1.Analysis of prognostic factors of extranodal NK/T-cell lymphoma treated with pegaspargase/L-asparaginase: a multicenter retrospective study.

Objective: To explore the prognostic factors of extracellular NK/T cell lymphoma (ENKTL) treated with pegaspargase/L-asparaginase. Methods: The clinical data of 656 ENKTL patients diagnosed at 11 medical centers in the Huaihai Lymphoma Working Group from March 2014 to April 2021 were retrospectively analyzed. The patients were randomly divided into two groups: a training set (460 cases) and a validation set (196 cases) at 7∶3, and the prognostic factors of the patients were analyzed. A prognostic scoring system was established, and the predictive performance of different models was compared. Results: Patients' median age was 46 (34, 57) years, with 456 males (69.5% ) and 561 nasal involvement (85.5% ). 203 patients (30.9% ) received a chemotherapy regimen based on L-asparaginase combined with anthracyclines, and the 5-year overall survival rate of patients treated with P-GEMOX regimen (pegaspargase+gemcitabine+oxaliplatin) was better than those treated with SMILE regimen (methotrexate+dexamethasone+cyclophosphamide+L-asparaginase+etoposide) (85.9% vs 63.8% ; P=0.004). The results of multivariate analysis showed that gender, CA stage, the Eastern Cooperative Oncology Group performance status (ECOG PS) score, HGB, and EB virus DNA were independent influencing factors for the prognosis of ENKTL patients (P<0.05). In this study, the predictive performance of the prognostic factors is superior to the international prognostic index, Korean prognostic index, and prognostic index of natural killer lymphoma. Conclusion: Gender, CA stage, ECOG PS score, HGB, and EB virus DNA are prognostic factors for ENKTL patients treated with pegaspargase/L-asparaginase.

Objective: This cross-sectional investigation aimed to determine the incidence, clinical characteristics, prognosis, and related risk factors of olfactory and gustatory dysfunctions related to infection with the SARS-CoV-2 Omicron strain in mainland China. Methods: Data of patients with SARS-CoV-2 from December 28, 2022, to February 21, 2023, were collected through online and offline questionnaires from 45 tertiary hospitals and one center for disease control and prevention in mainland China. The questionnaire included demographic information, previous health history, smoking and alcohol drinking, SARS-CoV-2 vaccination, olfactory and gustatory function before and after infection, other symptoms after infection, as well as the duration and improvement of olfactory and gustatory dysfunction. The self-reported olfactory and gustatory functions of patients were evaluated using the Olfactory VAS scale and Gustatory VAS scale. Results: A total of 35 566 valid questionnaires were obtained, revealing a high incidence of olfactory and taste dysfunctions related to infection with the SARS-CoV-2 Omicron strain (67.75%). Females(χ2=367.013, P<0.001) and young people(χ2=120.210, P<0.001) were more likely to develop these dysfunctions. Gender(OR=1.564, 95%CI: 1.487-1.645), SARS-CoV-2 vaccination status (OR=1.334, 95%CI: 1.164-1.530), oral health status (OR=0.881, 95%CI: 0.839-0.926), smoking history (OR=1.152, 95%CI=1.080-1.229), and drinking history (OR=0.854, 95%CI: 0.785-0.928) were correlated with the occurrence of olfactory and taste dysfunctions related to SARS-CoV-2(above P<0.001). 44.62% (4 391/9 840) of the patients who had not recovered their sense of smell and taste also suffered from nasal congestion, runny nose, and 32.62% (3 210/9 840) suffered from dry mouth and sore throat. The improvement of olfactory and taste functions was correlated with the persistence of accompanying symptoms(χ2=10.873, P=0.001). The average score of olfactory and taste VAS scale was 8.41 and 8.51 respectively before SARS-CoV-2 infection, but decreased to3.69 and 4.29 respectively after SARS-CoV-2 infection, and recovered to 5.83and 6.55 respectively at the time of the survey. The median duration of olfactory and gustatory dysfunctions was 15 days and 12 days, respectively, with 0.5% (121/24 096) of patients experiencing these dysfunctions for more than 28 days. The overall self-reported improvement rate of smell and taste dysfunctions was 59.16% (14 256/24 096). Gender(OR=0.893, 95%CI: 0.839-0.951), SARS-CoV-2 vaccination status (OR=1.334, 95%CI: 1.164-1.530), history of head and facial trauma(OR=1.180, 95%CI: 1.036-1.344, P=0.013), nose (OR=1.104, 95%CI: 1.042-1.171, P=0.001) and oral (OR=1.162, 95%CI: 1.096-1.233) health status, smoking history(OR=0.765, 95%CI: 0.709-0.825), and the persistence of accompanying symptoms (OR=0.359, 95%CI: 0.332-0.388) were correlated with the recovery of olfactory and taste dysfunctions related to SARS-CoV-2 (above P<0.001 except for the indicated values). Conclusion: The incidence of olfactory and taste dysfunctions related to infection with the SARS-CoV-2 Omicron strain is high in mainland China, with females and young people more likely to develop these dysfunctions. Active and effective intervention measures may be required for cases that persist for a long time. The recovery of olfactory and taste functions is influenced by several factors, including gender, SARS-CoV-2 vaccination status, history of head and facial trauma, nasal and oral health status, smoking history, and persistence of accompanying symptoms.

