1.Effect of compound anisodine combined with laser photocoagulation on hemorheology of diabetic retinopathy

AIM: To evaluate the effectiveness and safety of compound anisodine combined with laser photocoagulation in the treatment of diabetic retinopathy(DR).METHODS: A prospective cohort study was used to select 80 patients(160 eyes)diagnosed with severe non-proliferative diabetic retinopathy(NPDR)and proliferative diabetic retinopathy(PDR)in Beijing Tongren Hospital, Capital MedTcal University and Beijing Daxing District People's Hospital from May 2023 to July 2023. They were divided into control group(40 cases, 80 eyes)and observation group(40 cases, 80 eyes)by random number table method. The control group only received 532 nm laser panretinal photocoagulation(PRP)treatment, while the observation group received PRP treatment together with superficial temporal subcutaneous injection of compound anisodine. The clinical efficacy, changes in hemorheology, changes in retinal blood vessels, and incidence of adverse reactions in the two groups were observed before and at 2 mo after treatment.RESULTS: The visual acuity, fundus changes and hemorheological parameters of the two groups were analyzed before and after treatment. There were no significant differences in the two groups before treatment(all P>0.05). The best corrected visual acuity of the observation group was better than that of the control group at 2 mo after treatment(P<0.05), and the clinical curative effect of fundus was also better than that of the control group(all P<0.05). The hemorheological indexes of central retinal artery blood flow(peak systolic velocity and end diastolic velocity)in the observation group were higher than those of the control group(all P<0.05), and the blood flow resistance index was lower than that of the control group(P<0.05).CONCLUSION: Compound anisodine combined with 532 nm laser photocoagulation is safe and effective in the treatment of DR, and the visual recovery effect is better.

3.Current status of perceived health competence and related influencing factors in patients with hepatolenticular degeneration

Objective To investigate the level of perceived health competence in patients with hepatolenticular degeneration, also known as Wilson disease (WD), as well as related influencing factors. Methods From January to December 2023, the convenience sampling method was used to select 60 patients with WD as the subjects for this study among the patients who were hospitalized in The Affiliated Hospital of the Neurology Institute of Anhui University of Chinese Medicine, and General Information Questionnaire, Perceived Health Competence Scale, Health Literacy Scale, Perceived Social Support Scale, and Perceived Control Scale were used to investigate the influencing factors for perceived health competence in patients with WD. Results The total score of perceived health competence was (25.85±5.03) in the patients with WD, with an outcome expectancy score of (12.12±3.55) and a behavioral expectancy score of (13.73±3.33), and the multivariate analysis showed that degree of education and health literacy assessment were influencing factors for perceived health competence in the patients with WD. Conclusions There is a moderate level of perceived health competence in patients with WD, which is influenced by the factors such as degree of education and health literacy assessment.
4.Clinical manifestations and prognosis of patients with hemichorea-hemiballism: An analysis of 25 cases

Objective To investigate the cause,pathogenesis,clinical manifestations,treatment,and prognosis of secondary hemichorea based on related clinical features and examinations by summarizing the medical records of 25 patients with secondary hemichorea-hemiballism. Methods An analysis was performed for 25 patients with hemichorea-hemiballism who were admitted and treated from July 2018 to April 2024,including general status,clinical features,laboratory examinations,MRI or CT examination,treatment methods,and prognosis. Results Of all 25 patients,11 (44.0%) had acute cerebrovascular diseases [including 7 cases (28.0%) of cerebral infarction,3 cases (12.0%) of cerebral hemorrhage,and 1 case (4.0%) of moyamoya disease],13 (44.0%) had hyperglycemia,and 1 had unknown causes. Of all 25 patients with hemichorea-hemiballism after treatment of the primary disease and symptomatic treatment,18 patients had significant improvements in hemichorea symptoms,and 7 had disappearance of hemichorea symptoms. One patient experienced recurrence after half a year due to self-withdrawal of medication,and the other patients took medication regularly,with no recurrence at follow-up half a year later.Conclusion Hemichorea-hemiballism is more common in the elderly,and acute cerebrovascular disease is the most common cause of hemichorea-hemiballism,followed by hyperglycemia,with the basal ganglia as the main lesion site. In addition to the treatment of the primary disease,symptomatic treatment with haloperidol,clonazepam,or risperidone can significantly improve clinical symptoms and help to achieve a good prognosis.

