1.Effectiveness of intercostal nerve block and analgesia with bupivacaine liposomes in single-port thoracoscopic lung surgery: A prospective study

Objective To compare the analgesic effect, duration and incidence of adverse reactions of liposome bupivacaine (LB) and bupivacaine hydrochloride after intercostal nerve block in single-port thoracoscopic lung surgery. Methods In Department of Thoracic Surgery of the First Affiliated Hospital of Xinxiang Medical University between September 2023 and March 2024, 228 patients who needed to undergo thoracoscopic lung surgery were selected and divided into two groups by random number table method: a group B with bupivacaine hydrochloride (n=118), and a group LB with LB (n=110). Intraoperative intercostal nerve block was performed under endoscopy, and the time of first use of analgesic drugs after surgery, cumulative use of opioids 72 h after surgery, incidence of postoperative nausea and vomiting, length of stay and other indicators were evaluated and recorded. Results Visual analogue scale (VAS) scores at 4 h, 8 h, 12 h, 24 h, 48 h and 72 h in the LB group were significantly lower than those in the group B (P<0.05). The total number of activities within 48 h after surgery in the group B was significantly lower than that in the LB group (P<0.05), and the postoperative hospitalization stay in the LB group was shorter than that in the group B, but the difference was not statistically significant. There was no statistical difference between the two groups in postoperative adverse reactions. Conclusion Intercostal nerve block with LB during single-port thoracoscopic lung surgery can significantly reduce postoperative pain, improve quality of life, and promote recovery of the patients. It is worthy of clinical application.
2.Scoping review of research on social isolation in lung cancer survivors

Objective:To conduct a scoping review of the current research status, influencing factors, and assessment tools regarding social isolation in lung cancer survivors and provide research ideas for clinical medical and nursing staff to develop interventional measures.Methods:Based on the framework of a scoping review, systematic searches were conducted in databases including CNKI, Wanfang, VIP, CBM, Embase, PubMed, Web of Science, CINAHL, and Cochrane Library for studies on social isolation in lung cancer survivors, up to March 18, 2023.Results:Eighteen articles were included, with research distributed across three continents (Asia, North America, Europe) and in seven countries, including China, the USA, and Spain. Frequently used research tools included the University of California Los Angeles Loneliness Scale and the Cancer Loneliness Scale. Rates and severity of social isolation varied across different studies. Factors influencing social isolation in lung cancer survivors included demographic factors, disease-related factors, symptom factors, and psychosocial factors.Conclusions:Social isolation in lung cancer survivors can affect their quality of life to a certain extent. Although there are various tools for assessing social isolation in lung cancer survivors, there is a general lack of specific assessment tools. This suggests that medical and nursing personnel should consider developing and applying more specific research tools when developing related assessment tools in the future and develop personalized intervention measures based on influencing factors to help patients alleviate social isolation.
3.Epidemiological and etiological characteristics of active surveillance of foodborne illness

Objective To realize the epidemiological and pathogenic characteristics of foodborne illness in Shiyan in 2019 and provide targeted measures for the prevention and control work of the foodborne illness. Methods The active surveillance data of foodborne illness in shiyan in 2019 were collected and analyzed. The Salmonella strains isolated from the patients with foodborne illness were tested with serotyping,PFGE molecular classification and drug sensitivity test respectively according to《National foodborne illness surveillance workbook 2019》. Result A total of 3 191 cases of foodborne illness were analyzed. The pre-school children were the highest-risk group of foodborne diseases. Grain and grain product was the main suspicious contaminated food.The main location of eating place was family. A total of 330 samples were tested. The positive detection rate of foodborne illness was 20.30%(73/330).The main pathogen were Rotavirus and Salmonella,and the detection rates were 11.21%(37/330) and 4.85%(16/330) respectively. The pre-school children were the group with the highest detection rate, which was 57.27% (63/110). The dominant serotype of the 14 Salmonella strains isolated from patients were Salmonella typhimurium and Salmonella enteritis. 14 different genotypes were obtained through PFGE classification and clustering. 64.29% (9/14) of Salmonella strains were multidrug-resistant strains. The drug resistance rates of ampicillin and tetracycline were the highest, which were 71.43%(11/14)and 64.29%(10/14)respectively. Conclusions The publicity and education of food safety should be strengthened and the surveillance network system of food-borne disease should be further improved according to the epidemiological and pathogenic characteristics of foodborne illness in Shiyan in 2019, and the use of clinical antibiotics should be controlled reasonably as well.
4.A scoping review of application of blood flow restriction training in rehabilitation of patients undergoing anterior cruciate ligament reconstruction

