1.The influence of cumulative fluid balance on volume status in patients with heart failure

Objective To analyze the correlation between cumulative fluid balance volume and increasing plasma volume and aggravated clinical congestion symptoms in patients with heart failure on admission for 1~7 d,and to explore the application value of cumulative fluid balance volume in predicting volume overload.Methods Using the convenience sampling method,235 heart failure patients hospitalized from October 2022 to February 2023 in a total of 3 tertiary hospitals in Nanjing,Lianyungang,Jiangsu Province,and Zhumadian,Henan Province,were selected and classified into an increasing/decreasing plasma volume group and an aggravated/alleviated clinical congestion symptoms group.General information,clinical characteristics,and 1~7 d cumulative fluid balance volume of the study subjects were collected to compare the differences in cumulative fluid balance volume between the 2 groups.Logistic regression was used to analyze the effect of cumulative fluid balance volume on plasma volume and clinical congestion symptoms.Receiver operating characteristic curves were used to analyze the optimal cutoff value of cumulative fluid balance volume for predicting increasing plasma volume and aggravated clinical congestion symptoms.Results Excluding 15 pat ients who were discharged early and 3 patients with inaccurate records of fluid intake and excretion,a total of 217 patients were included.The incidence of positive fluid balance was lowest on day 1,at 65.90%,with the smallest cumulative volume of(0.235±0.983)L;the highest incidence of positive balance occurred on day 6,at 75.58%,with the largest cumulative volume of(2.444±5.445)L.The cumulative fluid balance at 4~7 d in the increasing plasma volume group was higher than that in the decreasing plasma volume group,and the difference was statistically significant(P<0.05);the cumulative fluid balance at 4~7 d was an independent risk factor for plasma volume elevation,and a cumulative fluid balance of 2.308,3.361,3.518,and 3.702 L at 4~7 d was predictive of a plasma volume elevation,and areas under receiver operating characteristic curve were 0.686,0.721,0.647,and 0.766,respectively.The cumulative fluid balance for 4~7 d in the aggravated clinical congestion symptoms group was higher than that in the alleviated clinical congestion symptoms group,and the difference was statistically significant(P<0.05);the cumulative fluid balance for 4~7 d was an independent risk factor for the aggravated clinical congestion symptoms,and a cumulative fluid balance for 4~7 d of 2.574,3.383,4.995,and 4.235 L predicted aggravated clinical congestion symptoms,and area under receiver operating characteristic curve was 0.640,0.693,0.654,and 0.720,respectively.Conclusion The incidence of cumulative positive fluid balance in heart failure patients is high,and the amount of cumulative fluid balance can better predict the occurrence of volume overload,and the increase of plasma volume precedes the appearance of clinical congestion symptoms.It is suggested that heart failure patients with more than 3 d of cumulative positive balance should be closely monitored,and when the cumulative positive balance exceeds 2.308 L,measures should be taken in time to drain excessive fluid,so as to avoid the incidence of volume overload.
2.Change in corneal endothelial cell density and morphology in different stages of keratoconus

