1.Inhibition of HDAC3 Promotes Psoriasis Development in Mice Through Regulating Th17

ObjectiveTo investigate the influence of histone deacetylase 3 (HDAC3) on the occurrence, development of psoriasis-like inflammation in mice, and the relative immune mechanisms. MethodsHealthy C57BL/6 mice aged 6-8 weeks were selected and randomly divided into 3 groups: control group (Control), psoriasis model group (IMQ), and HDAC3 inhibitor RGFP966-treated psoriasis model group (IMQ+RGFP966). One day prior to the experiment, the back hair of the mice was shaved. After a one-day stabilization period, the mice in Control group was treated with an equal amount of vaseline, while the mice in IMQ group was treated with imiquimod (62.5 mg/d) applied topically on the back to establish a psoriasis-like inflammation model. The mice in IMQ+RGFP966 group received intervention with a high dose of the HDAC3-selective inhibitor RGFP966 (30 mg/kg) based on the psoriasis-like model. All groups were treated continuously for 5 d, during which psoriasis-like inflammation symptoms (scaling, erythema, skin thickness), body weight, and mental status were observed and recorded, with photographs taken for documentation. After euthanasia, hematoxylin-eosin (HE) staining was used to assess the effect of RGFP966 on the skin tissue structure of the mice, and skin thickness was measured. The mRNA and protein expression levels of HDAC3 in skin tissues were detected using reverse transcription real-time quantitative polymerase chain reaction (RT-qPCR) and Western blot (WB), respectively. Flow cytometry was employed to analyze neutrophils in peripheral blood and lymph nodes, CD4+ T lymphocytes, CD8+ T lymphocytes in peripheral blood, and IL-17A secretion by peripheral blood CD4+ T lymphocytes. Additionally, spleen CD4+ T lymphocyte expression of HDAC3, CCR6, CCR8, and IL-17A secretion levels were analyzed. Immunohistochemistry was used to detect the localization and expression levels of HDAC3, IL-17A, and IL-10 in skin tissues. ResultsCompared with the Control group, the IMQ group exhibited significant psoriasis-like inflammation, characterized by erythema, scaling, and skin wrinkling. Compared with the IMQ group, RGFP966 exacerbated psoriasis-like inflammatory symptoms, leading to increased hyperkeratosis. The psoriasis area and severity index (PASI) skin symptom scores were higher in the IMQ group than those in the Control group, and the scores were further elevated in the IMQ+RGFP966 group compared to the IMQ group. Skin thickness measurements showed a trend of IMQ+RGFP966>IMQ>Control. The numbers of neutrophils in the blood and lymph nodes increased sequentially in the Control, IMQ, and IMQ+RGFP966 groups, with a similar trend observed for CD4+ and CD8+ T lymphocytes in the blood. In skin tissues, compared with the Control group, the mRNA and protein levels of HDAC3 decreased in the IMQ group, but RGFP966 did not further reduce these expressions. HDAC3 was primarily located in the nucleus. Compared with the Control group, the nuclear HDAC3 content decreased in the skin tissues of the IMQ group, and RGFP966 further reduced nuclear HDAC3. Compared with the Control and IMQ groups, RGFP966 treatment decreased HDAC3 expression in splenic CD4+ and CD8+ T cells. RGFP966 treatment increased the expression of CCR6 and CCR8 in splenic CD4+ T cells and enhanced IL-17A secretion by peripheral blood and splenic CD4+ T lymphocytes. Additionally, compared with the IMQ group, RGFP966 reduced IL-10 protein levels and upregulated IL-17A expression in skin tissues. ConclusionRGFP966 exacerbates psoriatic-like inflammatory responses by inhibiting HDAC3, increasing the secretion of the cytokine IL-17A, and upregulating the expression of chemokines CCR8 and CCR6.

Recent studies have provided new insights into the association between MASLD and the risk of developing chronic kidney disease (CKD). Furthermore, alterations in microbiota composition and metabolite profiles, notably altered BAs, have been described in studies investigating the association between MASLD and the risk of CKD. This narrative review discusses alterations of specific classes of metabolites, BAs, fructose, vitamin D, and microbiota composition that may be implicated in the link between MASLD and CKD.

