1.The influence of renally inappropriate medication on the frailty of elderly patients with diabetes

OBJECTIVE To investigate the effects of renally inappropriate medication (RIM) on the frailty of elderly patients with diabetes. METHODS The data of elderly patients with diabetes mellitus admitted to a third-grade class A hospital in Yunnan province from January to December 2022 were collected, and Beers criteria (2019 edition) and Chinese version of FRAIL scale were used to evaluate RIM and the frailty of the patients; the patients were divided into the trial group (with RIM) and the control group (without RIM) according to whether there was RIM. The propensity score matching was used to balance confounding factors between two groups, and the influence of RIM on the frailty of elderly diabetic patients was analyzed by the Logistic regression model. RESULTS Among the 367 patients, 80 patients (21.80%) had RIM, the drugs involved RIM were spironolactone (82.56%), rivaroxaban (13.95%) and gabapentin (3.49%). After reaching the balance between groups using the propensity score matching method, the incidence of frailty was 77.94% in trial group and 27.94% in control group (P<0.001); the difference was not statistically significant in other confounding factors between the two groups (P>0.05). Results of Logistic regression analysis showed that the risk of frailty in the experimental group was 3.118 times that of the control group (odds ratio was 3.118,95% confidence interval was 1.758-5.530, P<0.001). CONCLUSIONS RIM is a risk factor for the frailty of elderly patients with diabetes, which can be considered as an indicator for early identification and screening of the frailty of elderly diabetes patients.

3.Role of gut microbiota in the pathogenesis and treatment of chronic kidney disease

Chronic kidney disease (CKD) is a serious health problem worldwide, whereas there is still no efficient cure. The gut microbiota plays a crucial role in maintaining human health and disease resistance, and multiple studies have confirmed that the gut microbiota is closely related to the occurrence and development of CKD. Starting from the "gut-kidney axis" theory, this article provides a systematic review of the changes in gut microbiota composition and function in patients with CKD, such as a decrease in the abundance of butyrate-producing bacteria Roseburia and Faecalibacterium prausnitzii. Besides that, the article explores the mechanisms by which the gut microbiota affects CKD progression, such as inflammation and immunity, and also describes the application methods of using the gut microbiota as a therapeutic target for CKD, such as fecal microbiota transplantation, microecologics, and dietary therapy, in order to provide microbial- based targets for the clinical diagnosis and treatment of CKD.
4.Expression of hsa_circ_0001459 in peripheral blood mononuclear cells of patients with Epstein-Barr virus infection and its clinical significance

Objective:To detect the expression of has_circ_0001459 in peripheral blood mononuclear cells (PBMCs) of Epstein-Barr virus (EBV)-infected individuals and evaluate its diagnostic value for EBV infection.Methods:The expression profiles of circRNAs in PBMCs of patients with EBV infection were obtained by whole-transcriptome sequencing. Differentially expressed circRNAs with statistical significance were selected based on the criteria of ∣log 2Fold Change∣≥1 and P<0.05. Based on the composition, length and primer specificity, hsa_circ_0001459 with high expression was selected for further research. The relative expression of hsa_circ_0001459 in PBMCs of 60 patients with EBV infection and 45 healthy people was detected by real-time fluorescent quantitative PCR. Receiver operating characteristic (ROC) curve and Kappa analysis were used to analyze the diagnostic value of hsa_circ_0001459 expression for EBV infection. Results:The expression of hsa_circ_0001459 showed an up-regulation trend in patients with EBV infection as compared with that in the healthy people, and the results were consistent with the sequencing results. The area under the ROC curve for screening EBV-infected individuals was 0.83(95%CI: 0.75-0.91) with the sensitivity of 0.80(95%CI: 0.66-0.89) and the specificity of 0.77(95%CI: 0.65-0.86). Kappa analysis indicated that hsa_circ_0001459 was moderately consistent with EBV DNA in the diagnosis of EBV infection (κ=0.51).Conclusions:Hsa_circ_0001459 tended to be highly expressed in PBMCs of EBV-infected individuals, which might be uses as a diagnostic marker for EBV infection.

Objective To explore the effect and mechanism of miR-581 on the autophagy of ovarian cancer SKOV3 cells. Methods miR-581 mimics and miR-581 NC were transfected into SKOV3 cells, and the transfection efficiency was detected by qRT-PCR. After successful transfection, Western blot was used to detect autophagy-related proteins expression in SKOV3 cells. Objective:To translate the Efficacy of Self-Efficacy for Managing Symptoms (SEMSX) based on the Patient-Reported Outcomes Measurement Information System (PROMIS) into Chinese, and to test the reliability and validity of the scale among Chinese patients with nasopharyngeal carcinoma.Methods:The English version of PROMIS-SEMSX was translated into Chinese according to the FACIT translation method. The reliability and validity of the scale were evaluated. From January to April in 2021, the convenient sampling method was adopted to select 205 patients with nasopharyngeal cancer in Zhejiang Cancer Hospital for investigation.Results:The Chinese version of PROMIS-SEMSX scale consisted of 28 items in 4 dimensions, which were consistent with confidence level(with 7 items), symptom management(with 5 items), the impact of symptoms on life(9 items), seeking and understanding support(7 items). The calibration correlation validity was 0.935. The total Cronbach α coefficient of the scale was 0.966, and the half-reliability of the total scale was 0.891. The Cronbach α coefficient of each dimension was 0.910-0.938, and the total retest reliability coefficient of the scale was 0.757.Conclusions:The Chinese version of PROMIS-SEMSX has good reliability and validity in patients with nasopharyngeal carcinoma, and provides a new tool for patients with nasopharyngeal carcinoma to evaluating self-efficacy symptom management.
7.Research progress of scan-associated anxiety in tumor patients