Objective:To assess the changes in corneal endothelial cell density (CD) and morphology in patients with different stages of keratoconus.Methods:A cross-sectional study was conducted.One hundred and nineteen patients (199 eyes) with keratoconus who were treated in the Eye Hospital of Shandong First Medical University were included from March 2018 to October 2021.The 199 eyes were classified into stage Ⅰ (111 eyes of 58 cases), stage Ⅱ (41 eyes of 30 cases), stage Ⅲ (47 eyes of 31 cases) keratoconus groups according to the Amsler-Krumeich classification.In the same period, 25 age- and sex-matched healthy subjects (50 eyes) were enrolled as a normal control group.Corneal topography and anterior segment parameters such as keratometry (K), central corneal thickness (CCT), thinnest corneal thickness (TCT), anterior chamber depth (ACD), corneal diameter and corneal volume were obtained by Pentacam 3-dimensional anterior segment imaging and analysis system.The corneal endothelial CD, percentage of hexagonal cells (6A), average cell area (AVE), maximum cell area (MAX), minimum cell area (MIN), cell area standard deviation (SD) and cell area coefficient of variation (CV) in the central area were evaluated by non-contact specular microscopy.The correlation between corneal endothelial CD, morphological parameters and corneal topographic parameters was analyzed by Spearman rank correlation.This study adhered to the Declaration of Helsinki and was approved by the Ethics Committee of Shandong Eye Hospital (No.SDSYKYY201803). All patients were informed of the purpose and methods of the study and written informed consent was obtained before any medical examination.Results:The CD of the normal control group and stage Ⅰ, Ⅱ, Ⅲ keratoconus groups was 2 941(2 809, 3 072), 2 825(2 667, 3 030), 2 747(2 475, 2 903) and 2 370(2 142, 2 525) cells/mm 2, respectively.With the progression of keratoconus, CD decreased gradually, and there was a significant difference in CD among the four groups ( H=94.862, P<0.001). There were significant differences in CV and 6A among the four groups ( H=45.018, 20.421; both at P<0.001). CV was significantly higher in stage Ⅲ keratoconus group than that of the normal control group and stage Ⅰ and Ⅱ keratoconus groups and 6A was significantly lower in stage Ⅲ keratoconus group than that of the normal control group and stage Ⅰ keratoconus group (all at P<0.05). With the progression of keratoconus, MAX, MIN, AVE and SD increased gradually, and there were significant differences in MAX, MIN, AVE and SD among the four groups ( H=37.905, 32.437, 110.182, 72.941; all at P<0.001). MAX and MIN in stage Ⅲ keratoconus group were significantly higher than those in stage Ⅰ keratoconus groups and normal control group (all at P<0.05). AVE and SD in stage Ⅲ keratoconus group were significantly higher than those in normal control group and stage Ⅰ and Ⅱ keratoconus groups (all at P<0.05). In patients with keratoconus, CD was moderately positively correlated with CCT ( rs=0.47, P<0.001) and TCT ( rs=0.53, P<0.001), and was moderately negatively correlated with mean keratometry (Km) ( rs=-0.59, P<0.001).6A was weakly positively correlated with CCT ( rs=0.18, P=0.01) and TCT ( rs=0.22, P=0.002), and was weakly negatively correlated with Km ( rs=-0.32, P<0.001). CV was weakly negatively correlated with CCT ( rs=-0.35, P<0.001) and TCT ( rs=-0.37, P<0.001), and was moderately positively correlated with Km ( rs=0.48, P<0.001). There was no correlation between CD, CV, 6A and ACD, or corneal volume. Conclusions:As the keratoconus progresses, the cornea protrudes and becomes thinner with CD and 6A decreasing while CV increasing.Corneal topographic parameters are related to the density and morphology of corneal endothelial cells.
3.Investigation on the knowledge, attitude and behavior of medical staff in emergency rescue for public health emergencies under regular pandemic prevention and control

ObjectiveTo investigate the status quo of emergency rescue for public health emergencies by medical staff under regular pandemic prevention and control, analyze its influencing factors, and provide evidence for formulating intervention measures. MethodsAn online questionnaire was conducted among the medical staff of Jinshan District public medical institutions, and the possible influencing factors were analyzed by multiple linear stepwise regression analysis. The questionnaire included general information and knowledge, attitude, and behavior of emergency rescue. ResultsA total of 1,614 valid questionnaires were collected. The total average score of emergency rescue performance was (4.01±0.83), which was at a high level. The average scores of emergency rescue knowledge and attitude were (4.07±0.80) and (4.21±0.70) respectively, which were at a high level, and the average scores of emergency rescue behavior were (3.25±1.23), which was at a medium level. Regression analysis, age, department, occupation, hospital level, whether to participate in emergency training, whether to participate in emergency drills were the main influencing factors of emergency rescue knowledge and practice (P<0.05). ConclusionUnder the situation of major public health emergencies, the knowledge, attitude and practice of emergency rescue of medical personnel are generally at a good level, and the behavior of emergency rescue is at a medium level. Emergency rescue training and drills should be strengthened for medical personnel of different ages, occupations, departments, levels of hospitals, and sections.
4.Differences in the detection rates of depression and anxiety among middle school students in different time periods

