1.Effect of Yiqi Huoxue Tongluo Decoction on miR-126a-5p and VEGF signaling pathway in cervical spondylotic myelopathy model rats

Objective To investigate the effect of Yiqi Huoxue Tongluo Decoction on microRNA-126a-5p(miR-126a-5p)and vascular endothelial growth factor(VEGF)signaling pathway in cervical spondylotic myelopathy model rats.Methods Thirty healthy male SD rats were divided into the sham operation group,the model group and the traditional Chinese medicine(TCM)group by random number table method.Cervical spondylotic myelopathy models were prepared in the model group and the TCM group.The TCM group was given intragastric administration of Yiqi Huoxue Tongluo Decoction,while the sham operation group and the model group were given intragastric administration of normal saline for 12 weeks.After intervention,the threshold of mechanical stimulation and retraction time of thermal stimulation in each group were measured by behavior tests.Rats were sacrificed to collect intervertebral disc tissue for hematoxylin-eosin(HE)staining and observe the number of vascular buds in intervertebral disc.Rat intervertebral disc annulus fibrosus cells were subjected to terminal dexynucleotidyl transferase-mediated dUTP nick end labeling(TUNEL)staining.The miR-126a-5p and VEGF mRNA of rat intervertebral disc tissue were detected by real-time fluorescence quantitative polymerase chain reaction(RT-PCR).The expression of VEGF protein of rat intervertebral disc tissue was detected by Western blot assay.Results Compared with the sham operation group,the number of vascular buds in intervertebral disc was decreased in the model group and the TCM group.The cell destruction of intervertebral disc annulus was obvious in rats,and apoptosis was high and cell density decreased.Mechanical stimulation threshold decreased,and mechanical stimulation threshold decreased.The level of miR-126a-5p was decreased,and the expression levels of VEGF mRNA and protein were increased.Compared with the model group,the number of vascular buds in intervertebral disc was increased in the TCM group.The destruction of intervertebral disc annulus cells was alleviated in rats.The apoptosis of annulus fibrosus cells in intervertebral disc decreased and cell density increased.The threshold of mechanical stimulation increased,and the retraction time of thermal stimulation was prolonged.The level of miR-126a-5p increased,and the expression levels of VEGF mRNA and protein decreased(P<0.05).Conclusion The mechanism of Yiqi Huoxue Tongluo Decoction in the treatment of cervical spondylotic myelopathy may be related to the up-regulation of miR-126a-5p expression and the down-regulation of VEGF expression.
2.Cytopathological characteristics of SMARCA4-deficient thoracic undifferentiated tumors in serous effusion

Objective:To investigate the clinicopathological characteristics of SMARCA4-deficient thoracic undifferentiated tumors, and the diagnostic value of the cells in serous effusion.Methods:Eleven cases of SMARCA4-deficient tumor were collected from the Affiliated Hospital of Hebei University, China from January 2018 to July 2023, which were diagnosed using cell block of serous effusion. The clinical, histopathological, immunohistochemical and molecular genetic features were reviewed, along with related literature.Results:All the 11 patients were males with ages ranging from 54 to 77 years (median 64 years). Nine patients were smokers and two had an unknown smoking history. Most of them complained of cough and dyspnea with pleural effusion. The primary tumor sites included lung (9 cases), thoracic wall (1 case), and mediastinum (1 case), while 3 patients had a history of lung surgery. Histologically, tumor cells were large and pleomorphic, with increased nuclear-cytoplasmic ratio. They also showed round nuclei, conspicuous nucleoli, and basophilic cytoplasm in serous effusion. Immunohistochemically, tumor cells in all cases were negative for SMARCA4/BRG1, CKpan and CK7, but positive for SMARCB1/INI1. Some of the cases were positive for CD34 (7/11), synaptophysin (4/11) and SALL4 (2/11). Histologically, monotonous tumor cells formed solid sheets or anastomosing islands with poor cell adhesion and rhabdoid morphology. Brisk mitotic figures were accompanied by large areas of necrosis. Some cases focally exhibited syncytia, and some had bright cytoplasm and vesicular chromatin. The immunohistochemical profiles in the tumor tissues were consistent with those of cytology. Six cases were negative for PD-L1 (22c3). Among the 6 cases analyzed by targeted next generation sequencing, concurrent SMARCA4 and TP53 mutations were detected in all 6 cases. Some of the 6 tumors showed mutations of STK11, CDKN2A, and MET, and amplification of ERBB2, exon deletion of BRCA2, etc. Follow-up information was available in all cases and ranged from 2 to 24 months. The patients showed metastases to various sites, including lymph node, liver, kidney, adrenal gland, brain, bone and other sites. Four patients died of the tumor. The survival time of 4 patients who underwent radical resection or radiofrequency ablation was more than 13 months.Conclusions:SMARCA4-deficient thoracic sarcoma is a rare but highly aggressive tumor with dismal prognosis and rhabdomyoid features. It is difficult to diagnose this disease using only serous effusion samples. This tumor thus warrants careful consideration. Accurate diagnosis can greatly improve early diagnosis and treatment of these tumors.
3.Endogenous metabolite nicotinamide alleviates spontaneous lupus nephritis progression in a mouse model

