1.4 Weeks of HIIT Modulates Metabolic Homeostasis of Hippocampal Pyruvate-lactate Axis in CUMS Rats Improving Their Depression-like Behavior

		                        			ObjectiveTo investigate the role of 4-week high-intensity interval training (HIIT) in modulating the metabolic homeostasis of the pyruvate-lactate axis in the hippocampus of rats with chronic unpredictable mild stress (CUMS) to improve their depressive-like behavior. MethodsForty-eight SPF-grade 8-week-old male SD rats were randomly divided into 4 groups: the normal quiet group (C), the CUMS quiet group (M), the normal exercise group (HC), and the CUMS exercise group (HM). The M and HM groups received 8 weeks of CUMS modeling, while the HC and HM groups were exposed to 4 weeks of HIIT starting from the 5th week (3 min (85%-90%) Smax+1 min (50%-55%) Smax, 3-5 cycles, Smax is the maximum movement speed). A lactate analyzer was used to detect the blood lactate concentration in the quiet state of rats in the HC and HM groups at week 4 and in the 0, 2, 4, 8, 12, and 24 h after exercise, as well as in the quiet state of rats in each group at week 8. Behavioral indexes such as sucrose preference rate, number of times of uprightness and number of traversing frames in the absenteeism experiment, and other behavioral indexes were used to assess the depressive-like behavior of the rats at week 4 and week 8. The rats were anesthetized on the next day after the behavioral test in week 8, and hippocampal tissues were taken for assay. LC-MS non-targeted metabolomics, target quantification, ELISA and Western blot were used to detect the changes in metabolite content, lactate and pyruvate concentration, the content of key metabolic enzymes in the pyruvate-lactate axis, and the protein expression levels of monocarboxylate transporters (MCTs). Results4-week HIIT intervention significantly increased the sucrose preference rate, the number of uprights and the number of traversed frames in the absent field experiment in CUMS rats; non-targeted metabolomics assay found that 21 metabolites were significantly changed in group M compared to group C, and 14 and 11 differential metabolites were significantly dialed back in the HC and HM groups, respectively, after the 4-week HIIT intervention; the quantitative results of the targeting showed that, compared to group C, lactate concentration in the hippocampal tissues of M group, compared with group C, lactate concentration in hippocampal tissue was significantly reduced and pyruvate concentration was significantly increased, and 4-week HIIT intervention significantly increased the concentration of lactate and pyruvate in hippocampal tissue of HM group; the trend of changes in blood lactate concentration was consistent with the change in lactate concentration in hippocampal tissue; compared with group C, the LDHB content of group M was significantly increased, the content of PKM2 and PDH, as well as the protein expression level of MCT2 and MCT4 were significantly reduced. The 4-week HIIT intervention upregulated the PKM2 and PDH content as well as the protein expression levels of MCT2 and MCT4 in the HM group. ConclusionThe 4-week HIIT intervention upregulated blood lactate concentration and PKM2 and PDH metabolizing enzymes in hippocampal tissues of CUMS rats, and upregulated the expression of MCT2 and MCT4 transport carrier proteins to promote central lactate uptake and utilization, which regulated metabolic homeostasis of the pyruvate-lactate axis and improved depressive-like behaviors. 
		                        		
		                        		
		                        		
		                        	
2.Four Weeks of HIIT Modulates Lactate-mediated Synaptic Plasticity to Improve Depressive-like Behavior in CUMS Rats

		                        			ObjectiveThis study aimed to investigate the effects of 4-week high-intensity interval training (HIIT) on synaptic plasticity in the prefrontal cortex (PFC) of rats exposed to chronic unpredictable mild stress (CUMS), and to explore its potential mechanisms. MethodsA total of 48 male Sprague-Dawley rats were randomly divided into 4 groups: control (C), model (M), control plus HIIT (HC), and model plus HIIT (HM). Rats in groups M and HM underwent 8 weeks of CUMS to establish depression-like behaviors, while groups HC and HM received HIIT intervention beginning from the 5th week for 4 consecutive weeks. The HIIT protocol consisted of repeated intervals of 3 min at high speed (85%-90% maximal training speed, Smax) alternated with one minute at low speed (50%-55% Smax), with 3 to 5 sets per session, conducted 5 d per week. Behavioral assessments and tail-vein blood lactate levels were measured at the end of the 4th and 8th weeks. After the intervention, rat PFC tissues were collected for Golgi staining to analyze synaptic morphology. Enzyme-linked immunosorbent assays (ELISA) were employed to detect brain-derived neurotrophic factor (BDNF), monocarboxylate transporter 1 (MCT1), lactate, and glutamate levels in the PFC, as well as serotonin (5-HT) levels in serum. Additionally, Western blot analysis was conducted to quantify the expression of synaptic plasticity-related proteins, including c-Fos, activity-regulated cytoskeleton-associated protein (Arc), and N-methyl-D-aspartate receptor 1 (NMDAR1). ResultsCompared to the control group (C), the CUMS-exposed rats (group M) exhibited significant reductions in sucrose preference rates, number of grid crossings, frequency of upright postures, and entries into and duration spent in open arms of the elevated plus maze, indicating marked depressive-like behaviors. Additionally, the group M showed significantly reduced dendritic spine density in the PFC, along with elevated levels of c-Fos, Arc, NMDAR1 protein expression, and increased concentrations of lactate and glutamate. Conversely, BDNF and MCT1 contents in the PFC and 5-HT levels in serum were significantly decreased. Following HIIT intervention, rats in the group HM displayed considerable improvement in behavioral indicators compared with the group M, accompanied by significant elevations in PFC MCT1 and lactate concentrations. Furthermore, HIIT notably normalized the expression levels of c-Fos, Arc, NMDAR1, as well as glutamate and BDNF contents in the PFC. Synaptic spine density also exhibited significant recovery. ConclusionFour weeks of HIIT intervention may alleviate depressive-like behaviors in CUMS rats by increasing lactate levels and reducing glutamate concentration in the PFC, thereby downregulating the overexpression of NMDAR, attenuating excitotoxicity, and enhancing synaptic plasticity. 
		                        		