5.Follow-up Analysis of 267 Cases of Extracardiac Conduit Total Cavopulmonary Connection in a Single Center

ObjectiveTo explore the outcomes and surgical experiences of 267 cases of extracardiac conduit total cavopulmonary connection (TCPC) in patients with functional single ventricles. MethodsClinical data were collected from 267 patients who underwent extracardiac conduit TCPC at Guangdong Provincial People's Hospital from October 2004 to August 2021. Among them, 185 were male (69.3%) and 82 were female (30.7%). The age was 5.71 (4.08-10.90) year, and the weight was 17.5 (14.5-26.2) kg. A median sternotomy approach was used, and the surgery was completed under cardiopulmonary bypass. The preoperative, intraoperative and postoperative follow-up data of the patients were collected, and the survival prognosis and its influencing factors were analyzed. ResultsThe operative time was 330.0 (267.5-405.0) min, the cardiopulmonary bypass time was 124.0 (96.0-163.0) min, and the aortic cross-clamp time was 48.0 (0.0-81.0) min. The duration of mechanical ventilation after surgery was 8.7 (5.0-19.1) h, and the hospital stay was 34.0 (28.0-49.0) d. The follow-up period was 8.0 (4.6-11.0) year, with 8 early deaths (3.0%), 20 late deaths (7.5%). The independent risk factors of fatality in patients after extracardiac conduit TCPC included male, heterotaxy syndrome, preoperative pulmonary artery pressure, intraoperative blood loss, the duration of mechanical ventilation, and the 48 h average vasoactive-inotropic score. ConclusionsThe extracardiac conduit TCPC is quite effective in treating patients with functional single ventricle. The early postoperative fatality is low, while the late fatality is relatively high. Therefore, it is necessary to pay close attention to the long-term survival situation of this group of patients. The analysis of risk factors emphasizes the importance of perioperative refined assessment and individualized treatment. It may help further improve the therapeutic outcomes of such surgeries, including selecting appropriate patients, performing careful operations, ensuring meticulous hemostasis, and shortening the duration of postoperative mechanical ventilation.
6.Efficacy and safety of hypertonic dextrose prolotherapy for patients with post-herpetic neuralgia

Objective·To investigate the efficacy and safety of hypertonic dextrose prolotherapy(DPT)in the treatment of postherpetic neuralgia.Methods·Seventy-eight patients with postherpetic neuralgia who visited the Department of Pain of The Affiliated Hospital of Xuzhou Medical University from June 2019 to December 2022 were selected.The patients were randomly assigned to a control group and a research group in a 1∶1 ratio,with 39 patients in each group.The control group was treated with traditional analgesic solution,while the research group was treated with traditional analgesic solution combined with DPT.Visual analog scale(VAS)was used to evaluate the patients'pain level before and after treatment,flow cytometry was used to measure the patients'T-cell subsets,and enzyme-linked immunosorbent assay(ELISA)was used to measure the levels of C-reactive protein(CRP),interleukin-6(IL-6),and IL-10 cytokines.The VAS scores were compared between the two groups of patients before and at 1,2,4,8,and 12 weeks after treatment.CD4+/CD8+,CRP,IL-6,IL-10 levels,and the incidence of adverse reactions before and 2 weeks after treatment were compared between the two groups.Results·There was no statistically significant difference in sex ratio,age,and disease duration between the two groups of patients.The VAS scores of the two groups of patients at 1,2,4,8,and 12 weeks after treatment were significantly lower than those before treatment,and the differences were statistically significant(all P<0.05).The VAS scores of the research group at 1,2,4,8,and 12 weeks after treatment were significantly lower than those of the control group(all P<0.05).There was no statistically significant difference in basal CD4+/CD8+,CRP,IL-6 and IL-10 levels between the two groups of patients.IL-6 and CRP levels in the research group were significantly lower after treatment than those in the control group,and the differences were statistically significant(all P=0.000).CD4+/CD8+ and IL-10 levels were significantly higher in the research group than those in the control group after treatment,and the difference was statistically significant(all P=0.000).No adverse reactions such as local nerve damage,epidural hematoma,infection,pneumothorax or allergy occurred in both groups of patients during the treatment.Conclusion·DPT can significantly reduce the pain of PHN patients,improve patients'T lymphocyte subpopulations and cytokine expression,and can be safely applied to the clinic.