Objective:
To investigates the prevalence of depression and anxiety among middle school students at different times, and to provide a reference for mental health promotion among adolescents.
Methods:
A total of 1 505 middle school students, selected from Chongqing in 2021 by using multi stage stratified random sampling method, were surveyed by using the Center for Epidemiologic Studies Depression Scale(CSE-D) and Zung Self Rating Anxiety Scale (SAS).
Results:
At the beginning of the semester, 434(27.6%) and 601 students( 38.2 %) reported depressive and anxiety symptoms,respectively. While at the end of the semester showed that 463 cases (30.8%) were prone to depression, and 653 cases ( 43.4 %) reported depressive and anxiety symptoms,respectively. The detection rate of depressive symptoms at the end of the semester was significantly higher than that at the beginning of semester. (43.4%, 38.2 %, χ 2=8.55, P <0.05). Depressive of female students at the end of the semester( 52.8 %) was higher than that at the beginning of the semester(46.3%)( χ 2=6.19, P <0.05) The detection rate of anxiety symptoms in senior high school students at the end of the semester(44.9%) was significantly higher than that of at the beginning of the semester(28.6%) ( χ 2= 43.33 , P <0.01). No significant difference in anxiety symptoms boys and junior high school students between the beginning and the end of the semester ( χ 2=2.34, 0.71, P >0.05).
Conclusion
Substantial changes are observed in anxiety symptoms among female middle school students and depressive and anxiety symptoms among high school students in different periods of a semester. When investigating depression and anxiety of middle school students, time effect should be considered.
5.Effect of WeChat intervention on medication compliance, psychotic symptoms and recurrence rate of schizophrenic patients in community: a Meta-analysis

ObjectiveTo systematically review the efficacy of intervention by WeChat on medication compliance, psychotic symptom and recurrence rate of schizophrenic patients in community. MethodsDatabases including PubMed, the Cochrane Library, CBM, CNKI, Wanfang Data and VIP were searched electronically from January 1, 2011 to November 1, 2020 to collect randomized controlled trials (RCTs) about the effects of WeChat intervention on community schizophrenic patients. After two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies, the meta-analysis was performed with Stata 12.0 software. ResultsA total of 381 articles were retrieved and finally 10 RCTs were included, including 1 251 patients with WeChat intervention group 641 cases and routine health education group 610 cases. Meta-analysis showed that compared with the conventional health education group, the WeChat intervention group had higher medication compliance (OR=3.05,95% CI:1.98~4.69,P<0.01), lower PANSS score (SMD=-1.05,95% CI:-1.46~-0.64,P<0.01) and relapse rate (OR=0.34,95% CI:0.24~0.48,P<0.01). ConclusionThe interactive intervention based on WeChat platform can effectively improve the medication compliance of patients with schizophrenia in the community, help to reduce the severity of psychotic symptoms and the recurrence rate.
6.Risk factors and treatment outcome of recurrent acanthamoeba keratitis after corneal transplantation