Objective:To review the scope of research on the application of blood flow restriction training in the rehabilitation of patients undergoing anterior cruciate ligament (ACL) reconstruction.Methods:According to the research method of scoping review, computer searches were conducted on China National Knowledge Infrastructure, Wanfang Database, China Biology Medicine disc, VIP, Cochrane Library, PubMed, Embase, with the search period was from eatablishment of databases to December 31, 2022. The included literatures were screened, summarized and analyzed.Results:A total of 16 literatures were included, all of which were randomized controlled trials. The basic content of blood flow restriction training intervention included 8 aspects, such as intervention time, training intensity, training amount, training mode, frequency, interval time, training cycle and blood flow restriction pressure. The effect evaluation mainly involved two kinds of indexes, namely safety indicators and efficacy indicators.Conclusions:Blood flow restriction training can effectively enhance muscle strength and improve knee joint function in patients undergoing ACL reconstruction. Future studies should focus on exploring the best intervention strategy, formulating standardized and unified evaluation criteria and providing the best blood flow restriction training program for patients undergoing ACL reconstruction.
5.Analysis of the status quo and influencing factors of nurse′s knowledge, attitude and practice line on adult oxygen inhalation therapy

Objective:To investigate the knowledge, attitude and practice of adult oxygen inhalation therapy among nurses and analyze its influencing factors.Methods:A self-designed questionnaire was used to investigate the status of knowledge, attitude and practice of adult oxygen inhalation therapy among 1 410 nurses from many tertiary, secondary and lower hospitals in Chongqing, Jiangsu, Sichuan, Shanxi and other provinces and cities from December 2020 to January 2021 by convenient sampling method.Results:The score of nurses′ knowledge of adult oxygen inhalation therapy was 1-39 (23.10 ±4.82) points, the standard score was (44.42 ±9.27) points, the score of excellent was 0, the score of moderate was 1.8%(25/1 410), the score of poor was 98.2%(1 385/1 410). The score of nurses′ attitude of adult oxygen inhalation therapy was 6-30 (25.06 ±4.37) points, the standard score was (83.53 ±14.58) points, positive attitude accounted for 42.9%(605/1 410), neutral attitude accounted for 54.6%(770/1 410), negative attitude accounted for 2.5%(35/1 410). The score of nurses′ practice of adult oxygen inhalation therapy was 9-54 (33.82 ±6.32) points, and the standard score was (75.12 ±13.99). Positive practice accounted for 21.2%(299/1 410), neutral practice accounted for 70.4%(993/1410) points, negative practice accounted for 8.4%(118/1 410). The knowledge and attitude of nurses to adult oxygen inhalation therapy were positively correlated with practice( r=0.193, 0.554, both P<0.01). Multiple linear regression analysis showed that nurses′ knowledge, attitude and training frequency were the influencing factors of adult oxygen inhalation therapy practice( t=3.17, 23.33, 8.64, all P<0.01). Conclusions:Nurses should strengthen the study and training of adult oxygen inhalation therapy, correct attitude and improve practice, reduce or avoid the occurrence of excessive oxygen therapy and oxygen therapy complications, standardize clinical oxygen therapy to ensure the safety and effectiveness of oxygen therapy.

Netrin-1, an axon guidance factor, and its receptor UNC5B play important roles in axonal development and angiogenesis. This study examined netrin-1 and UNC5B expression in kidneys with diabetic kidney disease (DKD) and investigated their roles in angiogenesis. Netrin-1 and UNC5B were upregulated in streptozotocininduced DKD Wistar rats, and their expression was compared with that in healthy controls. However, exogenous netrin-1 in UNC5B-depleted human renal glomerular endothelial cells (HRGECs) inhibited cell migration and tubulogenesis. This effect was likely associated with SRC pathway deactivation. Netrin-1 treatment also eliminated the pro-angiogenic effects of exogenous VEGF-165 on UNC5B-silenced HRGECs. These results indicate that UNC5B antagonizes netrin-1 and that UNC5B upregulation contributes partly to enhancing angiogenesis in DKD. Therefore, introducing exogenous netrin-1 and depleting endogenous UNC5B are potential strategies for reducing the incidence of early angiogenesis and mitigating kidney injury in DKD.
7.Mechanism of the occurrence and development of leukemia regulated by intestinal microecology

