1.Morita therapy for misophonia: a case report

Misophonia is a psychophysiological and behavioral disorder characterized by an individual's low tolerance to specific sounds, leading to intense negative emotional experiences and physiological responses. Currently, there is no standardized and universally effective treatment for misophonia in clinical practice worldwide. This article reports the case of an 18-year-old male patient with misophonia who showed poor response to sertraline combined with exposure and response prevention therapy. Subsequently, the patient received 8 weeks of Morita therapy (once a week, 50 minutes per session), with symptomatic improvement. By presenting this case, we explore the potential efficacy of Morita therapy in treating misophonia, aiming to provide a reference for its clinical management. 2.Establishment and of preliminary verification of automatic auditing rules for routine coagulation assays

Objective:To establish auto verification rules for the routine coagulation assays,and to provide reference for clinical laboratories to improve the quality and efficiency of results verification.Methods:A total of 24,510 specimens of sodium citrate anticoagulation routine coagulation test from the laboratory departments of eight hospitals including the First Medical Center,Chinese PLA General Hospital during January to March 2020 were collected and randomly divided into a rule establishment group and a rule verification group,with 6,670 specimens in the rule establishment group,including 2,056 Delta checks,and 17,840 specimens in the rule validation group,including 3,210 Delta checks.The activities of prothrombin time(PT),activated partial thromboplastin time(APTT),fibrinogen(Fib),thrombin time(TT),D-dimer(DD)and/or antithrombin(AT)were detected by Stago STA R Max automatic coagulation analyzer and supporting reagents.Taking the manual verification results as the standard,the auto verification and manual false negative rate(invalid verification),false positive rate(invalid interception),pass rate,positive coincidence rate,negative coincidence rate,verification consistency rate and specimen turnaround time(TAT)of the two groups were calculated.Results:The auto verification rules and the application process were preliminarily established,including internal quality control,alarm information,auto verification scope,critical value and deviation value inspection.In the rule establishment group,the single item pass rate was 82.6%-92.4%,and the overall pass rate was 73.8%.The consistency rate between auto verification and manual verification was 98.2%,and the positive coincidence rate and negative coincidence rate were 24.4%and 73.8%,respectively.In the rule verification group,the single item pass rate was 86.4%-91.5%,and the overall review pass rate was 71.5%.By simulating the application of auto verification rules,the average TAT of two hospitals among the eight hospitals was shortened by 1.5 hours and 2.1 hours,respectively.Conclusion:The application of auto verification rules can reduce workload of manual verification,and significantly shorten the TAT,and improve the report efficiency of the laboratory.
3.Three-dimensional finite element analysis of distal proximal occlusal defect of short crown molar restored with endocrown

BACKGROUND:For teeth with normal dental crown height,pulp cavity retention crown restoration with different depths of the pulp cavity and different repair materials affects the stress and flexural strength of tooth tissue.For short crown molar defects,the research on pulp cavity repair mainly focuses on clinical observation and in vitro flexural strength experiments. OBJECTIVE:To establish a three-dimensional finite element model for short crown molar restored by the endocrown after root canal treatment to analyze the effects of different pulp cavity retention depths and different repair materials on the distribution and size of dentin equivalent stress. METHODS:Based on establishing the complete model of the short crown mandible first molar,a three-dimensional finite element model was established for repairing the distal adjacent defect of the short crown molar with different pulp cavity retention depths(h=2,3,4 mm)and different repair materials(zirconia,lithium disilicate).Under the oblique loading,the equivalent stress distribution was observed.The peak value of dentin equivalent stress and the mean value of equivalent stress near the bottom of the mesial pulp cavity wall were calculated. RESULTS AND CONCLUSION:(1)Equivalent stress concentration areas:The stress of complete short crown molar and restored models mainly concentrated in the mesial root mesial neck and mesial root lingual neck.The stress concentration area was found in the mesial pulp cavity wall corresponding to the bottom layer of restored models,and the stress concentration was obvious in the 4 mm retention depth group.(2)Under the same repair material,the peak value of dentin equivalent stress was the lowest at 3 mm for all models after repair.The average value of equivalent stress near the bottom of the mesial pulp cavity wall was lowest at 3 mm.(3)Under the same retention depth,there was no significant difference between the two materials in the dentin equivalent stress peak and the mean value near the bottom of the mesial pulp cavity.(4)The results showed that under the conditions of this experiment,the endocrown was used to repair the defect of the short crown molar and the retention depth was 3 mm,which was more beneficial to protect the remaining dental tissue.The selection of zirconia or lithium disilicate as the repair material had little effect on the dentin stress.
4.Epidemiological analysis of influenza in Jiading District, Shanghai, 2013‒2023

