Apnoeic oxygenation(AO)is the uptake of oxygen from the lungs by the patient's blood to maintain oxygenation in the absence of spontaneous breathing or mechanical positive pressure ventilation.During the administration of general anesthesia, the use of AO prolongs the patient's safe apnea time on top of preoxygenation before voluntary respiration ceases after induction and a safe airway is established, which may reduce the number of attempts at endotracheal intubation, and decrease the incidence of airway injury, hypoxemia, and hemodynamic decompensation.Over the past decade, more and more AO techniques have been applied in different clinical settings, such as perioperative airway management and"tube-free anesthe-sia".In order to deepen the understanding of AO and its applications, this article reviews the latest research progress on the mechanism, method and clinical application of AO for clinical applications provide refer-ence.

Objective:To analyze the types and functions of CD34 + cells in full-thickness skin defect wounds of normal mice and diabetic mice by single-cell RNA sequencing. Methods:This study was an experimental study. The CD34 + cell lineage tracing mouse was produced, and the visualization of CD34 + cells under the fluorescent condition was realized. Six male CD34 + cell lineage tracing mice aged 7-8 weeks (designated as diabetic group) were intraperitoneally injected with streptozotocin to establish a diabetic model, and full-thickness skin defect wounds were prepared on their backs when they reached 13 weeks old. Another 6 male CD34 + cell lineage tracing mice aged 13 weeks (designated as control group) were also subjected to full-thickness skin defect wounds on their backs. On post-injury day (PID) 4, wound tissue was collected from 3 mice in control group and 2 mice in diabetic group, and digested to prepare single-cell suspensions. CD34 + cells were screened using fluorescence-activated cell sorting, followed by single-cell RNA sequencing. The Seurat 4.0.2 program in the R programming language was utilized for dimensionality reduction, visualization, and cell clustering analysis of CD34 + cell types, and to screen and annotate the marker genes for each CD34 + cell subpopulation. Kyoto encyclopedia of genes and genomes (KEGG) and gene ontology (GO) enrichment analysis was performed to analyze the differentially expressed genes (DEGs) of CD34 + fibroblasts (Fbs), smooth muscle cells (SMCs), keratinocytes (KCs), and chondrocyte-like cells (CLCs) in the wound tissue of two groups of mice for exploring cellular functions. Results:On PID 4, CD34 + cells in the wound tissue of both groups of mice were consisted of 7 cell types, specifically endothelial cells, Fbs, KCs, macrophages, T cells, SMCs, and CLCs. Among these, Fbs were further classified into 5 subpopulations. Compared with those in control group, the proportions of CD34 + endothelial cells, Fbs subpopulation 1, Fbs subpopulation 4, KCs, and CLCs in the wound tissue of mice were increased in diabetic group, while the proportions of CD34 + Fbs subpopulation 2, Fbs subpopulation 3, and SMCs were decreased. The marker genes for annotating CD34 + CLCs, endothelial cells, Fbs subpopulation 1, Fbs subpopulation 2, Fbs subpopulation 3, Fbs subpopulation 4, Fbs subpopulation 5, KCs, macrophages, SMCs, and T cells were respectively metastasis-associated lung adenocarcinoma transcript 1, fatty acid binding protein 4, Gremlin 1, complement component 4B, H19 imprinted maternally expressed transcript, Dickkopf Wnt signaling pathway inhibitor 2, fibromodulin, keratin 5, CD74 molecule, regulator of G protein signaling 5, and inducible T-cell co-stimulator molecule. KEGG and GO enrichment analysis revealed that, compared with those in control group, DEGs with significant differential expression (SDE) in CD34 + Fbs from the wound tissue of mice in diabetic group on PID 4 were significantly enriched in terms related to inflammatory response, extracellular matrix (ECM) organization, regulation of cell proliferation, and aging (with Pvalues all <0.05), DEGs with SDE in CD34 + SMCs were significantly enriched in terms related to cell migration, apoptotic process, positive regulation of transcription, and phagosome (with P values all <0.05), DEGs with SDE in CD34 + KCs were significantly enriched in terms related to mitochondrial function, transcription, and neurodegenerative diseases (with P values all <0.05), and DEGs with SDE in CD34 + CLCs were significantly enriched in terms related to rhythm regulation, ECM, and viral infection (with P values all <0.05). Conclusions:CD34 + cells display high heterogeneity in the healing process of full-thickness skin defect wounds in both normal mice and diabetic mice. The significantly enriched functions of DEGs with SDE in CD34 + cell subpopulations in the wound tissue of the two mouse groups are closely related to the wound healing process.

