1.Immunotherapy for small cell lung cancer: progresses and challenges

		                        			[摘  要]  小细胞肺癌(SCLC)是肺癌中恶性程度最高的一种类型,其在全部肺癌病例中约占13%~15%。初治SCLC患者虽然对化疗和放疗具有较高的敏感性,但容易复发,导致患者生存率较低。2018年以来,随着IMpower133和CASPIAN研究相继成功,SCLC进入了免疫治疗时代,化疗联合免疫检查点抑制剂已成为广泛期SCLC的一线治疗标准方案。同时,免疫治疗在局限期SCLC中也取得了初步成功。SCLC的免疫治疗虽然取得了一些进展,但免疫治疗整体上带来的生存获益有限,缺乏有效的预测生物标志物,对复发/难治性SCLC缺少治疗手段,SCLC免疫治疗未来仍面临诸多挑战。本文回顾国内外SCLC免疫治疗临床研究的最新进展,分析不同类型的免疫治疗药物、生物标志物及新型免疫治疗靶点,探讨SCLC免疫治疗的联合策略和发展前景。

		                        			Surgical operation is the main treatment of oral and maxillofacial tumors.Dysphagia is a common postoperative complication.Swal-lowing disorder can not only lead to mis-aspiration,malnutrition,aspiration pneumonia and other serious consequences,but also may cause psychological problems and social communication barriers,affecting the quality of life of the patients.At present,there is no systematic evalua-tion and rehabilitation management plan for the problem of swallowing disorder after oral and maxillofacial tumor surgery in China.Combining the characteristics of postoperative swallowing disorder in patients with oral and maxillofacial tumors,summarizing the clinical experience of ex-perts in the field of tumor and rehabilitation,reviewing and summarizing relevant literature at home and abroad,and through joint discussion and modification,a group of national experts reached this consensus including the core contents of the screening of swallowing disorders,the phased assessment of prognosis and complications,and the implementation plan of comprehensive management such as nutrition management,respiratory management,swallowing function recovery,psychology and nursing during rehabilitation treatment,in order to improve the evalua-tion and rehabilitation of swallowing disorder after oral and maxillofacial tumor surgery in clinic.
		                        		
		                        		
		                        		
		                        	
3.Effects of group cognitive behavioral therapy on clinical efficacy and executive function in patients with generalized anxiety disorder

		                        			BackgroundPrevious studies have found that patients with generalized anxiety disorder (GAD) have impaired performance in executive function, and group cognitive behavioral therapy (CBT) has been shown to be effective in alleviating negative affect in patients with GAD, while its efficacy on executive function remains unclear. ObjectiveTo explore the efficacy of group CBT on anxiety symptom and executive function in GAD patients, so as to provide references for the rehabilitation program for GAD. MethodsA total of 80 consecutive patients with GAD who were hospitalized in Sleep and Psychosomatic Medical Center of Shiyan Taihe Hospital from March 2021 to August 2022 and met the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5) diagnostic criteria for GAD were enrolled, and they were assigned into study group (n=40) and control group (n=40) using random number table methods. All patients were subjected to routine medication treatment and regular health education, based on this, study group received group CBT once a week (6 weeks, 60 to 90 minutes per session). At the enrollment and after 6 weeks of treatment, patients were assessed using Hamilton Anxiety Scale (HAMA) and Frontal Assessment Battery (FAB). ResultsANOVA with repeated measures on HAMA score revealed a significant time effect (F=1 870.320, P<0.01), no significant group effect and no significant time×group interaction effect (F=1.254, 0.293, P>0.05). Significant time effect, group effect and time×group interaction effect were reported on FAB scores (F=311.190, 4.399, 7.021, P<0.05 or 0.01). Further analysis indicated that FAB scores of both groups after treatment were higher than those at baseline (t=200.569, 115.401, P<0.01).And the FAB score of study group was higher than that of control group after treatment (t=-3.211, P<0.01). ConclusionGroup CBT combined with medication treatment for GAD may alleviate the anxiety symptoms and improve executive function in GAD patients. 4.Research progress in preparation for future care for the elderly

		                        			The continued aging of the population has led to a surge in long-term care needs for the elderly.Proactive preparation for future care(PFC)for the elderly can effectively meet the care needs caused by aging,improve their physical and mental health,and promote the elderly to maximize their physiological,psychological and social functions,finally realizing healthy aging.This paper reviews the theoretical basis,concept connotation,measurement tools,influencing factors and intervention strategies of PFC for the elderly,and puts forward the prospects for future research,so as to provide references for potential mechanism and empirical research of PFC for Chinese seniors.
		                        		