ObjectiveTo analyze the epidemiological characteristics and trends of influenza‑like illness (ILI) in Jiading District from 2013 to 2023, to explore the prevalence patterns, so as to provide a scientific basis for a better prevention and control of influenza. MethodsData of ILI cases, pathogenetic surveillance results and pathogen detection data from influenza surveillance network laboratories in Jiading District from 2013 to 2023 were collected for statistical analysis. ResultsFrom 2013 to 2023, the overall proportion of medical visits for ILI cases in Jiading District was 2.70%. ILI cases were mainly distributed in the age group of ≥25 years, with a seasonal prevalence characteristic, mainly concentrated in the winter and spring, with an occasional small peak in the summer. A total of 12 423 specimens were tested for pathogenetic surveillance and monitoring, 3 651 of which were tested positive for nucleic acid detection, with a positive rate of 29.39%. The dominant strain was influenza A (H3N2) virus, accounting for 55.05%. There were statistical significant differences in the positive detection rates by different streets and townships(χ2=24.73,P<0.05). The influenza network laboratory isolated and cultured influenza viruses from 3 154 nucleic acid‑positive samples sent by national influenza sentinel surveillance hospitals using 2 methodsMadin‑Darby canine kidney (MDCK) cells and specific pathogen free (SPF) chicken embryos, respectively, with a total isolation rate of 39.51%, and the isolation rate of MDCK cells (35.45%) was higher than that of SPF chicken embryos (5.36%). From 2013 to 2023, a total of 14 ILI outbreaks were reported in Jiading District, mainly in primary schools, kindergartens and secondary schools. ConclusionInfluenza in Jiading District, Shanghai, is mainly prevalent in the winter and spring, with different subtypes of influenza viruses alternating or co⁃circulating in different monitoring year. It is recommended to promote influenza vaccination, further improve influenza prevention and control measures, enhance health promotion to the age group of ≥25 years in winter and spring, and strengthen surveillance and monitoring on ILI outbreaks in collective units such as primary schools and childcare institutions, so as to reduce the disease burden.
5.Task-oriented nursing information system upgrading

Objective This study aims to establish a stable and efficient nursing information system in the context of smart hospital development.Methods Leveraging the hospital's health information exchange(HIE)platform,we optimized data collection strategies.Focusing on"patient's admission,transfer,and discharge",we analyzed nursing tasks at each link and created a comprehensive management task list based on the timeline.Key nursing steps incorporated intelligent assistive reminders and decision-making functions.The upgraded system was evaluated by comparing pre-and post-upgrade metrics:structured rates of documentation,daily time spent on the document entry,timeliness of rounds,rate of timely medication administration,and nursing satisfaction.Results The nursing documentation became highly structured and configurable,with daily documentation time reduced by approximately 1.16 hours.There were significant improvements in the timeliness of rounds and the rate of timely medication administration,alongside high nursing satisfaction.Conclusion The task-oriented nursing pattern can enhance nurs-ing efficiency and safety,facilitating a shift in the nursing model.
6.Transcriptome analysis of baicalin interfering with IBV replication in CEK cells in vitro

