1.Effect of Shenlong Dingji Formula (参龙定悸方) on the Quality of Life in Patients with Paroxysmal Atrial Fibrillation of Qi-Yin Deficiency and Phlegm-Stasis Obstructing Collaterals Syndrome

ObjectiveTo explore the effectiveness and safety of Shenlong Dingji Formula (参龙定悸方) on the quality of life in patients with paroxysmal atrial fibrillation (PAF) of qi-yin deficiency and phlegm-stasis obstructing collaterals syndrome. MethodsA total of 60 patients with PAF of qi-yin deficiency and phlegm-stasis obstructing collaterals syndrome were recruited and randomly divided into a treatment group and a control group, with 30 patients in each group. The control group received standard western medicine treatment, while the treatment group was additionally given Shenlong Dingji Formula orally, one dose per day. Both groups were treated for 4 weeks. The primary outcome measure is the Atrial Fibrillation Effect on Quality of Life (AFEQT) score including scores of four dimensions,i.e. atrial fibrillation-related symptoms, treatment concerns, daily activities, and treatment satisfaction. The secondary outcome measures included the frequency and duration of symptomatic atrial fibrillation episodes and traditional Chinese medicine (TCM) syndrome scores covering symptoms such as palpitations, chest tightness, fatigue, shortness of breath, reluctance to speak, spontaneous sweating, stabbing pain, and insomnia. These indicators were assessed at baseline (before treatment), after 2-week of treatment, after 4-week of treatment, and 4 weeks after the end of treatment (follow-up). Additionally, safety indicators before and after treatment and adverse events occurring during the trial were recorded to evaluate safety. ResultsA total of 56 patients completed the study, with 28 in each group. Primary outcome indicators: 1) the treatment group showed significant improvement in the total score of the AFEQT scale, with significantly higher total scores after 2-week treatment, 4-week treatment, and follow-up compared to the previous time point (P<0.05). In the control group, the AFEQT score significantly increased only after 4-week treatment compared to baseline (P<0.05). In the treatment group, the AFEQT scores after 2-week, 4-week treatment, and during follow-up were all higher than those of the control group at the corresponding time points (P<0.01). 2) In the treatment group, there was no statistically significant difference in the AFEQT treatment satisfaction dimension score during follow-up compared to that after 4-week treatment (P>0.05). However, the scores for all other dimensions at each time point were higher than those at the previous time point (P<0.05). In the control group, the scores for the atrial fibrillation-related symptom dimension were higher after 2-week and 4-week treatment than those of the previous time points (P<0.05). For the treatment satisfaction dimension, significant increases were observed only after 2-week and 4-week treatment compared to baseline (P<0.05). Secondary outcome indicators: 1) In the treatment group, the frequency and duration of symptomatic atrial fibrillation episodes decreased significantly at each time point compared to the previous time point (P<0.05), except for the duration of trial fibrillation at follow-up. In the control group, the frequency of episodes decreased significantly at all time points compared to baseline (P<0.05), while the duration of trial fibrillation showed a significant reduction at follow-up compared to those after 2-week treatment (P<0.05). 2) In the treatment group, TCM syndrome scores significantly reduced after 2-week treatment, 4-week treatment, and during follow-up compared to the previous time point and baseline (P<0.05). In the control group, significant reductions were observed only after 4-week after treatment and during follow-up (P<0.05). The TCM syndrome scores in the treatment group were lower than those in the control group at the same time points (P<0.01). No adverse events occurred during the trial in either group, and safety indicators showed no significant changes after treatment. ConclusionShenlong Dingji Formula effectively improves the quality of life, alleviates TCM syndromes, and reduces the frequency and duration of symptomatic atrial fibrillation in patients with PAF of qi-yin deficiency and phlegm-stasis obstructing collaterals syndrome, and demonstrates good safety.

