1.Surveillance for Aedes albopictus in Guangzhou City from 2021 to 2023

Objective To investigate the population density and seasonal fluctuations of Aedes albopictus in Guangzhou City, Guangdong Province, from 2021 to 2023, so as to provide insights into A. albopictus control and management of dengue fever. Methods The surveillance of A. albopictus density was performed in all surveillance sites assigned across all streets (townships) in Guangzhou City during the period from January to December from 2021 to 2023. The surveillance frequency was twice every half month from May to September, and once every month for the rest of a year. In each surveillance period, A. albopictus mosquito larvae were captured from indoor and outdoor small water containers in residential areas, parks, medical facilities, schools, other government sectors and social organizations, construction sites, special industries and others for mosquito species identification. Adult mosquitoes were captured using electric mosquito suction apparatus for species identification and gender classification. Adult mosquitoes and mosquito eggs were collected with mosquito and egg traps at the breeding and dwelling places of Aedes mosquitoes for identification. The mosquito oviposition index (MOI), Breteau index (BI), adult mosquito density index (ADI) and standard space index (SSI) were calculated. The A. albopictus density was classified into grades 0, 1, 2 and 3 in each surveillance site, with Grade 0 density defined eligible, and the eligible rate of A. albopictus density was calculated at all surveillance sites each year from 2021 to 2023. In addition, the changing trends in MOI, SSI, BI and ADI of A. albopictus were analyzed in Guangzhou City from 2021 to 2023. Results The eligible rates of A. albopictus density were 61.69%, 68.75% and 55.15% in surveillance sites of Guangzhou City from 2021 to 2023 (χ2 = 297.712, P < 0.001), and appeared a tendency towards a reduction followed by a rise each year, which gradually reduced since January, maintained at a low level during the period between May and October, and gradually increased from November to December. The MOI, SSI, BI and ADI of A. albopictus all appeared a tendency towards a rise followed by a reduction in Guangzhou City during the period between January and December from 2021 to 2023. The BI of A. albopictus peaked in the first half of June in 2021 (4.03), the first half of July in 2022 (3.89) and the last half of August in 2023 (5.02), and the SSI of A. albopictus peaked in the last half of June in 2021 (0.93), the last half of May in 2022 (0.59), and the last half of June (0.94) and the first half of September in 2023 (1.12). In addition, the MOI of A. albopictus peaked in the first half of May in 2021 (8.64), the first half of June in 2022 (8.96), and the last half of May (10.21) and the last half of June in 2023 (10.89), and the ADI of A. albopictus peaked in the first half of June in 2021 (3.41), the last half of June in 2022 (4.06), and the first half of July in 2023 (3.61). Conclusions The density of A. albopictus is high in Guangzhou City during the period from May to October, and the risk of local outbreak caused by imported dengue fever is high. Persistent intensified surveillance of the density and seasonal fluctuation of A. albopictus is recommended and timely mosquito prevention and control is required according to the fluctuation in the A. albopictus density.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
3.Value of contrast-enhanced ultrasound combined with transvaginal ultrasound in predicting high-risk endometrial cancer

Objective:To explore the application value of contrast-enhanced ultrasound (CEUS) combined with transvaginal ultrasound features and quantitative parameters in evaluating high-risk endometrial cancer (EC).Methods:Retrospective analysis was made on 69 EC patients who received CEUS examination and were confirmed by surgery and pathology in Beijing Shijitan Hospital, Capital Medical University from December 2017 to September 2022. According to postoperative pathology, the patients were divided into low-risk group ( n=38) and high-risk group ( n=31). The differences in CEUS, transvaginal ultrasound features and quantitative parameters between the two groups were compared, relevant parameters that with predictive value for high-risk EC were screened, and these parameters were scored. Results:①There were differences in lesion size (thick diameter, long diameter), vascular morphology, and color blood flow score between high and low risk ECs (all P<0.05). ②There were differences in CEUS parameters [perfusion mode, enhancement intensity, area under curve(AUC)] between high and low risk EC groups (all P<0.05). ③The areas under the ROC curve for diagnosing high-risk EC were 0.79, 0.69, 0.69, and 0.62, respectively, based on the critical values of lesion thickness diameter ≥1.85 cm, lesion length diameter ≥2.05 cm, ultrasound contrast quantification parameter AUC ≥859 au, and enhancement intensity ≥29.4 dB. ④Using statistically significant parameters for scoring, the sensitivity and specificity for diagnosing high-risk EC with the score ≥5, were 70.97% and 89.47%, respectively. Conclusions:The combination of CEUS and transvaginal ultrasound is a feasible method for predicting high-risk EC. CEUS parameters (enhanced intensity, AUC, and " focal" perfusion mode) are related to high-risk EC. The combination of CEUS and transvaginal ultrasound helps to pre-evaluate the pathological prognostic factors of endometrial malignant lesions before surgery, providing a basis for clinical follow-up treatment.
4.Effect of indoor and outdoor environmental factors on allergic and infectious diseases in children

