1.Evaluation of Animal Models with Diminished Ovarian Reserve Based on Clinical Disease and Syndrome Characteristics of Traditional Chinese Medicine and Western Medicine

ObjectiveWith the development of society and the increasing demand for pregnancy at an advanced age, diminished ovarian reserve (DOR) has attracted wider attention. This disease affects the normal menstruation of women and poses a threat to their mental health. Current research on DOR is still in the initial stage, and advancing research progress in DOR necessitates the construction of effective and clinically relevant animal models. Building on existing literature and integrating the diagnosis criteria of DOR in traditional Chinese medicine (TCM) and Western medicine, this study summarized, analyzed, and evaluated existing models, providing a reference for optimizing DOR animal models. MethodsA retrospective analysis of literature on DOR-related animal models was conducted, and the diagnostic criteria of DOR in TCM and Western medicine were sorted out. By means of evaluation methods of animal models, the clinical relevance of each model to TCM and Western medicine was assessed. ResultsDOR animal models included those based on natural factors, iatrogenic factors, immune factors, metabolic factors, and environmental factors. Among them, the model based on iatrogenic factors, especially models induced by Tripterygium wilfordii preparations, cyclophosphamide, and cisplatin, had a high degree of relevance to both Western medical diseases and TCM syndromes and was confirmed as the optimal modeling method for studying DOR at present. ConclusionThe existing DOR modeling methods mostly have a high degree of clinical relevance to Western medical diseases, while the number of DOR animal models with clear TCM syndromes is very limited. TCM plays an indispensable role in exploring DOR treatment methods. Therefore, the development of DOR animal models related to TCM syndromes should be strengthened, further exploring the potential of TCM and providing strong theoretical support for the application of TCM in DOR treatment.
2.Risk assessment of human Spirometra mansoni infections and cross-sectional study on knowledge, attitude and practice towards sparganosis in endemic areas of Henan Province

Objective To assess the risk of human Spirometra mansoni infections and investigate the knowledge, attitude and practice (KAP) towards sparganosis mansoni among residents in Henan Province, so as to provide insights into formulation of the sparganosis mansoni control measures. Methods Qinling Village in Fugou County of Zhoukou City, Bali Village in Yancheng District of Luohe City, Duzhai Village in Puyang County of Puyang City and Doushan Village in Luoshan County of Xinyang City were sampled as survey sites in Henan Province from July to August 2023, and more than 40 frogs were sampled from ponds or streams in each survey site for detection of Sparganum mansoni infections. At least 150 residents were sampled using a cluster sampling method from each survey site, and the sero-prevalence of anti-S. mansoni IgG antibody was estimated. In addition, a questionnaire survey was conducted on the KAP towards sparganosis mansoni among participants, and the proportion of eligible KAP, rate of correct KAP and KAP scores were calculated. Results A total 229 frogs were collected from 4 survey sites in 2023, and the overall prevalence of S. mansoni infection was 4.37% (10/229) in frogs, with 7.75% (10/129) prevalence in wild frogs and 0 in farm-bred frogs. A questionnaire survey was performed among 649 residents sampled from 4 survey sites, and 649 serum samples were collected. The seroprevalence of anti-S.mansoni IgG antibody was 0.15% (1/649) and the overall proportion of eligible KAP was 23.73% (154/649) among participants. There were age- (χ2 = 30.905, P = 0.000), educational level- (χ2 = 41.011, P = 0.000), and occupation-specific proportions of eligible KAP among participants (χ2 = 10.721, P = 0.005), and the proportion of eligible KAP decreased with age (χ2 trend = 22.717, P = 0.000) and increased with education levels (χ2 trend = 40.025, P = 0.000). The rates of correct KAP towards sparganosis mansoni were 40.81% (2 119/5 192), 96.66% (1 882/1 947) and 63.81% (3 727/5 841) (χ2 = 1 913.731, P = 0.000) among residents, respectively. The rates of correct KAP towards sparganosis mansoni varied significantly among survey sites (χ2 = 136.872, 42.347 and 255.157; all P values= 0.000, with the highest rate of correct knowledge (51.94%, 748/1 440) and practices (75.86%, 1 229/1 620) in Yancheng District of Luohe City and the highest rate of correct attitudes in Puyang County of Puyang City (99.11%, 446/450) (all P values< 0.05). Conclusions There is still a high transmission risk of sparganosis mansoni in Henan Province, and the KAP towards sparganosis mansoni is required to be improved among residents.
3.Construction and practice of an intelligent management system for preoperative anemia based on multidisciplinary collaboration

