1.Clinical Advantages of Traditional Chinese Medicine in Treatment of Pediatric Diseases

The traditional Chinese medicine (TCM) in pediatric care has a long history,proven efficacy,and distinctive characteristics.The China Association of Chinese Medicine has organized a series of youth salons to discuss the clinical advantages of treating diseases.Experts at this seminar proposed that the superior disease categories in pediatric TCM are significant for showcasing the unique strengths and advantages of TCM in the treatment of pediatric diseases,enhancing diagnostic and treatment levels,inheriting TCM knowledge,promoting the integration of TCM and Western medicine,and meeting patients' needs.The strengths of pediatric TCM are summed up as "having what others do not,excelling at what others have,being special in what others excel at,and ensuring safety in what others specialize in." The scope of superiority in pediatric TCM covers multiple systems,including respiratory,digestive,endocrine,psychological,and dermatological systems.This article summarized the advantages of TCM in treating 13 diseases discussed in the salon,such as upper respiratory tract infections,coughs,pneumonia,allergic rhinitis,bronchial asthma,atopic dermatitis,functional dyspepsia,functional constipation,enuresis,marginal short stature,simple obesity,attention deficit hyperactivity disorder,and tic disorders.The overall advantages were reflected in three aspects:First,the holistic concept and treatment based on syndrome differentiation in TCM highlight the advantage of treating the root causes of diseases,making the treatment methodical and precise.Second,most traditional Chinese herbs are natural and have strong safety profiles.Third,TCM treatment methods are abundant,especially the external treatment methods,which are widely used in pediatrics,highly accepted by parents,and have definite efficacy,as well as good safety and compliance.
2.Application of Shear Wave Elastography in the Kidneys of Children with Henoch - Schonlein Purpura

Objective To explore the value of shear wave elastography(SWE)in the kidneys of children with Henoch-Schonlein purpura(HSP).Methods A total of 59 cases with HSP admitted to the Second Affiliated Hospital of Wenzhou Medical University from November 2021 to August 2022 were selected and divided into simple HSP group(29 cases)and Henoch-Schonlein purpura nephritis(HSPN)group(30 cases).50 normal children during the same period were included as control group.The Young′s modulus(YM)of right renal cortex and medulla were compared among the three groups.receiver operating characteristic curve(ROC curve)was plotted to evaluate the diagnostic efficiency of SWE for simple HSP.Correlation analysis and multiple linear regression analysis were performed for the YM value of right renal cortex and medulla of all children with HSP and related indexes.Results There was statistical significance in YM values of right renal cortex and medulla among the three groups(P<0.05).The YM value of right renal cortex of the simple HSP group was higher than that in the control group and HSPN group(P<0.05).The YM value of right renal medulla of the simple HSP group were higher than that in the control group(P<0.05).The YM value of right renal cortex was better than the YM value of right renal medulla in the diagnostic efficiency of simple HSP.The YM value of right renal cortex and medulla both had a linear regression relation-ship with the right renal volume.Conclusion The YM value of right renal cortex and medulla were higher than those of healthy children,with the progression of the disease the YM values decreasing.It may become a new technology for routine screening and follow-up moni-toring of renal involvement in children with HSP.
3.Reliability and validity of the Chinese version of Intermittent Catheterization Difficulty Questionnaire

Objective:To translate the Intermittent Catheterization Difficulty Questionnaire (ICDQ) into Chinese, and to test its reliability and validity in neurogenic bladder patients.Methods:After translation, back-translation, cross-cultural debugging, expert consultation and pre-investigation, a Chinese version of ICDQ was formed. A total of 248 patients with neurogenic bladder clean intermittent self-catheterization who were treated and followed up in the First Hospital of Shanxi Medical University in October 2022 were selected as research objects by the convenient sampling method, and the Chinese version of ICDQ was used for investigation. The critical ratio method was used for project analysis. Content validity and structure validity were used to test the validity of the questionnaire. Cronbach's α coefficient, split half reliability coefficient and retest reliability coefficient were used to test the reliability of the questionnaire. A total of 248 questionnaires were sent out in this study, and 238 were effectively collected, with a recovery rate of 95.97% (238/248) .Results:The Cronbach's α coefficient of the Chinese version of ICDQ was 0.857, the split half coefficient was 0.711, and the retest reliability coefficient was 0.954. The content validity index of the Chinese version of ICDQ item level was 0.860 to 1.000, and the content validity index of the scale level was 0.930. Seven common factors were extracted by exploratory factor analysis, and the cumulative variance contribution rate was 77.38%. The results of confirmatory factor analysis showed RMSEA < 0.080, GFI, AGFI, TLI, CFI > 0.800. Conclusions:Through confirmatory factor analysis, the Chinese version of ICDQ shows that the model fitting indicators of the scale meet the corresponding requirements, indicating that the scale has high structural validity and overall model fitting. It can be used as an evaluation tool for intermittent self catheterization difficulties in patients with neurogenic bladder.

