1.Visual analysis of the impact of T cells on rheumatoid arthritis in the past decade based on multiple analysis methods

Objective Through a multi-software visual analysis of the literature on the influence of T cells on rheumatoid arthritis(RA)in recent ten years,the research hotspot and frontier development in this field were summarized.Methods The Chinese and English literature on the influence of T cells on RA from 2012 to 2022 years was retrieved from CNKI and Web of Science database as the research object.CiteSpace and VOSviewer software were used to analyze the number of publications,authors and keywords.Results 519 articles in Chinese and 861 in English were retrieved.The results showed that the number of articles in Chinese increased slowly from 2020 to 2022 years,while the overall trend in English was stable.Keyword analysis shows that it is predicted that future research in this field will focus on the pathogenesis of T cells in RA,the mechanism of bone destruction in RA,disease activity,oxidative stress.Conclusion The influence of T cells on RA has attracted much attention in the past,present and future,and has great research value.However,due to the differences in research priorities at home and abroad,the teams should interact positively and communicate with each other to reveal the internal mechanism of RA and provide theoretical basis for targeted therapy.
2.Mechanism of Bone Destruction in Rheumatoid Arthritis Based on the Theory of Kidney Deficiency and Essence Deficiency

Rheumatoid arthritis is a common clinical autoimmune disease characterized by persistent synovitis and pannus formation. In late stage, irreversible destruction and deformation of bone and joint may occur. In this paper, the authors believe that kidney deficiency and essence deficiency is the core mechanism of rheumatoid arthritis bone destruction, and its disease evolution law is summarized as "marrow reduction, flesh flaccid, collaterals blocked". On the basis of modern medical understanding of bone destruction in rheumatoid arthritis, it is considered that the mechanism in Chinese medicine of "marrow reduction, flesh flaccid, collaterals blocked" ultimately leads to bone destruction, is similar to that in the western medicine of abnormal differentiation of osteoclasts, high expression of nuclear factor-κB receptor activator of ligand, and abnormal expression of inflammatory factors. This point of view may provide a more comprehensive and scientific understanding of the key pathogenic mechanism of bone destruction in rheumatoid arthritis.
3.Epidemiological characteristics of injury deaths among children and adolescents in China, 2010-2020

Objective:
To explore the epidemic characteristics of injury related deaths in children and adolescents aged 1-24 years old in China from 2010 to 2020, so as to provide a basis for the formulation of policies and measures related to the control of injuries and deaths among children and adolescents.
Methods:
The data were sourced from the China Death Cause Monitoring Dataset from 2010 to 2020. Annual percentage change (APC) and average annual percentage change (AAPC) of injury deaths in China in this age group during the period 2010 to 2020 were analyzed by Join point regression.
Results:
From 2010 to 2020, the standardized death rate of injury showed a decreasing trend (AAPC=-6.90%, t =4.58, P <0.01). The standardized death rates of male and rural injuries showed an overall downward trend, with AAPC rates of -8.37% and -7.79%( t =11.87, 10.34, P <0.01). An increasing trend was observed in the 20-24 year-old age group during 2010-2018 (APC=18.11%, t =6.50, P <0.01). The death rate from injuries was higher in males than females, and higher in rural areas compared with urban areas ( χ 2=16 483.64, 3 268.65 , P <0.01). A downward trend was observed in accidental falls and suicide, the overall standardized mortality rate of traffic accidents, accidental poisoning, fire, drowning, homicide and other injuries (AAPC=-10.22%, -6.21%, -7.50%, -7.94%, -9.01% , -10.97%, t =16.23, 7.29, 2.53, 9.32, 7.88, 4.58, P <0.05).
Conclusion
From 2010 to 2020, the overall injury standardized mortality rate in the 1-24 year-old age group shows a decreasing trend, but it remains at a relatively high level. Prevention efforts should be continuously strengthened, especially for urban areas, and should focus on women and those aged 20-24 years old, as well as accidental falls and suicide prevention.

Objective To study the value of postmortem imaging on the diagnosis of intestinal perforation.Method Postmortem imaging(PMCT and PMCTA)data of 2 intestinal perforation deaths(and 4 controlled cases)were reviewed retrospectively.Diagnosing capacities of intestinal perforation by postmortem imaging method were further investigated.Results PMCT is sensitive in detecting the free air and liquid induced by intestinal perforation.PMCT can sometimes detect the gravity-dependent purulent secretions in the abdominopelvic cavity.PMCTA can visualize the extravasation of contrast agent from the perforation,which can be used to locate the accurate perforation region.Conclusion Postmortem imaging method(PMCT and PMCTA)is an important tool for the diagnosis of intestinal perforation,which can not only be used as a forensic diagnosis method,but is also useful to locate the perforation site before an forensic autopsy.
5.A case of brucellosis complicated with capillary leak syndrome and multiple organ dysfunction syndrome

Brucellosis can lead to pathological changes of multiple systems. Capillary leak syndrome (CLS) is a clinical syndrome caused by different reasons, mainly characterized by hypotension, hypoproteinemia and systemic edema. The condition is critical and the clinical manifestations are complex, and multiple organ dysfunction syndrome (MODS) may occur in severe cases. CLS caused by brucellosis is extremely rare. The diagnosis and treatment of a patient with brucellosis complicated with CLS and MODS was analyzed in this paper, in order to improve the knowledge of clinicians about brucellosis and its complications.
6.Role and mechanism of IL-6/STAT3/Th17 signaling pathway in the intervention of forsythia in acute lung injury of septic rats