		                        		
		                        		
		                        	
5.A Comprehensive Study of the Association between LEPR Gene rs1137101 Variant and Risk of Digestive System Cancers

		                        			Objective The leptin receptor,encoded by the LEPR gene,is involved in tumorigenesis.A potential functional variant of LEPR,rs1137101(Gln223Arg),has been extensively investigated for its contribution to the risk of digestive system(DS)cancers,but results remain conflicting rather than conclusive.Here,we performed a case-control study and subsequent meta-analysis to examine the association between rs1137101 and DS cancer risk. Methods A total of 1,727 patients with cancer(gastric/liver/colorectal:460/480/787)and 800 healthy controls were recruited.Genotyping of rs1137101 was conducted using a polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)assay and confirmed using Sanger sequencing.Twenty-four eligible studies were included in the meta-analysis. Results After Bonferroni correction,the case-control study revealed that rs1137101 was significantly associated with the risk of liver cancer in the Hubei Chinese population.The meta-analysis suggested that rs1137101 is significantly associated with the risk of overall DS,gastric,and liver cancer in the Chinese population. Conclusion The LEPR rs1137101 variant may be a genetic biomarker for susceptibility to DS cancers(especially liver and gastric cancer)in the Chinese population.
		                        		
		                        		
		                        		
		                        	
6.Study on the fit testing for the workers wearing hearing protection device in machinery manufacturing enterprises

		                        			Objective:To investigate the occupational noise hazards in five machinery manufacturing enterprises, and to evaluate the individual noise reduction values and influencing factors of workers wearing hearing protection device (HPD) by individual fit testing.Methods:From November 2021 to January 2022, 5 machinery manufacturing enterprises in Bao'an District of Shenzhen were selected to conduct an occupational health survey to understand the noise exposure level of workers. The 3MTM E-A-RfitTM fitness test system was used to test the baseline individual sound attenuation value level (PAR) of the daily wear of the ear protecters for 485 workers in typical noise working positions. Workers whose PAR values could not meet the requirements of noise reduction at work were instructed to wear and repeated tests were conducted. PAR results of the workers before and after the intervention were collected and analyzed.Results:The noise workers who received the suitability test were mainly distributed in 24 types of work, the job noise exposure level was 80.2 dB (A) ~ 95.0 dB (A), and the job noise excess rate was 52.5% (138/263). The median baseline PAR [ M ( Q1, Q3) ] for 485 workers was 6.0 (0.0, 14.0) dB. The baseline PAR of male workers, those with more than 15 years of working experience, those with more than 15 years of using ear guards, those who considered ear guards comfortable to wear, those with college degree or above, and those exposed to noise level 90 dB (A) were higher, and the difference was statistically significant ( P<0.05). A total of 275 workers (56.7%) did not pass the baseline PAR test, and there was no statistically significant difference in the intervention rate of workers in different noise groups ( P>0.05). PAR in subjects who did not pass baseline after intervention increased from 0.0 (0.0, 3.0) dB to 15.0 (12.0, 18.2) dB. Conclusion:The workplace noise hazard of machinery manufacturing enterprises is serious, and there is a great difference between the baseline PAR and the nominal value of the hearing guard worn by the noise exposed workers. 7.Expression of enolase and laminin in the vagina of bacterial vaginosis of mice

		                        			To investigate the differences in enolase and laminin levels in vaginal epithelial tissues between mice successfully infected withGardnerellaand mice not infected with Gardnerella, providing information for further exploration of the correlation between enolase and laminin levels and the incidence of bacterial vaginosis.
		                        		
		                        			Methods:
		                        			Gardnerella strains isolated, purified, and identified from vaginal secretions of patients with bacterial vaginosis were used to infect the vagina of mice and establish a mouse model of bacterial vaginosis. Successful and failed mice was defined as successful and failed groups, respectively. Differential expression of enolase and laminin in the vaginal epithelial tissue of two groups of mice was detected by Western blot. Modeling success rate was statistically analyzed, and the expression differences of enolase and laminin was compared between two groups.
		                        		
		                        			Results:
		                        			One strain of Gardnerella vaginalis infected 10 SPF grade KM mice, 7 mice met the diagnostic criteria for bacterial vaginosis, and 3 mice failed to model, with a success rate of 70%. Western blot was used to detect protein expression levels, and the levels of laminin and enolase in the successfully modeled mouse vaginal epithelial tissue were significantly higher than those in the failed modeling group, with statistical differences between the two groups(P<0.05).
		                        		