Objective To construct an NZB/W(F1)mouse model of systemic lupus erythematosus and evaluate the effects of nicotinamide on each index of lupus nephritis pathogenesis of NZB/W(F1)mice in order to provide data for research on the role of nicotinamide in the treatment of lupus nephritis.Methods Female NZB/W(F1)mice were obtained by crossing male NZW mice with female NZB ones.Urine samples were collected using metabolic cages and proteinuria test strips were used to detect proteinuria.Blood samples were collected through the orbital venous plexus in mice.Enzyme-linked immunosorbent assay(ELISA)was used to detect the level of anti-dsDNA antibody.Levels of serum creatinine and urea nitrogen and liver function indexes were detected using an automatic blood analyzer.Hematoxylin-eosin staining and immunofluorescence technique were used to detect the pathological state of the kidney.Results The levels of proteinuria,double-stranded DNA antibodies,serum creatinine,and urea nitrogen were gradually increased during the natural course of the disease in female NZB/W(F1)mice,indicating that the lupus nephritis disease model was constructed in female NZB/W(F1)mice.Compared to the control group,nicotinamide feeding could obviously decrease the level of proteinuria(P=0.0070),inhibit the production of double-stranded DNA antibodies(P=0.0325),and retard the progression of serum creatinine(P=0.0067)and urea nitrogen indexes(P=0.0166)in serum.In addition,the pathological state of the kidney in the nicotinamide feeding group was significantly alleviated compared with the control group.Conclusion A lupus nephritis disease model is constructed in NZB/W(F1)mice.Nicotinamide feeding can obviously alleviate the disease state of lupus nephritis in NZB/W(F1)mice.
4.Effects of multipoint and multiple paravertebral nerve block on analgesia,serum indexes and complications in patients undergoing radical resection of lung cancer

Objective To investigate the analgesic effect of multi-site multiple paravertebral nerve blocks on patients undergoing radical lung cancer surgery and the impact on their serum indices and prognosis.Methods Ninety-eight cases of patients undergoing thoracoscopic radical lung cancer surgery in our hospital were selected from March 2021 to January 2023 and divided into 2 groups according to the difference in anaesthesia.Among them,42 patients in the study group were treated with preoperative and postoperative multipoint paravertebral nerve block combined with general anaesthesia,while 56 patients in the control group were treated with preoperative multipoint paravertebral nerve block combined with general anaesthesia,and the analgesic effect and other differences between the two groups were compared.Results In the study group,extubation time,recovery time,propofol dosage and postoperative VAS score were(150.2±15.2)min,(19.1±5.1)min,(738.2±36.5)mg and(4.2±0.7)points,respectively.The control group was(151.9±12.6)min,(22.6±7.3)min,(902.1±49.0)mg and(2.3±0.3)min,respectively,and the difference between the two groups was statistically significant(P<0.05).The differences of heart rate,mean arterial pressure and before and after half an hour after surgery in control group were higher than those in study group,and the differences of norepinephrine,epinephrine and before and after surgery were higher than those in study group,with statistical significance(P<0.05).The total complication rate of the study group was 7.2%,and that of the control group was 23.2%.The QoR score of the control group was lower than that of the study group,and the difference between the two groups was statistically significant(P<0.05).Conclusion Preoperative and postoperative multi-point paravertebral nerve block combined with general anaesthesia has a better analgesic effect on patients undergoing radical lung cancer surgery,which can not only reduce the inflammatory reaction of the patients,but also have less impact on their haemodynamic indexes and lower rate of postoperative adverse reactions,and ultimately improve the quality of life of the patients.
5.Normal reference range of ultrasound blood flow parameters and correlation with growth after pediatric parental liver transplantation: a retrospective study using single center big data

