1.Modified calcaneal plate combined with suture anchors in treatment of comminuted fracture of split-type greater tuberosity of humerus(Liu-Gang type IV)

BACKGROUND:Numerous scholars have previously researched certain greater tuberosity fractures and the procedures used to treat them.Few researchers,however,have studied the comminuted split fracture of the greater tuberosity of the humerus(Liu-Gang type IV)with rotator cuff tear in great detail. OBJECTIVE:To compare the clinical therapeutic effect of open repair position modified calcaneal plate combined with suture anchors and proximal humeral internal locking system(PHILOS)plate in the treatment of comminuted fracture of split-type greater tuberosity of humerus combined with rotator cuff tears(Liu-Gang type IV). METHODS:Case data of 30 patients with comminuted fracture of split-type greater tuberosity of humerus combined with rotator cuff tears(Liu-Gang type IV)from May 2012 to May 2022 were retrospectively analyzed.They were divided into the modified calcaneal plate combined with suture anchor group(group A)and the PHILOS with#2 Johnson group(group B),with 15 cases in each group.Intraoperative blood loss,surgical time,and incision length of all patients were recorded.Pain visual analog scale score,Constant-Murley score,as well as shoulder joint abduction,forward flexion,external rotation,and dorsal expansion activities during the last follow-up(>1 year)were evaluated. RESULTS AND CONCLUSION:(1)The surgical incision length and operation time were shorter,and blood loss was less in group A than those in group B(P<0.05).(2)No significant difference in visual analog scale score and Constant-Murley score was detected between the two groups(P>0.05).(3)During the last follow-up,forward flexion in group A was better than that in group B(P<0.05).No significant difference in abduction,external rotation,and dorsal expansion was determined between group A and group B(P>0.05).(4)In terms of complications,there was 1 case of shoulder joint pain and discomfort in group A(7%),2 cases of subacromial impingement syndrome,2 cases of upward movement of nodules,and 2 cases of shoulder joint pain(40%)in group B.There were significant differences in complication rates between the two groups(P=0.031).(5)In summary,the modified calcaneal plate combined with suture anchors in the treatment of comminuted fracture of split-type greater tuberosity of humerus combined with rotator cuff tears(Liu-Gang type IV)could better restore the forward flexion function of the shoulder joint and has a small incision,less blood loss,shorter operation time and fewer complications.
2.Biological Functions of Selenoprotein Iodothyronine Deiodinase and its Expression in Osteoarthritis

Objective Iodothyronine deiodinases (DIOs) are important selenoproteins that play a key role in the bone and joint diseases. Osteoarthritis (OA) is the most prevalent joint disease especially in elders. This bioinformatic analysis was performed to explore the role of DIOs in OA pathogenesis. Methods The biological functions of selenoprotein DIOs were analyzed by bioinformatic techniques, including GenCLip 3.0, Database for Annotation, Visualization and Integrated Discovery (DAVID), STRING, Cytoscape, and Network Analyst. The expression of DIOs in the healthy individuals and OA patients was determined by mining OA-related microarray data in the gene expression omnibus (GEO) database of National Center for Biotechnology Information and performing a Meta-analysis of the data with Review Manager 5.3. Results Cluster analysis revealed that the function of the DIOs was associated with thyroid hormone receptor and iodothyronine; GO analysis showed that DIOs were mainly involved in biological processes, such as ethanol metabolism and phenol-containing compound metabolism and primarily involved in the cytochrome P450 metabolism of exogenous organisms and thyroid hormone signaling; SULT1A1 was the core node of the PPI network; miRNAs and thyroid hormones had some iterations with DIO1 and DIO2; Meta-analysis showed that DIO3 expression was significantly up-regulated in OA patients (SMD = 0.31, 95%CI: 0.03, 0.59, P = 0.03). Conclusions The main biological functions of DIOs were closely associated with the regulation of thyroid hormone. And the up-regulated expression of DIO3 may have crucial impact on the occurrence of OA.
3.Effects and Mechanism of Qiwei Qinggan Powder on Hepatic Fibrosis Based on Proteomics

