reliable and scientific, which can provide reference basis for further understanding the needs of the elderly of integrated home care services, improving the quality of medical care and old-age care services for the elderly.
4.Correlation analysis between meteorological factors and onset of acute cerebral infarction in Dalian

Objective To provide more reference base for the prevention and treatment of acute cerebral infarction through the correlation study between the onset of acute cerebral infarction and meteorological factors during the same period in Dalian area.Methods From January 1,2015 to December 31,2015,the data of hospitalized medical records and meteorological data during the same period in 3 5 7 8 consecutive patients with acute cerebral infarction admitted to three tertiary hospitals in Dalian were analyzed retrospectively.The seasonal regularity of acute cerebral infarction in Dalian area was analyzed descriptively.A single retrospective 1 ∶ 1 matched case-crossover study design was used to analyze the effect of mean air temperature lag 0-3 days on the onset of acute cerebral infarction.Pearson correlation analysis was used to analyze the influence of meteorological factors of the day on the onset of acute cerebral infarction,at the same time,the relationship between stroke subtypes and meteorological factors was investigated.Results (1) The incidence of acute cerebral infarction was higher in winter (December,January,and February) and it was lower in summer (from June to August).(2) Hysteresis analysis:when the temperature was reduced by 1 ℃each time,the OR values of acute cerebral infarction lagging behind 0 and 1 d were 1.034 (95% CI 1.012-1.056) and 1.025 (95% CI 1.008-1.042) respectively.There was significant difference (all P < 0.05).The correlation between the temperature of the day of onset and the onset of acute cerebral infarction was the highest.(3) The onset of cerebral infarction was negatively correlated with the average temperature of the day and hours of sunshine (r =-0.392,-0.260,all P < 0.01),and it was positively correlated with the daily average air pressure (r =0.403,P < 0.01).Among them,the correlation of the type of cardiogenic embolism and each meteorological factor was the highest.The correlation coefficients of daily average temperature,daily air pressure,and hours of sunshine were-0.836,0.733,and-0.629,respectively (all P < 0.01).Conclusion A cold and high air pressure may trigger the onset of acute cerebral infarction,especially cardiogenic cerebral infarction.

Objective:
To evaluate the prognostic value of 18F-fluorodeoxyglucose positron emission tomography (PET)/computed tomography (CT) in patients with diffuse large B cell lymphoma (DLBCL) undergoing autologous hematopoietic stem cell transplantation (auto-HSCT).
Methods:
Forty-eight patients with DLBCL treated at Peking University Cancer Hospital between November 2010 and December 2014 were assessed. All patients underwent PET/CT scanning prior to or after auto-HSCT. Correlation analysis was done based upon patients characteristics, PET/CT scan results and survival.
Results:

Objective:
To evaluate the clinical characteristics and survival outcomes of patients with de novo grade 3 or transformed follicular lymphoma (FL).
Methods:
Fifty-two patients treated at Peking University Cancer Hospital between January 2009 and September 2017 were assessed, including 28 patients with FL 3A grade, 13 patients with FL 3B grade, 11 patients with transformed FL. Baseline characteristics, survival and prognostic factors were analyzed.
Results:

Minor stroke is a common type of acute ischemic stroke. Early large-scale thrombolytic studies regarded minor stroke as a contraindication of intravenous thrombolysis. In recent years, more and more retrospective studies have shown that some minor stroke patients can still benefit from intravenous thrombolysis. Some studies are starting to identify patients who are more likely to be treated with thrombolysis, but there is still a lack of evidence-based evidence. This article summarizes the thrombolytic study of minor stroke, hoping to provide some references for nerve physicians.
8.Effects of transitional care on growth and development among late preterm infants

Objective To explore the effects of transitional care on growth and development among late preterm infants.Methods From January 2015 to December 2016, we selected 100 late preterm infants in Hangzhou Red Cross Hospital. All of them were divided into observation group and control group with the random number table, 50 cases in each group. On discharge from hospital, control group adopted routine health guidance and telephone follow-up. Observation group carried out transitional care. And then, we compared the growth and development of infants between two groups after discharge from hospital.Results Physical growth such as weight, height and head circumference of preterm infants in observation group was higher than those in control group in the first, third and sixth month after birth (P<0.05). In the sixth month, the standard-reaching rate of physique of observation group and control group was 57.14% and 31.91% respectively with a significant difference (χ2=6.174,P<0.05). In the sixth month, there were two cases with readmission to hospital in observation group and 9 in control group with a significant difference (χ2=5.368,P<0.05). The parents' satisfaction of preterm infants of observation group was 87.75% significantly higher than that of control group (χ2=20.030, P<0.001).Conclusions The model of transitional care can promote growth and development of late preterm infants, reduce the morbidity rate and readmission rate of late preterm infants, and improve parents' satisfaction.

Objective:
To investigate the clinical features, diagnosis, treatment and prognosis of primary intestinal lymphoma (PIL) .
Methods:
The characteristics, diagnosis, treatment methods, and follow-up outcomes of 99 PIL patients, diagnosed in Peking university cancer hospital between Nov.1,1995 and Nov. 30, 2013.
Results:
There were 65 males and 34 females with a median age of 50 years. The majority of clinical manifestation were non-specific gastrointestinal symptoms, 67.68% of cases presented abdominal pain, 26.26% with acute abdomen. The most common primary sites of ileum and ileocecus were identified in 21 cases, respectively. The positive rate of endoscopic was only 24.24%, and 69 cases were diagnosed by operation. 71 patients (71.72%) were stageⅠ-Ⅱand 28 patients (28.28%) were stage Ⅳ. Hodgkin’s lymphoma was not found in all patients. Of the 99 cases, 77 were B-cell origin (77.78%) and 22 were T-cell origin. 55 cases (55.56%) were diagnosed with diffuse large B cell lymphoma (DLBCL) . 60 cases presented IPI score 0-1 point. The median overall survival (OS) was 100.0 months, and 5 year overall survival (5y-OS) was 53.5%. OBJECTIVETo compare the prognostic value of different models in patients with early-stage diffuse large B-cell lymphoma (DLBCL).
METHODSEarly-stage DLBCL patients diagnosed from January 2000 to December 2012 were analyzed retrospectively. All patients received with at least 2 cycles of immunochemotherapy R-CHOP regimen (rituximab, cyclophosphamide, doxorubicin, vincristine, prednisone) with or without radiotherapy. The prognostic value of international prognostic index (IPI) , revised IPI (R-IPI) and enhanced IPI (NCCN-IPI) was compared.
RESULTSNinety-seven cases of early-stage DLBCL were included in the study. The median age was 58 years (15-88 years) with a median follow-up of 34.7 months (range 7.3-77.4 months). The expected 5-year overall survival (OS) for entire group was 82%. There was no patient in the high risk group according to IPI or NCCN-IPI. According to IPI, the 5-year OS in the low, low intermediate, high intermediate risk groups were 95%, 38% and 60%, respectively. According to R-IPI, the 5-year OS in the very good, good, and poor risk groups were 93%, 75% and 60%, respectively. According to NCCN-IPI, the 5-year OS in the low, low intermediate, high intermediate risk groups were 92%, 85% and 29%, respectively.
CONCLUSIONNCCN-IPI would be of an ideal prognostic model for early-stage DLBCL patients.