Objective:To obtain the normal reference range of hemodynamic ultrasound parameters after pediatric liver transplantation through big data query and statistical analysis, and compare their changes with age.Methods:The clinical liver transplantation ultrasound imaging database software V1.0 independently developed by Tianjin First Central Hospital was used to query the ultrasound hemodynamic parameters of 0-14 years old pediatric patients after parental liver transplantation from December 2012 to December 2022, including portal vein diameter (PVD) on the 1st day, 7th day, 1st month, 6th month, 1st year, 5th year, and 10th year after surgery. The changes in ultrasound blood flow parameters such as portal vein velocity (PVV), hepatic artery peak velocity (S), hepatic artery end diastolic velocity (D), and left hepatic vein velocity (LHVV) with postoperative time in different age groups were analyzed, the differences between age groups were compared, and statistical analysis was perform to obtain the 95% reference value range for each parameter.Results:A total of 731 pediatric patients aged 0-14 years who underwent parental liver transplantation were included in this study, with a total of 5 283 monitoring results. The ultrasound hemodynamic parameters PVV, hepatic artery S, D, and LHVV were highest at 7th day after surgery, and gradually showed a sustained and slightly decreasing trend with the prolongation of postoperative time. At the same time, there was no statistically significant difference in PVV, hepatic artery S, D, and LHVV among different age groups (all P>0.05). Conclusions:This study obtains the trend of ultrasound hemodynamic parameters after pediatric liver transplantation with prolonged follow-up time, and compared them among different age groups.After pediatric liver transplantation, there is no significant change in liver hemodynamic parameters with increasing age. Obtaining reference value ranges for various parameters in different age groups is of great clinical significance for early detection and diagnosis of postoperative vascular complications. Pediatric liver transplantation is more complex than adult liver transplantation, and in clinical monitoring, more attention should be paid to the dynamic changes of transplanted liver blood flow, combining with individual patient status, to provide imaging support for clinical diagnosis.
6.Multi-parameter prediction model based on blood routine in children with influenza A

Objective To establish and validate a risk prediction model based on multiple blood routine parameters for preliminary differential diagnosis of influenza A and influenza like illness(ILI)in children.Methods Children with influenza A(n=2 686)and ILI(n=1 369)who were treated in Department of Pediatrics,The First Affiliated Hospital of Naval Medical University(Second Military Medical University)from Jul.1,2022 to Jun.30,2023 were enrolled,and their clinical and laboratory features were collected for retrospective analysis.According to age,patients were divided into 2 subgroups:1 year≤age≤6 years and 6 years<age≤16 years.Patients in each subgroup were randomly divided into training set(70%)and internal validation set(30%).Children with influenza A(n=204)and ILI(n=404)who were treated in Department of Pediatrics of The Second Affiliated Hospital of Naval Medical University(Second Military Medical University)and Naval Hospital of PLA Eastern Theater Command from Jul.1,2022 to Jun.30,2023 were selected as the external validation set.Multivariate logistic regression analysis was performed on the training set to obtain the independent influencing factors of influenza A.The prediction model based on these factors were displayed as a nomogram.Receiver operating characteristic(ROC)curve,Hosmer-Lemeshow test,and decision curve analysis(DCA)were used to evaluate the performance of the model from 3 aspects:discrimination,calibration,and clinical practicality,respectively.The diagnostic performance of the model was verified in both internal validation set and external validation set.Results In the subgroup of 1 year≤age≤6 years,age,white blood cell count,lymphocyte count and C reactive protein were the independent influencing factors of influenza A(all P<0.01);the area under the curve(AUC)value of the established nomogram prediction model for identifying influenza A was 0.746 in the training set,0.771 in the internal validation set,and 0.753 in the external validation set;the predicted probability of the model was highly consistent with the actual probability(P=0.216);and taking intervention measures within a threshold probability range of 16%-60%could yield net benefits.In the subgroup of 6 years<age≤16 years,gender,white blood cell count and lymphocyte count were the independent influencing factors of influenza A(all P<0.01);the AUC value of the established nomogram prediction model for identifying influenza A was 0.733,0.747 in the internal validation set,and 0.753 in the external validation set;the predicted probability of the model was highly consistent with the actual probability(P=0.06);and taking intervention measures within a threshold probability range of 12%-58%could yield net benefits.Conclusion This risk prediction model based on easily obtainable blood routine parameters shows good diagnostic performance for influenza A,with high accuracy and clinical practicality.
7.Application of E-learning digital platform combined with brainstorming teaching in nursing practice teaching of otolaryngology and head and neck surgery

