1.Effects of music assisted drug treatment on behavioral and psychological symptoms and cognitive function in the elderly with moderate to severe dementia

		                        			BackgroundWith the acceleration of population aging in China, studies have documented an increased prevalence of dementia in the elderly. Behavioral and psychological symptoms and impaired cognitive function are important problems affecting the quality of life in the elderly with dementia. It has been noted that there are certain limitations on the management of problems mentioned above by drug treatment alone, while its combination with music intervention is emerging as an effective approach. However, the application of this approach in the elderly with moderate to severe dementia is still in its relative infancy. ObjectiveTo investigate the effect of music assisted drug treatment on behavioral and psychological symptoms and cognitive function in the elderly with moderate to severe dementia, so as to provide insights for the development of effective intervention. MethodsA randomized controlled study was utilized to enroll a coherent of 43 elderly patients with moderate to severe dementia who were hospitalized in Geriatric Psychiatry of Deyang Mental Health Center from January to July 2023 and met the International Classification of Diseases, tenth edition (ICD-10) diagnostic criteria for dementia. Eligible subjects were divided into study group (n=22) and control group (n=21) based on random allocation using SPSS 26.0. Both groups received routine drug treatment and nursing care, while the study group added music intervention, which consisted of two 30-minute sessions per week for 8 weeks. Neuro Psychiatric Inventory (NPI) and Mini-Mental State Examination (MMSE) were administered to all patients before and after intervention. ResultsA total of 36 patients completed the study, including 20 case in study group and 16 case in control group. No statistical difference was noted in baseline NPI score and MMSE score between two groups (P>0.05). After intervention, study group reported a statistical reduction in NPI total score [58.00 (49.00, 79.25) vs. 78.50 (55.00, 95.50), Z=-3.902, P<0.01)] along with a clinically unremarkable increase in MMSE score (P>0.05) compared with baseline data. After intervention, NPI total score and scores on sub-domains including agitation/aggression, depression/dysphoria, anxiety and aberrant motor behavior were all significantly lower in study group than those in control group (Z=-2.183, -2.438, -2.691, -3.716, -2.250, P<0.05 or 0.01), while there was no significant difference in MMSE score between two groups (P>0.05). ConclusionMusic assisted drugintervention may ameliorate behavioral and psychological symptoms in the elderly with moderate to severe dementia, while no significant improvement is documented on cognitive function. 		                        			Ferroptosis is a kind of programmed cell death, involves multi-molecular and multi-signaling pathways, and the core processes is iron overload and lipid peroxidation. It affects cancer, neurological disease, cardiovascular disease, digestive system disease, and many other diseases. While, ischemia-reperfusion injury is an extremely complex pathological process, including ischemia-reperfusion injury of the heart, brain, liver and kidney, etc., which often occurs in shock, organ surgery and organ transplantation, seriously affecting human health, and there is no effective radical treatment. However, ferroptosis is closely related to ischemia-reperfusion injury so that it is possible to find the prevention and treatment measures of ischemia-reperfusion injury by exploring the internal relationship between them. Therefore, the author tries to summarize the signaling pathways of ferroptosis, and explores the relationship between ferroptosis and ischemia-reperfusion injury of heart, brain, liver and kidney by moderating the ferroptosis targets properly to find the effective ways to treat cancer, improve inflammation and relieve other diseases.
		                        		
		                        		
		                        		
		                        	
3.Confidential unit exclusion in Guangzhou from 2009 to 2022

		                        			【Objective】 To investigate the condition of confidential unit exclusion(CUE) in Guangzhou, so as to ensure blood safety. 【Methods】 The number of CUE donors, demographic characteristics of CUE donors, reasons for CUE, and response time of CUE after blood donation in Guangzhou from 2009 to 2022 were statistically analyzed. 【Results】 From 2009 to 2022, the response ratios of CUE was 0.006 2% (260/4 170 984) and the ratios had statistically significant difference between different years(P<0.05). For the response ratios of CUE, no statistically significant difference was noticed in gender and occupation (P>0.05), but statistically significant differences were found in age, number of blood donations, education background, and marital status (P<0.05). Blood donors aged 18~30 (0.007 3%, P<0.05) and first-time blood donors (0.010 8%, P<0.05) were the main groups of CUE. High risk sexual behavior (28.46%, 74/260) was the primary reason for CUE. The CUE response peak was within 72 hours after blood donation, and the response ratios within 24-72 hours after blood donation was the highest (68.46%, 178/260). 【Conclusion】 CUE is a crucial measure to ensure blood safety. Detailed pre-donation health consultations are suggested for blood donors aged 18-30 and first-time blood donors so as to better excluding high-risk blood donors. 4.Study on improvement mechanism of caudatin on liver injury in rats