		                        		
		                        		
		                        	
3.Study on chemical composition,pharmacodynamic consistency and mechanism between Hugan qingzhi formula decoction and its formulated granules

		                        			OBJECTIVE To evaluate the contents of characteristic components in Hugan qingzhi formula (HGQZ) decoction and formulated granules and the pharmacodynamic consistency of them on high-fat diet-induced non-alcoholic fatty liver disease (NAFLD) model mice, and explore their potential underlying mechanisms of action. METHODS Liquid chromatography-tandem mass spectrometry was used to analyze and compare the contents of six characteristic components in HGQZ decoction and formulated granules. Male C57BL/6 mice were randomly divided into normal control group, model group, HGQZ decoction low- dose and high-dose groups (13, 26 g/kg, calculated by crude drugs), and HGQZ formulated granules low-dose and high-dose groups (13, 26 g/kg, calculated by crude drugs), with 6 mice in each group. Except for the normal control group, which was fed a regular diet, the mice in the other groups were fed a high-fat diet for 20 weeks to establish the NAFLD model; at the same time, the mice in each group were gavaged with the corresponding drugs/water once. The fasting blood glucose (FBG) levels, glucose and insulin tolerance, body weight, liver index, white adipose Δ 基金项目 国家自然科学基金项目(No.82074078);广东省基础 tissue index, brown adipose tissue index, as well as lipid levels (total cholesterol, triglycerides) and liver function indicators (aspartate transaminase, alanine transaminase) were measured. Additionally, histopathological changes and lipid accumulation in liver tissues were observed. The serum samples of mice in the model group, HGQZ decoction high-dose group and HGQZ formulated granules high-dose group were taken for metabolomics analysis, and validation of the underlying mechanisms was conducted. RESULTS There were no statistically significant differences in the contents of ginsenoside Rb1, typhaneoside, isorhamnetin-3-O-neohesperidoside, hyperoside, nuciferine, and 23-acetylalismol B between HGQZ decoction and HGQZ formulated granules (P>0.05). Compared with the model group, the hepatic histopathological changes in mice were alleviated in both the HGQZ decoction group and all dose groups of HGQZ formulated granules. Inflammatory cell infiltration and lipid vacuoles were reduced. Additionally, there was a general improvement in FBG levels, glucose tolerance, insulin tolerance, body weight, liver index, white/brown adipose tissue index, lipid levels, and liver function indicators (P<0.05). However, no statistically significant differences were observed between these treatment groups (P>0.05). There were 234 and 136 differentially expressed serum metabolites identified in the model group versus HGQZ decoction high-dose group, and model group versus HGQZ formulated granules high-dose group, respectively. After taking the intersection, 65 common differentially expressed metabolites were obtained, which were enriched in metabolic pathways such as purine metabolism and tricarboxylic acid cycle metabolism. Among these, the content of citrate in the model group was significantly lower than that in both the HGQZ decoction group and HGQZ formulated granules high-dose group (P<0.05). Both high-dose HGQZ decoction and formulated granules could significantly elevate the phosphorylation levels of AMP-activated protein kinase (AMPK) (P<0.05). CONCLUSIONS HGQZ decoction and formulated granules contain comparable amounts of characteristic components, and both exhibit equivalent efficacy on NAFLD model mice. 4.Investigation on the current status and optimization strategies for the standardized on-the-job training for community clinical pharmacists in Shanghai

