1.Recent advance in myelin injury and pro-myelin remodeling in improving cognitive dysfunction

Cognitive dysfunction often coexists with age-related neurodegenerative diseases, leading to decline in the quality of life of individuals and bringing burden on families and society. Myelin sheath is one of the important structures of neurons, and myelin damage can be seen in patients with cognitive dysfunction resulting from neurodegenerative diseases. Early intervention of myelin damage, such as promoting OLs differentiation and miRNA-based therapy, can help pro-myelin remodeling, thereby improve cognitive dysfunction and reduce incidence of cognitive dysfunction in neurodegenerative diseases. This article reviews the recent advance in relationship between myelin injury and cognitive function, and pro-myelin remodeling in improveing cognitive dysfunction, in order to provide references for treatment of cognitive dysfunction.
2.Association of Interpregnancy Weight Change with Recurrence of Gestational Diabetes Mellitus

Objective To explore the association between interpregnancy weight change and the recurrence of gestational diabetes mellitus(GDM).Methods This study included women who had two delivery records and a history of GDM during their previous pregnancy at the Guangdong Women and Children Hospital between January 2017 and July 2022.Interpregnancy weight change was defined as the difference between the prepregnancy BMIs of two consecutive pregnancies.Interpregnancy weight change was categorized into four groups(<-1,-1 to<1,1 to<3,≥3 kg/m2),with the-1 to<1 kg/m2 group serving as the reference.Logistic regression analyses were used to assess the associations of interpregnancy weight changes with GDM recurrence and changes in glucose values during two consecutive pregnancies.Results Among 703 participants,326 individuals(46.4%)experienced GDM recurrence.Compared with the groups with an interpregnancy BMI change of-1 to<1 kg/m2,the groups with a BMI change of 1 to<3 or ≥3 presented an increased risk of GDM recurrence,with odds ratio(OR)[95%confi-dence intervals(CI)]of 2.16(1.52-3.08)and 2.56(1.44-4.56),respectively.The risk of GDM recurrence significantly in-creased with increasing interpregnancy weight gain(P for trend<0.01).However,there was no significant association between a BMI change<-1 kg/m2 and GDM recurrence,with an OR(95%CI)of 0.95(0.57-1.57).An increase of 1 kg/m2 in the in-terpregnancy BMI was associated with a higher risk of elevated fasting blood glucose and 1 h plasma glucose,with OR(95%C1)of 1.15(1.05-1.26)and 1.21(1.10-1.33),respectively.Conclusion Women with a history of GDM are at a high risk of GDM recurrence,and interpregnancy weight gain is associated with an increased risk of GDM recurrence.These findings under-score the importance of health education among women with a history of GDM who plan to conceive again and emphasize the im-portance of weight management in reducing the recurrence of GDM.
3.Research progress on preoperative evaluation of patients undergoing day surgery

Day surgery, as a new perioperative management model that is safe, efficient, and consumes a small amount of medical resources, has been widely recognized in recent years. With the requirements of China's new healthcare system reform and the promotion of national policies, there is a significant increase in hospitals carrying out day surgery, and they are about to enter a new stage of rapid development. Adequate preoperative evaluation of patients undergoing day surgery is an important link in improving perioperative care during day surgery. This article reviews the importance, location and personnel, methods, timing and content of day surgery preoperative evaluation, and current situation of day evaluation in China, in order to provide reference for clinical workers related to day surgery.
4.Summary of the best evidence for preoperative evaluation and management of adult day surgery patients