OBJECTIVE:To investigate the anti- hepatic fibrosis (HF)effects of Qiwei qinggan powder and explore its possible mechanism. METHODS :Male Wistar rats were randomly divided into blank group ,HF model group ,Qiwei qinggan powder low-dose,medium-dose and high-dose groups [ 135,270,405 mg/(kg·d),by total amount of crude drugs] ,with 12 rats in each group. Except for blank group ,other groups were given 50% CCl4-peanut oil solution intragastrically (2 mL/kg,twice a week ,for consecutive 8 weeks) to induce HF model. At same time , blank group and model group were given constant volume of 0.5% CMC-Na solution intragastrically ;administration groups were given relevant medicine intragastrically ,once a day ,for consecutive 8 weeks. General situation of rats were observedand liver morphology was observed after last administration and hepatic indexes were detected. The contents of liverfunction indexes (ALT,AST,ALP,HYP)in serum and the expression of α-SMA in hepatic tissue were determined , and HE and Masson staining were performed to observe the histopathology. Using the difference multiple of expression quantity as the index ,TMT technology was used to screen the differentially expressed protein in medicine group (combining the liver tissue samples of Qiwei qinggan powder groups )and HF model group. Uniprot-GOA database and KAAS ,KEGG mapper online tools were used to analyze GO and KEGG pathway enrichment. RESULTS :The rats in the blank group were in good health ;the liver was bright red and smooth ,the liver lobules were intact ,no degeneration and necrosis ,inflammatory cell infiltration or fibrous tissue proliferation was found. Compared with blank group ,the rats in HF model group had poor diet ,depressed spirit ,disordered and lusterless fur ;the liver was dark red or yellow with rough surface ,hard texture ,inflammatory cell infiltration ,fiber tissue destruction ,bridge connection and so on ;the hepatic index ,the contents of liver function indexes and the expression of α-SMA were increased significantly (P<0.05). Compared with HF model group ,above symptoms of rats were improved to different extent in different dose groups of Qiwei qinggan powder ;hepatic index in Qiwei qinggan powder low-dose group ,the content of ALP in high-dose group ,the contents of ALT,AST and HYP and the expression of α-SMA in different dose groups were decreased significantly (P<0.05). A total of 42 differentially expressed proteins related to HF were screened ,of which 15 were up-regulated and 27 were down-regulated in expression,including fatty acid binding protein 4(FABP4),cholesterol 7α-hydroxylase(CYP7A1). The results of enrichment analysis showed that the differentially expressed proteins were mainly enriched in extracellular space ,blood particles and other cell parts,involving the molecular functions of oxidoreductase activity and fatty acid binding ,the biological processes of the regulation of heterotypic cell adhesion ,protein activation cascade ,as well as retinol metabolism ,arachidonic acid metabolism ,PPAR and other signal pathway. CONCLUSIONS :Qiwei qinggan powder can reduce the hepatic index ,ALT,AST,ALP and HYP contents in serum ,down-regulate the expression of α-SMA,improve the degree of inflammation and fibrosis of liver tissue ,and have a certain protective effect on rats. The anti-HF mechanism of it involves multiple targets and signal pathways ,such as FABP 4, CYP7A1 and PPAR.
4.Efficacy of minimally invasive pulmonary surfactant administration in preterm infants with neonatal respiratory distress syndrome: a multicenter clinical trial

