Climate change is a significant environmental concern in the 21st century. It can directly and indirectly affect public health, making it a major public health problem. This review focused on the relationship between meteorological factors and health-related behaviors such as physical activity, sleep, dietary behavior, and social contact behavior. The results indicated that meteorological factors such as temperature, relative humidity, wind speed, atmospheric pressure, and rainfall are associated with the four health behaviors. Physical activity levels are negatively associated with temperature, relative humidity, and rainfall. Sleep quality tends to decrease under extreme temperatures and extreme weather events. Dietary behavior is influenced by high and low temperatures, low pressure, and daylight duration, which in turn affects appetite and food intake. Social contact frequency decreases in response to extreme temperatures, increased rainfall, and extreme weather events. However, there are differences in the strength and direction of the associations between meteorological factors and health-related behaviors in various studies. Research on the combined effects of multiple meteorological factors exposure on health-related behaviors is rare, and the mechanisms underlying the associations are unclear. Therefore, there is a need for more multi-center, large-scale studies to explore the biological and behavioral mechanisms behind these associations, which will help clarify the complex effects of meteorological factors on human health behaviors and provide scientific evidence for policy-making, thereby mitigating the negative impact of climate change on public health.

Falls are one of the most common injuries, with a high prevalence and risk of disability, which poses the greatest threat to the health of the elderly. Falls are influenced by a variety of factors, including environmental factors such as slippery pavements, poor meteorological conditions, and inadequate lighting. The Sixth Assessment Report of the United Nations Intergovernmental Panel on Climate Change (IPCC) stated that climate change has become more frequent and stronger in recent years, which may threat people's health and lives. This review summarized the progress of epidemiological studies on the association between meteorological factors (e.g. temperature, precipitation, frost, and snowfall) and falls, and found that most of the studies focused on the association between temperature and falls, and the results of different studies varied greatly. While the studies on the association between other meteorological factors and falls were relatively few. Moreover, the mechanisms behind the associations were unclear. Therefore, we point out that it is necessary to further conduct large-sample and multi-centre studies to evaluate the effects of exposure to multiple meteorological factors, and further to explore the mechanisms of the associations, which may provide scientific evidence for better response to climate change.

Objective:To study the clinical value of blood neutrophil gelatinase-associated lipocalin (NGAL) in the early diagnosis and prognostic evaluation of late-onset sepsis in very/extremely low birth weight infants (VLBWI/ELBWI).Method:From January 2017 to December 2019, VLBWI/ELBWI older than 3 days admitted to NICU of our hospital were prospectively enrolled in the study. The infants were assigned into suspected-sepsis group and non-infection (control) group according to their clinical symptoms and laboratory indicators. In the suspected-sepsis group, complete blood count, C-reactive protein (CRP), procalcitonin (PCT) and blood culture were examined on the 1st day of disease onset and blood NGAL was examined on the 1st day of disease onset, 3rd day of treatment and 2nd week of treatment. In the control group, blood NGAL was examined at the time of enrollment. The suspected-sepsis group was later assigned into sepsis group and non-sepsis infection group and the sepsis group was further assigned into mild sepsis group and severe sepsis group according to the severity of the disease. Blood NGAL levels between the sepsis group and the non-sepsis infection group on the 1st day of onset and the control group were compared. The dynamic changes of NGAL in the sepsis group and the non-sepsis infection group at different time points were compared and analyzed. ROC curve of NGAL level on the first day of onset predicting sepsis was drawn.Result:(1) On the 1st day of disease, the sepsis group (n=106) had higher level of NGAL compared with non-sepsis infection group (n=121) and the control group (n=84). Non-sepsis infection group had significantly higher level of NGAL compared with the control group ( P<0.05). (2) A gradual decrease of NGAL was found in both sepsis and non-sepsis infection group. Significantly higher level of NGAL in sepsis group was found comparing with non-sepsis infection group at different time points ( P<0.05). (3) For blood culture positive and negative patients in the sepsis group, no statistically significant differences existed in NGAL,CRP, PCT levels on the 1st day of disease onset ( P>0.05).(4) The NGAL level in the severe sepsis group was significantly higher than the mild sepsis group on the 1st day of disease onset ( P<0.05). However,CRP and PCT showed no differences between the two groups. (5) On the 1st day of disease onset, to establish the diagnosis of sepsis, the area under the ROC curve of NGAL level was 0.852. The sensitivity and specificity of cut-off value 205.25 ng/ml were 84.0% and 66.9%, respectively. Conclusion:The serum NGAL level is elevated in VLBWI/ELBWI with late-onset sepsis. The more severe the sepsis, the more elevated the NGAL level. NGAL has certain predictive value for late onset sepsis in VLBWI/ELBWI.