		                        			OBJECTIVE To investigate the improvement mechanism of caudatin on liver injury of rats. METHODS SD rats were randomly divided into blank group, model group, caudatin low-dose and high-dose groups (25, 50 mg/kg), with 6 rats in each group. Diethylnitrosamine (DEN) was injected intraperitoneally three times per week for eight weeks to establish liver injury model of rats. At 5th week of modeling, the rats received relevant medicine or 0.5% sodium carboxymethylcellulose intragastrically for 4 weeks. The levels of liver function indexes [alanine transaminase (ALT), aspartate transaminase (AST), total protein (TP) and total bilirubin (TBI)] and inflammatory factors [interleukin (IL-6), tumor necrosis factor α (TNF-α), IL-1β] in serum were detected; the histopathological morphological changes of rat liver were observed; the positive protein expressions of nuclear factor κB (NF-κB) and 78 kDa glucose regulatory protein (Grp78) in liver tissue were also determined; the expressions of endoplasmic reticulum stress-related protein Grp78, C/EBP homologous protein (CHOP), activating transcription factor 6 (ATF6) and inositol requiring enzyme 1α (IRE1α) and the level of protein kinase R-like endoplasmic reticulum kinase robertluoyi@126.com (PERK) in liver tissue were detected. RESULTS Compared with blank group, serum levels of ALT, AST, TBI, IL-6, TNF-α and IL-1β and positive expressions of NF-κB and Grp78 in liver tissue as well as protein expressions of Grp78, CHOP, ATF6 and IRE1α, PERK protein phosphorylation level were all increased significantly in model group (P<0.05), while the serum level of TP was decreased significantly (P<0.05). The disordered structure of liver lobule, swollen liver cells, unclear intercellular boundary were observed and accompanied by inflammatory cell infiltration. Compared with model group, most of the above indexes were significantly reversed in caudatin groups (P<0.05); the structure of hepatic lobule was relatively complete and clear, the cells were arranged orderly, and the infiltration of inflammatory cells was also reduced. CONCLUSIONS Caudatin has a significant improvement effect against DEN-induced liver injury in rats, the mechanism of which may be associated with inhibiting endoplasmic reticulum stress and inflammatory reaction.
		                        		
		                        		
		                        		
		                        	
5.Epidemiological characteristics and risk factors of bronchiectasis in patients with chronic obstructive pulmonary disease in Suining area

		                        			Objective  To analyze the epidemiological characteristics and risk factors of bronchiectasis in patients with chronic obstructive pulmonary disease (COPD) in Suining area, and to provide theoretical basis for COPD prevention and treatment of bronchiectasis.  Methods  A total of 582 patients with ACUTE exacerbation of COPD (AECOPD) admitted to grade A Hospitals in Suining area from 2019 to 2020 were selected as the investigation objects. According to whether the patients had bronchiectasis, they were divided into control group (without bronchiectasis, n=485) and observation group (with bronchiectasis, n=97). The risk factors of bronchiectasis in COPD patients were analyzed by univariate analysis and logistic regression. The clinical data of the two groups were compared, including age, sex, whether there were other diseases, respiratory tract infection, proportion of purpuric sputum, colonization rate of Pseudomonas aeruginosa.  Results Of 16.84% (49/291) AECOPD patients occurred Bronchiectasis. The colonization rate of Pseudomonas aeruginosa in observation group was significantly higher than that in control group (P<0.05). The values of FVC and FEV1/FVC in observation group were significantly lower than those in control group (P<0.05). The levels of serum CRP and PCT in the observation group were significantly higher than those in the control group (P<0.05). Male (OR=2.515), high proportion of GOLD grade III/IV (OR=3.654), smoking (OR=3.472), diabetes (OR=3.829) and bacterial infection (OR=4.159) were independent risk factors for bronchiectasis in COPD patients (P<0.05).  Conclusion  COPD patients in Suining area have a high risk of bronchiectasis.The lung function was declined significantly.It has a high colonization rate of Pseudomonas aeruginosa. Patients who are male, have a high proportion of GOLD class III/IV, smoke, have diabetes, and have bacterial infections should be given interventions that can reduce the risk of bronchiectasis in COPD patients.