Objective: To improve the efficiency and standardization of preoperative anemia diagnosis and treatment by establishing a systematic intelligent management platform for preoperative anemia. Methods: A multidisciplinary collaborative model was adopted to develop a preoperative anemia management system that integrates intelligent early warning, standardized treatment pathways, and quality control. The system utilizes natural language processing technology to automatically capture laboratory data and establish evidence-based medical decision support functions. A pre-post study design was employed to compare changes in preoperative anemia screening rates, preoperative anemia intervention rates, reasonable use of iron supplements, and perioperative red blood cell transfusion rates before and after system implementation. Results: After system implementation, the standardization of anemia diagnosis and treatment significantly improved: 1) Screening effectiveness: The anemia screening rate increased to 50.00% (an increase of 27.24%); 2) Intervention effectiveness: The anemia treatment rate rose to 56.30% (an increase of 14.02%); 3) Treatment standardization: The reasonable use rate of iron supplements increased to 55.33% (an increase of 21.02%); the red blood cell transfusion rate decreased to 18.29% (a decrease of 4.07%), and the amount of red blood cell transfusions was reduced by 291 units. Conclusion: This system achieves full-process management of preoperative anemia through information technology, significantly enhancing the standardization of diagnosis and treatment as well as intervention effectiveness, providing an effective solution for perioperative anemia management.

Objective:To investigate the protective mechanism of TRPV4 channel inhibitor on blood-brain barrier(BBB)damage after traumatic brain injury(TBI).Methods:The TBI rat model was established.TRPV4 channel inhibitor HC067047 or PKC-δ inhibitor Rottlerin was used to detect changes in BBB permeability,neurological function score,and the expression of microvascular endothelial tight junction proteins ZO-1 and ZO-2 in brain injury areas after TBI.Results:Compared with the Sham group,BBB permeability significantly increased,brain neurological function score significantly decreased,and the expression of ZO-1 and ZO-2 significantly decreased in TBI group(P<0.05).Compared with the TBI group,after administration of HC067047 or Rottlerin,changes in BBB permeability,brain neurological function score,the expression of ZO-1 and ZO-2 were partially reversed(P<0.05).Conclusions:TBI-induced BBB injury may be mediated by TRPV4 channel regulating PKC-δ signaling pathway to affect the expression of tight junction proteins ZO-1 and ZO-2.Inhibition of TRPV4 channel function or PKC-δ signal molecule can partially alleviate BBB damage induced by TBI.This study may provide new ideas for the treatment of clinical TBI.
6.Long-Term Survival Trend of Gynecological Cancer:A Systematic Review of Population-Based Cancer Registration Data

Gynecological cancer significantly affect the health of women.This review aimed to describe the global patterns and trends in the survival of patients with gynecological cancers.We searched PubMed,Embase,Web of Science,SinoMed,and SEER for survival analyses of cancer registration data of cervical,endometrial,and ovarian cancers published between 1980 and 2022.Globally,the highest 5-year observed survival rate for cervical cancer was 76.5%in Anshan,Liaoning,China(2008-2017).The 5-year observed survival rates of endometrial and ovarian cancers were higher in Finland(1995-1999,82.5%)and Singapore(1988-1992,62.0%).The 5-year relative survival rate of cervical cancer patients was higher in Haining,Zhejiang,China(2011-2014,85.8%).Korea ranked first at 89.0%and 64.5%for endometrial and ovarian cancers,respectively.Survival rates have improved for cervical,endometrial,and ovarian cancers.Patients aged≥75 years and those with advanced-stage disease had the worst 5-year survival rates.Survival rates were better for squamous cell carcinoma in cervical cancer,for endometrial carcinoma and mucinous adenocarcinoma in endometrial cancer,and for germ cell and sex-cord stromal tumors in ovarian cancer.Over the past four decades,the survival rates of gynecological cancers have increased globally,with notable increases in cervical and endometrial cancers.Survival rates are higher in developed countries,with a slow-growing trend.Future studies should focus on improving survival,especially in ovarian cancer patients.
7.Effects of Gengxin Decoction (更欣汤) on Oestrogen Receptors and Orexin A in the Nucleus Accumbens of the Hypothalamus Nodularis Papillaris in Perimenopausal Insomnia Model Mice

