1.Treatment of Diabetic Retinopathy with Blood Stasis, Collateral Obstruction, and Qi and Yin Deficiency Syndrome with Tongluo Mingmu Capsules: A Randomized, Double-blind, and Multi-center Phase Ⅲ Clinical Trial

ObjectiveTo evaluate the clinical efficacy and safety of Tongluo Mingmu capsules in the treatment of diabetic retinopathy with blood stasis, collateral obstruction, and Qi and Yin deficiency syndrome. MethodA randomized, double-blind, positive-control, and multi-center clinical trial design method was used. 416 patients with diabetic retinopathy with blood stasis, collateral obstruction, and Qi and Yin deficiency syndrome in four test centers were included (the ratio of the treatment group to the control group was 3∶1). On the basis of standardized hypoglycemic treatment, the treatment group was given both four Tongluo Mingmu capsules and two Calcium Dobesilate capsule agents three times a day, while the control group were given both two Calcium Dobesilate capsules and four Tongluo Mingmu capsule agents three times a day. The course of treatment was 12 weeks. The curative effect of Tongluo Mingmu capsules was evaluated by comparing the comprehensive curative effect of diabetic retinopathy, traditional Chinese medicine(TCM) syndrome score, corrected visual acuity, fundus changes, fundus fluorescence angiography, and other curative effect indexes before and after treatment in the two groups. At the same time, general examination, laboratory examination, and adverse events were performed to evaluate the safety of the drug. ResultThe baseline demographic data and disease characteristics of the treatment group and the control group were balanced and comparable, with the difference not statistically significant. After 12 weeks of treatment, the total effective rate of the comprehensive curative effect of diabetic retinopathy in the treatment group (61.0%, 189/310) was better than that in the control group (44.1%, 45/102), and the difference was statistically significant (χ2=8.880, P<0.01). The total effective rate of TCM syndromes in the treatment group (88.4%, 259/293) was better than that in the control group (69.9%, 65/93), and the difference was statistically significant (χ2=17.927, P<0.01). The disappearance rate of dry eyes (χ2=8.305), dull complexion (χ2=4.053), lassitude (χ2=10.267), shortness of breath (χ2=8.494), and dry stool (χ2=8.657) in the treatment group was higher than that in the control group, and the difference between the groups was statistically significant (P<0.05, P<0.01). In terms of improving corrected visual acuity (χ2=8.382), fundus changes (χ2=6.026) , the treatment group was significantly better than the control group (P<0.05). During the trial, the incidence of adverse events in the treatment group and the control group was 1.3% and 2.9%, respectively. There was no significant difference between the two groups. In addition, there were no serious adverse events and adverse events leading to withdrawal in both groups. ConclusionTongluo Mingmu capsules can improve the comprehensive curative effect of diabetic retinopathy and enhance the efficacy of TCM syndromes, visual acuity, fundus changes, and fundus fluorescein angiography, with great safety. Therefore, it can provide a new alternative therapeutic drug for patients with diabetic retinopathy.
2.Analysis of the prevalence of norovirus infection in children with single center acute gastroenteritis during 2021-2022

Objective:To investigate the prevalence of norovirus and the correlation between norovirus infection and stool traits and clinical symptoms in children with acute gastroenteritis during 2021-2022.Method:A total of 2 195 anal swab samples were collected from cases of acute gastroenteritis in Beijing Children 's Hospital Affiliated to Capital Medical University from January 2021 to December 2022. PCR fluorescent probe assay was performed to detect norovirus,the clinical information and test results were performed by statistical analysis. Results:The detection rate of norovirus was 19.09%(419/2 195)in 2 195 children. The detection rates of male and female were 17.80%(244/1 371)and 21.24%(175/824),respectively,with statistically significant differences between genders( χ2=3.945, P<0.05). The positivity rate of noroviruses was higher in aged 1 month to 3 years[29.09%(64/220)~40.69%(94/231)],especially in 1 year[40.69%(94/231)]. The norovirus detection rate ranged from 2.44%(2/82)to 35.84%(62/173)in different months,with a high detection rate between January and March,higher in the winter and spring[28.36%(150/529),21.07%(146/693)],showing a clear seasonal distribution.Clinical symptoms of both vomiting and diarrhea were significantly more common among norovirus positive than negative children,the difference was statistically significant( χ2=57.29, P<0.05). Conclusion:In the high incidence season of winter and spring,for infants and young children aged 1 month to 3 years with diarrhea accompanied by vomiting symptoms,especially those aged 1 year,it is recommended to undergo norovirus related testing in time. Early treatment and isolation measures should be taken for diagnosed children to reduce cross infection and prevent outbreaks.