8.Status quo and influencing factors of symptom management self-efficacy based on patient reported outcomes in nasopharyngeal carcinoma patients

Objective:To explore the status quo of symptom management self-efficacy based on patient reported outcomes of nasopharyngeal carcinoma patients and analyze its related factors.Methods:From January to April 2021, 230 patients with nasopharyngeal carcinoma who were treated in Zhejiang Cancer Hospital were selected as the study subject by convenient sampling. The General Information Questionnaire and Patient-Reported Outcome Measurement Information System-Self-Efficacy for Managing Symptoms (PROMIS-SEMSX) were used to investigate. Single factor analysis and multiple linear regression analysis were used to explore the influencing factors of symptom management self-efficacy in patients with nasopharyngeal carcinoma.Results:A total of 230 questionnaires were distributed, 220 were recovered, and 205 were valid. The score of symptom management self-efficacy of patients with nasopharyngeal carcinoma was (97.74±20.00) , which was above the middle level, and the lowest score dimension was confidence, which was (23.52±5.91) . Multiple linear regression showed that gender, education level, medical expense payment method, economic level, tumor stage, course of disease, tumor recurrence and metastasis were the influencing factors of symptom management self-efficacy of nasopharyngeal carcinoma patients ( P<0.05) . A total of 41.5% of the total variation was explained. Conclusions:The self-efficacy of symptom management in patients with nasopharyngeal carcinoma needs to be improved. The medical and nursing staff should focus on the patients with low educational level, heavy economic burden, late tumor, recurrence and metastasis, and focus on providing specific health guidance to alleviate their discomfort symptoms during treatment, and improve their self-efficacy of symptom management.
9.Results of non-invasive prenatal testing for 2473 women with twin pregnancy.

OBJECTIVE:
To assess the value of non-invasive prenatal testing (NIPT) for the detection of fetal chromosomal aneuploidies in women with twin pregnancy.
METHODS:
A total of 2473 women with twin pregnancy underwent the NIPT test to assess the risk for fetal chromosomal aneuploidies from January 2016 to September 2019. Those with a high risk by NIPT were confirmed by amniocentesis or chorionic villus sampling. All cases were followed up to evaluate the positive prediction value of NIPT for twin pregnancies.
RESULTS:
Among the 2473 women, the NIPT test has identified 31 cases (1.25%) with a high risk for fetal chromosomal aneuploidies, which included 5 cases of trisomy 21, 1 case of chromosome 21 deletion, 4 cases of trisomy 18, 7 cases of sex chromosome abnormality and 14 cases of microdeletion and microduplication. By invasive prenatal diagnosis or chromosomal karyotyping analysis of neonates, 5 cases of trisomy 21, 3 cases of trisomy 18, 1 case of sex chromosome abnormality, and 2 cases of microdeletion and microduplication were confirmed, which yielded a positive predictive value of 100%, 75%, 25% and 25%, respectively.
CONCLUSION
NIPT can be used for the screening of fetal chromosomal aneuploidies in women with twin pregnancy with high accuracy. The method is non-invasive, safe and effective for the screening of fetal chromosomal aneuploidies, in particular trisomy 21.

10.Is it necessary to choose NIPT-plus for pregnant women who opt for non-invasive prenatal testing? A study of 50 cases.

OBJECTIVE:
To explore whether it is necessary to choose NIPT-plus for the prenatal screening of pregnant women.
METHODS:
The results of NIPT and NIPT-plus sequencing data, fetal DNA concentration, prenatal diagnosis and pregnancy outcome of 50 pregnant women were compared.
RESULTS:
Compared with NIPT, NIPT-plus attained similar fetal DNA concentration and a 4.4-fold increase in sequencing data. NIPT was able to detect 4 cases of 21-trisomy, 2 cases of 18-trisomy, and 9 cases of sex chromosome aneuploidies (SCAs) signaled by NIPT-plus, but missed one 18-trisomy, and failed to detect rare chromosome aneuploidies (RCAs) and microdeletion/microduplication syndromes (MMS). The PPVs of NIPT-plus for 21-trisomy, 18-trisomy, SCAs, MMS and RCAs were 100%, 100%, 44.4%, 30.4% and 0%, respectively. And those of NIPT for 21-trisomy, 18-trisomy, and SCAs were 100%, 100%, and 44.4%, respectively.
CONCLUSION
It is necessary for pregnant women to select NIPT-plus to improve the detection rate of common trisomies, SCAs and disease-specific MMS, therefore reduce the occurrene of birth defect.
