OBJECTIVE To assess the correlation between the steady-state minimal concentration (cmin) and 24 h area under the drug concentration-time curve (AUC24)/minimal inhibitory concentration (MIC) ratio (AUC24/MIC) of vancomycin in pediatric patients, and analyze independent risk factors for treatment failure. METHODS Data of hospitalized children treated with vancomycin and receiving therapeutic drug monitoring in our hospital from January 2021 to July 2024 were retrospectively collected and divided into success group and failure group according to whether the treatment was successful or not. Spearman correlation analysis was used to analyze the correlation between cmin and AUC24/MIC of vancomycin, and one-way and multifactorial Logistic regression analyses were used to screen the independent risk factors for vancomycin treatment failure. RESULTS A total of 59 children were included, with 41 in the success group and 18 in the failure group. Compared with the failure group, AUC24/MIC of vancomycin was significantly higher in the success group (P=0.038), but there was no statistically significant difference in the cmin of the two groups (P>0.05); cmin of vancomycin was significantly positively correlated with AUC24/MIC (r=0.499, P<0.001), but it has a certain efficacy in predicting the achievement of the AUC24/MIC standard (≥400) (area under the receiver operator characteristic curve=0.696), with an optimal cutoff value of 6.05 mg/L determined by the Youden index. The efficacy of AUC24/ MIC in predicting treatment failure was superior to cmin (areas under the receiver operator characteristic curve were 0.671 vs. 0.523, P were 0.038 vs. 0.684), with higher sensitivity (83.3% vs. 66.7%). Hypoproteinemia and AUC24/MIC≤369.1 were independent risk factors for vancomycin treatment failure (P<0.05). The incidence of nephrotoxicity was 3.4%. CONCLUSIONS There is a significant positive correlation between cmin and AUC24/MIC of vancomycin in pediatric patients; hypoproteinemia and AUC24/MIC≤369.1 are independent risk factors for vancomycin treatment failure in children.

Objective:To investigate the effect of astragaloside IV (AS-IV) regulating the signal axis of stromal cell-derived factor-1α (SDF-1α)/CXC chemokine receptor 4 (CXCR4) on the mobilization of bone marrow endothelial progenitor cells (EPCs) to peripheral blood in diabetes skin ulcer (DSU) rats. Methods:Twenty four SPF grade male Sprague Dawley (SD) rats were selected to make the model of type 2 diabetes rats by intraperitoneal injection of 30 mg/kg 1% (plastid ratio) streptozotocin, and then round full-thickness skin with a diameter of 2 cm was cut on both sides of the waist and back to make the skin ulcer model of diabetes rats. After that, they were randomly divided into AS-IV group (50 mg/kg AS-IV), blocker group (50 mg/kg AS-IV+ 5 mg/kg AMD3100) and model group. At the same time, a blank group ( n=8) was set up, The drug was administered via intraperitoneal injection, and the model group and blank group were treated with 0.9% NaCl of equal volume. On the 10th day, peripheral blood, femoral bone marrow, and wound neovascularization tissues of rats were collected. The number of EPCs in peripheral blood of each group of rats was measured by flow cytometry, and the protein expression of SDF-1α and CXCR4 in peripheral blood, femoral bone marrow, and wound neovascularization tissues of rats was detected by enzyme-linked immunosorbent assay (ELISA); At the same time, the wound healing rates of each group were tested. Results:On the 10th and 21st day after modeling, the wound healing rate of each group of rats was compared. The blank group healed the fastest, while the model group healed the slowest. The AS-IV group had better healing than the model group and the blocker group, and the difference was statistically significant (all P<0.05). On the 10th day after modeling, the positive rates of peripheral blood EPCs in the white group, AS-IV group, and blocker group were significantly higher than those in the model group (all P<0.05), while the positive rates of peripheral blood EPCs in the blocker group were significantly lower than those in the AS-IV group (all P<0.05). On the 10th day after modeling, the protein expression of SDF-1α and CXCR4 in the wound, serum, and bone marrow of the model group was the lowest, while the protein expression in the blank group was the highest (all P<0.05). The protein expression of SDF-1α and CXCR4 in the wound, serum, bone marrow of the AS-IV group was significantly higher than that of the blocker group and model group, and the difference was statistically significant (all P<0.05). Conclusions:Astragaloside IV can promote the mobilization and migration of endothelial progenitor cells from bone marrow to peripheral blood in diabetes ulcer rats by regulating SDF-1α/CXCR4 signal axis, and can participate in angiogenesis of diabetes ulcer wounds as seed cells to promote the healing of diabetes skin ulcers.