Objective:To analyze the risk factors of bronchopulmonary dysplasia(BPD)in very preterm infants(VPI), and to provide scientific basis for the prevention and treatment of BPD in VPI.Methods:A prospective multicenter study was designed to collect the clinical data of VPI in department of neonatology of 28 hospitals in 7 regions from September 2019 to December 2020.According to the continuous oxygen dependence at 28 days after birth, VPI were divided into non BPD group and BPD group, and the risk factors of BPD in VPI were analyzed.Results:A total of 2 514 cases of VPI including 1 364 cases without BPD and 1 150 cases with BPD were enrolled.The incidence of BPD was 45.7%.The smaller the gestational age and weight, the higher the incidence of BPD( P<0.001). Compared with non BPD group, the average birth age, weight and cesarean section rate in BPD group were lower, and the incidence of male infants, small for gestational age and 5-minute apgar score≤7 were higher( P<0.01). In BPD group, the incidences of neonatal respiratory distress syndrome(NRDS), hemodynamically significant patent ductus arteriosus, retinopathy of prematurity, feeding intolerance, extrauterine growth restriction, grade Ⅲ~Ⅳ intracranial hemorrhage, anemia, early-onset and late-onset sepsis, nosocomial infection, parenteral nutrition-associated cholestasis were higher( P<0.05), the use of pulmonary surfactant(PS), postnatal hormone exposure, anemia and blood transfusion were also higher, and the time of invasive and non-invasive mechanical ventilation, oxygen use and total hospital stay were longer( P<0.001). The time of starting enteral nutrition, cumulative fasting days, days of reaching total enteral nutrition, days of continuous parenteral nutrition, days of reaching 110 kcal/(kg·d) total calorie, days of reaching 110 kcal/(kg·d) oral calorie were longer and the breastfeeding rate was lower in BPD group than those in non BPD group( P<0.001). The cumulative doses of amino acid and fat emulsion during the first week of hospitalization were higher in BPD group( P<0.001). Multivariate Logistic regression analysis showed that NRDS, invasive mechanical ventilation, age of reaching total enteral nutrition, anemia and blood transfusion were the independent risk factors for BPD in VPI, and older gestational age was the protective factor for BPD. Conclusion:Strengthening perinatal management, avoiding premature delivery and severe NRDS, shortening the time of invasive mechanical ventilation, paying attention to enteral nutrition management, reaching whole intestinal feeding as soon as possible, and strictly mastering the indications of blood transfusion are very important to reduce the incidence of BPD in VPI.
5.Role of long non-coding RNA 068 in the migration of melanoma cells

