1.Action Mechanism of Resolving Dampness and Phlegm of Pinelliae Rhizoma Praeparatum Based on Interconnection Between Lung and Large Intestine

ObjectiveTo investigate the effects of Pinelliae Rhizoma Praeparatum (PRP) on lung tissue, gut microbiota, and short-chain fatty acid (SCFA) metabolism in a model of mice with cold fluid retention in the lung and explore its mechanism of action in resolving dampness and phlegm based on the interconnection between the lung and large intestine. MethodsFifty female ICR mice were randomly divided into a normal group, model group, positive control group (Xiaoqinglong granules, 6.5 g·kg-1), and high-dose and low-dose PRP decoction groups (3.0, 1.5 g·kg-1), with 10 mice in each group. A model of mice with cold fluid retention in the lung was established using ovalbumin (OVA) sensitization combined with cold-water immersion. Drug interventions were conducted from day 18 to day 33 for 15 consecutive days. The airway resistance value of the mice was measured using a non-invasive pulmonary function analyzer. Phlegm-resolving effects were evaluated via a microplate reader. Eosinophil and neutrophil counts in bronchoalveolar lavage fluid (BALF) were analyzed using an automated hematology analyzer. Serum levels of total immunoglobulin E (IgE), interferon-γ (IFN-γ), interleukin-4 (IL-4), and BALF levels of interleukin-6 (IL-6) and interleukin-8 (IL-8) were quantified by enzyme-linked immunosorbent assay (ELISA). Lung histopathology was assessed using hematoxylin-eosin (HE) staining. Immunohistochemistry (IHC) was employed to detect mucin 5AC (MUC5AC) and aquaporin 5 (AQP5) protein expression in lung tissue. Gut microbiota composition was analyzed via agarose gel electrophoresis, and fecal SCFA levels were measured by gas chromatography-mass spectrometry (GC-MS). ResultsCompared with the normal group, the model group exhibited significantly increased airway resistance value (RI) (P<0.05), elevated eosinophil and neutrophil counts and IL-6 and IL-8 levels in BALF (P<0.05), increased serum IgE and IL-4 levels (P<0.05), with reduced IFN-γ levels (P<0.05). It also showed thickened bronchial walls, widened alveolar septa, narrowed lumens, and mucus plugs in lung tissue, upregulated MUC5AC protein expression and downregulated AQP5 protein expression (P<0.05), decreased relative abundance of beneficial gut microbiota (Firmicutes, Clostridia, Clostridiales, Lactobacillaceae, and Lactobacillus), and increased abundance of harmful microbiota (Bacteroidetes, Bacteroidia, Bacteroidales, Muribaculaceae, and Muribaculum). In addition, the model group presented reduced fecal SCFA levels (acetate, propionate, and butyrate) (P<0.05). After the intervention of PRP decoction, compared to the model group, all drug administration groups showed decreased RI (P<0.05), increased phenol red excretion, declined eosinophil and neutrophil counts and IL-6, IL-8, IgE, and IL-4 levels (P<0.05), and improved IFN-γ levels (P<0.05) and lung pathology improved. The MUC5AC protein expression decreased (P<0.05), and the AQP5 protein expression increased (P<0.05). The disorder of gut microbiota was improved, and the diversity of gut microbiota was restored, with a significantly increased relative abundance ratio of beneficial microbiota (P<0.05) and a significantly reduced relative abundance ratio of harmful microbiota (P<0.05). The SCFA levels (acetate, propionate, and butyrate) increased (P<0.05). The efficacy indicators of serum inflammatory factors (IgE, IL-4, and IFN-γ), phlegm-resolving effect, airway resistance, total pathological score, and the protein expression of MUC5AC and AQP5 were correlated with gut microbiota and SCFAs. ConclusionPRP decoction alleviates cold-phlegm syndrome by modulating the gut-lung axis, promoting beneficial gut microbiota, enhancing SCFA production, restoring the balance of gut microbiota, and suppressing respiratory inflammation. This study provides novel insights into the TCM theory of interconnection between the lung and large intestine.