3.Extracellular vesicles carrying non-coding RNA regulate the activation of osteoclasts

BACKGROUND:It has been demonstrated that osteoclast activation plays an important role in skeletal system-related diseases.The mechanism of regulation of osteoclast activation by extracellular vesicles carrying non-coding RNA has not been fully elucidated. OBJECTIVE:To review and summarize relevant literature in and outside China,and to review the regulation of osteoclast activation by different non-coding RNAs in extracellular vesicles in different diseases,so as to provide a certain direction for subsequent research. METHODS:"Non-coding RNA,miRNA,lncRNA,circRNA,snoRNA,osteoclasts,extracellular vesicles,exosome,microparticle,apoptotic bodies"were used as search terms to search the databases of CNKI,WanFang,and VIP."Extracellular vesicles,exosome,microparticle,apoptotic bodies,non-coding RNA,miRNA,lncRNA,circRNA,snoRNA,osteoclast"were used as search terms to search PubMed.Finally,71 articles were included. RESULTS AND CONCLUSION:(1)The activation of osteoclasts is affected by many factors,among which the specific mechanism of non-coding RNA regulating osteoclast activation is not clear.(2)Extracellular vesicles can be secreted by osteoblasts,bone marrow mesenchymal stem cells,tumor cells and other cells.As a carrier of intercellular communication,extracellular vesicles can carry non-coding RNA to regulate osteoclast activation.(3)In the current studies on the regulation of osteoclast activation by extracellular vesicles carrying non-coding RNA,most of the diseases are osteoporosis,followed by tumor bone metastasis,and most types of non-coding RNA are miRNA.(4)There are relatively few studies on the regulation of extracellular vesicles carrying lncRNA and circRNA and snoRNA on osteoclast activation,and the regulatory mechanism is mainly ceRNA mechanism.(5)In conclusion,an in-depth study of the regulatory mechanism of extracellular vesicles carrying non-coding RNA on osteoclast activation is helpful to find key targets for the treatment of skeletal system-related diseases.
4.Evaluation of Innovation and Sustainable Development Ability for Traditional Chinese Medicine Preparations in Medical Institutions

OBJECTIVE
To evaluate the innovation and sustainable development ability of traditional Chinese medicine(TCM) preparation in medical institutions, and to provide reference for the decision-making of administrative departments and the sustainable development of TCM preparation in hospitals.
METHODS
Based on the information data of 133 medical institutions in Zhejiang Province, analytic hierarchy process was used to construct the evaluation system of innovation and sustainable development ability of TCM preparation of medical institutions, and the prediction model of evaluation system of TCM preparation room/center was built by back propagation neural network.
RESULTS
The evaluation index system for innovation and sustainable development of TCM preparations in medical institutions included 4 second-level indexes and 19 third-level indexes. Among them, the number of varieties developed, the number of over one million varieties, the number of registered varieties, the way of research and development, and the area of preparation room had higher weight values, which were 0.15758, 0.12928,0.09343, 0.07879 and 0.07458, respectively. The entropy weight method and analytic hierarchy process index weights were used to construct the back propagation neural network. The overall correlation coefficients of the training models of the Levenberg-Marquardt algorithm and the Scaled Conjugate Gradient algorithm were 0.98983 and 0.93480, respectively.
