Traditional Chinese Medicine (TCM), as a treasure of the Chinese nation, plays a significant role in maintaining public health. In 2019, the Central Committee of the Communist Party of China and the State Council proposed for the first time the establishment of a TCM registration and evaluation evidence system that integrates TCM theory, "personal experience" and clinical trials (referred to as the "Three-in-One" System) to promote the inheritance and innovation of TCM. Subsequently, the National Medical Products Administration issued several guiding principles to advance the improvement and implementation of this system. Owing to the complexity of its implementation, there are still differing understandings within the TCM industry regarding the positioning of the "Three-in-One" Registration and Evaluation Evidence System, as well as the connotation and value orientation of the "personal experience." To address this, Academician WANG Qi, President of the TCM Association, China International Exchange and Promotion Association for Medical and Healthcare and TCM master, led a group of academicians, TCM masters, TCM pharmacology experts and clinical TCM experts to convene a "Seminar on Promoting the Implementation of the ′Three-in-One′ Registration and Evaluation Evidence System for Chinese Medicinals." Through extensive discussions, an expert consensus was formed, clarifying the different roles of the TCM theory, "personal experience" and clinical trials within the system. It was further emphasized that the "personal experience" is the core of this system, and its data should be derived from clinical practice scenarios. In the future, the improvement of this system will require collaborative efforts across multiple fields to promote the high-quality development of the Chinese medicinal industry.

ObjectiveTo explore the effect of Buzhong Yiqitang on the immune imbalance of helper T cell 17 (Th17)/regulatory T cell (Treg) and Notch1 signaling pathway in mice with autoimmune thyroiditis (AIT). MethodA total of 60 8-week-old NOD.H-2h4 mice were randomly divided into the normal group, model group, western medicine group (selenium yeast tablet, 32.5 mg·kg-1), and low-dose (4.78 g·kg-1·d-1), middle-dose (9.56 g·kg-1·d-1), and high-dose (19 g·kg-1·d-1) Buzhong Yiqitang groups, with 10 mice in each group. The normal group was fed with distilled water, and the other groups were fed with water containing 0.05% sodium iodide for eight weeks. After the animal model of AIT was formed spontaneously, the mice were killed under anesthesia after intragastric administration for eight weeks. Serum anti-thyroglobulin antibodies (TGAb), thyroid-stimulating hormone (TSH), free triiodothyronine (FT3), and free thyroid hormone (FT4) were detected by enzyme-linked immunosorbent assay (ELISA), and thyroid tissue changes were observed by hematoxylin-eosin (HE) staining. The mRNA and protein expressions of retinoid-related orphan receptor-γt (RORγt), interleukin (IL)-17, forkhead box P3 (FoxP3), IL-10, Notch1, and hair division-related enhancer 1 (Hes1) in thyroid tissue were detected by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) and Western blot, respectively. ResultCompared with the normal group, the thyroid structure of the model group was severely damaged, and lymphocytes were infiltrated obviously. The levels of serum TGAb, FT3, and FT4 contents were significantly increased, and TSH content was significantly decreased (P<0.01). The mRNA and protein expression levels of RORγt, IL-17, Notch1, and Hes1 were significantly increased, while those of FoxP3 and IL10 were significantly decreased in the model group (P<0.01). Compared with the model group, thyroid structural damage and lymphocyte infiltration were improved in the treatment groups, and serum TGAb, FT3, and FT4 contents were significantly decreased. TSH content was increased, and mRNA and protein expression levels of RORγt, IL-17, Notch1, and Hes1 were decreased. mRNA and protein expression levels of FoxP3 and IL-10 were increased to different degrees (P<0.05, P<0.01), and the middle-dose Buzhong Yiqitang group had the most significant intervention effect. ConclusionBuzhong Yiqitang can alleviate the thyroid structural damage in AIT mice, and its mechanism may be related to improving the abnormal differentiation of Th17/Treg immune cells and inhibiting the activation of the Notch1 signaling pathway.
4.Effect of resistance training on neck pain and neck function in aircraft carrier student pilots