Objective To investigate the prevalence of allergic and infectious diseases in children, and to assess the influence of indoor and outdoor environmental factors on these two common diseases in children. Methods A questionnaire was used to investigate the prevalence of allergic and infectious diseases in 140 children of 7 years old in Laizhou Bay, Shandong Province. Logistic regression was used to analyze the associations between indoor and outdoor environmental factors and children’s allergic and infectious diseases, respectively. Results The prevalence of previous eczema and other allergic diseases for the past year in children was 37.9% and 15.0%, respectively, and the prevalence of infectious diseases for the past year was 35.7%. As for allergic diseases, eye irritation due to outdoor air ( OR =2.977; 95% CI : 1.407‒6.296) and nose irritation due to outdoor air ( OR =5.147; 95% CI : 1.272‒20.827) were the risk factors for previous eczema in children. Indoor musty taste increased the risks of urticaria ( OR =4.306; 95% CI : 1.062‒17.454) and previous eczema ( OR =3.853; 95% CI : 1.080‒13.743). The use of cockroach killers indoors increased the risk of rhinitis ( OR =6.102; 95% CI :1.297‒28.697). As for infectious diseases, having outdoor pollution sources increased the risk of gastrointestinal infection ( OR =4.937; 95% CI : 1.050‒23.216), and the use of mosquito coils and clothing mothproofing agents increased the risks of respiratory ( OR =6.333; 95% CI : 1.397‒28.714) and gastrointestinal infections ( OR =3.218; 95% CI : 1.074‒9.644), respectively. However, we did not find associations between indoor passive smoking and allergic or infectious diseases. Conclusion Except outdoor passive smoking, all the other indoor and outdoor environmental factors increase the risks of children’s allergies and infectious diseases.

Background Heavy metals are widely present in the environment, difficult to degrade, and bioaccumulative. Children's physical development are not mature, and exposure to heavy metals which may cause irreversible harm to them. However, the current research conclusions are still inconsistent. Therefore, it is very important to pay attention to the impact of heavy metal pollutants on physical development. Objective To detect the level of heavy metal exposure of school-age children in Laizhou Wan, Shandong Province, and to explore the relationship between heavy metal exposure level and physical development indexes of school-age children. Methods The subjects of this study were 318 children born to pregnant women recruited from Laizhou Wan Birth Cohort in Shandong Province from September 2010 to December 2013. The school-age children at age 7 were followed up from July to September 2019. Their urine samples were collected and the levels of arsenic (As), cadmium (Cd), lead (Pb), and mercury (Hg) in urine were detected by inductively coupled plasma mass spectrometry (ICP-MS). Novel Coronavirus Pneumonia (NCP) is a class B infectious disease, which is prevented and controlled according to class A infectious diseases. Recently, children′s NCP cases have gradually increased, and children′s fever outpatient department has become the first strategic pass to stop the epidemic. Strengthening the management of the fever diagnosis process is very important for early detection of suspected children, early isolation, early treatment and prevention of cross-infection. This article proposes prevention and control strategies for fever diagnosis, optimizes processes, prevents cross-infection, health protection and disinfection of medical staff, based on the relevant diagnosis, treatment, prevention and control programs of the National Health and Health Commission and on the diagnosis and treatment experience of experts in various provinces and cities. The present guidance summarizes current strategies on pre-diagnosis; triage, diagnosis, treatment, and prevention of 2019-nCoV infection in common fever, suspected and confirmed children, which provide practical suggestions on strengthening the management processes of children′s fever in outpatient department during the novel coronavirus pneumonia epidemic period.
7.Value of cell-bound complement activation products in the diagnosis of systemic lupus erythematosus

Objective:
To investigate the values of T lymphocyte-bound complement activation products such as T-C3d and T-C4d, B lymphocyte-bound complement activation products such as B-C3d and B-C4d and erythrocyte-bound complement activation products such as E-C3d and E-C4d in the diagnosis of systemic lupus erythematosus (SLE).
Methods:
Peripheral blood samples from 68 SLE patients, 70 patients with non-SLE autoimmune diseases and 68 healthy controls were collected randomly, and the expression levels of T-C4d, B-C4d, E-C4d, T-C3d, B-C3d and E-C3d in these samples were detected by flow cytometry. Meanwhile, antinuclear antibodies (ANA), anti-double stranded DNA antibodies (anti-dsDNA), peripheral blood cell count and other markers were also detected. The differences of cell-bound complement activation products in three groups were analyzed with the area under the receiver operating characteristic curve (AUC), nonparametric test, sensitivity and specificity.
Results:
The specific median fluorescence intensity (SMFI) of T-C4d, B-C4d, E-C4d, T-C3d, B-C3d and E-C3d in SLE patients were significantly higher than those in the patients with non-SLE autoimmune diseases and healthy controls (all P<0.05). The SMFI (median \[P 25, P 75\]) of T-C4d, B-C4d and E-C4d in SLE patients were 3.8(1.2, 13.1), 22.1(6.2, 67.9) and 19.6(1.8, 52.4), respectively. The SMFI of T-C4d, B-C4d and E-C4d in SLE patients with reduced red blood cells and/or lymphocytes were significantly higher than that with normal red blood cell and lymphocyte count (all P<0.05). The AUCs of T-C4d, B-C4d, E-C4d, T-C3d, B-C3d and E-C3d were 0.711, 0.763, 0.663, 0.631, 0.611 and 0.615, respectively (all P<0.05). The sensitivity of the combination of T-C4d with B-C4d (73.5%) in the diagnosis of SLE was superior to that of anti-dsDNA (36.8%).
Conclusion
The cell-bound complement activation products (CB-CAPs) are specifically expressed in SLE patients, and their combination detection is helpful for the diagnosis of SLE.
8.Expression of IFN-γ and IL-10 in peripheral blood and intestinal mucosa of patients with diarrhea post-infectious irritable bowel syndrome