Objective:To investigate the risk factors and treatment outcome of recurrent Acanthamoeba keratitis (AK) after corneal transplantation. Methods:A serial case-observational study was carried out.Twenty-eight eyes of 28 patients with AK who underwent corneal transplantation in Shandong Eye Hospital from January 2012 to January 2019 were enrolled.All the eyes received corneal transplantation from failing to respond to topical and systemic anti- Acanthamoeba medical therapy, including 13 eyes that received penetrating keratoplasty (PKP) and 15 eyes that received lamellar keratoplasty (LKP). The corneal lesion was removed by a trephine with a diameter of 0.5 mm over infiltration area during PKP or LKP.The clinical features of recurrent AK were summarized, including recurrence time, site and signs, and the risk factors of AK recurrence were analyzed.Local and systemic anti- Acanthamoeba medical therapy was performed in all relapsed eyes, and secondary surgery was performed for the eyes with poor response to medication.The therapeutic outcome of recurrent AK was evaluated.The study adhered to the Declaration of Helsinki.This study protocol was approved by an Ethics Committee of Shandong Eye Hospital (No.201112). Results:In the 28 eyes, 7 eyes (25%) appeared recurrent AK after keratoplasty, including 2 eyes after PKP and 5 eyes after LKP.There was no significant difference in the recurrence rate between the two methods ( P=0.396). The recurrence rate of eyes that had used glucocorticoids drugs before operation was 57.14% (4/7), which was significantly higher in comparison with 14.29% (3/21) of eyes without glucocorticoids before surgery ( P=0.043). The recurrence rate of eyes with ulcer diameter ≥8.2 mm was 50.00% (5/10), which was significantly higher than 11.11% (2/18) of eyes with ulcer <8.2 mm ( P=0.036). The recurrent lesions began at the edge of implant bed accounted for 85.71% (6/7), and the recurrent lesions located below graft accounted for 14.29% (1/7). In 7 eyes with recurrent AK, 6 eyes were completely cured.Among recurrent AK eyes after LKP, 2 eyes were cured by long-term medical therapy, and 2 eyes were cured by extended-diameter LKP, and another 1 eye was cured by conjunctival flap covering surgery.One eye with recurrent AK after PKP was cured by extended-diameter PKP. Conclusions:The risk factors of recurrent AK after surgery are application of glucocorticoids before surgery and big lesions.Recurrent AK after surgery is curable by individualized therapy targeting to different clinical characteristics.
7.A systematic review of the effects of limited fluid intake on the prognosis of patients with heart failure

Objective:To systematically review the effects of limited fluid intake on the prognosis of patients with heart failure.Methods:RCTs about the effects of limited fluid intake on the prognosis of patients with heart failure published up to March 31, 2021 were retrieved from PubMed, The Cochrane Library, CINAHL, Embase, Web of Science, CNKI, Wanfang, VIP, and SinoMed. Two independent researchers were employed to extract data and evaluate the quality of included literature. Rev Man 5.3 was used for Meta-analysis, sequential analysis to evaluate the reliability and authenticity of the research results, and the Egger's test for publication bias.Results:A total of 7 articles were included, with a sample size of 867 cases. Meta-analysis showed that the body weight [ MD=-3.04, 95% CI (-4.70, -1.38) , P<0.001], B-type natriuretic peptide [ MD=-249.32, 95% CI (-305.00, -193.63) , P<0.001], blood creatinine [ MD=-22.03, 95% CI (-24.98, -19.09) , P<0.001], readmission rate [ OR=0.30, 95% CI (0.20, 0.45) , P<0.001] of the limited fluid intake group in the chronic phase of heart failure were lower than those in the control group; the body weight [ MD=1.41, 95% CI (-3.73, 6.55) , P=0.59], B-type natriuretic peptide [ MD=64.52, 95% CI (-50.01, 179.06) , P=0.27], serum creatinine [ MD=3.83, 95% CI (-9.69, 17.36) , P=0.58], readmission rate [ OR=1.21, 95% CI (0.65, 2.27) , P=0.55] of the acute fluid intake group were not statistically different from those in the control group. Conclusions:Limiting fluid intake in the chronic phase of heart failure can effectively improve the patients' heart and kidney function and the prognosis of the disease. In the acute phase of heart failure, it may be necessary to combine multiple treatments to keep the patients at the best volume state.
8.True feelings of comprehensive function impairment based on ICF among chronic heart failure patients: a qualitative research

Objective:To deeply explore the comprehensive function impairment of chronic heart failure (CHF) patients and analyze its influencing factors.Methods:From November 2018 to January 2019, this study selected medical workers (doctors and nurses) , CHF patients or their family members in Department of Cardiovascular at a Class Ⅲ Grade A hospital in Jiangsu Province as subjects by purposive sampling. Semi-structured interview was carried out for the subjects with the phenomenological method. Colaizzi analysis was used to extract concepts. Two researchers matched the concept with the international classification of functioning, disability and health (ICF) based on Cieza link rules. This study took ICF category no more than 5% of the former after two interviews as the sample size saturated standard.Results:Finally, a total of 28 interviewees were included. A total of 80 ICF categories were linked, 31 (38.8%) of them in body function, 5 (6.2%) in body structure, 28 (35.0%) in activity participation and 16 (20.0%) in environmental factors. Other 482 concepts could not be linked with ICF categories with health status (30.7%) for the most followed by personal factors (27.4%) .Conclusions:ICF could be used to describe the comprehensive function impairment of CHF patients and its influencing factors. Being linked with multitudinous ICF categories indicates the complex function impairment and the numerous influencing factors. This study will be beneficial to carry out the individualized, detailed and dynamic treatment and nursing based on comprehensive evaluation for CHF patients.
9.Construction of different functional graded care programs based on ICF for elderly patients with chronic heart failure

