Objective:To investigate the effect of Mediterranean diet on blood glucose control and cardiovascular risk factors in patients with type 2 diabetes.Methods:As to December 2021, the PubMed, Cochrance Central Register of Controlled Trials and Cochrance Database, Cochranc Library, Embase, China National Knowledge Infrastructure and Wanfang Medical Network system were searched for clinical randomized controlled trials(RCTs) of Mediterranean diet in patients with type 2 diabetes to conduct Meta-analysis The main observation index were cardiovascular risk factors, and the mean difference and its 95% confidence interval were used to estimate the effect size.Results:There were six RCTs, and 1181 patients met the inclusion criteria and entered the Meta-analysis. Compared with the control group, the intervention group can significantly reduce the level of systolic blood pressure ( MD=-1.20, 95% CI-2.21 to -0.19) and diastolic blood pressure ( MD=-4.17, 95% CI-7.12 to -1.22) in patients with type 2 diabetes mellitus, but there were no significant difference in the level of TC ( MD=2.92, 95% CI-0.84 to-6.67), HDL ( MD=2.33, 95% CI-0.27 to -4.92) and LDL ( MD=-2.34, 95% CI-5.67 to -0.99) between the two groups (all P>0.05). Conclusions:The meta-analysis provided evidence the Mediterranean diet showed the beneficial improvements in blood pressure glycemic control, but the effect of Mediterranean diet on lipid profile was not significant, which needed further verification.

【Objective】 To investigate the clinical features and gene analysis of one pedigree with multiple endocrine neoplasia type 2A （MEN2A） so as to clarify the diagnosis and classification of the disease， guide treatment and prevention， and improve prognosis. 【Methods】 The clinical data of a 36-member MEN2A family， including 6 probands， with medullary thyroid carcinoma， were investigated， and the peripheral blood genomic DNA of 28 family members （blood sample of one proband was not collected） was extracted. PCR amplification was performed on exons 8， 10， 11， 13， 14， 15 and 16 of the RET gene， and the products were directly sequenced. 【Results】 Review of the medical history showed that two probands with medullary thyroid carcinoma were accompanied with hyperparathyroidism， and one family member had pheochromocytoma. The RET gene mutation test confirmed that 13 family members， consisting of 5 probands and 8 family members， had the RET proto-oncogene exon 10 missense mutation. The heterozygous missense had mutation c.1852T>A， leading to the conversion of cysteine （TGC） at position 618 to serine （AGC） （Cys618Ser）. All subjects carrying RET gene Cys618Ser mutation had abnormal thyroid ultrasound change， accompanied with elevated calcitonin levels. Subjects carrying wild type of RET gene had normal calcitonin levels. The family was finally diagnosed with MEN2A by RET gene detection. 【Conclusion】 RET gene detection plays key role in the diagnosis and treatment of patients with MEN2A family and has guiding value in the follow-up and prognosis of asymptomatic carriers. There is a positive correlation between calcitonin level and the RET protooncogene mutation Cys618Ser. Patients suspected of MEN2A should be screened in time.

Objective:To assess the efficacy of different non-drug smoking cessation interventions on smoking cessation among the high-risk populations of lung cancer screening by network meta-analysis.Methods:PubMed, MEDLINE (Ovid), PsychINFO, CNKI, WanFang and VIP databases were searched for randomized controlled clinical trials published from January 2010 to September 2020.According to the eligibility criteria, the retrieval literature was screened, the quality evaluation and data extraction were conducted, then, the statistical analysis was performed by using the Stata 14.0 software.Results:A total of 28 trials were included, including 34 640 cases of high-risk population, involving intervention measures, including 5R short quit smoking intervention, cognitive behavior therapy, award model quit smoking intervention, motivational interview, network intervention, telephone intervention, incentive mechanism plus telephone intervention, network plus telephone intervention, conventional treatment and blank group.The results of network meta-analysis showed that on the 7-day follow-up of 6 months, according to the score of SUCRA, the ranking of smoking cessation rate was 5R short smoking cessation intervention (0.88), cognitive behavior therapy (0.85), AWARD model smoking cessation intervention (0.80), incentive mechanism plus telephone intervention (0.73), motivational interview (0.53), Internet plus telephone intervention (0.40), Internet intervention (0.37), telephone intervention (0.31), conventional treatment (0.13) and blank group (0.03). However, on the 7-day smoking cessation rate of 12-month follow-up, the ranking of smoking cessation rate was 5R short smoking cessation intervention (0.94), AWARD model smoking cessation intervention (0.81), motivational interview (0.51), network intervention (0.40), telephone intervention (0.19) and conventional treatment (0.14).Conclusions:The existing evidence shows that no matter the length of follow-up, 5R short smoking cessation intervention has the best effect on smoking cessation in high-risk population of lung cancer screening.