3.Pathogen spectrum of febrile respiratory syndrome among hospitalized children in Shanghai from 2019 to 2020

Objective:To understand the pathogenic spectrum and clinical characteristics of respiratory syndrome among hospitalized children in Shanghai, to provide reference for disease prevention and clinical diagnosis.Methods:Twenty-two pathogens were detected in 634 lower respiratory tract aspirates by using fluorescence quantitative PCR combined with the determination method of dissolution curve, and chi-square test was used for statistical analysis of different detection rates.Results:Among the 634 inpatient specimens, 555 were positive for pathogen nucleic acid, with a total positive rate of 87.54%. The detected pathogens were mainly Mycoplasma pneumoniae (MP), adenovirus (ADV), respiratory syncytial virus (RSV), influenza A virus (FluA) and Boca virus (BoV), and the ratio of multiple pathogen infection was greater than single pathogen infection, the pathogen type of multiple infection was ADV, RSV, FluA or BoV co-infection with MP; in the age distribution, FluA has a higher detection rate in the " <2 year old" and " 2-5 years old" age groups, and RSV had a significantly higher detection rate in the " <2 year old" age group compared with the other two age groups, the detection rate of coronavirus (CoV) in " 2-5 years old" and " >5 years old" is higher than that in " <2 year old" age group; combined with the analysis of clinical symptoms, RSV was found in non-febrile patients. The detection rate of RSV and PIV in cough patients was higher than that in non-cough patients, and the detection rate of RSV in wheezing patients was higher than that in non-wheezing patients. H3N2 in RSV and FluA had a high probability of causing pneumonia; FLuA, FluB and RSV had obvious epidemic peaks in winter and spring.Conclusions:We should strengthen the continuous monitoring of respiratory pathogens such as MP, ADV, RSV, FluA and BoV for the purpose of better understanding the changes in their epidemiological characteristics and pathogenicity, and provide the scientific data for the prevention and control of respiratory diseases in children.
4.Correlation analysis and breeding application of agronomic traits and quality indexes in Dendrobium nobile.

The present study aimed to explore the correlation between agronomic traits and quality indexes of Dendrobium nobile and its application value in agricultural breeding. The cultivated strains of D. nobile in Hejiang-Chishui producing areas were extensively collected,and the main agronomic traits and quality indexes were measured. The agronomic traits with significant correlation with quality indexes were screened out by the correlation analysis,and then the parental lines and self-bred F_1 generation plants were furtherverified. Among 96 lines of D. nobile,the content of soluble polysaccharides showed a significant negative correlation with dendrobine( P < 0. 01),and no significant correlation with agronomic traits in stems and leaves. The content of dendrobine exhibited a significant positive correlation with the stem width-thickness ratio( at the largest cross section; P < 0. 01),and no significant correlation with other agronomic traits. Regression analysis further verified the positive correlation between dendrobine content and stem width-thickness ratio( R2> 0. 9). Two lines,JC-10 and JC-35,with significant differences in stem width-thickness ratio were screened out( P <0. 05). The corresponding F1 generation plants by self-pollination both showed that the dendrobine content was higher with greater stem width-thickness ratio( P < 0. 01). The experimental results suggested that within a certain range,the dendrobine content was higher in D. nobile with flatter stem. Therefore,in the breeding of D. nobile,this specific trait could be used for screening plants with high content of quality indexes such as dendrobine.

5.Arthroscopic surgery combined with platelet-rich plasma for meniscus injury:a Meta analysis.