Targeting multiple immune mechanisms may overcome therapy resistance and further improve cancer immunotherapy for humans. Here, we describe the application of virus-like vesicles (VLV) for delivery of three immunomodulators alone and in combination, as a promising approach for cancer immunotherapy. VLV vectors were designed to deliver single chain interleukin (IL)-12, short-hairpin RNA (shRNA) targeting programmed death ligand 1 (PD-L1), and a dominant-negative form of IL-17 receptor A (dn-IL17RA) as a single payload or as a combination payload. Intralesional delivery of the VLV vector expressing IL-12 alone, as well as the trivalent vector (designated CARG-2020) eradicated large established tumors. However, only CARG-2020 prevented tumor recurrence and provided long-term survival benefit to the tumor-bearing mice, indicating a benefit of the combined immunomodulation. The abscopal effects of CARG-2020 on the non-injected contralateral tumors, as well as protection from the tumor cell re-challenge, suggest immune-mediated mechanism of protection and establishment of immunological memory. Mechanistically, CARG-2020 potently activates Th1 immune mechanisms and inhibits expression of genes related to T cell exhaustion and cancer-promoting inflammation. The ability of CARG-2020 to prevent tumor recurrence and to provide survival benefit makes it a promising candidate for its development for human cancer immunotherapy.
3.Study on the quality of Bolbostemmatis Rhizoma standard decoction by HPLC fingerprint combined with quantitative analysis of multi-components by single marker

Objective:To establish the HPLC fingerprint of Bolbostemmatis Rhizoma standard decoction; To determine the three effective components with similar structure by quantitative analysis of multi-components by single marker (QAMS); To evaluate the quality of Bolbostemmatis Rhizoma standard decoction.Methods:HPLC was adopted to establish the fingerprints of 15 batches of Bolbostemmatis Rhizoma standard decoction. The Chromatographic column was Waters XBridge Phenyl (4.6 mm×250 mm, 5 μm). The mobile phase was acetonitrile-0.1% phosphoric acid solution with gradient elution. Cluster analysis (HCA) and principal component analysis (PCA) were conducted based on the relative peak area of common peaks. The same method as the fingerprint was used to establish QAMS of tubeimoside A, B, C on Bolbostemmatis Rhizoma standard decoction.Results:There were 14 common peaks in the fingerprint of Bolbostemmatis Rhizoma standard decoction. It was confirmed that the peak 3 was L-tryptophan, the peak 11 was tubeimoside B, the peak 12 was tubeimoside C, and the peak 13 was tubeimoside A. 15 batches of Bolbostemmatis Rhizoma standard decoction from different origins were divided into 3 categories by HCA and PCA. There was no significant difference between QAMS and the external standard method (ESM) through the system suitability inspection. Conclusion:This method is accurate, reliable and has good specificity, which can effectively evaluate the quality of Bolbostemmatis Rhizoma standard decoction.
4.Textual analysis of China’s traditional Chinese medicine emergency management policy based on three-dimensional analysis framework

OBJECTIVE To analyze the traditional Chinese medicine (TCM) emergency management policy texts in China, reveal the characteristics, problems and improvement directions of Chinese medicine emergency management policies in China, and provide references and lessons for improving the level of Chinese medicine emergency management. METHODS Twenty-four TCM emergency management policy texts issued at the central level from 2016 to 2023 were coded and analyzed using Nvivo11 software to construct a three-dimensional analysis framework based on policy tools, stakeholders and policy strength. RESULTS In the policy tools dimension, the environmental type was the most (46.74%), the supply type was the second (31.80%), and the demand type was the least (21.46%); in the stakeholder dimension, there were more healthcare institutions (40.63%) and government departments (31.25%), and fewer healthcare workers (14.84%) and residents (13.28%); in the policy strength dimension, the overall policy strength was poor, and the differences in effectiveness across policy instruments and stakeholders were more significant. The cross-cutting results showed that there was a certain degree of mismatch in policy instruments, stakeholders and policy strength. CONCLUSIONS The use of supply-oriented policy tools is slightly lacking, and the use of policy tools should be optimized in a coordinated manner; the distribution of stakeholders is relatively unbalanced, and synergies among stakeholders should be enhanced; the overall strength of policies is poor, and the top-level design of relevant policies should be improved.
5.Application Progress of Electrochemical Methods in Quality Control of Traditional Chinese Medicine