In order to understand the difference and expression of genes in CEK cells before and af-ter baicalin interferes with IBV,further reveal and analyze the mechanism of baicalin interfering IBV replication in CEK cells in vitro.After IBV infected CEK cells for 2 h,9.75 mg/L baicalin liq-uid was added to interfere with CEK cells,which was recorded as the baicalin(H-IBV)group,and three replicate wells were set in the control group and the IBV(IBV)infection group.After 36 h culture,cell samples were collected and subject to transcriptome for sequencing.The results showed that there were 102 differentially expressed genes in H-IBV group compared with IBV in-fection group,among which 48 genes weresignificantly up-regulated and 54 genes were significantly down-regulated.Through functional annotation in GO and KEGG databases,it was found that dif-ferentially expressed genes were mainly annotated in biological processes such as cellular proces-ses,biological regulation,metabolism,and secondary pathways such as viral infectious diseases,signal transduction and interaction.Retinol metabolism pathway,phospholipid transfer to mem-brane,IL-27 mediated signaling pathway,MDA5/RIG-I and Toll-like receptor signaling pathway were significantly enriched in CEK cells,and the production of type Ⅰ interferon and interferon al-pha and the process of antiviral infection were also positively regulated.There were more differen-tial genes enriched in nucleic acid catalysis,immune system,and reaction,and interbiological reac-tion.Through the STRING network interaction map,it was found that most immune-related genes could form a 36-node interaction network centered on IRF7,TLR3 and STAT1.Therefore,com-pared with IBV group,the differentially expressed genes after baicalin treatment were mainly an-notated and enriched in the biological process,and the immune system and response were en-hanced,mainly through the positive regulation of IRF7 in the MDA5/RIG-I receptor signaling pathway and the inhibition of TLR3 signal transduction in the Toll-like receptor signaling path-way.Positive regulation of IL-27 mediated pathway and regulation of JAK-STAT signaling path-way were supplemented by activation of the expression of IRF7,TLR3,STAT1 and other related genes,and interaction with corresponding downstream proteins to promote the expression of IFN-α and regulatory cytokines,coupled with negative regulation of viral(defense)response and viral processes.Thus,baicalin interferes with IBV replication in CEK cells.
7.Efficacy of layered imbricate reset of orbital septum tissue through palpebral margin incision combined with thermal compression in lower eyelid rejuvenation

Objective:To explore the clinical effect of rejuvenation of lower eyelid through orbital septum fat and orbital repositioning combined with thermal compression.Methods:A total of 423 patients who underwent skin approach blepharoplasty with skin laxity accompanied by different degrees of lacrimal groove and palpebromalar groove deformity admitted to the Plastic Surgery Department of Hemangioma and Vascular Malformation, the Plastic Surgery Hospital of Chinese Academy of Medical Sciences, from April 2020 to January 2023 were selected, including 403 females and 20 males, aged 33-68 (45.0±11.1) years. During the operation, the combined subcutaneous and suborbicularis oculi approaches were used to separate the prezygomatic space and premaxillary space. In the space, the orbital septum fat flap was fixed with the orbital septum flap under the infraorbital margin by lmbricate suture, and the orbital septum was tightened with energy hemostasis device to reshape and strengthen the anatomical structure of the lower eyelid. The patients were followed up to evaluate the incision healing and complications after surgery.Results:All the 423 patients were followed up for 1-48 months. The patients were healed in one stage, without lower eyelid retraction, lower eyelid ectropion and other complications. The incision of the lower eyelid was concealed and smooth, and the depression of lacrimalgroove and palpebromalar groove and lower eyelid relaxation were improved significantly.Conclusions:For patients with lower eyelid bag skin laxity and different degrees of lacrimal groove and palpebromalar groove depression, the surgical design is reasonable, and the structure of orbital septum fat and orbital septum is reset, and the effect is reliable.
8.Feasibility of low tube voltage scanning technology in CT-guided cryoablation therapy of liver cancer

