Journal of Preventive Medicine 2025;37(2):163-167,172
Objective:
To explore the value of body measurement indicators in screening for metabolic syndrome (MS), so as to provide the basis for early detection and prevention of MS.
Methods:
The individuals who were 18 years old or above and underwent physical examinations at a tertiary hospital of Xinjiang Uygur Autonomous Region were selected using the convenience sampling method. Demographic information, lifestyle, waist circumference (WC) and blood biochemical indicators were collected through questionnaire surveys, physical examinations and laboratory tests. The associations of WC, waist-to-hip ratio (WHR), waist-to-height ratio (WHtR), body roundness index (BRI), abdominal volume index (AVI), lipid accumulation product (LAP) and visceral adiposity index (VAI) with MS were analyzed using a multivariable logistic regression model. The screening values of these body measurement indicators for MS were assessed by receiver operating characteristic (ROC) curves.
Results:
A total of 37 921 individuals were recruited, including 20 666 males (54.50%) and 17 255 females (45.50%), and had a median age of 41.00 (interquartile range, 20.00) years. There were 7 988 cases of MS, with a detection rate of 21.06%. Multivariable logistic regression analysis showed that WC, WHR, LAP and VAI were statistically associated with MS (all P<0.05), and the risk of MS increased with the levels of these indicators (all Ptrend<0.05), after adjusting for confounding factors (gender, age, lifestyle, etc.). The area under the ROC curve (AUC) of LAP for screening MS was the highest among single indicators, at 0.930 (95%CI: 0.927-0.933), and the optimal cut-off value was 43.13, Youden's index was 0.720, the sensitivity was 88.01%, and the specificity was 84.04%. The combined indicators of WC×VAI for screening MS had the highest AUC, which was 0.937 (95%CI: 0.935-0.939), and the optimal cut-off value was 0.17, Youden's index was 0.730, the sensitivity was 91.88%, and the specificity was 81.07%. The DeLong test showed that the AUC of WC×VAI for screening MS was higher than that of WC, WHR, LAP and VAI, respectively (all P<0.05).
Conclusion
The value of combining WC and VAI for screening MS is higher than other body measurement indicators.

BackgroundSleep disorder can adversely affect human physical and mental health, with important implications for socioeconomic development. And higher vocational college students are subject to various pressures and stimuli exerted by academic studies, job seeking and family socioeconomic status, which may contribute to the onset insomnia symptom or sleep disorder. ObjectiveTo discuss the sleep status and influencing factors among higher vocational college students, and to provide references for improving their sleep status. MethodsIn January and February, 2022, a stratified random sampling method was utilized to enroll 3 300 students from five higher vocational colleges in Wenjiang district of Chengdu city. All the participants were assessed using Insomnia Severity Index (ISI), Patient Health Questionnaire Depression Scale-9 item (PHQ-9) and Generalized Anxiety Disorder Scale-7 (GAD-7). Pearson correlation analysis was conducted to test the correlation of ISI with PHQ-9 and GAD-7 scores, and the Logistic regression analysis was performed to identify the influencing factors of insomnia symptom in higher vocational college students. ResultInsomnia symptom was detected in 81.90% (2 497 / 3 300) of higher vocational college students. The detection rate of insomnia symptom among vocational college students yielded statistical difference in terms of gender, family socioeconomic status, being an only child or having siblings, degree of psychological pressure exerted by academic studies or job seeking, amount of hours spent on Internet per day, frequency of physical exercise, and the presence or absence of anxiety symptom and depression symptom (χ2=21.032, 22.172, 8.983, 75.939, 36.781, 32.350, 54.512, 86.561, P<0.01 or 0.05). Among higher vocational college students, ISI score was positively correlated with GAD-7 and PHQ-9 scores (r=0.620, 0.714, P<0.01), and GAD-7 score was positively correlated with PHQ-9 score (r=0.824, P<0.01). Gender, family socioeconomic status, degree of psychological pressure exerted by academic studies or job seeking, amount of hours spent on Internet per day, frequency of physical exercise, and the presence or absence of anxiety symptom and depression symptom were effective factors in predicting the insomnia symptom in higher vocational college students (P<0.01 or 0.05). ConclusionFemale gender, average and below average family socioeconomic status, moderate-to-severe degree of psychological pressure exerted by academic studies or job seeking, spending 2~5 h, 5~7 h or >7 h on Internet per day, inadequate physical exercise and presence of depressive symptom are the risk factors for insomnia symptom in higher vocational college students.[Funded by Sichuan Provincial Primary Health Development Research Center in 2022, North Sichuan Medical College (number, SWFZ22-C-89)]