Objective Analysis of hybrid cardiac and non-cardiac operations. Methods 11 patients received hybrid operations during 2000 ~2016 in our hospital were summarized, meanwhile another 11 patients undergoing only cardiac sur-gery served as controls. The level of CK-MB, TNI and visual analogue pain scores were recorded. The hospitalization expenses and postoperative hospital time were compared between the two groups .Results There was no death during hospitalization . There were no serious complications such as wound infection, delayed wound healing, postoperative bleeding requiring re-thora-cotomy and hemostasis, malignant arrhythmia, cerebral infarction and so on.Longer operation time[(358 ±79)min vs.(224 ±56)min, P<0.01], more allogeneic red blood cells transfusion[(3.2 ±1.6)U/case vs.(1.1 ±0.8)U/case, P<0.05] and more plasma transfusion[(515 ±234)ml/case vs.(284 ±92)ml/case, P<0.01]in hybrid operation group than those in the control group. There was no significant difference about CK-MB and TNI between two groups perioperatively(P>0.05). The wound pain was mild after cardiac surgery , and the wound pain was moderate after hybrid operation , which had little effect on rehabilitation exercises such as cough.4 days longer of postoperative hospital stay and 23 thousand yuan of hospitalization cost in hybrid operation group than those in the control group. One patient diagnosed of unstable angina pectoris and sigmoid colon cancer with intestinal obstruction was failed to resection of the tumor after received pericardial stripping and coronary ar-tery bypass surgery. The patient died three weeks after surgery due to intestinal obstruction and cachexia .The remaining 10 pa-tients were underwent successful surgery, cured and left hospital. No serious cardiovascular events, tumor recurrence and me-tastasis were found as followed up for 6-20 months. Conclusion Our practices show that hybrid cardiac and non-cardiac op-erations can be done safely and effectively.
10.The association between depressive symptoms and fall accidents among middle-aged and elderly people in China.
Environmental Health and Preventive Medicine 2018;23(1):42-42
BACKGROUND:
Depressive symptoms are a worldwide health problem. However, the research about the effect of depressive symptoms on the fall among the Chinese mid-aged and elderly people is lacking. Therefore, this study aims to investigate the association between depressive symptoms and fall accidents among middle-aged and elderly people in China.
METHODS:
This study was conducted based on 12,527 sets of data from China Health and Retirement Longitudinal Survey (CHARLS). The 2011 depressive symptoms data and the 2013 fall data were chosen for this study. The depressive symptom-related data was assessed by the Chinese version of Center for Epidemiologic Studies Depression scales (CES-D). Individuals were divided into subgroups according to gender (male or female), age (45-59, middle-aged or ≥ 60, elderly people), and residence (rural or urban). The odds ratios (ORs) were compared between subgroups using multivariable logistic regression analysis method.
RESULTS:
The adjusted OR value (OR = 1.19 [95% CI 1.07-1.33]) shows there is a significant association between depressive symptoms and subsequent fall accidents. The ORs of the female, elderly people, rural, and urban subgroups are 1.31 (95% CI 1.11-1.55), 1.24 (95% CI 1.08-1.43), 1.17 (95% CI 1.02-1.33), and 1.25 (95% CI 1.04-1.49), respectively, which reveals that this association is also statistically significant in these subgroups.
CONCLUSIONS
This study shows that there is a significant association between depressive symptoms and their subsequent fall accidents among the Chinese middle-aged and elderly people.