Objective:To retrieve, evaluate, and integrate the best evidence for preoperative evaluation and management of adult day surgery patients.Methods:The guidelines, evidence summary, systematic review, expert consensus and randomized controlled trial on preoperative evaluation and management of adult day surgery patients were systematically searched on Guidelines International Network, Chinese Clinical Guidelines Library, Clinical Practice Guideline of Canadian Medical Association, Scottish Intercollegiate Guidelines Network, Yimaitong, New Zealand Guidelines Group, China Ambulatory Surgery Alliance, National Institute for Health and Clinical Excellence, Chinese Medical Journal Network, Agency for Healthcare Research and Quality, UpToDate, Cochrane Library, Australian Joanna Briggs Institute Evidence-Based Healthcare Center Database, British Medical Journal (BMJ) Best Practice, PubMed, Web of Science, CINAHL, Elsevier, Embase, Medline, WanFang Data, China National Knowledge Infrastructure, China Biomedical Literature Database and other databases. After conducting methodological quality evaluation, evidence was extracted and integrated based on the theme. The search period was from the establishment of the database to May 15, 2023. Two researchers trained in evidence-based knowledge conducted a rigorous literature quality evaluation, evidence extraction, and integration of all included literature.Results:A total of 12 articles were included, including two guidelines, six evidence summaries, three expert consensus and one randomized controlled trial. A total of 24 pieces of evidence were collected from five aspects, including evaluation timing and evaluation method, evaluation personnel, evaluation location, evaluation content, education and training, and quality improvement.Conclusions:The best evidence for preoperative evaluation and management of adult day surgery patients provides evidence basis for clinical medical and nursing staff to conduct preoperative evaluation and management comprehensively and effectively.
5.Evaluation on application of virtual reality technology in dental implant process training

Objective:To explore the application effect and evaluation of virtual reality technology in oral implant therapy training.Methods:In November 2018, one adult patient (female, 36 years old) with missing right mandibular first molar was treated in the Department of Implantology, School and Hospital of Stomatology, Fujian Medical University. The three-dimensional virtual models of mandible and implant surgery tools were established, and the virtual reality software (Unity 3D 5.5.1) was imported. Combined with the virtual reality head mounted display, a virtual reality training system simulating the dental implant treatment process was independently developed. Ten refresher doctors and 20 graduate students in Department of Implantology, School and Hospital of Stomatology, Fujian Medical University from September 2018 to December 2019 were recruited as the experimental objects (no clinical experience was found). According to the level and seniority of doctors, they were randomly divided into virtual training group and conventional training control group, which made the two groups comparable, with 15 in each group. Subjective scores (including anatomical structure, surgical field of vision, cavity preparation, implant placement and process mastery) were given after the corresponding training in the two groups, and the virtual reality training system was used to test. The mesial and distal direction, buccolingual direction, depth and angle deviation of implants before and after the training were analyzed, and the differences between the two groups were compared.Results:The subjective scores of five dimensions in the virtual training group were significantly higher than those in the conventional training control group ( P<0.05). In the virtual training group, the mesial and distal, buccolingual, depth and angle deviation of implants were (0.73±0.33), (0.78±0.41), (0.61±0.32) mm and 6.66°±3.87°. All of them were significantly lower than those in the control group [(0.85±0.32), (1.12±0.38), (0.89±0.24) mm and 9.68°±3.74°] ( P<0.05). Conclusions:The self-developed virtual reality system of oral implant has good application effect, good operability and predictability. It can be effectively carried out in implant education and training, and it can strengthen skills of doctors, and is conducive to the practical operation.
6.Evidence-based practice of non-drug management of chemotherapy induced nausea and vomiting in patients with gynecological malignant tumors

Objective:To apply the best evidence of non-drug management of chemotherapy induced nausea and vomiting (CINV) in clinical practice, and evaluate the effect of practice.Methods:After searching domestic and foreign databases, 14 articles were finally included, including 3 systematic reviews, 3 evidence summaries and 8 guidelines. According to the evidence-based continuous quality improvement model of Joanna Briggs Institute (JBI) Evidence-based Nursing and Health Care Center, we conducted the review before and after evidence application, obstacle factors, available resources and strategy analysis, and evaluation of the effect of evidence application. From March to May 2020, convenience sampling was used to select 52 inpatients with gynecological chemotherapy in the Affiliated Hospital of Qingdao University and 26 clinical nurses as subjects for baseline review. From June to August 2020, 42 inpatients with gynecological chemotherapy and 26 clinical nurses (same as before) were selected for re-review.Results:The management process, nursing routine, nursing path, and risk assessment and follow-up form for CINV were improved, and an evidence-based education video was produced. After application of the evidence, the compliance of nurses' review indicators improved, and the differences were statistically significant ( P<0.05) . The scores of nurses on the non-drug management evidence-based knowledge questionnaire for gynecological CINV increased, and the difference was statistically significant ( P<0.05) . The incidence of acute CINV decreased, and the difference was statistically significant ( P<0.05) . The incidence of delayed CINV decreased, and the difference was not statistically significant ( P> 0.05) . Conclusions:The development of the evidence-based practice project promotes the improvement of related processes, routines, and paths, enriches the forms and content of CINV non-drug antiemetic education, standardized nurses' behavior, improves nurses' compliance with evidence-based practice, and reduces the incidence of acute nausea and vomiting in patients.
7.The significance of seminal plasma polymorphonuclear neutrophil elastase and semen quality in male infertility