Objective:Currently, patients with pre-exsiting donor-specific antibody(DSA)are prone to antibody-mediated rejection(AMR)after surgery and are at a relatively high risk of postoperative complications and graft failure. The risk of postoperative complications and graft failure is relatively high. This study aims to discuss the clinical outcome of DSA-positive kidney transplantation and analyze the role and safety of preoperative pretreatment in DSA-positive kidney transplantation, providing single-center treatment experience for DSA-positive kidney transplantation. Methods:We retrospectively analyzed the clinical data of 15 DSA-positive kidney transplants in the Department of Renal Transplantation of First Affiliated Hospital of Zhengzhou University from August 2017 to July 2022.Eight cases were organ donation after citizen's death(DCD)kidney transplant recipients,of which 3 cases in the early stage were not treated with preoperative desensitisation therapy(DCD untreated group,n=3),and 5 recipients were treated with preoperative rituximab desensitisation(DCD preprocessing group,n=5).The remaining 7 cases were living related donors recipients(LRD)who received preoperative desensitisation treatment with rituximab and plasma exchange(LRD preprocessing group,n=7).We observed and recorded the incidence of complications with changes in renal function and DSA levels in the recipients and the survival of the recipients and transplanted kidneys at 1,3 and 5 years, and to compare the differences in recovery and postoperative complications between 3 groups. Results:All 15 recipients were positive for preoperative panel reactive antibody(PRA)and DSA and were treated with methylprednisolone+rabbit anti-human thymocyte immunoglobulin induction before kidney transplantation.DCD untreated group all suffered from DSA level rebound, delayed renal graft function(DGF)and rejection reaction after surgery. After the combined treatment,DSA level was reduced and the graft renal function returned to normal. The DCD preprocessing group were all without antibody rebound,1 recipient developed DGF and the renal function returned to normal after plasmapheresis, and the remaining 4 recipients recovered their renal function to normal within 2 weeks after the operation. In the LRD preprocessing group,2 cases had antibody rebound and 1 case had rejection, but all of them recovered to normal after treatment, and DSA was maintained at a low level or even disappeared. The incidence of DGF and rejection in the DCD untreated group were significantly higher than that in the DCD preprocessing group and the LRD preprocessing group;and there were no significant difference in the incidence of postoperative haematuria, proteinuria, bacterial and fungal infections, and BK virus infection between the 3 groups(all P>0.05).A total of 11 of the 15 recipients were followed up for more than 1 year,6 for more than 3 years, and 1 for more than 5 years, and the survival rates of both the recipients and the transplanted kidneys were 100%. Conclusion:Effective preoperative pretreatment with desensitization therapy can effectively prevent antibody rebound in DSA-positive kidney transplantation and reduce perioperative complications.

ObjectiveTo explore the possible mechanism of Yupingfeng Granules (玉屏风散) in preventing and treating chronic obstructive pulmonary disease (COPD) from the perspective of “lung-gut axis”. MethodsThirty-two male Wistar rats were randomly divided into normal group, model group, roxithromycin group and Yupingfeng Granules group, with 8 rats in each group. Except for the normal group, the rat model of COPD was prepared by intratracheal instillation of lipopolysaccharide (LPS) combined with smoking for 12 weeks. Since the fifth week of modeling, the roxithromycin group and the Yupingfeng Granules group were given 31.5 mg/(kg·d) and 1.575 g/(kg·d) of corresponding drugs respectively by gavage, and normal group and model group were given 10 ml/(kg·d) physiolo-gical saline. Sample was collected 24 hours after the last administration. The pathological changes of lung tissue were observed using HE staining; Ultrahigh performance liquid chromatography quadrupole time of flight mass spectrometry (UHPLC-QTOFMS) was used to detect the differential metabolites in alveolar lavage fluid (BALF) in all groups but roxithromycin group;16S rDNA sequencing technology was used to detect the changes of intestinal flora, and the association analysis was conducted between the differential metabolites and the differential flora. ResultsCompared with the normal group, the model group showed an increase in goblet cells in the small bronchial wall, disappearance of the smooth muscle layer of the bronchial wall, and infiltration of inflammatory cells; compared with the model group, roxithromycin group showed slight alveolar interstital edema, and obviously reduced inflammatory cell, while no obvious alveolar interstital edema was observed in the Yupingfeng Granules group, showing a small amout of inflammatory cell infiltration. The results of the BALF differential metabolite screening showed that compared with the normal group, 12 substances were upregulated and 19 substances were downregulated in the model group; compared with the model group, 37 substances in the Yupingfeng Granules group were upregulated and 43 substances were downregulated KEGG analysis yielded a total of 2 metabolic pathways, glycerophospholipid metabolism, and unsaturated fatty acid biosynthetic metabolism; compared with the model group, choline, acetylcholine, glycerol-3-phosphate, glycerophosphate choline, palmitic acid, and arachidonic acid showed an upward trend, while stearic acid and docosahexaenoic acid showed a downward trend in Yupingfeng Granules group (P<0.05). The results of the intestinal flora showed that, there are 80 different species between the normal group and the model group, and 65 different species between the model group and Yupingfeng Granules group. Among the top 5 species with relative abundance levels, compared with the model group, the level of Prevotella_9,Ruminococcaceae_UCG-005,Ruminiclostridium_6 increase, and Lactobacillus,Bacteroides decrease(P<0.05).The results of the correlation analysis showed that, in the normal and model groups, arachidonic acid was negatively correlated with Oribacterium(r=

