1.Renal abscess induced by fish bone ingestion: a case report

Renal abscess caused by fish bone ingestion is extremely rare and has not been reported in the literature. A male patient presented with a 1-week history of flank pain and a 2-day history of fever. Urinary ultrasound and CT scan showed an irregular hypodense lesion in the left kidney and blurred thickening of the descending colon wall. Three-dimensional CT reconstruction images revealed a needle-like foreign body, which perforated from the descending colonic lumen to the left kidney. The patient had accidentally eaten fish bone one week prior. On the basis of clinical data, the diagnosis of renal abscess caused by foreign body was suspected. Accordingly, laparotomy was performed, the abscess was drained, and the colon was repaired. The foreign body was confirmed to be fish bone. The postoperative condition of the patient was uneventful, and the patient remained well in the 3 months' follow-up without any further complaints.
2.Mechanisms of Danggui Shaoyaosan in Improving Cognitive Ability in SAMP8 Mice by Regulating Gut Microbiota via 16S rDNA Sequencing

ObjectiveTo analyze the effects of Danggui Shaoyaosan (DSS) on the gut microbiota of the Alzheimer's disease (AD) model in SAMP8 mice based on 16S rDNA sequencing. MethodTwenty-four SAMP8 mice aged seven months were randomly divided into low-, medium-, and high-dose DSS groups (14.4, 28.8, 57.6 g·kg-1·d-1) and a model group according to a random number table, with six rats in each group. Six SAMR1 mice of the same age were assigned to the normal group. After intragastric administration for eight consecutive weeks, 16S rDNA sequencing was performed to detect the gut microbiota of feces in mice. Morris water maze was employed to assess the directional navigation and space exploration ability of mice. Nissl staining was performed to observe the pathological changes of neurons in the hippocampal CA1 area. Enzyme-linked immunosorbent assay (ELISA) was adopted to measure the protein content of hippocampal amyloid β-protein (Aβ) and hyperphosphorylated Tau (p-Tau). ResultCompared with the normal group, the model group presented a declining α diversity (P<0.05), markedly altered β diversity, prolonged escape latency (P<0.05), reduced number of platform crossings and cumulative duration in the targeted quadrant (P<0.05), decreased neurons and Nissl bodies in the CA1 hippocampal area, and up-regulated Aβ and p-Tau expression (P<0.05). However, DSS intervention enhanced the α diversity, and medium- and high-dose DSS, especially the medium-dose DSS, could result in α diversity similar to the control group. Moreover, at the phylum level, the abundance of Firmicutes increased (P<0.05), while the abundance of Bacteroidetes and Proteobacteria decreased (P<0.05). At the genus level, the abundance of Lactobacillus and other genera increased (P<0.05), while the abundance of Bacteroides, Helicobacterium, Rikenella, Parabacteroides, Sutterella, and Mucilaginibacter decreased (P<0.05). The DSS groups also showed shortened escape latency (P<0.05), increased number of platform crossings and cumulative duration in the targeted quadrant (P<0.05), increased Nissl bodies (P<0.05), and reduced Aβ and p-Tau content (P<0.05). Pearson correlation analysis showed that the abundance of Mucilaginibacter, Bacteroides, and Sutterella was negatively correlated with the cognitive ability of SAMP8 mice, while the abundance of Lactobacillus and Butyricimonas was positively correlated with the cognitive ability of SAMP8 mice. ConclusionDSS can improve the cognitive ability of SAMP8 mice, and its mechanism may be related to the regulation of gut microbiota diversity and community composition.
3.High expression of FABP4 in colorectal cancer and its clinical significance.

OBJECTIVES:
To investigate the relationship between the fatty acid-binding protein 4 (FABP4) and colorectal cancer (CRC).
METHODS:
Using an enzyme-linked immunosorbent assay (ELISA), we measured the expression of FABP4 in plasma of 50 patients who underwent surgery for CRC from October 2017 to May 2018 and 50 healthy controls. The content of the visceral fat area (VFA) as seen with abdominal computed tomography (CT) scanning was measured by ImageJ software. The expression levels of FABP4, E-cadherin, and Snail proteins in CRC and adjacent tissues were determined by immunohistochemistry.
RESULTS:
The mean concentration of plasma FABP4 of CRC patients was higher than that of the control group (22.46 vs. 9.82 ng/mL;
CONCLUSIONS
High LPA and VFA were risk factors for increased plasma FABP4 in CRC patients. FABP4 protein was highly expressed in CRC tissues and associated with TNM stage, differentiation, and lymph node metastasis of CRC. The level of FABP4 in CRC tissue was correlated with E-cadherin and Snail expression, suggesting that FABP4 may promote CRC progression related to epithelial-mesenchymal transition (EMT).
4.Efficacy and Safety of Xianling Gubao Capsule Combined with Glucosamine in the Treatment of Knee Osteoar- thritis:A Meta-analysis