ObjectiveTo explore the possible mechanism of Gengxin Decoction (更欣汤) in the treatment of perimenopausal insomnia from the perspective of the expression of oestrogen receptor and orexin A (OXA) in hypothalamic tuberomammillary nucleus (TMN). MethodsFemale ICR mice with regular estrous cycle were selected to establish perimenopausal insomnia mice models by injecting 4-vinylcyclohexene diepoxide (VCD) combined with para-chlorophenylalanine (PCPA) suspension intraperitoneally. After successful modeling, 75 mice models were randomly divided into model group, zopiclone group and low-, middle-, and high-dose Gengxin Decoction, with 15 mice in each group, and 15 normal mice were set as blank group. On the next day after successful modelling, mice in the low-, middle-, and high-dose Gengxin Decoction were given Gengxin Decoction granules 1.15, 2.3 and 4.6 g·kg-1·d-1 by gavage at 9:00 AM every day, mice in the zopiclone group were given zopiclone suspension 1.13 mg·kg-1·d-1 by gavage at the same time point, and mice in the blank and model groups were given distilled water 1 ml·kg-1·d-1 by gavage at the same time point, for 14 days in all groups. During the experimental period, general conditions of the mice in each group, such as daytime activities, mental state, diet and coat colour, were observed; twenty-four hours after the final intervention, the sleep latency and sleep duration of mice were detected by sodium pentobarbital synergy test, and the levels of serum estradiol (E2)、progesterone (P)、follicle stimulating hormone (FSH) and OXA protein in TMN of hypothalamus were detected by ELISA, and the protein optical density values of estrogen receptor α (ERα), OXA and ERβ in TMN was detected by immunofluorescence staining. ResultsThe circadian rhythm of mice was regular in the blank group; the circadian rhythm of mice was disordered in the model group, and they remained active during the day and night; the mental state of mice in each group of Gengxin Decoction was improved compared with that before, which showed that the daytime activity was less than before, and the coat color was slightly yellow but shiny, and the mental state of mice was significantly improved in the middle-dose Gengxin Decoction; the zopiclone group was less active during the day than before, had rough, less shiny hair, and was more alert. Compared with the blank group, the sleep latency of mice in the model group prolonged, the sleep duration shortened, serum E2 and P decreased, serum FSH and the expression of OXA protein in TMN region increased, the optical density of ERα and ERβ in TMN region decreased, and the optical density of OXA increased (P<0.05 or P<0.01). Compared with the model group, the sleep latency of the mice in the low-, middle-, and high-dose Gengxin Decoction groups and the zopiclone group shortened, the sleep duration prolonged (P<0.01); the serum E2 and P in the middle-, and high-dose Gengxin Decoction groups increased, FSH and the expression of OXA protein in TMN area decreased (P<0.05); the optical density values of ERα in TMN area increased in the medium-dose group, and the optical density value of OXA decreased in low- and middle-dose Gengxin Decoction groups, and the optical density values of ERβ in TMN area increased in low-, middle-, and high-dose Gengxin Decoction groups and the zopiclone group (P<0.05 ). Compared with the zopiclone group, mice in the low-dose Gengxin Decoction group had longer sleep latency and shorter sleep duration, and mice in the middle- and high-dose Gengxin Decoction groups had higher serum E2 level and ERβ optical density value in the TMN area, and lower serum FSH and OXA level in the TMN area (P<0.05). Compared with the low-dose Gengxin Decoction group, the sleep latency of mice in the middle- and high-dose Gengxin Decoction groups shortened, and the serum FSH level reduced (P<0.01). ConclusionGengxin Decoction may inhibit arousal and improve perimenopausal insomnia by modulating the distribution of estrogen receptors and OXA in the TMN region, and inhibiting the overactivation of the orexin system in the TMN region.
8.Construction and preliminary application of an illness perception intervention program for patients with chronic obstructive pulmonary disease

