1.Effects of orlistat on the viability of human gallbladder cancer cells

Objective:To investigate the effects of orlistat on the viability of human gall-bladder cancer (GBC) cells.Methods:The experimental study was conducted. The human GBC NOZ cells with high expression of FSAN was screened out through in vitro cultivating human GBC-SD, SGC-996 and NOZ cells. The cell proliferation assay, clone formation assay and protein detection experiment were used to analysis of the effects of orlistat on the viability of human GBC cells. Cell grouping: NOZ cells cultured with medium were set as the control group, cultured with medium + 10 μmol/L orlistat were set as the low-dose orlistat group, cultured with medium + 100 μmol/L orlistat were set as the high-dose orlistat group, respectively. Observation indicators: (1) expression of FASN protein in human GBC cells; (2) effects of orlistat on the proliferation of human GBC NOZ cells; (3) effects of orlistat on apoptosis of human GBC NOZ cells. Measurement data with normal distribution were represented as Mean± SD, the ANOVA test was used for comparison between groups and the least significant difference method was used for pairwise comparison. Results:(1) Expression of FASN protein in human GBC cells. Results of western blot showed that the relative expression of FASN protein in human GBC NOZ, GBC-SD and SGC-996 cells was 0.57±0.06, 0.12±0.04 and 0.10±0.02, respectively, showing a significant difference among them ( F=115.67, P<0.05). There were significant differences between the NOZ cells and the GBC-SD or the SGC-996 cells ( P<0.05), and there was no significant difference between the GBC-SD cells and the SGC-996 cells ( P>0.05). (2) Effects of orlistat on the proliferation of human GBC NOZ cells. ① Results of cell proliferation assay showed that the absorbance value of NOZ cells was 2.34±0.12, 1.57±0.08 and 1.07±0.13 in the control group, low-dose orlistat group and high-dose orlistat group, respectively, showing a significant difference among them ( F=205.88, P<0.05). ② Results of clone formation assay showed that the number of NOZ cells clones was 257±23, 153±11 and 83±11 in the control group, low-dose orlistat group and high-dose orlistat group, respectively, showing a significant difference among them ( F=92.64, P<0.05). ③Results of western blot showed that the relative expression of Cyclin-D1 protein of NOZ cells was 2.31±0.10, 1.52±0.05 and 1.23±0.11 in the control group, low-dose orlistat group and high-dose orlistat group, respectively, showing a significant difference among them ( F=120.73, P<0.05). The relative expression of CDK-4 protein of NOZ cells was 1.58±0.04, 1.21±0.02 and 1.19±0.04 in the control group, low-dose orlistat group and high-dose orlistat group, respectively, showing a signifi-cant difference among them ( F=110.45, P<0.05). (3) Effects of orlistat on apoptosis of human GBC NOZ cells. Results of western blot showed that the relative expression of Bcl-2 protein of NOZ cells was 1.07±0.03, 0.36±0.03 and 0.15±0.02 in the control group, low-dose orlistat group and high-dose orlistat group, respectively, showing a significant difference among them ( F=1 242.93, P<0.05). The relative expression of Bax protein of NOZ cells was 0.51±0.03, 0.38±0.05 and 1.38±0.04 in the control group, low-dose orlistat group and high-dose orlistat group, respectively, showing a signifi-cant difference among them ( F=583.51, P<0.05). Conclusion:Orlistat can inhibit the growth of human GBC NOZ cells and promote their apoptosis.

The popularization of medical insurance in Panama has been basically completed. The coverage of health insurance is high, but with the problem of polarization. The major causes of death in Panama are is chemic heart disease, stroke, AD and diabetes. In recent years, the chronic kidney disease and COPD are increasing significantly. Panamanian native medicine, considered as complementary and alternative medicine, is recognized by local laws and usedby most of indigenous groups and rural people. Traditional Chinese Medicine (TCM) in Panama is at the starting stage, and there is no relevant legislation. However, this country attaches great importance to indigenous traditional medicine, and more and more people gradually accept it. TCM has great development potential in this country. It is suggested to promote the development of TCM in Panama. By culture as the pathway, exploring various forms of cooperation and exchange, taking the research and development of traditional herbal medicine as the breakthrough point, and increasing the spead of acupuncture usage, etc.
3.Comparison of prognostic value between gastric cancer and esophageal adenocarcinoma staging system in evaluation of the patients with advanced Siewert type Ⅱadenocarcinoma of esophagogastric junction