OBJECTIVE:
To evaluate clinical efficacy of arthroscopic with platelet-rich plasma (PRP) in treating meniscus injury.
METHODS:
From January 2015 to December 2019, clinical control study on repair meniscus injury by arthroscopic with PRP between arthroscopic were searched by PubMed, Science Direct, Cochrane library, Chinese Journal Full-text Database, Wanfang and VIP database. Literature screening, data extraction and quality evaluation according to inclusion and exclusion criteria. Visual analogue scale(VAS) of knee joint and Lysholm score at 1, 6 and 12 months after opertaion between two groups were compared, and Western Ontario and McMaster University Osteoarthritis Index (WOMAC) at 3, 6 and 12 months after opertaion between twogroups were also compared.
RESULTS:
Totally 9 literatures and 329 patients with meniscal injuries were screened, include 146 patients treated by arthroscopic with PRP and 183 patients treated by arthroscopic. There were no statistical differences in VAS between two groups at 1, 6 and 12 months after opertaion. There were differences in Lysholm score at 1 and 6 months after operation between two groups [
CONCLUSION
Arthroscopic with PRP for repair meniscus injury has short term efficacy of knee function and delay arthritis, while has similar effect in long term clinical efficacy and relieve pain.

6.Effect of SHIP-1 on Invasion, Migration and PI3K-AKT Signaling Pathway of Leukemic Cells.

OBJECTIVETo investigate the effect of SH2-containing inositol phosphatase-1 (SHIP-1) on the proliferation, invasion and migration of human leukemia cells as well as phosphatidylinositol-3 kinase (PI3K) / protein kinase B (AKT) signaling pathway.
METHODSThe overexpression vector pCDNA3.1-SHIP1 was transfected into THP-1 cells by Lipofectamine 2000. The experiment was divided into 3 groups: control group (untreated cells) and empty vector group (transfected with empty vector pCDNA3.1-NC) and overexpression group (transfected with overexpression vector pCDNA3.1-SHIP1). The cell proliferation was tected by CCK-8 assay, Transwell assay was used to evaluate the cell invasion and migration capabilities. The expressions of SHIP-1, AKT, phosphorylated AKT (pAKT), matrix metalloproteinase-9 (MMP-9) protein were analyzed by Western blot.
RESULTSThe expression of SHIP-1 in overexpression group was significantly higher than that in the control group(P<0.05). Compared with the control group, the absorbance of the cells in the empty vector group was not statistically different (P>0.05), and the absorbance in overexpression group decreased significantly(P<0.05). The cell numbers of invasion and migration were not significantly different between empty and control groups(P>0.05), but those in overexpression group were significantly lower than those in the control group(P<0.05). Compared with the control group, the expression of AKT, pAKT and MMP-9 in the empty vector group was not statistically different (P>0.05); the AKT protein in overexpression group was not significantly different (P>0.05), but the pAKT and MMP-9 significantly decreased(P<0.05).
CONCLUSIONSHIP-1 plays a role in inhibiting the proliferation, invasion and migration of leukemia cells, the mechanism probably relates with supressing the expression of MMP-9 by regulating PI3K/AKT signaling pathway.

7.Change of Plasma Interleukin-16 Level in Patients with Multiple Myeloma and Its Clinical Significance.

OBJECTIVETo investigate the change of plasma IL-16 level in patients with multiple myeloma(MM) and its clinical significance.
METHODSSixty-two patients with multiple myeloma were admitted in our hospital from June 2008 to June 2015. Forty healthy volunteers were selected as control group. The peripheral blood of all the patients and healthy volunteers were collected before the treatment of patients. The levels of IL-16, Cys-C, LDH and β2-MG were measured. ROC curve was used to analyze the optimal IL-16 thresholds in MM patients. Kaplan-Meier method was used to analyze the factors affecting overall survival.
RESULTSThe levels of IL-16, Cys-C, LDH and β2-MG in the MM group were significantly higher than those in the control group (P<0.05). The levels of IL-16, Cys-C, LDH and β2-MG in patients with different ISS were significantly different (P<0.05). The levels of IL-16, Cys-C, LDH and β2-MG in ISS III groups were higher than those in ISS I and ISS II groups(P<0.05). When the IL-16 concentration was 171.26 ng/L, the AUC was 0.787 (P<0.01), and the sensitivity and specificity were 82.25% and 75.80%, respectively, when the IL-16 threshold was predicted by ROC curve analysis. The 3-year overall survival rates of patients with IL-16≤171.26 ng/L and IL-16>171.26 ng/L were 91.93% and 51.61%, respectively (P<0.01). Multivariate analysis showed that the changes of IL-16 levels were significantly related with overall survival (P<0.01).
CONCLUSIONThe level of IL-16 in peripheral blood of patients with multiple myeloma has been cofirmed to be significantly elevated, and the elevated IL-16 is closely related with the prognosis.
8.Effects of Different Dosages of rhG-CSF on Duration of Aleucocytosis and Leukocytes Level after Chemotherapy of Patients with Hematologic Malignancies.