Objective:To explore the feasibility and safety of minimally invasive surfactant administration (MISA) in preterm neonates with respiratory distress syndrome (NRDS).Methods:In this multicenter prospective randomized controlled trial, 92 preterm infants with gestation age ≤30 weeks and diagnosed with NRDS were enrolled in 8 level Ⅲ neonatal intensive care units (NICU) in Beijing-Tianjin-Hebei Region from 1 st July 2017 to 31 st December 2018. They were randomly assigned to minimally invasive surfactant administration (MISA) group or endotracheal intubation surfactant administration (EISA) group according to random number generated by computer. Infants in both groups received calf pulmonary surfactant preparation at a dose of 70-100 mg/kg. The data of demography, perinatal situation, medication administration, complications, clinical outcomes in the two groups were compared with Chi-square test, Student′s t-test, Mann-Whitney U test or Fisher′s exact test. Results:Among the 92 preterm infants, 53 were males, 39 were females; 47 were in the MISA group (25 males), and 45 were in the EISA group (28 males). The gestational age and birth weight were (29.5±1.2) weeks and (1 271±242) g in all patients, (29.5±1.4) weeks and (1 285±256) g in the MISA group, and (29.6±0.9) weeks and (1 255±227) g in the EISA group. The duration of surfactant infusion and the length of whole procedure in the MISA group were significantly longer than that in the EISA group (60 (18, 270) s vs. 50 (30, 60) s, Z=3.009, P=0.003; 90 (60, 300) s vs. 60 (44, 270) s, Z=3.365, P=0.001). For the outcomes, the incidence of hemodynamically significant patent ductus arteriosus (hsPDA) and bronchopulmonary dysplasia (BPD) were lower in the MISA group than in the EISA group (36% (17/47) vs. 67% (30/45), χ 2=8.556, P=0.003; 26% (12/47) vs. 47% (21/45), χ 2=4.464, P=0.035). Conclusions:Minimally invasive surfactant administration is applicable in preterm infants ≤30 weeks gestational age with NRDS. Although the length of whole procedure is longer than route endotracheal administration, the benefit of decreasing the incidences of hsPDA and BPD outweighs this demerit.
5.Changes of serum cortisol level in ischemic stroke patients with obstructive sleep apnea syndrome

Objective To observe the serum cortisol level in ischemic stroke patients with obstructive sleep apnea syndrome (OSAS),and discuss the influence factors and its correlation with severity of cerebral infarction.Methods Two hundred ischemic stroke patients with onset of 6 h to 3 weeks,admitted to our hospital from July 2015 to April 2017,were recruited;all patients were monitored with polysomnography.According to apnea hypopnea index (AHI),all patients were divided into ischemic stroke without OSAS group (AHI<5/h,n=89) and ischemic stroke with OSAS group (AHI≥ 5/h,n=111).Moreover,according to AHI,patients from ischemic stroke with OSAS group were divided into three subgroups,namely,mild subgroup (5/h ≤AHI<15/h),moderate subgroup (15/h ≤AHI<30/h) and severe subgroup (AHI ≥30/h).According to National Institutes of Health Stroke Scale (NIHSS) scores,all subjects were divided into a group of NIHSS scores no more than 10 and a group of NIHSS scores>10.The general clinical data,biochemical indices,early morning blood pressure,serum cortisol level and sleeping parameters were detected and compared among the groups,and the main factors affecting serum cortisol levels were identified by multivariate linear regression analysis.Results (1) The serum cortisol level in ischemic stroke with OSAS patients ([195.41±75.31] μg/L) was significantly higher than that of ischemic stroke without OSAS patients ([158.65±77.28] μg/L,P<0.05);the serum cortisol level in ischemic stroke with mild OSAS subgroup ([227.32±75.12] μg/L) was significantly increased as compared with that in the ischemic stroke with moderate OSAS subgroup and ischemic stroke with severe OSAS subgroup ([191.27±71.50] μg/L and [175.21±75.13] μg/L,P<0.05).(2) The serum cortisol level of group of NIHSS scores>10 was significantly higher than that of group of NIHSS scores ≤ 10 (P<0.05).(3)AHI,NIHSS scores,longest duration of apnea,and lowest blood oxygen saturation at night had significant effects on serum cortisol levels.Serum cortisol levels increased with AHI (β=89.984,95%CI:71.325-108.644,P=0.000) and NIHSS scores (β=0.923,95%CI:0.377-1.468,P=0.001),increased with the longest sleep apnea (β=0.804,95%CI:0.262-1.325,P=0.000),and decreased with the lowest blood oxygen saturation at night (β=-0.709,95%CI:-0.290--0.041,P=0.000).Conclusion The serum cortisol level in cerebral infarction patients with OSAS was increased,and the higher the severity of cerebral infarction and OSAS is,the higher the serum cortisol level is.
6.Effects of low salt diet on gene expression in dog's heart.