Objective:To investigate the clinical significance of seminal plasma polymorphonuclear neutrophil elastase(PMNE) concentration and semen quality in male infertility.Methods:Semen samples of 941 patients admitted to the andrology department from June 2017 to December 2018 in Dalian Municipal Women and Children′s Medical Center were selected. According to the World Health Organization (WHO) fifth edition standards, the positive group had a leukocyte content ≥ 1 × 10 9/L, and the negative group had a leukocyte content < 1 × 10 9/L; the samples were divided into the inflammatory group and the non-inflammatory group with the PMNE concentration of 600 μg/L as the cut-off point, and the relationship between the PMNE, semen parameters and mycoplasma positive rate was analyzed; the samples were divided into sterile group and normal group according to WHO standard, and the relationships among semen routine parameters, PMNE concentration and mycoplasma positive rate were analyzed. Results:PMNE in the positive group was significantly higher than that in the negative group [(1823.29 ± 557.64) × 10 9/L vs. (480.60 ± 195.36) × 10 9/L], and PMNE concentration was positively correlated with leukocyte content ( P<0.05); the semen quality of the inflammatory group decreased, and the positive rate of mycoplasma was higher than that of the non-inflammatory group [(23.7%(71/299) vs. 11.5%(74/642)] ( P<0.05). The semen quality of the sterile group decreased, and the PMNE and mycoplasma positive rate were both higher than that in the normal group [(1 230.89 ± 489.09) × 10 9/L) vs. (596.78 ± 159.25) × 10 9/L, 34.7%(90/259) vs. 8.1%(55/682)]( P<0.05). Conclusions:There is a good correlation between the concentration of PMNE and the leukocyte content, which can be used as an index to evaluate the male reproductive system infection. The concentration of PMNE is closely related to semen quality, which has clinical value in the diagnosis of male infertility.
8.Prenatal diagnosis of anomalous origin of one pulmonary artery branch from the aorta and unilateral absence of pulmonary artery by echocardiography

Objective To improve the prenatal diagnosis and differential diagnosis ability for anomalous origin of one pulmonary artery branch from the aorta ( AOPA ) and unilateral absence of pulmonary artery ( U APA ) by comparing and analyzing different types of fetal echocardiography features . Methods Fetal echocardiographic features were analyzed retrospectively in fetus with prenatal diagnosis of U APA or AO PA between June 2012 and December 2018 ,and the different types of fetal echocardiography characteristics and main points of identification were summarized . Results A total of 6 fetuses with AOPA and 6 fetuses with U APA were diagnosed .Among the 6 fetuses with AOPA ,5 cases were anomalous origin of right pulmonary artery from the aorta ( AORPA ) ,including 3 cases with isolated AORPA and 2 cases combined with Berry syndrome ,and 1 case was anomalous origin of left pulmonary artery from the aorta ( AOLPA ) with an absent pulmonary valve . All 6 cases of AOPA had proximal form . Among the 6 fetuses with U APA ,4 cases were isolated unilateral absence of right pulmonary artery ,1 case was isolated unilateral absence of left pulmonary artery and 1 case was unilateral absence of left pulmonary artery combined with tetralogy of Fallot ( T OF) . T he main common characteristic sonographical findings included :there was no confluence at the bifurcation of the main pulmonary artery on multiple view s ,and the affected pulmonary branch was not attached to the main pulmonary artery . T he different characteristic sonographical findings in fetus included :the affected pulmonary artery of AO PA arosed from the right posterior or left posterior portion of the ascending aorta on three vessels and trachea view ( 3V T ) and long axial view of left ventricle , and 3V T and the coronary view of brachiocephalic trunk artery showed that the abnormal origin of pulmonary artery branch of U APA connected with the root of brachiocephalic trunk artery by the ipsilateral vertical arterial duct ( DA ) ,and color Doppler flow imaging showed that the blood flow of the affected pulmonary artery branch was isolated in the pulmonary field . Conclusions AOPA and UAPA have many similarities in the characteristics of fetal echocardiography . 3V T and long axial view of left ventricle are the best view s for displaying proximal AOPA ,3VT and the coronary view s of the root of brachiocephalic trunk artery are the best view s for displaying UAPA .