Chinese Journal of Practical Nursing 2022;38(10):797-801
The sacrum is a common site of stress injury. The occurrence of pressure ulcers not only leads to the aggravation of the patient′s condition, the prolongation of hospital stay, the increase of medical costs, the decline of life quality, but also aggravates the burden of society, family and medical staff. Early reasonable and effective application of sacral dressing can prevent the occurrence of pressure injury. In this study, the characteristics and effectiveness of different types of sacral dressings were reviewed by reviewing relevant literatures at home and abroad and summarizing the existing relevant studies, so as to provide guidance for clinical application.

Objective:To investigate the clinical efficacy of 3D printed osteotomy guide plate combined with "Jail" screw technique in the treatment of tibial plateau fractures involving external posterior condylar collapse. Methods:From January 2016 to January 2021, 41 patients (22 males and 19 females) with tibial plateau fractures involving external posterior condylar collapse were treated with 3D printed osteotomy guide plate combined with "Jail" screw technique and followed up. The age was 47.4±11.5 years (range, 22-69 years). According to Schatzker fracture type, 18 cases were type IV, 14 cases were type V and 9 cases were type VI. All fractures were closed, and 12 of them were complicated with lateral meniscus injury, but none of them were complicated with nerve and vascular injury. The time from injury to operation was 7.2±3.4 d (range, 4-17 d). All patients underwent 3D CT scanning and digital modeling before operation. According to the modeling results, a 1∶1 solid size fracture model was made by 3D printing, and the osteotomy guide plate and the "Jail" screw preset guide plate were designed. During the operation, the tibial lateral condyle osteotomy was performed with customized osteotomy guide plate. After reduction, the fixation of the fracture was performed with the preset guide plate using "Jail" screw. Postoperative fracture reduction was evaluated according to Rasmussen score, and knee function was evaluated by Hospital for Special Surgery (HSS) score. Results:All the 41 patients were followed up for 15.2±5.8 months (range, 6-26 months). Immediate postoperative radiographs showed good fracture reduction, and the average healing time was 14.1±1.2 weeks (range, 12-17 weeks). One year after operation, the Rasmussen score of knee joint was 17.4±1.6 points (range, 13-19 points), of which 31 cases were excellent, 8 cases were good, and 2 cases were fair, with an excellent/good rate of 95% (39/41). HSS scores was 87.3±5.6 points (range, 68-95 points), including 30 excellent cases, 10 good cases and 1 fair case, with an excellent/good rate of 98% (40/41). The range of motion of knee joint was 126.8°±3.8°. At the last follow-up, no serious complications such as common peroneal nerve injury, popliteal vascular injury, postoperative infection, or internal fixation failure occurred.Conclusion:3D printed osteotomy guide plate combined with "Jail" nail placement technique is an effective method for tibial plateau fractures involving external posterior condylar collapse, and the postoperative treatment results are satisfactory. The use of customized osteotomy guide plate is more accurate and less damaging. The use of "Jail" screw preset guide plate can ensure more accurate screw placement.