OBJECTIVETo investigate the effect of low-salt diet on gene expression of canine cardiac tissue.
METHODSMicroarray data (GSE17149) of heart tissue from dogs fed with low-salt diet was obtained from the Gene Expression Omnibus (GEO). The data were analyzed by Qlucore Omics Explorer 3.1, STRING 10.0, Genclip 2.0 and GCBI. The protein-protein interactions (PPI) of the differentially expressed genes in low-salt and control groups were conducted to screen the key node genes between the two groups.
RESULTSCompared with the control group, the gene expression profile of the dog heart tissue was changed in the low-salt diet group, and 1343 (3.12%) differentially expressed genes were found in 43 035 genes. The differentially expressed gene protein interaction analysis revealed a most obvious protein (NFKBIA) as the core, which might be related to inhibiting the differentiation of macrophage-derived foam cell and promoting cholesterol discharge and decomposition.
CONCLUSIONSGene expression of heart tissue in dogs fed with low-salt diet is significantly changed and low-salt diet may reduce the risk of cardiovascular disease by up-regulating the expression of NFKBIA.
7.New Way of Cadre Quality Improvement in Food and Drug Supervision System

After the structural reform in the food and drug supervision system, the knowledge structure and the professional re-quirements of the supervisors is unmatched, which hinder the fulfillment of regulatory responsibilities and the performance of adminis-trative efficacy. Effective ways for team building are actively explored throughout the country. The paper introduced the adult corre-spondence education of the supervisors in Zhejiang Ningbo food and drug supervision system to provide a good example for training work throughout the country and some useful suggestions for the quality improvement of the supervision team.
8.Role and related mechanism of S1P/S1P1 signal pathway during post conditioning of hypertrophic cardiomyocytes

Objective To study the role and mechanism of sphingosine-l-phosphate (S1P)/sphingosine-1-phosphate receptor 1 (S1P1) signal pathway during post conditioning of hypertrophic cardiomyocytes.Methods Neonatal rat cardiomyocytes were isolated and cultured,then stimulated by norepinephrine (NE) to induce cardiomyocytes hypertrophy.Using tri-gas incubator to create hypoxia and reoxygenation enviroment to mimic ischemia-reperfusion and postconditioning.Hypertrophic cardiomyoctyes were divided into five groups according to the presence or absence of various drugs and postconditiong and relevant signal pathways changes were detected:(1) IPost group (hypoxia + postconditioning);(2) IPost + S1P group (cells were pretreated with S1P (1 μmol/L) for 2 h before IPost);(3) IPost + W-146 + S1P group (cells in IPost + W-146 + S1P group were pretreated with S1P1 inhibitor W-146 (0.4 μmol/L) for 20 min);(4) IPost + PD98059 + S1P group (cells in IPost + S1P group were pretreated with MAPK antagonist PD98059 (125 μmo[/L) for 20 min);(5) IPost + LY-294002 + S1P group (cells in IPost + S1P group were pretreated with PI3K antagonist LY294002 (0.1 μmol/L) for 20 min).Apoptosis was detected by flow cytometry and protein expression of relevant signal pathways were detected by Western blot.Results (1) Apoptosis rate was significantly increased in hypoxia/reoxygenation (27.90 ± 4.49) % group compared with normal control group (7.97 ± 2.18) %,which could be significantly reduced in IPost group (15.90 ± 1.77) % (all P < 0.05).(2) Apoptosis rate and caspase-3 expression were both significantly lower in IPost + S1P and IPost + S1P + LY-294002 groups than in IPost and IPost + S1P + W-146 and IPost + S1P + PD98059 group (all P < 0.05).(3)p-ERK1/2 expression was significantly higher in IPost + S1P and IPost + S1P + LY-294002 group than in IPost and IPost + SIP + W-146 group and IPost + S1P + PD98059 group (all P < 0.05) while p-Akt expression was similar among IPost,IPost + S1P + W-146 and IPost + S1P +PD98059 groups.p-ERK1/2 and p-Akt levels in IPost + S1P + W-146 group and IPost + S1P + PD98059 were similar as in IPost group.Conclusions S1P can play protective role on NE induced cardiomyocytes hypertrophy during post conditioning through downregulating caspase-3 expression and reducing apoptosis rate via targeting S1P1 and activating ERK1/2 signal pathway.
9.Evaluation of the rapid trehalose test for the identification of the Candida glabrata