CONCLUSION
This study establishes a scientific and effective comprehensive evaluation system for the innovation and sustainable development ability of TCM preparations in medical institutions, which can realize the prediction and evaluation of the comprehensive ability of TCM preparations in medical institutions.
5.Correlation analysis between preschool children s emotional competence and home rearing environment

Objective:
To analyze the correlation between preschool children s emotional competence and home rearing environment in Shangrao City, so as to provide support for improving children s emotional competence development as well as their home rearing environment.
Methods:
A total of 1 242 children aged 3-6 years old from 10 kindergartens in Shangrao City were retrospectively investigated by stratified cluster random sampling method in December 2022, and the Children s Emotional Adjustment Scale-Preschool Version (CEAS-P) and the Home Nurture Environment Scale for children aged 3-6 were surveyed on parents of preschool children. The t-test was used to test the difference, Spearman correlation analysis and multiple linear regression were used to analyze the influencing factors of preschool children s emotional competence.
Results:
There were significant differences in emotional competence scores of preschool children for demographic indicators including age, place of residence, health status and whether they were only children ( F/t =5.98, 6.56, 38.00, 2.23, P <0.01). The emotional competence of preschool children was positively correlated with the home rearing environment ( r=0.62, P <0.01). Multiple linear regression analysis showed that diverse activities/play participation, social adaptation/self management, and emotional warmth/self expression in home rearing environment were positive predictors of children s emotional ability ( β =0.30, 0.28, 0.16), while neglect/intervention/punishment were negative predictors ( β =-0.09)( P <0.05).
Conclusions
The home rearing environment is a factor related to young children s emotional competence. It is suggested specific parenting initiatives such as enriching family activities and play, strengthening children s self adaptation and management, giving warmth and let children express emotions, and preventing child neglect, interference and punishment should be conducted to improve children s emotional competence.
6.Herbal Textual Research, Quality Evaluation and Phase Analysis of Tremolitum

In this research, a comprehensive examination of historical materia medica, medical literature, medical books and contemporary literature was conducted to systematically compile and verify the naming, origin, geographical variations, quality, medicinal properties and processing of Tremolitum, and combined with the physical analysis of 15 batches of samples, with a view to providing a basis for the quality evaluation. The evidences unequivocally support Yangqishi as the rectification of name, while alternative aliases include Baishi, Shisheng, Yangjushi and Yangshi. The primary source of Tremolitum has been the silicate hornblende mineral tremolite throughout recorded history, and its accompanied minerals are mainly chlorite, talc schist, anthophyllite asbestos, etc. In ancient times, the main production area of Tremolitum was situated in the "Yaoshan Mountain" region of Jinan, Shandong province. Presently, it is primarily produced in Hubei, Henan, Shanxi, Hebei and Shandong provinces. The ancient quality evaluation of Tremolitum includes "white color", "wolf teeth", "cloud head and rain foot", "heron hair", etc. While, modern materia medica mostly evaluates its quality in terms of color, luster and texture, including the characteristics of needle bundle, grayish-white, glossy and easy to be twisted and crushed. Tremolitum is slightly warm, non-toxic, and possesses a salty taste. It is associated with the kidney meridian, known for its benefits in nourishing the fire of the gate of life, warming the kidneys and strengthening the Yang. Common processing methods include fire calcination, elutriation, and processing with wine. Additionally, there is an exclusive processing method known as the "ascending Tremolitum method". Through the X-ray diffraction(XRD) analysis, the commercially available Tremolitum is mainly composed of tremolite[Ca2Mg5(Si4O11)2(OH)2], and the accompanied minerals are calcite and quartz, etc. Its color includes white, light gray, light green and so on, and it is weight and soft texture, irregular cross-section, with obvious fibrous texture and mostly glassy luster. Among the commercially available Tremolitum, samples with high content of tremolite are characterized by high quality features, such as "white color", "heron hair" and "ease of tearing". Also, Tremolitum is the Fe-bearing heterogeneous species of tremolite, and the two are often symbiotic. Therefore, by the herbal textual research and the comparison of the properties and composition of Tremolitum and its similar species, it has been verified that the primary source of Tremolitum is the silicate hornblende minerals tremolite and actinolite.