Objective:To investigate the mechanism of interleukin-6 (IL-6) /signal transducers and activators of transcriptional 3 (STAT3) /helper T cell 17 (Th17) signaling pathway in lung injury of rats with sepsis intervened by forsythia, with a view to provide experimental basis for the role and mechanism of forsythia in the treatment of sepsis.Methods:In July 2021, 30 healthy Wistar male rats were selected and randomly divided into sham operation group, model group and treatment group, with 10 rats in each group. The rat model of sepsis was established by cecal ligation and puncture. 2 h after recovery, the rats were given traditional Chinese medicine forsythia orally, twice a day at an interval of 12 h. The wet/dry mass ratio (W/D) of lung tissue was detected 24 h after surgery. The morphological changes of lung tissue were detected by HE staining. Flow cytometry was used to detect Th17 population in peripheral blood. The expression levels of IL-6 and interleukin-17A (IL-17A) in serum were determined by enzyme-linked immunosorbent assay. The mRNA expression levels of IL-17A and IL-6 in lung tissues were detected by reverse transcription-polymerase chain reaction. The expression levels of STAT3 and IL-17A in lung tissue were determined by Western blotting.Results:Compared with model group, the W/D of lung tissue in treatment group was decreased, and the difference was statistically significant ( P<0.05). HE staining of lung tissue showed that compared with the model group, the degree of lung lesion and injury was reduced in the treatment group. Compared with sham operation group, the proportion of Th17 cells in CD4 lymphocytes in peripheral blood of rats in model group was significantly increased ( P<0.05). Compared with model group, the proportion of Th17 cells in CD4 lymphocytes in peripheral blood of rats in treatment group was significantly decreased ( P<0.05). Compared with sham operation group, peripheral blood serum IL-6 and IL-17 of rats in model group were significantly increased ( P<0.05). Compared with model group, IL-6 and IL-17 in peripheral blood serum of rats in treatment group were decreased ( P<0.05). Compared with sham operation group, the expressions of IL-17A and IL-6 mRNA in lung tissue of model group were significantly increased ( P<0.05). Compared with model group, the expressions of IL-17A and IL-6 mRNA in lung tissue of rats in treatment group were significantly decreased ( P<0.05). Compared with sham operation group, the protein expressions of STAT3 and IL-17A in lung tissue in model group were significantly increased ( P<0.05). Compared with model group, the pritein expressions of STAT3 and IL-17A in lung tissue in treatment group were significantly decreased ( P<0.05) . Conclusion:Forsythia plays a role in alleviating lung injury by down-regulating the expressions of the signaling pathway IL-6/STAT3/Th17, which providing a new target for the treatment of sepsis induced lung injury.

10.Improved three-dimensional CTA reconstruction of perforator area in the application of anterolateral thigh perforator flap and deep inferior epigastric artery perforator flap

Objective:To investigate the clinical value of improved perforator area CTA three-dimensional reconstruction in design and harvest of anterolateral thigh perforator flap(ALTPF) and deep inferior epigastric artery perforator flap(DIEPF).Methods:Repairs of defects of oral and maxillofacial tumour resection with ALTPF for 8 patients and defects of breast tumour resection with DIEPF for 2 patients were performed from September 2021 to January 2022 in the Department of Hand and Microsurgery of Affiliated Hospital of Binzhou Medical College. According to the improved scanning parameters and drug administration protocol, patients underwent CTA scans of both thighs or abbomen before operation. The data of CTA were sent to GE AW 4.7 work station to produce three-dimensional reconstruction of perforator area and angiosome. The source artery and perforator were observed dynamically from the angiosome in the perforator area, and the specific data were measured. The perforator location was marked by HHD, and then according to the measurement data of CTA three-dimensional reconstruction marked the location, course of perforator and the course of source artery on the body surface. The data of source arteries and perforators explored during the operation were compared with preoperative three-dimensional reconstruction. The perforator locations of CTA were compared with the HHD. The harvest time and survival condition of flap were compared with the previous patients who only had the perforator location markers from HHD. The sizes of ALTPFs and DIEPFs were 4.0 cm×4.0 cm-15.0 cm×6.0 cm and 19.0 cm×7.5 cm-25.0 cm×10.0 cm, respectively. The survival of flaps and the healing of wound were observed in the postoperative follow-up in terms of appearance, texture, function of recipient site and the shape and function of the donor site.Results:Eight ALTPFs and 2 DIEPFs all survived without any adverse event. Both recipient and donor sites healed well without any complication. Seven femoral septocutaneous perforators, 2 musculocutaneous perforators and abdominal 3 perforators coursed directly, 2 tortuously perforators were seen from three-dimensional reconstruction. The types and origins of perforators explored during operation were basically consistent with three-dimensional reconstruction. The accuracy of CTA[(0.36±0.11) cm] was higher than HHD[(0.54±0.19) cm] for perforators location( t=-3.160, P<0.05). Compared with the previous group[(74.60±30.53) min], this group[(52.80±24.57) min] had a shorter time to cut out the flap of similar area( t=-9.179, P<0.05). In the previous group, one flap transfer was failed due to the thinner caliber of perforator and source artery. All the flaps survived with satisfactory outline and softness with good blood supply after 2-6 months of follow-up. The oral and maxillofacial functions were normal. The reconstructed breasts were symmetrical with the healthy side, and the shape was satisfactory. Only linear scars remained in the donor sites without dysfunction. Conclusion:The improved CTA three-dimensional reconstruction of perforator area can help to determine the detailed location, course and distribution of the perforators at the superficial fascia layer. It provides a reliable bases in the design and harvest of perforator flaps during operation, reduces the perioperative risks and has certain clinical values.