Objective:To investigate the application effect of E-learning digital platform combined with brainstorming teaching in nursing practice teaching of otolaryngology and head and neck surgery.Methods:A total of 41 nursing students who received practice teaching in Qilu Hospital of Shandong University from January 2022 to June 2022 were selected as control group, and 39 nursing students who received practice teaching from July 2022 to December 2022 were selected as observation group. The students in the control group received traditional nursing teaching, while those in the observation group received brainstorming teaching based on the E-learning digital platform. After teaching, the two groups were compared in terms of academic performance, daily problem-solving ability, self-learning ability, critical thinking ability, and degree of satisfaction with teaching. SPSS 24.0 was used to perform the chi-square test and the t-test. Results:Compared with the control group, the observation group had significantly higher theoretical and operational assessment scores [(90.11±5.43)/(88.19±5.33) vs. (82.67±5.09)/(81.33±4.27)], scores of positive problem-solving orientation and rational problem-solving dimensions of the problem-solving questionnaire, scores of each dimension of the self-learning ability scale and the critical thinking ability, and scores of classroom teaching, clinical teaching, support, and resources in the satisfaction scale, as well as significantly lower scores of negative problem orientation, avoidance style, and impulsive neglect style in the problem-solving questionnaire ( P<0.05). The observation group had significantly higher scores of classroom teaching, clinical teaching machine support, and resources than the control group. Conclusions:In the nursing practice teaching of otolaryngology and head and neck surgery, the application of E-learning digital platform combined with brainstorming teaching can effectively improve the performance of nursing interns and strengthen their problem-solving, self-learning, and critical thinking abilities, with a relatively high degree of satisfaction among nursing interns.
8.Effect of esketamine versus sufentanil on postoperative pain after anesthesia induction in obese patients un-dergoing laparoscopic sleeve gastrectomy

Objective To observe and compare of the effects of esketamine and sufentanil induction for general anesthesia on postoperative analgesia in obese patients undergoing laparoscopic sleeve gastrectomy.Methods Patients scheduled for elective laparoscopic sleeve gastrectomy between March 2023 and March 2024,irrespective of gender,aged between 20 and 50 years,with a BMI ranging from 30.0 to 50.0 kg/m2 and ASA Ⅰ or Ⅱ classification,were randomly allocated into two groups:the esketamine group(Group E)and the sufentanil group(Group S),each consisting of 32 cases.During anesthesia induction,Group E received a dose of esketamine at 0.5 mg/kg while Group S received sufentanil at a dosage of 0.5 μg/kg;the remaining protocol remained unchanged.Heart rate(HR),mean arterial pressure(MAP),and SpO2 were recorded at various time points:upon arrival(T0),prior to anesthesia induction(T1),immediately after induction(T2),during intubation(T3),at the conclusion of anesthesia adminis-tration(T4),and during extubation(T5).Postoperative pain scores using the Numeric Rating Scale(NRS)were assessed at specific intervals following surgery:one hour post-surgery(P0),six hours post-surgery(P1),twelve hours post-surgery(P2),twenty-four hours post-surgery(P3)and forty-eight hours post-surgery(P4).Additionally,intraoperative remifentanil consumption as well as the number of presses on the analgesic pump within forty-eight hours after surgery in both patient groups were documented along with any occurrences of adverse reactions.Results During the surgery,there was no statistically significant difference in mean arterial pressure(MAP)and heart rate(HR)between the two patient groups at T0~T5(P>0.05).At T2,both groups exhibited lower MAP and HR compared to T0;specifically,group S had a MAP of(91.81±8.94)mmHg and HR of(81.75±13.37)beats/min,while group E had a MAP of(93.69±9.96)mmHg and HR of(80.38±13.2)beats/min,with group E showing values closer to baseline levels.At T3,both groups experienced a transient increase in MAP and HR(P<0.05);specifi-cally,group S had a MAP of(97.56±8.96)mmHg and HR of(86.47±13.84)beats/min,while group E had a MAP of(101.03±8.29)mmHg and(89.41±15.32)times/min,with S group closer to baseline values.There was no statistically significant difference in the amount of remifentanil used during surgery between group S and group E(P>0.05),which were(2071.88±717.63)μg and(2093.75±718.39)μg,respectively.Compared with the postoperative conditions of the two groups,the NRS scores of group E(0.41±0.61±1.870.75,2.47±0.62)at P0,P1 and P2 were lower than those in group S(0.88±0.71,2.47±0.72,2.97±0.54),and the difference was statisti-cally significant(P<0.05).The number of intravenous analgesia pump presses was significantly reduced in group E after surgery(P<0.05),with the postoperative analgesic pump compressions occurring 11.25±2.70 times in group S and 8.56±2.23 times in group E.The incidence of postoperative nausea and vomiting(PONV)and hypotension in Group E(21.88%,15.63%)was lower than that observed in group S(46.88%,37.50%),demonstrating statistical significance(P<0.05).Conclusion In comparison to sufentanil induction,the utilization of esketamine anesthesia induction is deemed safe for laparoscopic sleeve gastrectomy in obese patients,effectively mitigating postoperative acute pain and reducing the incidence of PONV.
9.Baicalein:A potential GLP-1R agonist improves cognitive disorder of diabetes through mitophagy enhancement