Objective:To investigate the effect of long non-coding RNA 068 (lncRNA 068) on the migration of a melanoma cell line A375, and to explore its mechanism of action.Methods:From December 2015 to November 2020, 21 patients with pathologically confirmed cutaneous melanoma were collected from Department of Dermatology, Affiliated Hospital of Nantong University, and quantitative PCR (qPCR) was performed to determine the expression of lncRNA 068 in melanoma and paracancerous tissues. LncRNA 068 was overexpressed or knocked down via lentiviral transfection in A375 human melanoma cells in the following experiments. Specifically, A375 cells were divided into lentiviral vector (LV) -green fluorescent protein (GFP) group and LV-lncRNA 068 group to be transfected with a GFP-expressing LV and a LV containing lncRNA 068 respectively in the overexpression experiment, and were divided into LV-LacZ short hairpin RNA (shRNA) group and LV-lncRNA 068 shRNA group to be transfected with a LV containing the reporter gene LacZ-specific shRNA and a LV containing the lncRNA 068-targeting shRNA respectively in the low-expression experiment, with the LV-GFP group and LV-LacZ shRNA group serving as the control groups. Transwell and scratch assays were performed to evaluate cell migration, EdU cell proliferation assay and cell counting kit-8 (CCK8) assay to determine the proportion of proliferative cells and cell viability respectively, and immunofluorescence staining was conducted to evaluate epithelial-mesenchymal transformation in the above groups. Lentivirus-transfected A375 cells from the above groups were inoculated into the axillae of BALB/c nude mice, and tumor volume was measured and calculated every 3 days. After 30 days, all mice were sacrificed, and tumor tissues were resected to measure the tumor volume and weight. Cultured B16F10 cells were subcutaneously inoculated into the back and foot of BALB/c nude mice to construct mouse models of subcutaneously transplanted B16F10 melanoma. After 2 weeks, the mice were sacrificed, and qPCR and Western blot analysis were performed to determine the mRNA expression of inflammatory factors in transplanted B16F10 melanoma and paracancerous tissues, and expression of IκB kinase (IKK) /P65 signaling pathway-related proteins, respectively. Comparisons between 2 groups were done by t test, and comparisons of tumor volume and weight at different time points among groups were done by repeated measures analysis of variance. Results:qPCR showed that the relative expression of lncRNA 068 was significantly lower in human melanoma tissues and transplanted B16F10 murine melanoma tissues (0.414 ± 0.109, 0.717 ± 0.041, respectively) than in the corresponding paracancerous tissues (1.050 ± 0.103, 1.011 ± 0.023, t = 19.48, 10.83, respectively, both P < 0.001). Transwell and scratch assays both showed that the cellular migratory ability was significantly lower in the LV-lncRNA 068 group than in the LV-GFP group (both P < 0.01), and significantly higher in the LV-lncRNA 068 shRNA group than in the LV-LacZ shRNA group (both P < 0.05). Immunofluorescence assay showed significantly increased fluorescence intensity of E-cadherin and decreased fluorescence intensity of N-cadherin in the LV-lncRNA 068 group compared with the LV-GFP group (both P < 0.001), but significantly decreased fluorescence intensity of E-cadherin and increased fluorescence intensity of N-cadherin in the LV-lncRNA 068 shRNA group compared with the LV-LacZ shRNA group (both P < 0.05). In vivo tumor formation experiment in nude mice showed that there were no significant differences in the volume or weight of melanoma between the LV-lncRNA 068 group and LV-GFP group (both P > 0.05), as well as between the LV-lncRNA 068 shRNA group and LV-LacZ shRNA group (both P > 0.05). As qPCR and Western blot analysis showed, the mRNA and protein expression of interleukin-10 (IL-10) and claudin-1 in A375 cells were significantly higher in the LV-lncRNA 068 group than in the LV-GFP group (both P < 0.05), but significantly lower in the LV-lncRNA 068 shRNA group than in the LV-LacZ shRNA group (both P < 0.05). Compared with the paracancerous tissues, B16F10 melanoma tissues showed significantly decreased mRNA expression of IL-10 ( P < 0.01), but significantly increased mRNA expression of IL-6 and tumor necrosis factor-α, as well as protein expression of phosphorylated P65 and phosphorylated IKK ( P < 0.01) . Conclusion:Overexpression of lncRNA 068 can inhibit the migration of A375 melanoma cells, and may affect the development of inflammation and inhibit the epithelial-mesenchymal transformation by inhibiting the IKK/P65 signaling pathway.
6.Effects of early essential newborn care after vaginal delivery on breastfeeding: a prospective randomized controlled trial

Objective:To explore the influence of early essential newborn care (EENC) on breastfeeding after vaginal delivery.Methods:Women with full-term vaginal delivery at Jiaxing Women and Children's Hospital Affiliated to Wenzhou Medical University from January to June 2019 were enrolled in this prospective randomized controlled trial. These participants and their neonates were assigned randomly to receive early skin-to-skin contact for 90 min after delayed umbilical clamping (EENC group) or routine nursing care after delivery and skin-to-skin contact for 30 min (control group). The main outcomes were successful initiation and duration of first-time breastfeeding, initiation time of second-stage lactation, self-efficacy score of breastfeeding, acceptance and satisfaction score of breastfeeding, and the rate of exclusive breastfeeding at 24, 48, 72 h, and 42 d postpartum. The secondary outcomes included the onset time of first deep sleep of the newborn, the baby's first breast-fed stool, and the neonatal weight within 4 days of birth. Two independent samples t-test and Chi-square test were used for statistical analysis. Results:A total of 200 women with full-term vaginal delivery were recruited and 117 (61 in EENC group and 56 in control group) completed the study. (1)Main outcomes: Compared with the control group, the EENC group had earlier initiation of the first breastfeeding [(25.5±6.9) vs (33.2±7.2) min, t=5.902] and the second stage of lactation [(72.1±3.3) vs (78.6±4.3) h, t=9.504], longer duration of the first breastfeeding [(40.6±5.2) vs (32.6±6.1) min, t=7.806], and higher self-efficacy score [(122.5±2.1) vs (98.2±3.1) scores, t=50.660], acceptance score [(21.8±3.1) vs (15.3±4.3) scores, t=9.444], satisfaction score of breastfeeding [(23.2±2.1) vs (18.8±3.1) scores, t=9.278], and higher rate of exclusive breastfeeding at 24, 48 and 72 h after delivery[43% (26/61) vs 21% (12/56), 59%(36/61) vs 36%(20/56), 89%(54/61) vs 64%(36/56); χ 2 were 5.980, 6.353, and 9.663, respectively], all P<0.05. (2)Secondary outcomes: In the EENC group, the onset time of first deep sleep [(90.1±10.6) vs (118.3±9.9) min, t=7.645] and the the baby's first breast-fed stool [(57.1±6.4) vs (66.4±5.5) h, t=8.435] were earlier; neonatal weight at 3 d [(3 720±329) vs (3 558±412) g, t=2.382] and 4 d after birth [(3 778±289) vs (3 562±315) g, t=3.857] were significantly heavier, comparing with the control group (all P<0.01). Conclusions:EENC can improve multiple short-term breastfeeding indicators and maternal acceptance and satisfaction of breastfeeding in women after full-term vaginal delivery.
7.A multicenter survey of short-term respiratory morbidity in late-preterm infants in Beijing