Objective Motortherapy and neck muscle resistance training are used to study the effects of resistance training on neck pain level and surface myoelectric activity of student pilots of carrier aircraft.Methods We conducted an intervention experiment on the neck pain of the carrier-based aircraft student pilots.The experiment period was 3 months.The experimental subjects were male carrier-based aircraft student pilots aged from 23 to 35.Questionnaires,including the Cervical Dysfunction Index and the Chinese Physical Activity Questionnaire,were issued,followed by cervical muscle function tests,including cervical maximum muscle strength,cervical range of motion,and surface electromyography tests.The experimental subjects were randomly divided into the control group and the resistance training group,with 8 in each.The subjects in the control group recieved usual military training.Those in the resistance training group recieved targeted training programs developed by using the self-developed cervical spondylosis prevention training device in addition to usual military training.The programs were carried out for resistance training three times a week for 20 minutes in each.Cervical maximum muscle strength,cervical range of motion,and surface electromyography were tested again before the end of the intervention experiment.The data were analyzed by SPSS 22.0.The results were expressed by mean±standard deviation((-x)±s).P<0.05 was considered statistically significant.Results(1)After 12-week intervention,resistance training had a significant effect on visual pain analog scale(VAS).(2)After the intervention,the comparison results of the subjects'cervical spine dysfunction index(NDI)before and after their own tests showed that the level of cervical spine dysfunction was significantly improved.(3)After the intervention,the results of inter-group comparison of the subjects'own joint motion showed that the self-variation difference of the subjects in the resistance training group was significantly different in forward flexion,backward extension,left flexion,right flexion,right rotation and left rotation.(4)After the intervention,the comparison results between the two groups of subjects'own neck muscle strength showed that there was no significant difference in the self-change of the six movements of subjects in the resistance training group.(5)The root mean square(RMS)values in the four movement tests of flexion,extension,flexion and flexion before and after the intervention in the resistance training group showed no significant difference between the left and right sternocleidomastoid muscle and the right and right trapezius muscle.(6)The integrated electromyography(iEMG)in the four movement tests of flexion,extension,flexion and flexion before and after the intervention in the resistance training group showed no significant difference between the left and right sternocleidomastoid muscle and the right and right trapezius muscle superior bundle.Conclusion After 12 weeks of resistance training,VAS scores and NDI scores of resistance training subjects are significantly reduced,indicating that resistance training can alleviate neck pain and improve cervical spine dysfunction.12-week resistance training can enhance the strength of neck muscles of carrier-based aircraft student pilots and improve the anti-fatigue ability of neck muscles.Meanwhile,it can enhance the range of motion of neck joints,improve the load of the deep and shallow muscle fibers of the four tested muscles in the neck and optimize the activation mode of the deep and shallow muscles in the neck.
5.Intervention effect of badminton exercise on neck pain in aircraft carrier student pilots

Objective To explore the intervention effect of badminton exercise on neck pain in naval carrier-based aircraft flight trainees,and provide data support for the prevention and relief of neck pain in these trainees.Methods A total of 16 naval carrier-based aircraft flight trainees were selected as subjects based on inclusion and exclusion criteria,and randomly divided into the sports rehabilitation group and the control group,with eight participants in each group.The sports rehabilitation group engaged in badminton exercise three times a week for one hour each time,in addition to their regular military training.The control group received no other interventions apart from their regular military training.Neck Disability Index(NDI),Visual Analog Scale(VAS)for pain simulation,Cervical Range of Motion(CROM),maximum neck strength,Root Mean Square(RMS)value of surface electromyographic signals,and integrated electromyography(iEMG)values were used as evaluation indexes for the intervention effect of neck pain.Results There was a significant difference in NDI scores among members of the sports rehabilitation group before and after intervention(P<0.05).However,there was no significant difference observed in NDI scores among members of the control group before and after intervention(P>0.05).Furthermore,there was a significant difference in VAS scores among members of the sports rehabilitation group before and after intervention(P<0.01).The VAS scores of the control group members showed no significant difference compared to pre-intervention testing(P>0.05).The maximum range of motion in all directions increased for the exercise rehabilitation group,with significant differences observed in left and right flexion(P<0.05),but no significant differences were found in other directions(P>0.05)compared to pre-intervention levels.There was no significant change in maximum range of motion within the control group before and after intervention(P>0.05).The exercise rehabilitation group demonstrated a significant increase in neck muscle strength during forward flexion,backward extension,left flexion(P<0.01),and right flexion as well as left and right rotation values(P<0.05)compared to pre-intervention levels.There was no significant difference observed between RMS values on both sides of the sternocleidomastoid muscles during movement in four different directions for the exercise rehabilitation group(P>0.05).However,there was a significant difference observed on the left side of the upper trapezius muscle during forward and rightward movements(P<0.05),but not during backward or leftward movements(P>0.05).No statistically significant differences were found among any direction's iEMG values before or after intervention for four muscles tested except that iEMG value increased slightly without significance after intervention when performing forward bending,backward extension and rightward bending exercises within exercise rehabilitation groups(P>0.05).Conclusion After badminton training,neck pain among carrier-based fighter pilots is allivated while cervical dysfunction recovers somewhat.Furthermore,their neck flexibility increases along with their neck muscle strength while deep muscles are activated to maintain stability to effectively enhance fatigue resistance capacity within these muscle groups around their necks.