		                        			OBJECTIVE To systematically investigate the current status and effectiveness of the standardized on-the-job training program for community clinical pharmacists in Shanghai, and to provide a scientific basis for optimizing the training scheme. METHODS A questionnaire survey was conducted to collect the data from trainees and mentor pharmacists who participated in the program between 2016 and 2024. The survey examined their basic information, evaluations of the training scheme, satisfaction with training outcomes, and suggestions for improvement. Statistical analyses were also conducted. RESULTS A total of 420 valid responses were collected, including 340 from trainees and 80 from mentor pharmacists. Before training, only 30.29% of trainees were engaged in clinical pharmacy-related work, whereas this proportion increased to 73.24% after training. Most mentor pharmacists had extensive experience in clinical pharmacy (76.25% with ≥5 years of experience) and mentoring (78.75% with ≥3 teaching sessions). Totally 65.59% of trainees and 55.00% of mentor pharmacists believed that blended training yielded the best learning outcomes. Over 80.00% of both trainees and mentor pharmacists considered the overall training duration, theoretical study time, and practical training time to be reasonable. More than 95.00% of trainees and mentor pharmacists agreed that the homework and assessment schemes were appropriate. Trainees rated the relevance of training content to their actual work highly (with an average relevance score >4.5), though they perceived the chronic disease medication therapy management module as significantly more challenging than the prescription review and evaluation module and the home-based pharmaceutical care module. The average satisfaction score of trainees and mentor pharmacists with the training effectiveness of each project was above 4 points, indicating a high overall satisfaction. Inadequate provision of teaching resources was unanimously recognized by trainees and mentor pharmacists as the key area requiring improvement. CONCLUSIONS The standardized on-the-job training program for community clinical pharmacists in Shanghai has contributed to improving pharmaceutical services in community healthcare settings. 5.Establishment and validation of the determination method for the related substances in carbetocin injection

		                        			An HPLC analytical method was developed to determine the related substances in carbetocin injection. The method was performed on a Waters Xbridge C18 column (150 mm×3 mm, 3.5 μm) with 0.30 mg/mL ammonium acetate-19% acetonitrile aqueous solution as mobile phase A and mobile phase A-acetonitrile (1∶1) as mobile phase B. The detection wavelength was 220 nm. Gradient elution was performed at the flow rate of 0.8 mL/min and the column temperature of 60℃. The method was validated for system applicability, specificity, linearity and range, accuracy, with the results that the 9 impurities of carbetocin injection showed good linearity (R²>0.999) with peak area in their respective concentration range, and that the method had good precision (RSD<5%). This method is suitable for the simultaneous determination of carbetocin and its 9 impurities in carbetocin injection and can provide a theoretical basis for the quality control of the carbetocin injection.

		                        			[摘  要]  目的:筛选炎症性肠病(IBD)和结直肠癌(CRC)的潜在共病基因,探讨关键基因去整合素-金属蛋白酶8(ADAM8)与IBD及CRC发生发展的关系及其潜在机制。方法:基于从GEO数据库和TCGA数据库下载IBD及CRC患者的转录组数据及对应临床资料,通过差异分析、预后基因筛选及交集分析鉴定同致共病基因。用多个数据集分析和验证关键基因ADAM8在IBD及CRC中的表达特征及其与临床病理指标的关系。生存分析探究ADAM8在CRC中的预后价值。功能和通路富集分析探究ADAM8影响CRC进展的潜在机制,进一步用肿瘤微环境算法评估ADAM8与肿瘤微环境的关系。用免疫组化(IHC)法检测中国人CRC组织中的表达验证数据库数据。结果: ADAM8为IBD和CRC的潜在关键共病基因。ADAM8在IBD和CRC组织中均呈高表达(均P < 0.01),且其高表达提示疾病病理进展(P < 0.01)。高表达ADAM8的CRC患者总生存期(OS)更短(P < 0.05或P < 0.01)。ADAM8在中国人CRC组中也呈高表达(P < 0.01)。免疫细胞迁移、细胞因子产生及免疫受体相互作用等通路与ADAM8表达密切关联,ADAM8表达与中性粒细胞、巨噬细胞等免疫细胞浸润水平呈正相关(P < 0.01或P < 0.001)。结论:ADAM8在IBD和CRC组织中呈高表达,与患者预后、疾病进展及免疫细胞浸润密切关联,有望成为2种疾病的共同干预靶点。
		                        		
		                        		
		                        		