Objective To investigate the prognostic value between gastric cancer and esophageal adenocarcinoma staging system from International Union Against Cancer (UICC) / American Joint Committee on Cancer (AJCC) 8th edition in treatment of the patients with advanced Siewert type Ⅱ adenocarcinomas of esophagogastric junction (AEG) in China. Methods A total of 67 patients who were pathologically diagnosed as pT3-pT4 Siewert type Ⅱ AEG in Shanxi Provincial Cancer Hospital from November 2012 to September 2013 were enrolled. The stage distribution and survival of the patients in two staging systems were compared. The homogeneity and discriminative power of the two staging systems were compared by using likelihood ratio chi-square test and linear trend chi-square test. Log likelihood and C-index were used to evaluate the prognostic value of the two staging systems. Results The stage distribution in gastric cancer staging system of 67 patients was ⅡA (16.4%), ⅡB (14.9%), ⅢA (31.3%), ⅢB (34.3%), ⅢC (3.0%), and 5-year survival rates ofⅡA, ⅡB, ⅢA, ⅢB were 81.8%, 70.0%, 39.1% and 36.6%, respectively (χ2= 8.221, P= 0.042); the stage distribution of esophageal adenocarcinoma staging system was ⅡB (22.4%), ⅢB (40.3%) and ⅣA (37.3%), and 5-year survival rates were 80.0%, 43.7%and 40.0%, respectively (χ2=7.671, P=0.022). The likelihood ratio chi-square value and linear trend chi-square value of the gastric cancer classification system were more than those of esophageal adenocarcinoma staging system (7.855 vs. 6.692, 6.051 vs. 5.351), and Log likelihood estimation value of the gastric cancer classification system was less than that of esophageal adenocarcinoma classification system (241.324 vs. 242.045), and C index of the gastric cancer classification system was more than that of esophageal adenocarcinoma staging system (0.7289 vs. 0.7215). Conclusion UICC/AJCC 8th edition gastric cancer TNM staging system is superior to esophageal adenocarcinoma TNM staging system in predicting the prognosis of advanced Siewert typeⅡAEG.

5.Outcome of patients with osteosarcoma after recurrence and analysis of factors affecting the long-term survival——A long-term follow-up study of recurrent patients by multicenter

Objective To investigate the outcome and affecting factors of patients with osteosarcoma after recurrence.Methods Retrospective analysis was used to analyze the achieved data and prognosis-related factors of 72 patients with extremity osteosarcoma after recurrence from June 2000 to September 2012 through multicenter joint treatment.37 males and 35 females were included.The overall age was 7 to 61 years old (average age22.2 years old).Classification based on the tumor recurrent parts:42 cases (58.3%) on femur,18 cases (25%) on tibia,7 cases (9.7%) on humerus and 5 cases (6.9%) on fibula.Classification based on metastases:14 cases of simple recurrence (19.4%),58 cases (80.6%) of recurrence with metastasis:29 cases (40.3%) of recurrence occurred first,9 cases (12.5%) of metastasis occurred firstly,20 cases (27.8%) of simultaneous recurrence with metastasis;12/14 patients (16.7%) of simple recurrence had long-term survival,but 2 died (2.8%).Six of 58 patients (8.3%) of recurrence with metastasis had long-term survival but 52 died (72.2%).Results The overall 10-year postrelapse survival rate of the 72 patients was 25%.Kaplan-Meier univariate analysis revealed that postrelapse survival was significantly influenced by the continued treatment after recurrence and the number of postoperative chemotherapy,but it was not significantlyinfluenced by age,gender and the number of preoperative chemotherapy.Multivariate analysis revealed that the continued treatment after recurrence and the number of postoperative chemotherapy were independent prognostic factors (P=0.002,P=0.007).At the same time,according to the survival curve grouped by different indicators,the number of chemotherapy after recurrence has no significant effect on TFS.However,the continued treatment after recurrence has obvious statistical significance on improving TFS and OS of patients (P=0.026,P=0.002).Conclusion Resection and standard postoperative chemotherapy had significant effects on the postrelapse survival in patients with osteosarcoma.There are good prospects for further multimodal therapy and multicenter cooperation for osteosarcoma after recurrence.

Objective To develop a highly sensitive luminescent reagent for bloodstain testing at forensic crime scenes.Methods Based ontheprincipleof ECL luminescence and the ping-pong conjugate activation principle of chemical electronic chain,this project developed a new type of highly sensitive luminescent reagent for bloodstain testing by usingthe uniform design of experimental methods to optimize the conditions andsynthesize several new compounds.Results The bloodstain testing luminescent reagentdeveloped in this project has high sensitivity andlongluminescence time.In the case of blood samples diluted by 1,000 times,reading the fluorescence withChemiScope 3300 chemiluminescence imaging system,the maximumvalue of gray scale reached 56,and the luminescence time lasted for 10 minutes.Conclusion The project has successfully developed a highly semitivebloodstain testing reagentthat could be applied to crime scene investigation.
7.Clinical analysis and imaging features of 5 cases of anti GABA-B receptor encephalitis