3.Effect analysis of trihalomethane reduction in the raw water from Qingcaosha reservoir using various water treatment processes

ObjectiveTo investigate the content of trihalomethanes (THMs) in treated water after different water treatment processes and their correlations with premanganate index, so as to provide data support for the renovation of water production process and optimization of water quality improvement. MethodsFrom 2022 to 2023, seven centralized water supply units using raw water from Qingcaosha reservoir were selected as the testing sites, among which three units with the conventional treatment process, two units with the advanced treatment process, and two units with the advanced treatment process combined CO2 treatment. Monthly water quality testing data were collected, focusing on testing the concentration variations of THMs, trichloromethane, dibromochloromethane, bromodichloromethane, bromoform, and permanganate index. ResultsThe comparison between conventional treatment process and advanced treatment process demonstrated that the conventional treatment process exhibited significantly higher concentrations of trihalomethanes, trichloromethane, bromodichloromethane, and permanganate index in water samples (all P<0.05). When comparing conventional treatment process with advanced treatment process combined with carbon dioxide treatment, the conventional treatment process showed significantly elevated levels of trihalomethanes, dibromochloromethane, bromodichloromethane, and permanganate index (all P<0.05). No statistically significant differences were observed in the comparison of various indicators between advanced treatment process and advanced treatment process combined with carbon dioxide treatment for any of the measured parameters (all P>0.05). Analysis of seasonal variations revealed that finished water during the high-temperature period (May to November) contained significantly higher concentrations of trihalomethanes, trichloromethane, bromodichloromethane, and tribromomethane compared to the low-temperature period (December to April of the following year) (all P<0.05). Significant positive correlations were identified between permanganate index and trihalomethanes (r=0.213, P=0.007), permanganate index and dibromochloromethane (r=0.186, P=0.019), permanganate index and bromodichloromethane (r=0.243, P=0.002), permanganate index and tribromomethane (r=0.193, P=0.014). ConclusionCompared to the conventional water treatment process, advanced treatment process and advanced treatment combined with CO2 injection process can significantly reduce the concentrations of THMs in the treated effluent water. Besides, the generation of THMs is affected by seasonal temperatures, with higher concentrations of THMs, trichloromethane, bromodichloromethane, and bromoform being observed in the high-temperature season. Additionally, the permanganate index shows a significant positive correlation with THMs concentrations, indicating that the content of organic matter in the source of raw water contributes to the generation of THMs in the treated water.
4.Prenatal diagnosis and genetic analysis of two fetuses with Wolf-Hirschhorn syndrome

Objective:To explore the prenatal ultrasound phenotype and genetic basis of two fetuses with Wolf-Hirschhorn syndrome (WHS).Methods:A retrospective analysis was conducted on the ultrasound imaging data of two fetuses suspected for WHS at the Prenatal Diagnostic Center of Qingyuan People′s Hospital in July 2017 and August 2019, respectively. Amniotic fluid samples of the two fetuses were subjected to chromosomal karyotyping and chromosomal microarray analysis (CMA). This study was approved by Medical Ethics Committee of the Qingyuan People′s Hospital (Ethics No. IRB-2022-064).Results:Prenatal ultrasound examination of the two fetuses had consistently revealed WHS-associated traits including intrauterine growth restriction (IUGR), craniofacial abnormalities and cardiovascular anomalies. Karyotyping analysis suggested that both fetuses had harbored cryptic chromosomal translocations involving partial deletion of 4p. And parental verification revealed that it was de novo for fetus 1 and paternal for fetus 2. CMA has confirmed that fetus 1 had an approximately 8.7 Mb deletion at 4p16.3p16.1 and a 6.8 Mb duplication at 8p23.1p23.1, whilst fetus 2 had a 20.05 Mb deletion at 4p16.3p15.31 and a 7.66 Mb duplication at 9p24.3p24.1. The karyotype of fetus 1 was determined as 46, XN, der(4)t(4; 8)(p16.1; p23.1)dn.arr[hg19]4p16.3p16.1(68345_8721580)×1, 8p23.3p23.1(158048_6933745)×3, and that of fetus 2 was determined as 46, XN, der(4)t(4; 9)(p15.3; p24)dpat.arr[hg19]4p16.3p15.31(68345_20116061)×1, 9p24.3p24.1(208454_7868292)×3. Conclusion:The 4p deletion is probably the main cause for the WHS phenotype in both fetuses. WHS should be suspected when IUGR, renal anomalies, craniofacial and cardiovascular abnormalities are detected upon prenatal ultrasound screening.