Objective To establish an illness perception intervention program for patients with chronic obstructive pulmonary disease(COPD)and preliminarily analyse its application effect.Methods On the basis of systematic retrieval,a draft of the illness perception intervention program for patients with COPD was formed through content analysis.From May to August 2022,after 2 rounds of expert consultation,the illness perception intervention program for patients with COPD was formed.Patients with COPD who were hospitalized in the respiratory departments of 2 hospitals in Jiangsu Province from December 2022 to January 2023 were selected as the study subjects by convenience sampling method.The experimental group was treated with the illness perception intervention program,while the control group was treated with routine nursing.The effects of the 2 groups were compared by Brief Illness Perception Questionnaire,Modified British Medical Research Council、Brief Illness Perception Questionnaire、Acceptance and Action Questionnaire Ⅱ,COPD Assessment Test,COPD Self-management Scale before and after intervention,and a month after intervention.Results The illness perception intervention program for patients with COPD included 6 intervention topics and 24 specific intervention items.In the 2 rounds of correspondence,the expert group authority coefficient was 0.81,and the Kendall coordination coefficients were 0.19 and 0.16 in the second round(all P<0.001).In the intervention study,the recruitment rate of patients was 56%;the withdrawal rate was 15%;and the high compliance rate was 80%.Repeated measures analysis of variance showed that there was an interaction effect between the scores of Acceptance and Action Questionnaire Ⅱ of the 2 groups(P<0.05).The differences in the scores of Brief Illness Perception Questionnaire,COPD Assessment Test,Self-management Scale between the 2 groups were statistically significant in terms of time effect(P<0.05),and the differences in the scores of the Brief Illness Perception Questionnaire and Acceptance and Action Questionnaire Ⅱ between the 2 groups were statistically significant in terms of main effect(P<0.05).Conclusion The established illness perception intervention program for patients with COPD is scientific,feasible and effective,and it can provide practical references for clinical healthcare workers to promote illness perception and improve negative psychology in patients with COPD.
9.Implicit theories applied to health field:a scoping review

Objective To understand the current status of implicit theory derivation forms and applications in the health field at home and abroad,and to provide references for clinical practice and future research.Methods In accordance with the scoping review framework,PubMed,Embase,Web of Science,Cochrane Library,CINAHL,PsycINFO,CNKI,Wanfang,VIP,SinoMed were searched for relevant studies.The time limit for the search was from the establishment of the databases to 10 March 2024.Results A total of 45 articles were included in the study,including 24 articles on health behaviour and 21 articles on mental health,covering 14 derivations of implicit theories such as implicit health theory,implicit weight theory and implicit emotion theory.The main intervention process of implicit theories can be summarised as 3 major steps:belief-led intervention,attention examination and outcome testing.Conclusion The research value of implicit theory in the field of health has been gradually explored,and it is an important antecedent to improve individual health behaviors and mental health,and its brief and effective intervention model is more compatible with the characteristics of nursing work,so domestic and foreign nursing researchers can learn from the relevant research results to further optimize the strategies of health education,preventive guidance,and psychological care,and to carry out high-quality studies to validate them.
10.Clinical outcomes of allogeneic hematopoietic stem cell transplantation from matched sibling donor for myelofibrosis

Objective:To evaluate the efficacy and safety of matched sibling donor allogeneic hematopoietic stem cell transplantation (allo-HSCT) for the treatment of myelofibrosis (MF).Methods:In this case series, the clinical data of 18 patients with MF who received allo-HSCT in the Department of Hematology, Peking University People′s Hospital from December 2008 to December 2023 were retrospectively studied. Kaplan-Meier survival analysis and competitive risk model were used to evaluate the probabilities of 3-year overall survival (OS), disease-free survival (DFS), cumulative incidence of relapse (CIR), and transplant related mortality (TRM). The transplant related complications were also analyzed.Results:Among the 18 patients included, there were 12 males and 6 females, with a median age of 50 (range: 28-64) years. All 18 patients achieved neutrophil engraftment, and the time of neutrophil engraftment [ M ( Q1, Q3)] was 16.0 (11.8, 18.0) days. Twelve patients achieved platelet engraftment, and the platelet engraftment time was 21.0 (16.2, 43.2) days. Six patients had grade Ⅱ to Ⅳ acute graft-versus-host disease (GVHD), and six patients had chronic GVHD. The 3-year OS rate and DFS rate after transplantation were 62.2% and 52.2%, respectively. The 3-year CIR and TRM were 29.7% and 24.6%, respectively. Four patients died during follow-up, with the main cause of death being infections. Conclusion:Matched sibling allo-HSCT is a feasible option for the treatment of MF.