The intestinal flora in patients with leukemia is significantly different from healthy people. Some studies have found that intestinal flora can participate in leukemia progression by regulating the body's immune system and hematopoietic system and affecting the metabolic function, but the underlying molecular mechanism is not clear. Maintaining the intestinal microecological balance and exploring intestinal microbes that can be used as therapeutic targets have a profound significance for prolonging the survival of leukemia patients.
8.Domestic imatinib in treatment of chronic myeloid leukemia: a real world multicenter clinical research

Objective:To investigate the clinical therapeutic efficacy and safety of domestic imatinib mesylate in treatment of chronic myeloid leukemia (CML).Methods:The clinical data of 87 CML-chronic phase patients treated by domestic imatinib between December 2013 and March 2018 in the Affiliated Hospital of the Southwest Medical University were retrospectively analyzed. The clinical therapeutic efficacy and safety of imatinib was analyzed.Results:The completely hematologic reaction (CHR) rate was 95.4% (83/87), 97.7% (85/87) and 100.0% (87/87), respectively after treatment at 3, 6 and 12 months. The completely cytogenetic reaction (CCyR) rate was 31.0% (27/87), 42.5% (37/87) and 64.4% (56/87), respectively at 3, 6 and 12 months. At 3-month, BCR-ABL/ABL IS value was equal to or less than 10% in 44 patients (50.6%), BCR-ABL/ABL IS value was equal to or less than 0.1% in 4 patients (4.6%). At 6-month, BCR-ABL/ABL IS value was equal to or less than 1% in 39 patients (44.8%), BCR-ABL/ABL IS value was equal to or less than 0.1% in 13 patients (14.9%). The major molecular response rate at 12-month was 35.6% (31/87). The major hematologic adverse reaction was grade Ⅰ-Ⅱ hemacytopenia. The common non-hematology adverse reactions were edema, muscular and joints soreness, nausea and vomiting and diarrhea. Most patients didn't have severe adverse reactions. Conclusions:The clinical efficacy of CML patients treated with domestic imatinib is sure; CML patients are well-tolerated and the adverse reactions are mild. It is worthy of wide application in clinical treatment.
9.Qualitative research on clinical practice training experience of respiratory specialist nurses in Chongqing

Objective To know the perception and experience of respiratory specialized nurses on clinical practice training,and provide references and basis for perfecting the scheme of clinical practice training in further.Methods Personal in-depth interviews were conducted on 16 graduated respiratory specialized nurses and the subjects were refined as well.Results Six themes were obtained:clinical practice training time was insufficient;training content was rich but lack of novelty;flexible and diverse forms of training were expected;qualifications for the clinical teachers were required;traditional single clinical competency assessment methods,and specialist knowledge,the clinical skills,scientific research,teaching abilities could be improved.Conclusion In order to cultivate high-quality respiratory specialized nurses,clinical practice training scheme should be further modified and perfected.
10.Application effect of measuring central venous pressure with different catheter in LBWIs

Objective To discuss the effect of measuring central venous with peripherally inserted central catheter (PICC) in low birth weight infants (LBWIs).Methods Totally 100 LBWIs admitted in the intensive care unit ( ICU) of a tertiary hospital from January to Octomber 2015 were selected and were randomly divided into the PICC group and the central venous catheterization ( CVC) group, with 50 cases in each group. Patients in the PICC group received the main intravenous catheter, while patients in the CVC group received subclavian intravenous catheter. Data were collected for statistical analysis. The difference of central venous pressure and the incidence rate of complications in patients with different measure methods were compared between two groups.Results The central venous pressure of LBWIs in the PICC group was (8.26±2.18) cmH2O, and in the CVC group was (7.85±2.36) cmH2O (t=0.421,P>0.05); the incidence rates of catheter obstruction, phlebitis and puncture point bleeding of the PICC group were lower than that of the CVC group (χ2=5.01,6.93, 4.00;P<0.05).Conclusions PICC catheter measurement won′t affect the measure results of central venous pressure in LBWIs,and can reduce the incidence of complications related with central venous catheter in very low LBWIs, so it can be popularized for use.