OBJECTIVE:To systematically evaluate the efficacy and safety of Xianling gubao capsule combined with glucosamine in the treatment of knee osteoarthritis (KOA),and to provide evidence-based reference for clinical treatment of KOA. METHODS:Retrieved from Cochrane library ,PubMed,Embase,CNKI,Wanfang database ,CBM and VIP during the inception to Nov. 2020,randomized controlled trials (RCTs)about glucosamine (control group )combined with Xianling gubao capsule (trial group)in the treatment of KOA were collected. After data extraction ,Cochrane 5.1.0 system evaluator manual was used to evaluate the quality of the included clinical studies met inclusion criteria ,and Rev Man 5.4 software was used for Meta-analysis. RESULTS : A total of 11 RCTs with 970 patients were included in this study. Results of Meta-analysis showed that cure rate [OR =2.41,95%CI (1.48,3.93),P<0.001],marked effective rate [OR =1.95,95%CI(1.49,2.56),P<0.001],total effective rate [OR =4.00,95%CI (2.57,6.24),P<0.001],visual analogue scale (VAS)score [MD =-2.34,95%CI(-2.51,-2.17),P<0.001],knee function score [MD =31.32,95%CI(27.89,34.75),P<0.001],Japanese Orthopaedic Association (JOA)low back pain score [MD = 12.22,95%CI(9.68,14.76),P<0.001] and pain relief time [MD =-1.55,95%CI(-1.84,-1.25),P<0.001] of trial group were all significantly better than those of control group. The incidence of ADR in trial group was significantly lower than control group [OR =0.04,95%CI(0.02,0.12),P<0.001]. CONCLUSIONS :Xianing gubao capsule combined with glucosamine in the treatment of KOA is significantly better in cure rate ,marked effective rate ,total effective rate ,VAS,score,knee function score,JOA low back pain score and pain relief time ,and could significantly reduce the incidence of ADR.
5.Establishment and predictive efficiency of model for the effect and safety of endoscopic lithotripsy on pediatric upper urinary tract calculi

Objective:To establish a predictive model that can predict the effectiveness and safety of endoscopic surgery for pediatric upper urinary tract calculi, and evaluate its diagnostic efficacy through internal validation.Methods:The data was selected from the prospective database of pediatric upper tract calculi constructed by Beijing Friendship Hospital Affiliated to Capital Medical University from June 2014 to April 2019. A total of 348 children were recruited in this investigation including 250 boys and 98 girls, with a median age of 3.0 years(Interquartile Range 1.4, 7.0 years). Totally 375 endoscopic surgeries were performed, with an overall stone free rate (SFR) of 88.0% (330/375) and complication rate (CR) of 23.2% (87/375). This research used univariate and multivariate logistic regression analysis to evaluate and screen the predictors of SFR and CR. The nomogram of SFR and CR was established by using the selected predictive factors. The differentiation degree of the model was evaluated by the area under the curve (AUC), the consistency between the prediction probability and the actual risk was evaluated by the calibration curve, and the clinical benefits of the model application were assessed by the decision curve.Results:The results of multivariate analysis demonstrated that stone burden, operation duration, intraoperative perfusion, stone location and operative options were the postoperative predictors of SFR and CR for children. Besides, BMI was also a predictor of postoperative CR. The AUC of the model (model 1: SFR; model 2: CR) based on the above predictive factors was 0.81 and 0.73, respectively. The predictive probability of the calibration curve showed that the model had a good consistency with the actual value. The decision curve showed that the application of model 1 to predict SFR had significant clinical benefits when the threshold value was greater than 20%, and the application of model 2 to predict the postoperative CR had a significant clinical benefit when the threshold was greater than 10%.Conclusions:Nomogram based on stone burden, operation time, intraoperative perfusion, stone location and operative options was validated internally with preferable predictive power on the effectiveness and safety of pediatric endoscopic surgery. This model can be used to predict the clinical effects of pediatric endoscopic lithotripsy. BMI was also an important factor for the safety of pediatric endoscopic procedures.

Objective:
To understand the fruit consumption of adults of Qingdao and examine the association between fruit consumption and stroke.
Methods:
We analyzed baseline data and stroke incidence of the participants who were aged 30-79 years and had been enrolled into the China Kadoorie Biobank (CKB) study from Qingdao city. Cox proportional hazards regression model was conducted to estimate the association of fruit consumption with risk of stroke.
Results:
A total of 35 509 participants were investgated in the baseline survey. Ratio of male to female was 1∶1.27, and the average age was (50.3±10.2) years. Respondents with higher frequency of fruit consumption were younger, more women, with higher education level and higher income (

Stroke has increasingly become one of the three major diseases threatening human beings,among which acute ischemic stroke is the most common.Intravenous thrombolysis has become the first choice for acute ischemic stroke,but a large number of studies have shown that intravenous thrombolysis increases the risk of hemorrhagic transformation.The latest advances in clinical types,incidence,mechanism and risk factors of hemorrhagic transformation after thrombolysis with recombinant human tissue plasminogen activator are reviewed in this article,and the possible predictors of hemorrhagic transformation are discussed,aiming to provide new reference for clinical thrombolytic therapy.