Objective:To observe the role of Huaiqihuang Granules (HQ) in the long-term management of bronchial asthma in young children, and the effective effect on concomitant rhinitis. Methods:A prospective real-world multicenter study was conducted in children aged 2-5 years with asthma diagnosed in the outpatient department (from April 2016 to March 2019)who received either inhaled corticosteroid (ICS)/leukotriene receptor antagonist (LTRA)(control group); inhaled ICS/LTRA plus HQ(combination group), or HQ alone(HQ group). All patients were followed up at week 4, 8, 12 after treatment. The number of days with asthma symptoms, the frequency of severe asthma attacks, the level of asthma control, and the days with rhinitis symptoms in the last 4 weeks were recorded. Differences before and after treatment, and those among groups after treatment were compared using Kruskal- Wallis H test or Wilcoxon rank-sum test. Results:A total of 2 234 eligible patients were recruited, and 2 147 cases completed followed-up visits, including 477, 1 374 and 296 cases in the control group, combination group, and HQ group, respectively. After the treatment, all 3 groups showed significant declines in the days with asthma symptoms, frequency of severe asthma attack and the days with rhinitis symptoms (all P<0.01), and the rate of well-controlled asthma increased significantly ( P<0.01). It lasted until the end of follow-up. Among groups, patients in the combination group showed significantly less days of asthma symptoms than those of the other 2 group at week 8 and 12[0(0, 0.9) d vs.0(0, 0.3) d, P<0.05; 0(0, 0.1) d vs. 0(0, 1.0) d, P<0.01]. Patients in the combination group and HQ group showed a significantly lower rate of severe asthma attacks than that of the control group at week 12 [0(0, 1), 0(0, 1), 0(0, 2), all P<0.05]. The well-controlled rate of asthma in the combination group was significantly higher than that of the control group and HQ group at week 8 and 12 (89.6% vs. 85.9% vs.82.1%, H=15.28; 90.9% vs. 84.1% vs. 81.8%, χ2=29.32, all P<0.01). Conclusions:HQ can significantly alleviate symptoms of asthma and rhinitis, severe attack of asthma, and increase the control rate of asthma when used as an additional treatment or used alone.

Chinese Journal of Ocular Fundus Diseases 2023;39(9):735-740
Objective:To investigate the clinical settings, antibiotic susceptibilities, management and outcomes of streptococcal endophthalmitis. Methods:A retrospective observational case series study. Fifty six eyes of 56 patients diagnosed with streptococcal endophthalmitis in Eye & ENT Hospital, Fudan University from 2012 to 2022 were collected. The treatment followed the general principles of relevant guidelines, including pars plana vitrectomy (PPV), vitreous injection of antibiotics (IVI), vitreous injection of glucocorticoids and systemic application of antibiotics. The follow-up time was (11.9±17.0) months. Patients' clinical characteristics, pathogenic distribution and antibiotic susceptibilities, treatment and outcomes in their medical records were retrospectively collected and analyzed. Results:All 56 patients had monocular onset, including 39 (69.6%, 39/56) males and 17 (30.4%, 17/56) females, 26 (46.4%, 26/56) with left eyes involved and 30 (53.6%, 30/56) with right eyes involved. Their average age was (25.0±24.4) years. Ocular trauma was the leading cause of streptococcal endophthalmitis (73.2%, 41/56), followed by ophthalmic surgery (23.2%, 13/56) and endogenous infection (3.6%, 2/56). The streptococcal species included Streptococcus viridans (50.0%, 28/56), Streptococcus pneumoniae (18/56, 32.1%) and β-hemolytic Streptococcus (17.9%, 10/56). The susceptibility rates of Streptococcus to penicillin, cefatriaxone, vancomycin and levofloxacin were 66.0%, 57.1%, 94.1% and 92.4%, respectively. Patients received PPV+IVI and IVI as initial treatment were 49 eyes (87.5%, 49/56) and 7 eyes (12.5%, 7/56), respectively. Vitreous injection of glucocorticoids were performed in 17 eyes (30.4%, 17/56); and systemic antibiotics were applied in 52 cases (92.9%, 52/56). At the final follow-up, 47 eyes were recorded with visual acuity. Twenty (35.7%, 20/56) had best corrected visual acuity (BCVA)≥0.05 and 27 (48.2%, 27/56) had BCVA <0.05, of which 1 (1.8%, 1/56) had an eyeball enucleation. The etiology of endophthalmitis, streptococcal species, initial treatment with PPV, vitreous injection of glucocorticoids, and systemic antibiotics did not significantly affect patients' visual outcomes ( P>0.05). Timely visit to the hospital after the onset of symptoms (≤3 days) was significantly associated with achieving a final BCVA above 0.05 ( P=0.025). Conclusions:Ocular trauma was the primary cause of streptococcal endophthalmitis. Streptococcus viridans is the most common pathogenic bacterium. Streptococci had high susceptibility to vancomycin, but patients' visual outcomes were poor.

Objective:To evaluate the effect of imatinib on growth impairment in children with chronic myeloid leukemia (CML-CP) in the chronic phase. Methods:From July 2018 to July 2019, questionnaires were distributed to CML children aged <18 years at the time of diagnosis who were receiving imatinib for at least 3 months or to their parents in China. The height-for-age standard deviation score (HtSDS) and the difference of standard deviation integral (△HtSDS) were used to explore the change in height with imatinib therapy. Results:The data of 238 respondents were included; 138 (58.0% ) respondents were men. The median age at the first diagnosis of CML was 11.0 years (range, 1.4-17.9 years) , and 93 (39.0% ) respondents were at the prepuberty stage. At the time of completing the questionnaires, the median age was 15.0 years (range, 2.0-34.0 years) . The median duration of imatinib therapy was 28 months (range, 3-213 months) . Among all the respondents, the mean HtSDS when completing the questionnaires (-0.063±1.361) was significantly lower than that at the time of starting imatinib treatment (0.391±1.244) ( P<0.001) . Total 71.0% respondents showed growth impairment that was more common in those starting imatinib therapy at prepubertal age than in those starting at pubertal age. Multivariate analysis showed that younger at the start of imatinib therapy ( P<0.001) and longer duration of imatinib therapy ( P<0.001) were significantly associated with severe growth impairment on imatinib therapy. Conclusions:Imatinib induced growth impairment in children with CML-CP. Younger the age of initiation and longer the duration of imatinib therapy, more obvious the effect of imatinib on growth impairment.