Objective:To explore a rapid and cost-effective method for identification of Candida gla-brata through the comparison of two different methods , using molecular methods of sequencing as gold standard.Methods:From our clinic, 200 strains of suspected Candida glabrata were collected during the last 3 years and selected after incubation in CHROMagar Candida medium for 48 h.By comparing the results of the CHROMagar Candida medium, the identification of the rapid trehalose test for different kinds of strains were analyzed under incubation in the tubes for 3 h, 6 h, and 24 h at 37 ℃and 42 ℃, respectively .All the strains were identified to species level by methods of sequencing .The optimal time and temperature of the trehalose test for the identification of Candida glabrata were assessed .Two different methods, CHROMagar Candida medium and the rapid trehalose test , in identification of Candi-da glabrata were compared.Results:In all the 200 strains, Candida glabrata ferment trehalose with 3 h incubation under 42 ℃ were the optimal time and temperature for fermenting trehalose .The accuracy , sensitivity, and specificity of the rapid trehalose test were 99.00% (198/200), 98.66% (147/149) and 100.00%(51/51).The accuracy rate of CHROMagar Candida medium was 79.50%(159/200), the sensitivity and specificity were only 89.93%(134/149) and 49.02%(25/51), however, compared with the domestic current popular methods , the rapid trehalose test had better time efficiency ratio .Con-clusion:The evaluation results suggest that the rapid trehalose test has advantages in terms of operational convenience and low cost , and the results can be obtained in 3 h.Therefore, it has application value in clinical laboratory .
10.Moxibustion:its outlook in the intestinal flora and mucosal immunity for regulation of irritable bowel syndrome

BACKGROUND:The interaction and mutual influence of intestinal flora in the host intestine with mucosal immune barrier play an important role in the homeostasis regulation within the intestine. OBJECTIVE:To explore the role of intestinal flora in the pathogenesis of irritable bowel syndrome and the mechanism of moxibustion in the treatment of irritable bowel syndrome from the aspects of intestinal flora imbalance and mucosal immune activation. METHODS:With the key words of “irritable bowel syndrome, intestinal flora, intestinal immune, moxibustion” in Chinese and in English, respectively, a computer-based search was performed for articles published in CNKI, VIP and PubMed databases from October 2001 to October 2014. After the initial screening, the reserved articles were further detailed, summarized and concluded. RESULTS AND CONCLUSION:Totaly 37 articles were colected, including 15 in Chinese and 22 in English. The analysis results showed that, the imbalance of intestinal flora and mucosal immune can play an important role in the pathogenesis of irritable bowel syndrome, and the former one can affect the body’s feeling of rectal distension threshold and visceral sensitivity. Moxibustion excels at improvement of the symptoms of irritable bowel syndrome, particularly abdominal pain with high visceral sensitivity, by regulating the body’s physiological balance from multi-link and multi-target aspects.