Objective To observe the therapeutic effect and mechanism of polydatin on intrauterine adhesions(IUA)in rats. Methods Eight rats were randomly selected as the sham operation group from 40 fertile female SD rats, and the remaining 32 rats were given intrauterine mechanical injury combined with lipopolysaccharide infection to induce IUA model. The 32 model rats were randomly divided into model group,low-,medium-and high-dose groups of polydatin, with 8 rats in each group, and the corresponding intervention lasted for 14 days. The morphological structure of endometrium was observed by hematoxylin-eosin(HE)staining,the thickness of endometrium was measured and the number of endometrial glands was counted.Masson staining was used to observe endometrial fibrosis;enzyme-linked immunosorbent assay(ELISA)was used to detect the contents of tumor necrosis factor-α(TNF-α),interleukin(IL)-1β and IL-6 in endometrial tissue.Western Blot was used to detect the ptotein expressions of key proteins of Wnt/β-catenin signaling pathway, including β-catenin,glycogen synthase kinase 3β(GSK-3β),phosphorylated glycogen synthase kinase 3β(p-GSK-3β),Cyclin D1,and fibrosis-related proteins, including transforming growth factor β1(TGF-β1),α-smooth muscle actin(α-SMA),type I collagen(ColⅠ)and type Ⅲ collagen(Col Ⅲ).Results Compared with the sham operation group, the endometrial tissue of rats in the model group and the low-,medium-and high-dose groups of polydatin showed different degrees of pathological damage, the thickness of the endometrium and the number of glands decreased, and there were obvious different degrees of collagen fiber deposition. The levels of TNF-α,IL-1β,IL-6,the relative protein expressions of β-catenin,Cyclin D1 and the activation level of GSK-3β protein, the relative protein expressions of TGF-β1,α-SMA,ColⅠ and Col Ⅲ in endometrial tissue were increased(P<0.05).Compared with the model group, the pathological damage of endometrial tissue in the low-,medium-and high-dose groups of polydatin was improved, the endometrial thickness and the number of glands were increased, and the endometrial fibrosis was alleviated. In addition, the levels of the above indicators in the endometrial tissue were decreased(P<0.05)in a dose-dependent manner. Conclusion Polydatin can promote endometrial repair and inhibit excessive endometrial fibrosis in rats with IUA,and its mechanism may be related to the inhibition of Wnt/β-catenin signaling pathway activation.

Objective To systematically evaluate the application efficacy of failure mode and effect analysis(FMEA)management mode in the prevention and control of healthcare-associated infection(HAI).Methods Li-terature on the application of FMEA management mode in HAI prevention and control were retrieved from PubMed,Embase, the Cochrane Library,China National Knowledge Infrastructure(CNKI),VIP Database,Wanfang Data-base,and China Biomedical Literature Database(CBM).Two researchers independently screened the literature,ex-tracted data, and conducted cross checking.Risk and quality assessments were performed on the included studies of randomized controlled trials by ROB tool, the included cohort studies were scored by Newcastle-Ottawa(NOS)scale,and Meta-analysis was conducted by RevMan 5.4 software. Results A total of 22 studies involving 42 815 patients were included in the analysis, with 21 784 in the FMEA management mode group and 21 031 in the control group.Meta-analysis results showed that the incidence of HAI in the FMEA management mode group was lower than that in the control group(OR=0.31,95%CI[0.24,0.40]).Compared with the conventional management mode, incidences of superficial surgical site infection(OR=0.53,95%CI[0.36,0.78]),respiratory system infec-tion(OR=0.44,95%CI[0.35,0.56]),urinary system infection(OR=0.45,95%CI[0.38,0.53]),and blood system infection(OR=0.29,95%CI[0.18,0.45])in the FMEA management mode group were all lower(all P<0.01).Conclusion The application of FMEA management mode in HAI prevention and control can reduce the inci-dence of HAI,which should be actively promoted in hospital management.

Objective To observe the clinical efficacy and adverse drug reactions of recombinant human growth hormone on dwarfism in children with primary nephrotic syndrome. Methods Children with dwarfism in primary nephrotic syndrome were divided into control group and treatment group.Patients in control group were orally administered prednisone acetate tablets, with an initial dose of 2 mg·kg-1·d-1,at once,no more than 60 mg in a single day,and after a duration of 6 weeks of full dosage, the dosage was reduced by 2.5 mg every 2 weeks until the maintenance dose of 5-10 mg·d-1 was administered for 12 months.Patients in treatment group were injected subcutaneously with recombinant human growth hormone 0.15 U·kg-1 at 0.5 h before bedtime every night on the basis of control group for a period of 12 months. The levels of height, bone age, standard deviation fraction of height(HtSDS),insulin-like serum growth factor 1(IGF-1),insulin-like growth factor binding protein 3(IGFBP-3),and the incidence of adverse drug reactions were compared between the two groups. Results There were 63 cases in control group and 63 cases in treatment group. The height of the children in treatment group and control group after treatment were(146.48±6.76)and(138.62±4.95)cm;the HtSDS values were-1.72±0.18 and-1.97±0.20;the IGF-1 values were(158.86±18.24)and(113.14±15.88)ng·mL-1;IGFBP-3 values were(5.21±0.83)and(3.13±0.71)μg·mL-1,the differences were all statistically significant(all P<0.05).The incidence of adverse drug reaction in treatment group and control group were 9.52%(6 cases/63 cases)and 3.17%(2 cases/63 cases),with no statistically significant difference(P>0.05).Conclusion Recombinant human growth hormone has a definite clinical efficacy, high safety, and effective promotion of growth and development in the treatment of primary nephrotic syndrome in children with dwarfism.