7.Analysis of epidemiological characteristics of respiratory pathogens in children with influenza-like illnesses in a children′s hospital in Beijing from 2022 to 2023

To investigate the status and epidemiological characteristics of respiratory pathogens infections in children with influenza-like illnesses (ILI) in Beijing Children′s Hospital from 2022 to 2023. A dual amplification technique was used to detect nucleic acids of seven common respiratory pathogens, including influenza A virus (Flu A), influenza B virus (Flu B), mycoplasma pneumoniae (MP), respiratory syncytial virus (RSV), parainfluenza virus (PIV), adenovirus (ADV), and Chlamydia pneumoniae (CP), in outpatient and inpatient children (aged 0-18 years) with influenza-like symptoms who sought medical care at Beijing Children′s Hospital, from January 2022 to March 2023. A total of 43 663 children were included in the study, of which 27 903 tested positive for respiratory pathogens with a total detection rate of 63.91%. Flu A had the highest detection rate of 69.93% (27 332/39 084), followed by MP about 13.22% (380/2 875). The total detection rate of RSV, PIV and ADV was 7.69% (131/1 704). Flu B had a detection rate of 0.16% (64/39 084). No CP was detected in this study. A total of 7 cases of dual infections were detected, with a detection rate of 0.41% (7/1 704). The Chi-square test was used to analyze the differences in detection rates of pathogens among different genders, age groups, and different seasons. Among the seven pathogens, only Flu A had statistically significant differences in gender ( χ2=16.712, P<0.001). The detection rates of Flu A and MP showed an increasing trend with age (both P trend<0.001), while the detection rates of RSV and PIV showed a decreasing trend with age (both P trend<0.001). Flu A had its epidemic peak in winter and spring, with detection rates of 61.30% (3 907/6 374) and 77.47% (23 207/29 958) respectively; MP and PIV had higher detection rates in autumn (25.14% and 7.64% respectively); RSV showed a relatively higher detection rate in winter (8.69%); Flu B and ADV had lower detection rates throughout the study period (0.16% and 1.17% respectively). In conclusion, children with ILI in 2022-2023 were mainly infected with a single respiratory pathogen, and occasionally dual pathogen infections were observed. Among them, the detection rate of Flu A was the highest, and only Flu A showed a gender difference in detection rate. As the age of the children patients increased, the detection rate of Flu A and MP showed an increasing trend, while RSV and PIV showed a decreasing trend. The prevalence of Flu A, Flu B, MP, PIV, and RSV were seasonal.