Objective:To explore the clinical efficacy of endoscopic sleeve gastrectomy (ESG) in the treatment of obesity.Method:A 26 year old female patient was admitted on October 20, 2022 due to a progressive increase in weight for 2 years. Her body mass index (BMI) was 30.04 kg/m 2, body fat percentage was 39.2%, and visceral fat grade was 15. ESG was performed using the OverStitch SX endoscopic suture system. Result:The surgery was successful, with approximately 5 ml of intraoperative bleeding.The patient discharged on the first day after surgery. Two weeks after surgery, small bowel follow-through showed a tubular shape of the stomach. At 6 months after surgery, the BMI was 25.2 kg/m 2, body fat percentage was 32%, visceral fat grade was 10. The total body weight loss rate (%TBWL) at 6 months after surgery was 16%, and the excess weight loss rate (%EWL) was 54.5%. Conclusion:ESG is effective for the treatment of obesity.

7.Analysis of the effect of depression on the quality of life of patients with Parkinson's disease

Objective To explore the effect of depression in Parkinson's disease(PD)on the quality of life of patients.Methods The patients in this study were from the outpatient department and inpatients of PD specialty of Nanjing Brain Hospital from May 1,2022 to December 31,2022.The 199 patients were screened with Hamilton Depression Scale(HAMD-17)and 39 PD Quality of Life Questionnaire(PDQ-39).According to the HAMD-17 score,HAMD-17<14 was divided into PD group,and HAMD-17≥14 was divided into PD combined with depression group.The differences between the two groups in age,age of onset,course of disease,HAMD-17 score,PDQ-39 score,UPDRS-Ⅰ,UPDRS-Ⅱ,UPDRS-Ⅲwere compared,and the relationship between the PDQ-39 sub-dimension and HAMD-17 and the average score of the PDQ-39 sub-dimension of the two groups were further analyzed.Finally,the possible factors affecting the quality of life of patients were analyzed.Results The correlation analysis of each subdimension of HAMD-17 and PDQ-39 showed that except for physical discomfort,the other seven dimensions of PDQ-39 were positively correlated with HAMD-17 score,the differences were statistically significant(all P<0.05).Multiple linear regression analysis showed that in addition to HAMD-17,the influencing factors of quality of life were also associated with UPDRS-Ⅱ(activities of daily living).Conclusion PD complicated with depression is common,depressive symptoms can significantly affect the quality of life of patients.
8.Comparison of the diagnostic value of 18F-FDG PET/CT and WB-DWI in malignant tumor bone metastasis

Objective To compare the diagnostic value of 18F-fluorodeoxyglucose(18F-FDG)positron emission tomography(PET)/CT and MR whole body diffusion weighted imaging(WB-DWI)in malignant tumor bone metastasis.Methods A total of 100 patients with suspected malignant tumor bone metastasis admitted were selected as the research objects.All patients underwent 18F-FDG PET/CT and WB-DWI before treatment.Pathological examination and follow-up results were used as the gold standard,the diagnostic value of the two examination modalities in malignant tumor bone metastasis was analyzed.Results There were 93 patients with bone metastasis diagnosed by gold standard,with a total of 232 lesions.The detection rate of 18F-FDG PET/CT was 92.67%,which was not significantly different from that of WB-DWI(89.66%)(P>0.05).The sensitivity,positive predictive value and diag-nostic coincidence rate in malignant tumor bone metastasis of 18F-FDG PET/CT were 92.67%,98.62%and 98.71%,respectively,which were significantly higher than 89.66%,97.65%and 94.83%of WB-DWI,but the difference was not significant(P>0.05).The specificity and negative predictive value of 18F-FDG PET/CT were 82.35%and 45.16%,respectively,which were significantly higher than 70.59%and 33.33%of WB-DWI(P<0.05).The maximum standardized uptake value(SUVmax)of patients with bone metastasis was significantly higher than that of patients with benign lesions,and the apparent diffusion coefficient(ADC)value was significantly lower than that of patients with benign lesions(P<0.05).Receiver operating characteristic(ROC)curve showed that the diagnostic value of SUVmax combined with ADC[area under the curve(AUC)=0.912,P<0.05]was higher than that of single examination.Conclusion 18F-FDG PET/CT is more effective than WB-DWI in the diagnosis of malignant tumor bone metastasis,he combination of the two examinations can improve the diagnostic efficacy for malignant tumor bone metastasis.
9.Application and exploration of virtual reality technology in surgical practice and medical education