OBJECTIVETo compare the effects of different dosages of rhG-CSF on duration of aleucocytosis and white blood cell counts after chemotherapy of patients with hematologic malignancies.
METHODSNinety patients in our hospital from December 2011 to June 2016 were chosen as study objects, and all of them were divided into 3 groups: group A (rhG-CSF 200 µg/m), group B(rhG-CSF 300 µg/m) and group C(rhG-CSF 400 µg/m); 30 patients from January 2004 to January 2007 were chosen as control(control group). The WBCand its duration, WBCand its timepoint were compared among different groups. The infection rate, incidence of side reactions and total amount of rhG-CSF used in different groups were compared.
RESULTSIn control group, WBCwas(1.30±0.11)×10/L, its duration was (3.2±0.7)d, WBCwas(5.14±0.41)×10/L, and its time point was (26.1±1.8)d; these in group A were (3.14±0.23)×10/L,(2.7±1.0)d, (10.08±0.69)×10/L and (14.9±1.8)d respectively; these in group B were (3.11±0.32)×10/L, (0.9±0.5)d, (10.17±0.75)×10/L and(10.7±1.5)d respectively; these in group C were (3.15±0.30)×10/L,(0.5±0.3)d, (11.95±0.86)×10/L and (10.6±1.5)d, respectively. Compared with control group, the WBCand WBCwere both increased significantly, the duration of WBCwas shortened and the timepoint of WBCwas moved up(P<0.05). The infection rate of group C (3.33%(1/30)) was significantly lower than that of control group(33.33%(10/30))(P<0.05), total used amount of rhG-CSF and incidence of side reactions were not statistically different among group A,B,C(P>0.05).
CONCLUSIONCompared with low dosage of rhG-CSF, medium/high dosage of rhG-CSF can help to shorten duration of a leukocytosis after chemotherapy of patients.
9.Characteristics and Clinical Significance of CRLF2 Mutations in Adult Acute Lymphoblastic Leukemia.

OBJECTIVECytokine receptor-like factor 2(CRLF2) plays an important role in the development of normal B lymphocytes, which can mediate early B cell proliferation and survival. The aim of this study was to investigate the mutations of CRLF2 and its clinical significance in adult patients with acute lymphoblastic leukemia(ALL).
METHODSExons of CRLF2 were amplified, then the DNA was purified and sequenced; the frequency, position, types and clinical significance of CRLF2 mutations were analyzed.
RESULTS6 types of genetic alterations in CRLF2 were found, among them the R186S prompted better prognosis, while L86I, F232F and W255C associated with poor prognosis.
CONCLUSIONCRLF2 mutations may play an important role in the development and progressions of adult patients with ALL, and these genetic abnormalities may associate with clinical outcome.
10.Regulation of matrix metalloproteinase-9 protein expression by 1alpha,25-(OH)2D3 during osteoclast differentiation.

To investigate 1alpha,25-(OH)2D3 regulation of matrix metalloproteinase-9 (MMP-9) protein expression during osteoclast formation and differentiation, receptor activator of nuclear factor kappaB ligand (RANKL) and macrophage colony-stimulating factor (M-CSF) were administered to induce the differentiation of RAW264.7 cells into osteoclasts. The cells were incubated with different concentrations of 1alpha,25-(OH)2D3 during culturing, and cell proliferation was measured using the methylthiazol tetrazolium method. Osteoclast formation was confirmed using tartrate-resistant acid phosphatase (TRAP) staining and assessing bone lacunar resorption. MMP-9 protein expression levels were measured with Western blotting. We showed that 1alpha,25-(OH)2D3 inhibited RAW264.7 cell proliferation induced by RANKL and M-CSF, increased the numbers of TRAP-positive osteoclasts and their nuclei, enhanced osteoclast bone resorption, and promoted MMP-9 protein expression in a concentration-dependent manner. These findings indicate that 1alpha,25-(OH)2D3 administered at a physiological relevant concentration promoted osteoclast formation and could regulate osteoclast bone metabolism by increasing MMP-9 protein expression during osteoclast differentiation.