Non-alcoholic fatty liver disease(NAFLD)is a common chronic liver disease characterized by excess fat accumulation within liver cells.The main causes include obesity,diabetes,and hyperlipidemia.In recent years,NAFLD and other metabolic diseases have become global public health issues.Although some progress has been made in the drug treatment of NAFLD,the efficacy is limited and there are many adverse effects.As a treatment method with high safety and few adverse effects,exercise therapy has good application prospects in the treatment of NAFLD and other metabolic diseases.However,challenges remain in overcoming patients'low exercise compliance and in finding safe and effective exercise therapy drug targets.This article explores the mechanisms and application prospects of exercise therapy in the treatment of NAFLD and other metabolic diseases,summarizes the energy consumption,metabolic pathways,and inter-organ communication induced by exercise,aiming to provide useful references for clinical practitioners.
7.Xuebijing improves intestinal microcirculation dysfunction in septic rats by regulating the VEGF-A/PI3K/Akt signaling pathway

BACKGROUND:This study aims to explore whether Xuebijing(XBJ)can improve intestinal microcirculation dysfunction in sepsis and its mechanism. METHODS:A rat model of sepsis was established by cecal ligation and puncture(CLP).A total of 30 male SD rats were divided into four groups:sham group,CLP group,XBJ+axitinib group,and XBJ group.XBJ was intraperitoneally injected 2 h before CLP.Hemodynamic data(blood pressure and heart rate)were recorded.The intestinal microcirculation data of the rats were analyzed via microcirculation imaging.Enzyme-linked immunosorbent assay(ELISA)kits were used to detect the serum levels of interleukin-6(IL-6),C-reactive protein(CRP),and tumor necrosis factor-α(TNF-α)in the rats.Histological analysis and transmission electron microscopy were used to analyze the injury of small intestinal microvascular endothelial cells and small intestinal mucosa in rats.The expression of vascular endothelial growth factor A(VEGF-A),phosphoinositide 3-kinase(P13K),phosphorylated P13K(p-PI3K),protein kinase B(Akt),and phosphorylated Akt(p-Akt)in the small intestine was analyzed via Western blotting. RESULTS:XBJ improved intestinal microcirculation dysfunction in septic rats,alleviated the injury of small intestinal microvascular endothelial cells and small intestinal mucosa,and reduced the systemic inflammatory response.Moreover,XBJ upregulated the expression of VEGF-A,p-PI3K/total P13K,and p-Akt/total Akt in the rat small intestine. CONCLUSION:XBJ may improve intestinal microcirculation dysfunction in septic rats possibly through the VEGF-A/PI3K/Akt signaling pathway.
8.Changes in the microstructure and bone mineral density of vertebral tra-becular bone in the early stages of spinal Mycobacterium tuberculosis in-fection

Objective To observe and compare the changes of vertebral bone mineral density(BMD)in the early stages of spinal Mycobacterium tuberculosis infection.Methods Patients who underwent spinal surgery at Xiangya Hospital,Central South University from January 1 to December 31,2023 were continuously enrolled(spinal tuber-culosis group),based on gender matching,non-spinal tuberculosis surgical patients treated for spinal stenosis were selected as the control group.Dual-energy X-ray scans were performed on the enrolled patients,difference in verte-bral BMD between two groups of patients was compared.An animal model of spinal Mycobacterium tuberculosis in-fection(referred to as the animal model)was constructed,differences in microstructure of trabecular bone between spinal tuberculosis group and control group was compared,and the bone volume/tissue volume(BV/TV),the thickness of trabecular bone(Tb.Th),the number of trabecular bone(Tb.N),and sparse density of trabecular(Tb.Sp)were used as evaluation indexes to further analyze the bone quality differences between the diseased verte-brae and the neighboring vertebrae.Results 69 patients were included in the spinal tuberculosis group and the con-trol group,respectively.The BMD of patients in the spinal tuberculosis group(0.793[0.712,0.869]g/cm2)was lower than that of the control group(0.907[0.800,1.020]g/cm2),difference was statistically significant(P<0.05).Microstructure of trabecular bone BV/TV([18.4±5.4]%),Tb.Th([0.124±0.010]mm)in the spinal tuberculosis group of animal model were significantly altered compared with BV/TV([22.6±3.2]%),Tb.Th([0.160±0.017]mm)in the control group(both P<0.05).In the spinal tuberculosis group,microstructure of diseased vetebral trabecular bone BV/TV([25.5±6.7]%)and Tb.N([1.871±0.443]/mm)were significantly lower than BV/TV([26.6±6.8]%)and Tb.N([1.969±0.454]/mm)in the neighboring vertebrae,both with statistically difference(both P<0.05).Conclusion In the early stages of spinal Mycobacterium tuberculosis infec-tion,microstructure of vertebral trabecular bone can be altered,leading to a decrease in BMD.