		                        	
7.Research progress of narcolepsy

		                        			Narcolepsy(NP) is a rare central sleep disorder, and it is categorized into narcolepsy type1 (NT1) and narcolepsy type2(NT2).The main clinical symptoms of NP include daytime lethargy, cataplexy induced by emotions, and sleep paralysis, with a peak onset during adolescence (with an age of 8‒12 years). At present, the pathogenesis of NP remains unclear and is associated with various factors such as human leucocyte antigen (HLA), infection, and epigenetic silencing, and the selective loss of hypocretin neurons in the hypothalamus is the key pathophysiological mechanism of NP. Pharmacological intervention is currently the main treatment method, and diagnostic delay can be as long as more than 10 years, which not only affects the social activities, academic performance, and work of patients, but also leads to mental health issues such as anxiety and depression. 8.Exo70 Enhances Exosome Secretion by Modulating Multivesicular Body Trafficking to Promote Pancreatic Cancer Metastasis

		                        			Objective To investigate the role and molecular mechanisms of Exo70 in the metastasis of pancreatic cancer cells. Methods Stable cell lines with Exo70 knockdown or overexpression were established using lentiviral infection. The migration ability of pancreatic cancer cells was assessed by Transwell assay. The morphology, particle size, and secretion levels of exosomes were observed using transmission electron microscopy. Changes in the expression of Exo70 and exosomal marker proteins were detected by Western blot. The localization of multivesicular bodies (MVBs) was examined by immunofluorescence. Results Exo70 knockdown inhibited the migration ability of pancreatic cancer cells and substantially reduced the total amount of exosome secretion. By contrast, Exo70 overexpression enhanced the migration ability of pancreatic cancer cells. Immunofluorescence results showed that Exo70 knockdown induced the perinuclear accumulation of CD63 and LAMP2, leading to an increase in MVB accumulation and lysosomal degradation in pancreatic cancer cells. Conclusion Exo70 promotes the normal transport of MVBs and maintains exosome secretion. Its knockdown enhances the lysosomal degradation of MVBs, resulting in impaired exosome biogenesis and secretion and thereby inhibiting the migration of pancreatic cancer cells.
		                        		
		                        		
		                        		
		                        	
9.Evaluation of uncertainty in the determination of 19 elements in human urine by inductively coupled plasma mass spectrometry

		                        			Objective  To use direct dilution method to pretreat human urine, and to determine 19 elements in human urine using inductively coupled plasma mass spectrometry, and to evaluate the uncertainty of the entire experimental process.  Methods  The relevant mathematical models were established according to JJF 10591-2012 “Evaluation and Expression of Measurement Uncertainty” and CNAS-GL006 “Guidance for Evaluation of Uncertainty in Chemical Analysis”. Taking molybdenum as an example, the uncertainty sources in the determination of 19 elements in human urine by inductively coupled plasma method were analyzed and evaluated, including sample pretreatment, sample repeated measurement, standard solution preparation and standard curve fitting.  Results The extended uncertainty of molybdenum in human urine is 2.12μg/L, and the measurement result of molybdenum is (44.8±2.12)μg/L. The measurement result of 19 elements in human urine ranges from less than the detection limit to 601μg/L, and the extended uncertainty range is 0.38~33.6μg/L.Conclusion It was found from the calculation that the uncertainty of the determination result was mainly affected by the uncertainty of the sample repeated measurement and the standard curve fitting. By adjusting the range of standard curve and increasing the number of parallel sample measurement, the uncertainty was reduced and the quality of detection was improved.
		                        		
		                        		
		                        		
		                        	
10.Occurrence risk of enteral nutrition intolerance and its influencing factors in 302 elderly critically ill patients

		                        			Objective  To explore the occurrence risk of enteral nutrition intolerance and analyze its influencing factors in 302 elderly critically ill patients.  Methods The clinical case data of elderly critically ill patients in department of elderly cadres of the hospital were retrospectively analyzed from January 2019 to January 2024. According to the occurrence of enteral nutrition intolerance or not, they were divided into occurrence group (n=156) and non-occurrence group (n=146). The risk of nutritional intolerance in elderly critically ill patients was evaluated by feeding intolerance risk assessment form, and the influencing factors of enteral nutrition intolerance were analyzed by multivariate logistic regression analysis.  Results Among the 302 elderly patients with critical illness, 53.31% (161/302) had high risk of enteral nutrition intolerance, and 51.66% (156/302) had enteral nutrition intolerance. Multivariate logistic analysis revealed that CRP level>10mg/L, APACHE-II score≥20 points, Lac≥3mmol/L and hypoalbuminemia were risk factors in elderly critically ill patients (OR=1.806, 2.977, 8.232, 3.031, P=0.011, 0.001, 0.041, 0.047), and addition of dietary fiber was a protective factor for enteral nutrition intolerance (OR=1.652, P=0.037). Conclusion  The risk of enteral nutrition intolerance is high in elderly critically ill patients. Lac level, CRP level, hypoalbuminemia, and APACHE-II score of patients are independent risk factors for enteral nutrition intolerance, and addition of dietary fiber is a protective factor. It is necessary to take targeted interventions for patients according to the above factors to minimize the occurrence of enteral nutrition intolerance.