Objective:To construct the different functional graded care programs for elderly patients with chronic heart failure (CHF) .Methods:In March 2019, convenience sampling was used to select 24 nursing experts from 14 ClassⅢ Grade A hospitals in Northeast, North China, East China, South China, Central China and Northwest China as the subjects of correspondence. Based on the "International Classification of Functioning, Disability and Health" (ICF) , we took the function as the starting point to construct the different functional graded care programs for elderly CHF patients through systematic reviews, expert meeting and expert consultations.Results:In three rounds of questionnaires, the recovery rates were 100.00%, 100.00% and 95.83%; the authority coefficients of experts were 0.912, 0.914 and 0.913; the coordination coefficients were 0.348, 0.451 and 0.752, respectively; the differences were all statistical ( P<0.001) . The finally constructed different functional graded care programs for elderly CHF patients included heart function, respiratory function, water and electrolyte function, energy and drive function, emotional function, skin protection function, sleep function, and activity and participation function, totaling 8 ICF functions, 14 nursing measures and 161 nursing activity. Conclusions:The different functional graded care programs for CHF patients is comprehensive and systematic, which can not only improve nurses' nursing decision-making, standardize nursing behaviors, but also can be dynamically adjusted. It is scientific and feasible, and can satisfy the individualized and differentiated care demands of CHF patients with impaired function in different disease stages. At the same time, it also provides a theoretical framework for the future development of nursing information systems.
10.Study on the Improvement of Quality Standard for Citrus reticulatae Formula Granules

OBJECTIVE: To optimize and improve the quality standard for Citrus reticulata formula granules. METHODS: Totally 13 batches of C. Reticulata formula granules from 4 different manufacturers were used as trial samples, and qualitative identification of hesperidin and nobiletin in the samples were carried out by TLC according to the method of 2015 edition of Chinese Pharmacopoeia (part Ⅳ). The quantitative analysis of naringin, hesperidin, hesperetin, nobiletin and tangeretin in C. reticulatae formula granules were conducted by UPLC[The determination was performed on Waters Acquity UPLC BEH C18 column with mobile phase consisted of acetonitrile-0.2% phosphoric acid aqueous solution (gradient elution). The detection wavelength was set at 283 nm, and sample size was 3 μL]. RESULTS: The results of TLC showed that in the chromatograms of samples, same color spots were shown in the corresponding positions of the chromatogram of reference substance. The results of UPLC showed, that the linear range of naringin, hesperidin, hesperetin, nobiletin and tangeretin were 0.64-6.44, 15.78-157.80, 0.17-1.66, 2.08-20.85 and 2.04-20.43 μg/mL, respectively (all r≥0.999 2); the limits of detection were 0.03, 0.33, 0.10, 0.20 and 0.06 μg/mL; the limits of quantitation were 0.07, 1.34, 0.20, 0.60 and 0.22 μg/mL. The average recoveries were 99.4%, 99.6%, 99.7%, 99.7% and 99.7% (n=9); RSDs of precision (n=6), stability (n=7) and reproducibility (n=6) tests were all≤2.03%; naringin was detected in only 3 batches of samples from one manufacturer (the content ranged from 0.067 3 to 0.069.6 mg/g), while the other 4 components were detected in 13 batches of samples (the contents of them ranged 0.646 5-1.728 0, 0.102 6-0.290 5, 0.023 1-0.689 8, 0.018 2-0.270 7 mg/g). CONCLUSIONS: In this study, the quality standard of C. reticulata formula granules was improved by qualitative and quantitative methods, and the contents of hesperidin, hesperetin, nobiletin and tangeretin were not less than 0.60, 0.10, 0.02 and 0.01 mg/g, respectively.