Objective To investigate the effect of Anchusa italica Retz.extract on cough variant asthma(CVA)and its mechanism of action.Methods Forty-eight SD rats of each eight were randomly divided into blank group(equal amount of saline),model group(equal amount of saline),control group(250 mg·kg-1 prednisone acetate)and experimental-A1,-A5,-A7 groups(61.1 mg·kg-1 Anchusa italica Retz.extract A1,48.8 mg·kg-1 Anchusa italica Retz.extract A5,201.8 mg·kg-1 Anchusa italica Retz.extract A7),continuous gavage for 28 d;except for the blank group,the other groups were made with ovalbumin-complete Freund's adjuvant.The number of cough times of each rat was observed.The changes in the expression of Toll-like receptor 4(TLR4),myeloid differentiation factor(MyD88)and nuclear factor κB(NF-κB)proteins were observed in the lung tissue.Results The cough times in the blank group,model group,control group and experimental-A1,-A5,-A7 groups were 1.10±0.22,8.33±1.24,3.08±0.65,3.31±0.99,3.08±1.02 and 3.06±0.68;the relative expression levels of TLR4 protein were 0.61±0.01,0.84±0.04,0.66±0.02,0.64±0.04,0.64±0.03 and 0.69±0.02;the relative expression levels of MyD88 protein were 0.54±0.08,0.86±0.06,0.71±0.06,0.65±0.05,0.64±0.08 and 0.70±0.06;the relative expression levels of p-NF-κB protein were 0.48±0.11,0.94±0.06,0.80±0.08,0.68±0.04,0.68±0.06 and 0.78±0.09.Compared with the experimental-A1,-A5,-A7 groups,control group,normal group,the differences were statistically significant in the model group(all P<0.05).Conclusion The extract of Anchusa Italica Retz.can inhibit the symptoms of CVA rats,and the mechanism may be related to the inhibition of TLR4/MyD88/NF-κB signaling pathway.
9.Chairside digital design and manufacturing method for children's band and loop space maintainers

Objective This study proposes a chairside digital design and manufacturing method for band and loop space maintainers and preliminarily validates its clinical feasibility.Methods Clinical cases of 10 children requiring space maintenance caused by premature loss of primary teeth were collected.Intraoral scan data of the affected children were also collected to establish digital models of the missing teeth.Using a pediatric band and loop space maintainer de-sign software developed by our research team,a rapid personalized design of band and loop structures was achieved,and a digital model of an integrated band and loop space maintainer was ultimately generated.A chairside space maintainer was manufactured through metal computer numerical control machining for the experimental group,whereas metal 3D printing in the dental laboratory was used for the control group.A model fitting assessment was conducted for the space maintainers of both groups,and senior pediatric dental experts were invited to evaluate the clinical feasibility of the space maintainers with regard to fit and stability using the visual analogue scale scoring system.Statistical analysis was also performed.Results The time spent in designing and manufacturing the 10 space maintainers of the experimental group was all less than 1 h.Statistical analysis of expert ratings showed that the experimental group outperformed the control group with regard to fit and stability.Both types of space maintainers met clinical requirements.Conclusion The chairside digital design and manufacturing method for pediatric band and loop space maintainers proposed in this study can achieve same-day fitting of space maintainers at the first appointment,demonstrating good clinical feasibility and significant potential for clinical application.
10.Clustering analysis of risk factors in high-incidence areas of esophageal cancer in Yanting county

Objective To investigate the dietary patterns of rural residents in the high-incidence areas of esophageal cancer (EC), and to explore the clustering and influencing factors of risk factors associated with high-incidence characteristics. Methods A special structured questionnaire was applied to conduct a face-to-face survey on the dietary patterns of rural residents in Yanting county of Sichuan Province from July to August 2021. Univariate and multivariate logistic regression models were used to analyze the influencing factors of risk factor clustering for EC. Results There were 838 valid questionnaires in this study. A total of 90.8% of rural residents used clean water such as tap water. In the past one year, the people who ate fruits and vegetables, soybean products, onions and garlic in high frequency accounted for 69.5%, 32.8% and 74.5%, respectively; the people who ate kimchi, pickled vegetables, sauerkraut, barbecue, hot food and mildew food in low frequency accounted for 59.2%, 79.6%, 68.2%, 90.3%, 80.9% and 90.3%, respectively. The clustering of risk factors for EC was found in 73.3% of residents, and the aggregation of two risk factors was the most common mode (28.2%), among which tumor history and preserved food was the main clustering pattern (4.6%). The logistic regression model revealed that the gender, age, marital status and occupation were independent influencing factors for the risk factors clustering of EC (P<0.05). Conclusion A majority of rural residents in high-incidence areas of EC in Yanting county have good eating habits, but the clustering of some risk factors is still at a high level. Gender, age, marital status, and occupation are influencing factors of the risk factors clustering of EC.