Objective:To study the respiratory morbidity and the risk factors of respiratory complications in late-preterm infants.Methods:The data of 959 late-preterm infants in 21 hospitals in Beijing from October 2015 to April 2016 were collected.These infants were divided into the respiratory morbidity group (237 cases) and the control group (722 cases) according to whether they had short-term respiratory morbidity after birth.Clinical data of the two groups were compared.Results:Among the 959 late-preterm babies, 530 were male and 429 were female.Two hundred and thirty-seven cases (24.7%) developed short-term respiratory morbidity after birth.Infectious pneumonia developed in the most cases (81 cases, 8.4%), followed by transient tachypnea (65 cases, 6.8%), amniotic fluid aspiration (51 cases, 5.3%), and respiratory distress syndrome (24 cases, 2.5%) successively.All the infants recovered and discharged.There were no differences between gender and maternal age between 2 groups (all P>0.05). Compared with the control group, more late-preterm infants were delivered by cesarean section (73.4% vs.59.7%, χ2=14.43, P<0.001) and the 1-minute Apgar score was lower [(9.41±1.66) scores vs.(9.83±0.53) scores, t=5.40, P<0.001] in the respiratory morbidity group.The differences were statistically significant.There were more cases with maternal complications in the respiratory morbidity group that in the control group (66.7% vs.58.6%, χ2=4.877, P=0.027), but no difference in various complications between 2 groups was observed ( P>0.05). In the respiratory morbidity group, the most frequent complications were maternal hypertension and preeclampsia (27.8% vs.22.6%, χ2=2.728, P=0.099). There were no differences between 2 groups in gestational age, birth weight and birth length (all P>0.05). There were more infants small for gestational age and large for gestational age in the respiratory morbidity group than in the control group (18.8% vs.14.1%, 6.3% vs.2.4%, χ2=8.960, P=0.011). The duration of hospitalization of the respiratory morbidity group was significantly longer than that of the control group [(9.00±4.42) d vs.(6.82±4.19) d, t=6.676, P<0.001] since the infants with respiratory morbidity needed to be hospita-lized. Conclusions:Respiratory diseases occur in about 1/4 of late-preterm infants.Infants who are delivered by cesarean section and whose mothers are complicated with the maternal hypertension and preeclampsia should be monitored closely.Respiratory support should be provided for infants not appropriate for gestational age who are more likely to suffer from respiratory diseases, so that they can successfully pass through the transition period.
8.Identification of Chemical Constituents in Fengliaoxing Fengshi Dieda Wine