There is increasing evidence that the activation of glucagon-like peptide-1 receptor(GLP-1R)can be used as a therapeutic intervention for cognitive disorders.Here,we have screened GLP-1 R targeted com-pounds from Scutellaria baicalensis,which revealed baicalein is a potential GLP-1 R small-molecule agonist.Mitophagy,a selective autophagy pathway for mitochondrial quality control,plays a neuro-protective role in multiple cognitive impairment diseases.We noticed that Glp1r knock-out(KO)mice present cognitive impairment symptoms and appear worse in spatial learning memory and learning capacity in Morris water maze(MWM)test than their wide-type(WT)counterparts.Our mechanistic studies revealed that mitophagy is impaired in hippocampus tissue of diabetic mice and Glp1r KO mice.Finally,we verified that the cognitive improvement effects of baicalein on diabetic cognitive dysfunction occur through the enhancement of mitophagy in a GLP-1 R-dependent manner.Our findings shed light on the importance of GLP-1 R for cognitive function maintenance,and revealed the vital significance of GLP-1R for maintaining mitochondrial homeostasis.Furthermore,we identified the therapeutic potential of baicalein in the treatment of cognitive disorder associated with diabetes.
10.Descending necrotizing mediastinitis:a clinical analysis based on 10 years of published data in China

Objective To investigate the clinical characteristics,diagnosis,treatment,and prognosis of descending necrotizing mediastinitis(DNM)to provide a reference for the early diagnosis and timely treatment of DNM.Methods Data on DNM in China was electronically retrieved from the core databases and comprehensively reviewed from June 2012 to June 2023.The infection,pathogenic microorganisms,main symptoms,comorbidities and treatment methods of DNM were analyzed.Results The data of a total of 781 DNM patients,with an average age of(52.97±5.64)years,were retrieved,including 554 males and 227 females.Odontogenic source,tonsillitis,pharyngeal abscess,sialoadenitis,upper respiratory tract infection,foreign body injury,or iatrogenic traumatic procedures are common causes.Among these,odontogenic infection is the most common source.Streptococcus sp.(n=217)and Staphylococcus sp.(n=82)were most isolated,followed by Klebsiella pneumoniae and Pseudomonas aeruginosa(equally n=59).A total of 69.4%(542/781)of DNM patients recruited in this study were discovered to have various comorbidities,and more than one-third of these patients(n=185)had diabetes.Of the broad antibiotics,carbapenem was most frequently used as treatment,and vancomycin was the most frequently coadministered.The mediastinal drainage approach varies widely,and the optimal regimen is still unknown.Seventy-two patients were treated with video-assisted thoracoscopic/mediastinoscopic surgical drainage,22 patients were treated with percutaneous catheter drainage,30 underwent the transcervical approach,and 40 underwent thoracotomy.A total of 617 patients who were selected underwent the appropriate combined operation for surgical drainage according to the specific location of the infected focus.The overall mortality rate of all 781 DNM pa-tients included was 11.2%.Conclusion The most effective diagnosis and treatment of DNM is a high degree of clini-cal vigilance followed by prompt and adequate drainage with intensive care,including hemodynamic monitoring,nutri-tional support,computer tomographic scanning repeated as necessary,and combined use of systemic antibiotics.