Objective To monitor the susceptibility of clinical isolates to antimicrobial agents in tertiary hospitals in major regions of China in 2022.Methods Clinical isolates from 58 hospitals in China were tested for antimicrobial susceptibility using a unified protocol based on disc diffusion method or automated testing systems.Results were interpreted using the 2022 Clinical &Laboratory Standards Institute(CLSI)breakpoints.Results A total of 318 013 clinical isolates were collected from January 1,2022 to December 31,2022,of which 29.5%were gram-positive and 70.5%were gram-negative.The prevalence of methicillin-resistant strains in Staphylococcus aureus,Staphylococcus epidermidis and other coagulase-negative Staphylococcus species(excluding Staphylococcus pseudintermedius and Staphylococcus schleiferi)was 28.3%,76.7%and 77.9%,respectively.Overall,94.0%of MRSA strains were susceptible to trimethoprim-sulfamethoxazole and 90.8%of MRSE strains were susceptible to rifampicin.No vancomycin-resistant strains were found.Enterococcus faecalis showed significantly lower resistance rates to most antimicrobial agents tested than Enterococcus faecium.A few vancomycin-resistant strains were identified in both E.faecalis and E.faecium.The prevalence of penicillin-susceptible Streptococcus pneumoniae was 94.2%in the isolates from children and 95.7%in the isolates from adults.The resistance rate to carbapenems was lower than 13.1%in most Enterobacterales species except for Klebsiella,21.7%-23.1%of which were resistant to carbapenems.Most Enterobacterales isolates were highly susceptible to tigecycline,colistin and polymyxin B,with resistance rates ranging from 0.1%to 13.3%.The prevalence of meropenem-resistant strains decreased from 23.5%in 2019 to 18.0%in 2022 in Pseudomonas aeruginosa,and decreased from 79.0%in 2019 to 72.5%in 2022 in Acinetobacter baumannii.Conclusions The resistance of clinical isolates to the commonly used antimicrobial agents is still increasing in tertiary hospitals.However,the prevalence of important carbapenem-resistant organisms such as carbapenem-resistant K.pneumoniae,P.aeruginosa,and A.baumannii showed a downward trend in recent years.This finding suggests that the strategy of combining antimicrobial resistance surveillance with multidisciplinary concerted action works well in curbing the spread of resistant bacteria.
10.In Vitro Amplification of NK Cells from Feeder Layer Cells Expressing IL-21

Objective:To investigate the effect of feeder layer cells expressing interleukin(IL)-21 on the amplification of NK cells in vitro.Methods:The K562 cell line with IL-21 expression on its membrane was constructed by electroporation,and co-cultured with NK cells after inactivation.The proliferation of NK cells was observed.The killing function of the amplified NK cells in vitro was evaluated by the lactate dehydrogenase(LDH)and interferon-γ(IFN-y)release assay.A colorectal cancer xenograft model in NOD/SCID mice was established,and a blank control group,a NK cell group and an amplified NK cell group were set up to detect the tumor killing effect of amplified NK cells in vivo.Results:K562 cells expressing IL-21 on the membrane were successfully constructed by electroporation.After co-culturing with K562 cells expressing IL-21 on the membrane for 17 days,the NK cells increased to 700 times,which showed an enhanced amplification ability compared with control group(P<0.001).In the tumor cell killing experiment in vitro,there was no significant difference in the killing activity on tumor cells between NK cells and amplified NK cells,and there was also no significant difference in mice in vivo.Conclusion:K562 cells expressing IL-21 on the membrane can significantly increase the amplification ability of NK cells in vitro,but do not affect the killing function of NK cells in vitro and in vivo.It can be used for the subsequent large-scale production of NK cells in vitro.