OBJECTIVE:To identify t he chemical constituents of Fengliaoxing fengshi dieda wine. METHODS :An ultra-high performance liquid chromatography-quadrupole-electrostatic field orbital trap high-resolution mass spectrometry (UPLC-Q-Exactive- MS)technique was used for identifying chemical constituents of Fengliaoxing fengshi dieda wine. The determination was performed on Thermo Accucore aQ RP 18 column with mobile phase consisted of 0.1% formic acid-methanol (gradient elution )at the flow rate of 0.3 mL/min. The column temperature was 30 ℃,and the sample size was 2 μL. HESI source was adopted,ion mode was positive and negative ion mode ,nitrogen as sheath gas and auxiliary gas. The positive ion mode had spray voltage of 3.5 kV, capillary heating temperature of 350 ℃,sheath gas pressure of 35 psi,auxiliary gas pressure of 15 arb,and ion source heating temperature of 320 ℃. The negative ion mode had spray voltage of 3.2 kV,capillary heating temperature of 350 ℃, sheath gas pressure of 35 psi,auxiliary gas pressure of 15 arb,and ion source heating temperature of 300 ℃. The mass axis was calibrated by external standard method (mass error less than 5 ppm). The scanning range of the first mass spectrometry was m/z 80.0-1 200.0 (the resolution was 70 000),the scanning range of secondary mass spectrometry was m/z 80.0-1 200.0(the resolution was 17 500),and the collision voltage was 20,40,60 eV. Retrieved from CNKI ,VIP,PubMed and other database ,the chemical constituents information of each Chinese traditional medicine in Fengliaoxing fengshi dieda wine were collected to establish chemical constituents database. The structure of the compounds was identified on the basis of above constituents database ,the relevant literature ,retention time of reference substance and MS fragmentation regularity. RESULTS & CONCLUSIONS : Fifty-nine compounds were identified in Fengliaoxing fengshi dieda wine ,including 12 flavones(e.g. neoeriocitrin ,hesperidin.),8 alkaloids (e.g. baogongteng C or erycibellin , ephedrine,pseudoephedrine), 9 organic acids (e.g. chlorogenic acid , cryptochlorogenic acid ,neochlorogenic acid ),7 coumarins(e.g. xanthotoxol), 4 esters, 4 amino acids , and 15 other categories(including volatile oils ,terpenes,amides). UPLC- 5103YX11SF37(17)] Q-Exactive-MS technology can quickly and accurately identify the chemical constituents in Fengliaoxing fengshi dieda wine.
9.Effect of benzbromarone on body fat in patients with gout

Objective:To investigate the effect of urate-lowering therapy (ULT) on body fat and visceral fat areas in patients with gout and to analyze the related risk factors.Methods:A total of 140 patients with gout eligible for enrollment were recruited from the gout clinic in the Affiliated Hospital of Qingdao University from Sept. 2018 to Sept. 2019. After 2 weeks of washout, all patients were treated with benzbromarone for consecutive 12 weeks. The data of blood biochemical, body fat (BF), body fat percentage (BFP) and visceral fat area (VFA) were collected, and the differences before and after ULT as well as the risk factors affecting the changes of BF, BFP, and VFA were analyzed.Results:The diastolic blood pressure (DBP), alanine aminotransferase (ALT), serum uric acid (sUA), BF, BFP, and VFA were significantly decreased after ULT ( P<0.05). The results of multivariate stepwise linear regression analysis showed that sUA, creatinine (Cr) and glomerular filtration rate (eGFR) were independent risk factors affecting the improvement of BF and VFA (P<0.05). Lower serum uric acid levels in patients with gout before and after ULT indicated better improvement effect of body fat and visceral fat areas( P<0.05). Conclusion:Gout combined with hyperuricemia is closely related to visceral obesity. In addition to lowering the serum uric acid level, ULT could also benefit the body fat and visceral fat area in patients with gout to some extent.

Objective:To evaluate the clinical efficacy of human-like collagen scar repair gel in the treatment of proliferative hypertrophic scar.Methods:The clinical effect of smear-like silicone ointment on patients with hypertrophic scars after prolonged injury or 4-6 months after surgery, the difference of curative effects between 3-18 years old juvenile group and 19-45 years old adult group, were analyzed by multicenter randomized controlled clinical study. Among them, 58 cases were treated with human-like collagen scar repair gel, 39 cases in adult group and 19 cases in minor group; 30 cases were using Silicone Gel, 20 cases in adult group and 10 cases in minor group. The differences in scar color, thickness, blood vessel distribution, softness, patient's self-conscious symptoms, clinical efficacy scores and adverse reactions were analyzed before and after application of the products at 4, 8, 12 and 24 weeks.Results:The scores of the two groups before treatment were slightly higher than those of the control group except the softness and thickness score test group ( P<0.05). There was no significant difference in the remaining parameters ( P>0.05). The efficacy scores at the 4th, 8th, 12th and the 24th week after application of the two groups showed that there were significant differences in the therapeutic indicators after treatment for different time ( P<0.05) and age grouping. There was no significant difference between juvenile group and adult group ( P>0.05). There was a statistically significant difference in the time points of different clinical evaluation scores ( P<0.05). There was no interaction between the evaluation time and the experimental control group and age group ( P>0.05); and not affected by group or age grouping ( P>0.05), no adverse reactions occurred in either group. Conclusions:Human-like collagen scar repair gel is safe and effective in the treatment of proliferative hypertrophic scar, and the clinical efficacy is not inferior to Silicone gel.