5.Comparative burden of disease attributable to high BMI in Kunshan City between 2012-2023

Objective To analyze and compare the disease burden of high BMI in Kunshan City in different periods, and to provide a scientific basis for the prevention and control of overweight and obesity in Kunshan City. Methods Using the global burden of disease research method, the number of deaths attributable to high BMI and attributable YLL in Kunshan City were calculated using the survey data of chronic diseases and their risk factors and the data of the death registration system in Kunshan City. Results In 2023, R5.46% of deaths in Kunshan City were attributed to high BMI, with 345 attributable deaths, and attributable mortality rate and standardized attributable mortality rate were 39.16/100 000 and 33.82/100 000, Rrespectively. Attributable YLL rate and standardized attributable YLL rate were 692.35/100 000 and 604.46/100 000, respectively. High BMI caused a loss of 0.52 years of life expectancy per capita. Compared with 2012, PAF, standardized attributable mortality rate, standardized attributable YLL rate and life expectancy loss per capita of high BMI in 2023 increased by 121.95%, 100.71%, 57.05%, and 100%, respectively. Among different genders, PAF increased by 91.05% for males and 161.97% for females from 2012 to 2023. Among different diseases, cardiovascular and cerebrovascular diseases and cancers had the highest attributable disease burden, while diabetes, chronic kidney disease and Alzheimer's disease all had a significant increase. Conclusion The burden of disease attributable to high BMI has risen dramatically in Kunshan City, and the adverse health effects of overweight and obesity need to be reduced through scientific weight loss and comprehensive practical measures.

7.Disease Burden and Indirect Economic Burden Due to Lung Cancer from 1981 to 2020 in Kunshan, Jiangsu Province, China

Objective To identify the disease burden and indirect economic burden caused by lung cancer in Kunshan City, Jiangsu Province, from 1981 to 2020. Methods The incidence and death cases of lung cancer were obtained from cancer registry and death cause monitoring data. The disability adjusted life years (DALY) was used as the evaluation index for burden posed by lung cancer on health, and the indirect economic burden was calculated by a human capital method. Results From 1981 to 2020, a total of 9272 deaths due to lung cancer were reported in Kunshan, of which 7106 were males and 2166 were females. The DALY caused by lung cancer in the whole population were 3.81, 4.14, 4.38, and 9.46 in 1981–1990, 1991–2000, 2001–2010, and 2011–2020, respectively. The indirect economic burden caused by lung cancer was 10.515, 141.657, 813.794, and 6659.149 million yuan. From 2011 to 2020, the ratios of years of life lost due to premature mortality to DALY in males, females, and the general population were 92.42%, 95.15%, and 93.60%, respectively. Conclusion The health burden and indirect economic burden for lung cancer are substantial in the Kunshan City. Moreover, age-specific DALY and indirect economic burden are not exactly symmetrical, suggesting that an effective control strategy to lower cost is urgently needed, especially for individuals aged 40-59.
8.Advantages and features of nanocomposite hydrogel in treatment of osteoarthritis