磷脂酰肌醇蛋白聚糖3(glypican-3,GPC3)是一种锚定在细胞膜上的硫酸乙酰肝素(heparan sulfate,HS)蛋白多糖的家族成 员之一。GPC3激活经典的Wnt/β-连环蛋白(β-catenin)途径在肝癌(hepatocellular carcinoma,HCC)中表现出促癌基因的作用。尽 管GPC3在胎肝中含量丰富,在多种实体肿瘤中高表达,然而在成人正常组织中含量极少。选择靶点是肿瘤免疫治疗的关键。迄 今为止靶向GPC3的MRI、PET和近红外成像已被用于早期HCC检测。针对GPC3+实体瘤也已经开发了各种免疫治疗方案,一种 是基于抗GPC3抗体包括单克隆抗体、多肽疫苗、免疫毒素、双特异性抗体等,一种是靶向GPC3的CAR-T/NK疗法,其中部分已 进入I/II期临床试验。靶向GPC3有可能为实体瘤治疗提供新的工具。本文概述GPC3的结构、功能等生物学特性,介绍基于抗 GPC3抗体、CAR-T细胞开发的新策略,提供GPC3免疫疗法靶向实体瘤的证据。
10. Prenatal diagnosis of anomalous origin of one pulmonary artery branch from the aorta and unilateral absence of pulmonary artery by echocardiography

Objective:
To improve the prenatal diagnosis and differential diagnosis ability for anomalous origin of one pulmonary artery branch from the aorta (AOPA) and unilateral absence of pulmonary artery (UAPA) by comparing and analyzing different types of fetal echocardiography features.
Methods:
Fetal echocardiographic features were analyzed retrospectively in fetus with prenatal diagnosis of UAPA or AOPA between June 2012 and December 2018, and the different types of fetal echocardiography characteristics and main points of identification were summarized.
Results:
A total of 6 fetuses with AOPA and 6 fetuses with UAPA were diagnosed.Among the 6 fetuses with AOPA, 5 cases were anomalous origin of right pulmonary artery from the aorta (AORPA), including 3 cases with isolated AORPA and 2 cases combined with Berry syndrome, and 1 case was anomalous origin of left pulmonary artery from the aorta (AOLPA) with an absent pulmonary valve. All 6 cases of AOPA had proximal form. Among the 6 fetuses with UAPA, 4 cases were isolated unilateral absence of right pulmonary artery, 1 case was isolated unilateral absence of left pulmonary artery and 1 case was unilateral absence of left pulmonary artery combined with tetralogy of Fallot (TOF). The main common characteristic sonographical findings included: there was no confluence at the bifurcation of the main pulmonary artery on multiple views, and the affected pulmonary branch was not attached to the main pulmonary artery. The different characteristic sonographical findings in fetus included: the affected pulmonary artery of AOPA arosed from the right posterior or left posterior portion of the ascending aorta on three vessels and trachea view (3VT) and long axial view of left ventricle, and 3VT and the coronary view of brachiocephalic trunk artery showed that the abnormal origin of pulmonary artery branch of UAPA connected with the root of brachiocephalic trunk artery by the ipsilateral vertical arterial duct (DA), and color Doppler flow imaging showed that the blood flow of the affected pulmonary artery branch was isolated in the pulmonary field.
Conclusions
AOPA and UAPA have many similarities in the characteristics of fetal echocardiography. 3VT and long axial view of left ventricle are the best views for displaying proximal AOPA, 3VT and the coronary views of the root of brachiocephalic trunk artery are the best views for displaying UAPA.