Objective To investigate the effect of seasonal and meteorological factors on the onset of stroke in Qinghai Plateau area.Methods Patients with stroke admitted to Qinghai Provincial People's Hospital from December 1,2011 to November 30,2016 were enrolled retrospectively.The meteorological data provided by Qinghai Meteorological Bureau were used to analyze the distribution of the number of cases in different seasons in Qinghai area and the relationship between the incidence of stroke and plateau meteorological factors.Results A total of 9 412 patients with acute ischemic stroke were enrolled,aged 15～95 years.There were significant statistical differences in the number of cases of ischemic stroke,cerebral hemorrhage,and subarachnoid hemorrhage in different seasons (all P ＜ 0.05).Among them,the number of cases with ischemic stroke increased first and then gradually decreased with the changes of spring,summer,fall and winter,while cerebral hemorrhage and subarachnoid hemorrhage showed a trend of decreasing first and then increasing gradually.There were significant differences in the number of cases in different relative humidity,temperature,and temperature differences (all P ＜ 0.05).Conclusion There are seasonal differences in the incidence of stroke in the plateau area,which may be associated with the influence of plateau meteorological factors.

Objective: To clarify the prevalence, clinical features and molecular characteristics of germline GATA2 mutations in pediatric primary myelodysplastic syndromes (MDS) . Methods: Next-generation sequencing technology was used to detect mutations in GATA2 and other myeloid malignancy genes in 129 children with primary MDS from Jan. 2007 to Jan. 2018. The relationship between genotypes and phenotypes was analyzed. Results: Germline GATA2 mutations accounted for 8.5% (11/129) of all primary MDS cases, and 14.0% (11/50) of MDS with excess blasts (MDS-EB) and acute myeloid leukaemia with myelodysplasia-related changes (AML-MRC) . Compared with GATA2 wild-type patients, GATA2 mutated patients were older at diagnosis[8 (1-16) years old vs 6 years old (range: 1 month old-18 years old) , P=0.035]and higher risk of monosomy 7 (72.7%vs 5.2%, P<0.001) and classified into MDS-EB and AML-MRC compared with refractory cytopenia of childhood (RCC) (63.6%vs 36.4%, P=0.111) . The multivariate analysis showed SETBP1 mutation (P=0.041, OR=9.003, 95%CI 1.098-73.787) and isolated monosomy 7 (P=0.002, OR=24.835, 95%CI 3.305-186.620) were significantly associated with germline mutated GATA2. Overall survival (OS) and outcomes of hematopoietic stem cell transplantation (HSCT) were not influenced by GATA2 mutational status. Conclusions Our data identify germline GATA2 mutations have a high prevalence in older pediatric patients with monosomy 7, and high risk of progression into advanced MDS subtypes. GATA2 mutation status does not affect OS in pediatric primary MDS.

OBJECTIVE:To establish a method for simultaneous determination of eugenol and thymol in mummification pastes.METHODS:HPLC method was adopted.The determination was performed on Wonda Cract C18 column with mobile phase consisted of methanol-0.2％ phosphoric acid (gradient elution) at the flow rate of 1.0 mL/min.The detection wavelength was 274 nm,and column temperature was set at 25 ℃.The sample size was 10 μL.RESULTS:The linear ranges of eugenol and thymol were 0.02-0.19 mg/mL (r=0.999 7) and 0.03-0.30 mg/mL (r=0.999 6).RSDs of precision,stability and reproducibility tests were all lower than 2.0％.The recoveries were 95.85％-99.84％(RSD=1.11％,n=9) and 95.81％-100.91％(RSD=1.59％,n=9).CONCLUSIONS:The method is simple and accurate,and can be applied to simultaneous determination of eugenol and thymol in mummification pastes.