BACKGROUND:Nanocomposite hydrogel has great research prospects and application potential in the treatment of osteoarthritis. OBJECTIVE:To review the research progress of nanocomposite hydrogel in osteoarthritis and cartilage repair. METHODS:Databases such as CNKI and PubMed were searched.The English key words were"nanocomposite hydrogel,nanogel,osteoarthritis,cartage,physical encapsulation,electrostatic interaction,covalent crosslinking",and the Chinese key words were"nanocomposite hydrogel,nanogel,osteoarthritis,cartage,physical encapsulation,physical encapsulation,electrostatic effect,covalent cross-linking".After an initial screening of all articles based on inclusion and exclusion criteria,71 articles with high correlation were retained for review. RESULTS AND CONCLUSION:In cell or animal experiments,nanocomposite hydrogel has the effect of improving osteoarthritis.Nanocomposite hydrogel can promote cartilage repair,improve the internal environment of osteoarthritis,and achieve the therapeutic purpose of osteoarthritis by improving the mechanical environment between joints,carrying targeted drugs,and promoting the chondrogenesis of seed cells.At present,the research of nanocomposite hydrogel in osteoarthritis disease still has a huge space to play.It is expected to open up a new way for the clinical treatment of osteoarthritis by continuing to deepen the research of material preparation and actively carrying out cell and animal experiments.
9.Clinical and genetic diagnosis of hepatolenticular degeneration occurring in two consecutive generations

Objective To investigate the ideas for the clinical and genetic diagnosis of hepatolenticular degeneration (HLD) occurring in two consecutive generations. Methods The information on history of present illness, past history, and family history were collected from three patients with HLD who were admitted to our department from June 2017 to July 2023, and their first-degree relatives who might have this disease were examined in terms of liver function, serum ceruloplasmin (CP), basal 24-hour urinary copper excretion (urinary Cu), Kayser-Fleischer ring (KFR) of the cornea, abdominal ultrasound, and ATP7B gene testing. Results Among the parents or children of the probands in these three families, two were found to have elevated serum aminotransferases, three had a reduction in CP and an increase in urinary Cu, and 1 had fatty liver; all of them had compound heterozygous mutations in the ATP7B gene, and all tested negative for KFR. This study identified three families with HLD occurring in two consecutive generations and found a new pathogenic mutation. Conclusion CP, KFR, or abdominal ultrasound has certain limitations in screening pre-symptomatic HLD; urinary Cu test has a good sensitivity, and genetic testing can further improve the accuracy of diagnosis. Due to the high carrying rate of ATP7B pathogenic mutations in the population with HLD, HLD occurring in two consecutive generations should be taken seriously in clinical practice.
10.Influencing factors for peripheral neutropenia in patients with hepatolenticular degeneration after splenectomy

Objective To investigate the clinical features of neutropenia after splenectomy in patients with hepatolenticular degeneration, also known as Wilson's disease (WD), and related influencing factors. Methods The patients with WD who were hospitalized and underwent splenectomy from January 2018 to March 2023 were enrolled as subjects. The patients with an absolute neutrophil count of <1.7×109/L at 1 year after splenectomy were enrolled as observation group, and those with an absolute neutrophil count of ≥1.7×109/L were enrolled as control group. The two groups were compared in terms of the change in routine blood test results at 1 year after splenectomy, as well as the indicators such as general information,preoperative myelogram data, preoperative Child-Pugh score, preoperative spleen size, surgical procedures, and postoperative laboratory markers, and the influencing factors for the reduction in neutrophil count were analyzed. Results A total of 61 patients were included. At 1 year after splenectomy, both groups had significant increases in white blood cell count, neutrophil count, red blood cell count, and platelet count and a significant reduction in the percentage of neutrophils (P<0.05). After surgery, 39.34%(24/61) of the patients still had a neutrophil count lower than the normal level. Before surgery, compared with the control group,the observation group had significantly higher Child-Pugh score, total bilirubin(TBIL),aspartate aminotransferase, and prothrombin time and a significantly lower level of albumin(ALB)(P<0.05), and there was also a significant difference in the proportion of patients with ascites between the two groups(P<0.05). The multivariate analysis showed that preoperative Child-Pugh score, TBIL, and ALB were independent influencing factors for neutropenia. Conclusion Splenectomy cannot completely correct neutrophil level in WD patients, which may be associated with the degree of liver damage.