Objective To investigate the expression change of cytokines in peripheral blood and intestinal mucosa in the patients with diarrhea post-infectious irritable bowel syndrome(PI-IBS) and its relation with clinical symptoms scores.Methods Thirty outpatients and inpatients with diarrhea PI-IBS(observation group) and contemporaneous 30 individuals undergoing physical examination(control group) in the Hainan Provincial People's Hospital from January to December 2013 were selected.The peripheral blood mononuclear cells(PBMC) were separated and cultured.Then the levels of IFN-y and IL-10 in peripheral blood and cell culture supernatant fluid were detected by ELISA.The colonic mucosal tissue was taken by coloscopy.Then colonic mucosal IFN-γ and IL-10 protein expression was detected by immunohistochemistry staining.Furthermore,the correlationship between the level change of IFN-γ and IL-10 with clinical symptom score was analyzed by using the Spearman correlation method.Results Peripheral blod IL-10 and IFN-γ levels had no statistical difference between the two groups(P>0.05).Compared with the control group,in PBMC seperation and cuture,the IFN-γ level in the observation group was increased and IL-10 level was decreased,the difference was statistically signifieant(P<0.01).The intestinal main symptom score in the observation group had the positive correlation with IFN-γ expression level of PBMC culture supernatant fluid and colonic mucosal IFN-γ expression level(r=0.45,0.94,P<0.01),and had the negative correlation with IL-10 expression level(r=-0.52,-0.79,P<0.01).Conclusion The unbalance of IFN-γ and IL-10 level could be involved in the pathogenesis of diarrhea PI-IBS,which can serve as the observation indicators of disease activity.
9.Effect of denatonium benzoate on airway inflammation and remodeling in asthmatic mice

Objective To evaluate the effect of denatonium benzoaten on α-smooth muscle actin (α-SMA),subepithelial collagen and airway inflammation in asthmu mice.Methods Forty-five BALB/c mice were divided into 3 groups,normal control group (A group),asthma model group (B group),asthma model+ denatonium benzoaten group(C group);α-SMA detected by using immunohistochemistry,lung sections were stained with Masson to detect subepithelial collagen,HE stain method was used to observe the airway inflammation the images were analyzed with semi-quantitative computer.Results The deposition of α-SMA、subepithelial collagen and inflammation degree in C group was significantly reduced compared with B group,the difference were statistically significant(P<0.05).Conclusion Denatonium benzoaten can improve airway remodeling in asthmatic mice.
10.Investigation of the Habits and Understanding Situation and Demand of Knowledge of Drug Use among Pa-tients in Our Hospital

OBJECTIVE:To provide reference for the development and improvement of pharmaceutical care in our hospital. METHODS:By questionnaire investigation,habits and understarding situation and demand of knowledge of drug use among pa-tients in our hospital were randomly collected by face-to-face interview or network platform. A statistical analysis was carried out on obtained results. RESULTS:In this questionnaire investigation,289 questionnaires were sent out face-to-face,and 282 valid ques-tionnaires were returned with effective feedback rate of 97.58%. Totally 51 valid questionnaires were returned through network plat-form. A total of 333 valid questionnaires were returned through two ways. The results of questionnaire investigation showed that 44.14% of the surveyed patients would read the drug instructions before taking drugs;41.14% of the surveyed patients would pay more attention to ADR and cautions stated in drug instructions. More than 60% of the surveyed patients had various bad habits of drug use. When getting better,66.07% of the surveyed patients would stop using drugs or reduce the dosage. When having not im-proved,26.73% of them would change drugs or increase dosage.Consulting with the medical staff was the most common source(69.07%)of drug use knowledge,and it was also the most trusted source(84.08%). Among the knowledge of drug use, surveyed patients most expected to understand theADR and side effects(65.46%),and the selectionindication and main us-er,cautions,drug interaction in multiple useandusage and dosagewere also selected frequently,and the choices of some options were significantly affected by the characteristics of people,such as age and education degree(P<0.05). The most expected way to acquire drug use knowledge wasface-to-face consultation with medical staff (72.97%),followed bytelephone consulta-tionandnetwork consultation,and the choices of some options were significantly affected by the characteristics of people,such as age and education (P<0.05). CONCLUSIONS:Patients in our hospital haven't pay enough attention to drug instructions and their content,and bad habits of drug use still exist. Sources of drug use knowledge are diverse,but confidence of other sources is not enough except for source of medical staff. The demands for the knowledge of drug use are different,however,the present form and content of pharmaceutical care in our hospital cannot fully meet the needs of patients.