		                        			Flavonoids such as baohuoside I and icaritin are the major active compounds in Epimedii Folium(EF)and possess excellent therapeutic effects on various diseases.Encouragingly,in 2022,icaritin soft capsules were approved to reach the market for the treatment of hepatocellular carcinoma(HCC)by National Medical Products Administration(NMPA)of China.Moreover,recent studies demonstrate that icaritin can serve as immune-modulating agent to exert anti-tumor effects.Nonetheless,both production effi-ciency and clinical applications of epimedium flavonoids have been restrained because of their low content,poor bioavailability,and unfavorable in vivo delivery efficiency.Recently,various strategies,including enzyme engineering and nanotechnology,have been developed to increase productivity and activity,improve delivery efficiency,and enhance therapeutic effects of epimedium flavonoids.In this review,the structure-activity relationship of epimedium flavonoids is described.Then,enzymatic en-gineering strategies for increasing the productivity of highly active baohuoside I and icaritin are dis-cussed.The nanomedicines for overcoming in vivo delivery barriers and improving therapeutic effects of various diseases are summarized.Finally,the challenges and an outlook on clinical translation of epi-medium flavonoids are proposed.
		                        		
		                        		
		                        		
		                        	
7.Clinical application of infrared vascular imager in the transplantation of free arterialized venous flaps

		                        			Objective:To explore the feasibility and clinical effect of the infrared vascular imager in the transplantation of free arterialized venous flaps.Methods:A retrospective analysis was conducted with patient data of hand microsurgery using free arterialized venous flaps to repair hand wounds in the Second Affiliated Hospital of Wenzhou Medical University from March 2019 to November 2020. An arterialized venous flap was designed according to the Goldschlager’s Type Ⅲ flap design with the aid of a magnifying glass following the venous course. The flap was transfer to cover the wound of the recipient area. The reconstructive effect was observed postoperatively, including the blood supply, color, texture, survival of the flap, the flexion and extension of the affected finger, and the appearance of the forearm donor site. The hand function was evaluated by the Chinese Medical Association Hand Surgery Society’s upper limb function evaluation trial standard.Results:A total of 17 patients were included, including 11 males and 6 females, ranging in age from 19 to 68 years old, with an average age of 34.6 years. A total of 18 skin flaps were designed and elevated, and the flap size ranged from 2.0 cm × 3.0 cm to 8.0 cm × 9.0 cm. The patients were followed up 3 to 12 months after the operation, with an average follow-up of 6 months. Blisters appeared on the second day after the operation in 8 flaps, and the affected limbs were raised to maintain the integrity of the blisters. The blisters of the flaps subsided one week after the operation. Congestion occurred on the second day after operation in 7 flaps, which subsided after two weeks after routine treatment, such as raising the affected limb and removing some sutures. One flap developed progressive hypoperfusion one week after the operation, and eventually necrosis. Seventeen flaps survived, and one flap failure was observe. All the surviving skin flaps were free of bloat and had a good appearance. In two cases, the skin graft area was pigmented, and the flexion and extension activities of the affected fingers were suitable. Using the Chinese Medical Association Society of Hand Surgery’s upper limb function evaluation trial standard, 16 patients (excluding one flap failure) were evaluated for hand function, showing 11 cases were excellent and 5 cases were good.Conclusions:Infrared vascular imaging technology enables free arterialized venous flap transplantation to realize non-contact, non-invasive, simple operation, intuitive and accurate, and truly realizes rapid flap design under direct vision. 9.Genome of the Giant Panda Roundworm Illuminates Its Host Shift and Parasitic Adaptation

		                        			Baylisascaris schroederi,a roundworm(ascaridoid)parasite specific to the bamboo-feeding giant panda(Ailuropoda melanoleuca),represents a leading cause of mortality in wild giant panda populations.Here,we present a 293-megabase chromosome-level genome assembly of B.schroederi to infer its biology,including host adaptations.Comparative genomics revealed an evolutionary trajectory accompanied by host-shift events in ascaridoid parasite lineages after host separations,suggesting their potential for transmission and rapid adaptation to new hosts.Genomic and anatomical lines of evidence,including expansion and positive selection of genes related to the cuticle and basal metabolisms,indicate that B.schroederi undergoes specific adaptations to survive in the sharp-edged bamboo-enriched gut of giant pandas by structurally increasing its cuticle thickness and efficiently utilizing host nutrients through gut parasitism.Additionally,we characterized the secretome of B.schroederi and predicted potential drug and vaccine targets for new control strategies.Overall,this genome resource provides new insights into the host adaptation of B.schroederi to the giant panda as well as the host-shift events in ascaridoid parasite lineages.Our findings on the unique biology of B.schroederi will also aid in the development of prevention and treatment measures to protect giant panda populations from roundworm parasitism.