Objective To explore the effect of diabetic liaison nurses on controlling blood sugar levels in patients with hyperglycemia in department other than endocrinology. Methods Four hundred diabetic patients with high blood sugar were selected from January to December, 2014 in department other than endocrinology. They were divided randomly into 2 groups equally:the control group and the observation group. The control group received traditional nursing care, while blood sugar management was carried out by diabetic liaison nurse in the observation group. Result Pre-discharge sugar metabolism in the observation group was significantly better than that in the control group (P<0.05). Conclusion The diabetic liaison nurses in other departments than the endocrinology department can help control blood sugar levels in patients with hyperglycemiain.

OBJECTIVE:To provide reference for the establishment of quality standard for Jinbo capsule. METHODS:TLC was conducted for the qualitative identification of Phellodendron chinense and Melia toosendan in preparation;HPLC was used for the content determination of berberine hydrochloride in preparation;the column was Diamonsil C18 with mobile phase of acetoni-trile-0.1% phosphoric acid (50∶50,V/V) at flow rate of 1 ml/min,detection wavelength was 265 nm,column temperature was 25 ℃ and volume injection was 10 μl. RESULTS:TLC spots of P. chinense and M. toosendan in preparation were clear and well-separated. The linear range of berberine hydrochloride was 0.044-0.154 mg/ml(r=0.999 9);RSDs of precision,reproducibili-ty and stability tests were no more than 2.0%,recovery was 98.36%-102.15%(RSD=1.11%,n=9). CONCLUSIONS:The meth-od is simple,accurate and reproducible,and can be used for the quality control of Jinbo capsule.

OBJECTIVETo observe the efficacy of polyethylene glycol conjugated asparaginase (peg-asp) for induction treatment of children with newly diagnosed acute lymphoblastic leukemia (ALL).

METHODA total of 268 newly diagnosed children with ALL enrolled in CCLG-2008 from January, 2010 to August, 2012 were analyzed. Patients received either native Escherichia coli asparaginase or pegaspargase along with multiagent chemotherapy during remission induction treatment. Status of bone marrow aspiration was assessed on day 15, day 33 (M1, M2, M3).

RESULTOf the 268 patients stratified, 37.3% (n = 100) were SR, 32.1% (n = 86) were IR, and 31.6% (n = 82) were HR; 159 patients received native Escherichia coli asparaginase and 109 patients received pegaspargase. Characteristics of two groups in age, sex, blood count at diagnosis, immunophenotype and response to prednisolone had no significant difference (P > 0.05). Bone marrow status on day 15 in pegaspargase group was M1 in 70 (64.2%) cases, M2 in 23 (21.1%) and M3 in 16 (14.7%), while in native Escherichia coli asparaginase group, M1 in 112 (70.4%) cases, M2 in 21 (13.2%) and M3 in 26 (16.4%), respectively (χ(2) = 2.938, P = 0.230). Bone marrow status on day 33 was M1 in 105 (96.3%), M2 in 3 (2.8%) and M3 in 1 (0.9%) in pegaspargase group, while it was M1 in 154 (96.9%) cases, M2 in 5 (3.1%) and M3 in native Escherichia coli asparaginase group, respectively (χ(2) = 1.494, P = 0.474).

CONCLUSIONDomestic pegaspargase of our country can achieve the similar efficacy in induction treatment for ALL patients as compared with native Escherichia coli asparaginase. The drug could be considered as not only the choice for allergic patients but also a first-line alternative for new patients.

BCR/ABL negative classical myeloproliferative neoplasms including three diseases,namely polycythemia veta,essential thrombocythemia and primary myelofibrosis.Progresses have been made in the study of these dieases in children following the development of that in adult,especially the research of molecular pathogenesis.Differences exist in clinical and molecular characteristics between children and adult BCR/ABL negative classical myeloproliferative neoplasms.There are specific characteristics in the diagnoses and treatment of childhood myeloproliferative neoplasms.This paper reviews the characteristics of childhood BCR/ABL negative classical myeloproliferative neoplasms in clinical and molecular features,diagnoses and treatment.