		                        			Conclusion
		                        			Enolase and laminin may be involved in the occurrence of bacterial vaginosis, however, further research is needed to determine the mechanisms through which they trigger the occurrence and development of the disease.

9.Effect of Huoxue Yiqi Decoction Combined with Pinggan Qianyang Muzu Powder on Blood Lipid Metabolism and Cardiac Function in Female Patients with Essential Hypertension

		                        			Objective To investigate the effects of Huoxue Yiqi Decoction(composed of Carthami Flos,Crataegi Fructus,Salviae Miltiorrhizae Radix et Rhizoma,and Astragali Radix)combined with Pinggan Qianyang Muzu Powder(composed of Uncariae Ramulus cum Uncis,Prunellae Spica,Chrysanthemi Flos,Lonicerae Japonicae Flos and Cassiae Semen)on blood lipid metabolism and cardiac function in female patients with essential hypertension.Methods A total of 220 female patients with essential hypertension of qi deficiency and blood stasis type were randomly divided into observation group and control group,with 110 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with Huoxue Yiqi Decoction orally combined with Pinggan Qianyang Muzu Powder for foot bath on the basis of treatment for the control group.The course of treatment for the two groups covered one month.Before and after treatment,the two groups were observed in the changes of blood pressure,blood lipid,traditional Chinese medicine(TCM)syndrome scores and cardiac function indicators of left ventricular ejection fraction(LVEF)and stroke volume(SV).After treatment,the clinical efficacy of the two groups was evaluated.Results(1)After one month of treatment,the total effective rate of the observation group was 95.45%(105/110),and that of the control group was 84.55%(93/110).The intergroup comparison(tested by chi-square test)showed that the efficacy of the observation group was superior to that of the control group(P<0.01).(2)After treatment,the TCM syndrome scores of primary symptoms and secondary symptoms in the two groups were decreased when compared with those before treatment(P<0.05),and the decrease in the observation group was superior to that in the control group(P<0.01).(3)After treatment,the levels of systolic blood pressure(SBP)and diastolic blood pressure(DBP)in the two groups were lower than those before treatment(P<0.05),and the decrease of SBP and DBP levels in the observation group was significantly superior to that in the control group(P<0.01).(4)After treatment,the levels of blood lipid indicators of total cholesterol(TC),triglyceride(TG)and low density lipoprotein cholesterol(LDL-C)in the two groups were lower than those before treatment(P<0.05),and the levels of high density lipoprotein cholesterol(HDL-C)were higher than those before treatment(P<0.05).The decrease of TC,TG and LDL-C levels and the increase of HDL-C level in the observation group were significantly superior to those in the control group(P<0.01).(5)After treatment,the levels of cardiac function indicators of LVEF and SV in the two groups were higher than those before treatment(P<0.05),and the increase of LVEF and SV levels in the observation group was significantly superior to that in the control group(P<0.01).Conclusion The combination of Huoxue Yiqi Decoction and Pinggan Qianyang Muzu Powder exerts certain effect in the treatment of female patients with essential hypertension of qi deficiency and blood stasis type.The combined therapy is effective on controlling blood pressure,improving blood lipid metabolism and cardiac function indicators.
		                        		
		                        		
		                        		
		                        	
10.Study on the Method of"Unblocking Governor Vessel and Nourishing the Mind,Conducting Qi back to Its Source"on the Treatment of Multiple System Atrophy

		                        			Combined with clinical practice,it is found that the clinical manifestations of multiple system atrophy are complex and diverse,but the pathogenesis of traditional Chinese medicine can be summarized as kidney deficiency and empty marrow as the core,and yang deficiency as an important factor.Tongyuan therapy can communicate with the brain and kidney by unblocking and regulating the governor vessel,so that the kidney essence can nourish the brain marrow,stimulate the yang qi of the whole body,nourish the original spirit and the spirit of the zang-fu organs,and nourish the muscles of the limbs.Make much of use conception vessels and abdominal front Mu point to tonify the kidney and cultivate the vital qi,strengthen the spleen and replenish qi,so that the prenatal and postnatal essence can be regulated simultaneously.Clinically,it can be based on the combination of acupuncture and Chinese medicine,which has guiding significance for the treatment of multiple system atrophy.