8.Initial clinical outcomes of the comprehensive debulking surgery for one-stage reconstruction of multiple toes macrodactyly in children

Objective:To evaluate the initial clinical outcome of applying a comprehensive debulking procedure centered on the digital and plantar flaps with comparatively healthy proximal pedicle to reconstruct the hypertrophic toes and forefoots in one-stage surgery in multiple toes macrodactyly in children.Methods:The clinical data of children with macrodactyly deformity treated by the Department of Burn and Plastic Surgery in Children’s Hospital of Chongqing Medical University from January 2022 to October 2023 were retrospectively analyzed. Design toe and plantar arbitrary flaps with a few vascular perforating branches which pedicle at the comparatively healthy proximal side and resect all tissues beyond the normal length. In severe toe, a composite tissue nail-flap with the artery was dissected to reconstruct the toenail. Fat debulking on the surface of deep plantar fascia as well as within the osseofascial sheath, resecting partially of the fatty infiltrated nerve, transverse and longitudinal osteotomy to shorten and narrowed phalanxes without epiphyseal block, arthroplasty and flexor tendon tightening were performed. Finally, toes shaped by wrapping toe flaps, and pelmas restored by plantar flaps which both with comparatively healthy proximal pedicle. The operation time, healing time, and whether the toes and toenails are preserved were all recorded. The maximum circumference of both foot, the difference in shoe size, the presence of skin ulcers, wound scars, toe webbing morphology, the presence of basic sensation, and the satisfaction of parents were followed-up after surgery. The modified follow-up questionnaire was used to evaluate the surgical effect, the total score is 0-12 points, and the higher the score, the better the surgical outcome. Descriptive methods was used for statistical analysis, and the measurement data conforming to normal distribution were expressed as Mean±SD.Results:A total of 15 patients were enrolled, including 11 males and 4 females. The median age at the time of operation was 2.75 years (0.9-10.8 years). There were 11 cases with 2 toes and 4 cases with 3 toes, a total of 34 toes involved. The average surgical duration was 4.13 hours (3.25 hours for 2 toes and 5.00 hours for 3 toes). Only one severe case with 3 toes involved was amputated the biggest toe ray and totally 2 toe nail flaps were harvested unsuccessfully intraoperative. Except 3 toes had distal skin partially necrosis and 2 nail flaps were completely necrotic which healed after dressing changes, the rest of the toes were healing well after operation. The average follow-up time was 4.8 months (3-11 months), the difference of circumference was less than 1.5 cm, as well as in length was less than 0.5 cm between feet and all children were able to wear the appropriate shoes with same size. A total of 4 toes failed to retain toenails, and 1 had a very thin toenail. Except for one case with insufficient web depth, the other toe webs were close to normal. There was no skin ulcer occurred, the basic sensation of the feet was present, and wound scars were inconspicuous. The score of the modified questionnaire was 10.54±0.88, all parents were satisfied with the results.Conclusion:Comprehensive procedure centered on the digital and plantar flaps with comparatively healthy proximal pedicle for debulking multiple toes macrodactyly in children could completely excise distal hypertrophic tissues in a single operation, and the size of the affected feet and toes can be greatly reduced to wearing shoes of the same size, and have a high success rate in preserving the toes, as well as nails. The reconstructed feet, toes and toe webbing have near-normal morphology, hidden scar, no ulcers after walked and achieve a good initial effect.
9.Role of palmitoyltransferase modified NOD2 in brain injury after cardiopulmonary resuscitation in mice

Objective To investigate the role of nucleotide-binding oligomerization domain 2(NOD2)modified by palmitoyltransferase(ZDHHC5)in brain injury after cardiopulmonary resuscitation(CPR)in mice.Methods Twenty-four male C57BL/6J mice were divided into the blank group,the control group,the ZDHHC5-si group and the NOD2-si group,with 6 mice in each group.Except for the blank group without any treatment,CPR modeling was performed in the other three groups.At 24 h before CPR,mice in the ZDHHC5-si group and the NOD2-si group were injected with ZDHHC5 siRNA and NOD2 siRNA via tail vein,respectively.The modified neurological deficit scale(mNSS)was used to evaluate the neurological function at 24 h,48 h and 72 h in each group.Blood samples and brain tissue were collected 72 h after modeling.Real-time fluorescent quantitative PCR(qPCR)was used to detect ZDHHC5 and NOD2 in brain tissue.The protein expression levels of IL-1β,TNF-α and IL-6 in plasma were detected by enzyme-linked immunosorbent assay(ELISA).Colorimetric method and thiobarbituric acid(TBA)method were used to detect protein expression levels of MDA and MPO in brain tissue,respectively.Western blot assay was used to detect expression levels of Cleaved Caspase-3,ZDHHC5 and NOD2 in brain tissue.HE pathological sections of brain tissue were observed under light microscope.The pathological sections of brain tissue were observed by TUNEL under fluorescence microscope.Results Compared with the blank group,the mNSS score,the expression levels of IL-1β,TNF-α,IL-6,MDA and MPO,and the protein expression levels of Cleaved Caspase-3,ZDHHC5 and NOD2 were significantly increased(P<0.05),and brain tissue damage and cell apoptosis were aggravated in the other three groups.Compared with the control group,the above indicators were significantly decreased in the ZDHHC5-si group and the NOD2-si group(P<0.05),and brain tissue damage and cell apoptosis were significantly attenuated.Compared with the NOD2-si group,the above parameters were significantly decreased(P<0.05),and brain tissue damage and cell apoptosis were further attenuated in the ZDHHC5-si group.Conclusion In the mouse CPR model,NOD2 can produce palmitoylated NOD2 after regulated by ZDHHC5,which further promotes the release of inflammatory factors and causes neuronal apoptosis,ultimately damaging brain tissue and affecting neurological function.