Objective To investigate the clinical manifestations, laboratory and imaging features, treatments and prognosis of anti GABA-B receptor encephalitis. Methods We performed a retrospective analysis of the clinical data of five cases with anti GABA-B receptor encephalitis diagnosed by cerebrospinal fluid immunological examination. Results Five cases had acute onset. The initial clinical symptom was epileptic seizure. After epileptic attack, patients developed different degrees of cognition dysfunction, mental and behavioral disorders. Neuroimaging showed that the lesions mainly involved medial temporal lobe and hippocampus. The lesions ware equal or low density shadows in CT scan and long T 1 and T2 signals, T2 high signals on Flair, equal or high signals on DWI in MRI scan. Cranial MRI enhancement showed lesions with no enhancement in two tested cases. One case took PET-CT and data showed a low density shadow of temporal lobe and hippocampus with increased metabolism. CSF and blood immunological examinations showed CSF GABAB-R-AB positive (+), blood GABAB-R-AB positive (+). Five cases were associated with increased NSE. EEG was nonspecific and related to the conscious state of the patient. Lung CT showed enlarged mediastinal lymph nodes in 5 tested cases. Central lung cancers were found in five cases including three cases at onset and two cases during three month follow-up. Bronchoscopic biopsy identified small cell lung cancer in two examined cases. Conclusion Epileptic seizure is the most frequent initial symptoms of anti GABA-B encephalitis. Inflammatory response signals in the medial temporal lobe and hippocampus are image features of anti GABA-B encephalitis. The disease is closely related to lung cancer (Especially c small cell lung cancer, SCLC). Immunological examination of cerebrospinal fluid and the above characteristics are helpful for diagnosis.

OBJECTIVE:To discuss the way to realizing clinical pharmacists’prescribing rights,and to provide reference for the revise of related policies and regulations. METHODS:The clinical pharmacists prescribing rights and its necessity were inter-preted. Referring to pharmacists’prescribing rights in Canadian limited prescription mode,British dispensatory mode,United States consultative prescription mode,clinical pharmacists’prescribing rights in China were expounded. RESULTS & CONCLU-SIONS:It is necessary and feasible to achieve clinical pharmacists’prescribing rights in China. Clinical pharmacists’prescribing rights can be realized and the pharmaceutical role of clinical pharmacists can be played through conducting clinical pharmacist pre-scription training,establishing chronic disease,common disease,mild disease and other disease dispensatory,gradually revising the concept of“prescription”and“prescribing rights”,promoting the legislation of pharmacists and clinical pharmacists’prescrib-ing rights,promoting the realization of the prescription right of clinical pharmacist,prompting clinical pharmacist to play the role of pharmacy.
9.Comparative analysis of MRI features and pathological findings for renal clear cell carcinoma

Objective To study the relationship between MRI features and pathological characteristics of the renal clear cell carcinoma,and to conduct a comparative analysis.Methods 23 patients renal clear cell carcinoma who were proved by postoperative pathology were retrospectively analyzed. The preoperative MRI findings and the postoperative pathological characteristics were compared. Results MRI plain scan showed uniform signal was seen in 2 cases, and mixed signal was found in 21 cases with cystic and necrosis. Blood was seen in 11 cases and false envelope was present in 12 cases. Dynamic enhanced scanning revealed that solid part was enhanced obviously, it was lower than the renal cortex but higher than the renal medulla in cortical phase and continued to be enhanced in medullary phase and delay phase.Postoperative pathological characteristics showed that tumor profiles were yellow-white and cystic and necrotic structures were present within tumors, blood was found in 17 cases. Endoscopic tumor cells were present in solid nests or acinar structure, and mesenchyma was rich in thin-walled vessels involved in reticular interval.14 cases were found to have false envelope.Conclusion Renal clear carcinoma MRI imaging manifestion is related to organixational srructure. MR plain scan combined with dynamic enhanced scan has very high value in the diangnodis of renal cell carcoinoma.
10.Selection of range of regional lymphadenectomy for patients with T2 gallbladder cancer

Objective To investigate the efficacy of regional lymphadenectomy for patients with T2 gallbladder cancer. Methods From January 1990 to December 2009, 48 patients with T2 gallbladder cancer received regional lymphadenectomy following radical surgery at the Xinhua Hospital of Shanghai Jiaotong University, and their clinical data were retrospectively analyzed. Patients were divided into two groups according to the range of lymphadenectomy. Standard group (23 patients): lymph nodes in the regions of bile duct, common bile duct and hepatoduodenal ligament were dissected; extended group (25 patients): lymph nodes in the regions of hepatoduodenal ligament, head of pancreas, duodenum, portal vein, common hepatic artery and celiac axis were dissected).The condition of patients in the two groups were compared after the treatment. The morbidity and survival rate were analyzed by using Fisher exact test and Kaplan-Meier method, respectively, and the survival rates between the two groups were compared by using Log-rank test. Results No perioperative death was found in the two groups. The morbidities was 17% (4/23) in the standard group and 24% (6/25) in the extended group, with no significant difference between the two groups ( P > 0.05 ). The 5-year cumulative survival rate and median survival time were 40% and 29.8 months in the standard group, and 66% and 53.2 months in the extended group, with significant differences between the two groups ( x2 = 4. 687, P < 0.05 ). Conclusion Extended regional lymphadenectomy should be performed on patients with T2 gallbladder cancer if the primary lesions can be dissected radically.