The quality control of traditional Chinese medicine(TCM)is the core issue to ensure the modernization,industrialization and internationalization of TCM.Compared with other detection methods,electrochemical analysis method has many advantages such as high sensitivity,fast detection speed and low cost,making it an important means of quality control for TCM and having broad development prospects.This article reviewed the research progress of electrochemical methods in quality control of TCM in recent years,discussed the application of electrochemical fingerprinting technique in identification of TCM,and comprehensively summarized the application of electrochemical technology in analyzing effective components and harmful substances in TCM,including flavonoids,alkaloids,quinones,glycosides,heavy metals and pesticide residues.Finally,the development prospects of electrochemical methods in the field of quality control of TCM were discussed.

Objective:To explore the impact and cost-effectiveness of community stroke screening intervention mode on stroke risk.Methods:A total of 3 561 community people over 40 years old who participated in screening intervention in 2017,2019 and 2021 were selected as research objects,and stroke risk was divided into low risk,medium risk and high risk.A Markov model was established to explore the impact of screening intervention mode on stroke risk in community population.The cost increment during the phase I trial was calculated,and the life year increment was adjusted according to the quality estimate of previous studies.The cost-effectiveness increment ratio was calculated,and the screening intervention mode was evaluated,and univariate sensitivity analysis was performed.Results:Within a certain range,intervention screening could effectively shift the status of residents to the low-risk direction,and finally stabilize the distribution of low-risk,medium-risk and high-risk were 47.4%,31.0%and 21.6%.The incremental cost of interventional screening was 160 245 yuan,the incremental quality-adjusted life year was 151.129 yuan,and the incremental cost-effectiveness ratio(ICER)was 1 060.319 yuan/QALY,which was less than 1 times the per capita GDP.The intervention program was fully cost-effective.Conclusion:Screening intervention can promote the transformation of the commu-nity population to a low-risk state of stroke in the prevention stage,and this approach has good cost-effectiveness performance.It is recommended that the primary medical and health institutions that are not enough to fully implement the integrated process ser-vice of community prevention and treatment of stroke should first implement low-cost screening intervention.

9.Mendelian randomization analysis of the causal relationship between asthma, allergic rhinitis, and chronic sinusitis

Objective:To explore the causal relationship between asthma, allergic rhinitis (AR), and chronic sinusitis (CRS), using two sample Mendelian randomization (MR) analysis, thereby providing foundational evidences for the pathogenesis and treatment of CRS.Methods:The genetic variations in AR and asthma were used as instrumental variables, with genetic data from the Integrated Epidemiology Unit (IEU) Open database. A total of 14 283 asthma and 18 934 AR cases were included, with 98 300 and 64 595 corresponding normal control cases, respectively. For CRS, there were 3 236 CRSwNP and 8 524 CRSsNP, respectively, with 167 849 and 167 849 corresponding normal control cases, respectively. The genetic data were analyzed using the inverse variance weighting method (IVW), MR Egger method, weighted median method, and Cochran′s Q-test.Results:The IVW analysis showed that asthma increased the risk of both CRSwNP ( OR=482.8, 95% CI: 57.18-4 077.78, P<0.001) and CRSsNP ( OR=25.73, 95% CI: 9.79-67.56, P<0.001); AR significantly increased the risk of CRSsNP ( OR=5.40, 95% CI: 1.68-17.26, P=0.004), but not CRSwNP ( OR=7.38, 95% CI: 0.80-67.73, P=0.077). Conversely, neither CRSwNP nor CRSsNP increased the risk of asthma or AR. Conclusion:According to Mendelian genetic laws, asthma is a risk factor for CRSwNP and CRSsNP, while AR is a risk factor for CRSsNP.