Virtual reality (VR) is a technology that interacts computer-generated three-dimensional scenes with the user′s senses and has been gradually applied in the real medical field. Since the first time of successful attempt in 3D laparoscopic gastrointestinal surgery VR living video broadcast in 2016, the authors have experienced the exploration of the application of this technology (2016-2017) as well as the in-depth research phase (2018-2019). Nowadays, the VR application in our center has entered into the application practice phase (2020 to present). With the technology iteration and more and more application experience acquisition, our current VR application mainly focuses on two practice settings, including the clinical teaching scenarios for early exposure to acute appendicitis for medical undergraduate students and the preoperative communication to give pati-ents an early sense of the process in the operating theatre, and has achieved good application results. With continuous breakthroughs and innovations in software and hardware, we believe that more clinical VR scenarios and the course audience will be expanded to a wider group. The integration of some new hotspots such as Meta-Universe and ChatGPT will make up for the shortcomings in VR content scenarios and interactivity, and its application prospects are infinite.
10.Constructing a Syndrome Differentiation Model by Combining Latent Structure Analysis of Association Rules: An Example of Kidney Yang Deficiency Syndrome in Primary Osteoporosis

ObjectiveTo construct a quantitative differentiation model of traditional Chinese medicine (TCM) syndromes by taking primary osteoporosis (POP) with kidney yang deficiency syndrome as an example, and to provide methodological reference for the standardization of syndrome differentiation. MethodsHigh-frequency clinical features of POP were screened by descriptive statistical analysis, and strong association features of POP were obtained by association rule algorithm. On this basis, a latent structure (latent tree) model was established through latent structure analysis, and the implicit and explicit variables (features) related to POP with kidney yang deficiency syndrome were comprehensively clustered, and the clustering results were interpreted by the indexes of mutual information and cumulative information coverage, to explore the primary and secondary symptoms, and to deduce the categories of POP with kidney yang deficiency syndrome based on the probability of the features appearing in the various latent categories. Based on the categories, the clinical feature scores and identification thresholds were calculated, and the syndrome differentiation model of POP with kidney yang deficiency was initially constructed by combining the comprehensive judgment rules. Finally, the results of TCM professionals' judgment were used as the gold standard to further evaluate the effectiveness of the model in assisting the syndrome differentiation. ResultsThe 32 features strongly associated with POP were obtained, and the Bayes information critedon score of the further constructed latent tree model was -15291.93. Based on the mutual information and the cumulative information coverage, the main symptoms of POP with kidney yang deficiency syndrome were bone weakness, fatigue, pale tongue, clear urine, frequent nocturnal urination, cold limbs, thin pulse, white coating, and secondary symptoms were weakness, loss of libido, loose stool, frequent urination, lumbar and knee weakness, and fear of cold. From the probability of the occurrence of each clinical feature in different latent categories of POP with kidney yang deficiency syndrome, the state was introduced as S0 category (none/mild kidney yang syndrome)/ S1 category (moderate kidney yang syndrome)/ S2 category (severe kidney yang syndrome). Optimizing the preliminary rules of state identification and refining the state of S1 category, the results showed that among 970 patients with POP, there were 520 patients having no/mild kidney yang deficiency syndrome, 224 patients with moderate to mild kidney yang deficiency syndrome, 81 patients with moderate to severe kidney yang deficiency syndrome, and 145 patients with severe kidney yang deficiency syndrome. During the evaluation and validation process, the correct rate of the model assessment index was 0.8835, while the sensitivity was 0.7181, and the specificity was 0.9437. ConclusionCombined with the latent structure analysis of the association rule, the syndrome differentiation model for POP with kidney yang deficiency could be constructed, and the model shows a good quantitative identification effect, which can provide methodological supports for clinicians to improve the efficiency and accuracy of TCM diagnosis.