10.Relationship between serum cTnⅠ level and ECG QRS wave,and between serum CK-MB level and ECG QRS wave in elderly patients with chronic heart failure

Objective:To analyze the correlation between serum cardiac troponin Ⅰ(cTnⅠ)and electrocardiogram(ECG)QRS wave,and between creatine kinase isoenzyme MB(CK-MB)and ECG QRS wave in elderly patients with chronic heart failure(CHF).Methods:A total of 100 elderly CHF patients who admitted to Lu'an Hospital of Traditional Chinese Medicine of Anhui Province from September 2020 to October 2023 were selected as the observation group,and a total of 100 elderly healthy patients who underwent physical examination during the same period were selected as the control group.All of them received examinations of serum cTnⅠ,CK-MB and ECG.The time limit of QRS wave,serum cTnⅠ and CK-MB levels between control group and observation group were compared.The patients of the observation group were further divided into two subgroups based on whether occurred abnormal time limit of QRS wave,which included abnormal QRS wave group(52 cases,time limit of QRS wave ≥120 ms)and normal QRS wave group(48 cases,time limit of QRS wave<120 ms).The general data,serum cTnⅠ and CK-MB levels between the two groups were compared,and the correlations between serum cTnⅠ level and ECG QRS wave,and between CK-MB level and ECG QRS wave in elderly CHF patients were analyzed.Results:Compared with serum cTnⅠ(0.861±0.173),CK-MB(10.573±2.332)and time limit of QRS wave(104.475±13.351)of control group,the serum cTnⅠ(1.912±0.401)and CK-MB(23.272±4.801)of observation group were higher,and the time limit of QRS wave(126.761±17.452)was longer,and the differences of them between two groups were statistically significant(t=24.159,23.802,10.145,P<0.05),respectively.The ECG examination of observation group found that 52 patients occurred abnormal time limit of ECG QRS wave,and the average time limit of QRS wave of them was(141.641±12.581)ms,and 48 patients had normal time limit of ECG QRS wave,and the average time limit of QRS wave of them was(110.652±10.751)ms.The serum cTnⅠ and CK-MB levels of patients in abnormal time limit of QRS wave were higher than these of patients in normal time limit of QRS wave,and the difference was statistically significant(t=7.534,10.362,P<0.05),respectively.The result of general linear bivariate Pearson-test indicated that serum cTnⅠ and CK-MB were positively correlated with time limit of QRS wave(r=0.395,0.453,P<0.05),respectively.The result of logistic regression analysis indicated that serum cTnⅠ and CK-MB levels were related to the time limit of ECG QRS wave,which overexpression might be risk factor for abnormal time limit of ECG QRS wave(OR=4.444,1,445,P<0.05).Conclusion:The serum cTnⅠ and CK-MB levels of elderly CHF patients are abnormally high,and the overexpression of the two indicators is related to abnormal time limit of ECG QRS wave,which might be risk factor of abnormal time limit of QRS wave.