Objective To observe the feasibility of low tube voltage scanning technology in CT-guided cryoablation therapy of liver cancer.Methods Twenty-one patients who underwent routine CT(tube voltage 120 kV)-guided cryoablation of liver cancer(group A)were retrospectively enrolled,while low tube voltage(100 kV)CT-guided cryoablation of liver cancer was prospectively performed in 23 patients(group B).CT scanning were performed before and after ablation with automatic tube current mode(50-450 mA),and adaptive statistical iterative reconstruction(ASIR)algorithm was used for image reconstruction(reconstruction percentages of 40%)of both groups.The subjective scores and objective evaluations of CT image quality before and after treatment were compared between groups,so as the treatment effect and patient's radiation dose(volume CT dose index[CTDIvol]).Results No significant difference of subjective scores,objective evaluations of CT image quality nor treatment effect was detected between groups(all P>0.05).Before and after treatment,CTDIvol of patients in group A was significantly higher than that in group B(btoh P<0.001).Conclusion Low tube voltage scanning technology could be used for CT-guided cryoablation of liver cancer,which might ensure image quality without affecting evaluation on cryoablation effect and reduce radiation dose combined with ASIR algorithm.
9.Cross-face nerve grafting followed by free functional gracilis muscle transplantation in patients with long-standing facial paralysis

Objective:To investigate the clinical effect of stage treatment for long-standing facial paralysis with free functional gracilis transfer innervated by cross-face nerve grafting(CFNG).Methods:A retrospective study was carried out focusing on the case data of patients with long-standing facial paralysis who were treated by the free functional gracilis transfer innervated by CFNG in the Second Department of Plastic Surgery Hospital of the Chinese Academy of Medical Sciences from January 2014 to December 2020. CFNG was performed in the first stage approach and free functional gracilis transfer in the second stage approach. Auther utilized the modified House-Brackmann grading scale to record the patients’ lip angle movements postoperatively and measured the static and dynamic angle formed between the horizontal line of both lip corners and the vertical midline before and after surgery objectively. Paired t-test was utilized to evaluate the bilateral middle and lower facial symmetry changes, so as to estimate the surgical effect. The significance level was set at 0.05. Results:A total of 26 patients with long-standing facial paralysis were enrolled in this study, including 10 males and 16 females, aged from 16 to 28 years, with an average age of (24.8±3.5) years. The interval of staging operation was 6 to 12 months. The postoperative follow-up was 1-3 years, with an average of (34.9±11.7) months. All grafts survived well. No major complication occurred. 2 patients underwent gracilis muscle debulking due to the paralysis side facial bulking one year after the second stage operation. 1 patient underwent muscle fixation position adjustment due to the poor position of gracilis muscle fixation one year after the second stage operation. 1 patient underwent masseter nerve-CFNG end-to-side anastomosis 2 years after the second stage operation due to insufficient movement of the mouth. The facial symmetry of all patients was improved after the two-staged operations. The movement of lip corners was classified as excellent in 15 cases, good in 9 cases, and fair in 2 cases in terms of the modified House-Brackmann grading scale. According to the objective evaluation of oral angle movement, the static lip angle after surgery was higher than that before surgery (86.4°±3.3° vs. 82.3°±2.7°) and the difference was statistically significant ( t=5.27, P=0.017); the dynamic lip angle after surgery was higher than that before surgery (86.9°±2.8° vs. 80.1°±3.3°), and the difference was statistically significant ( t=7.55, P<0.001). Conclusion:Free functional gracilis transfer innervated by CFNG is an ideal technique for long-standing facial paralysis. It can effectively improve the facial dynamic of the affected side.
10.The application of the "cigarette core" product for oral mucosa roll in hypospadias patients

Objective:To evaluate the clinical application of the new "cigarette core" product for oral mucosa roll in hypospadias patients.Methods:From December 2018 to January 2022, patients who visited the Plastic Surgery Hospital, Chinese Academy of Medical Sciences and Peking Union Medical College were selected and underwent staged surgical repair. In the first stage, we used the oral mucosa to form the urethra. The length of the oral mucosa was 5.0-6.0 cm and the width was 1.0-1.2 cm. In the control group, we use the handmade gauze roll; in the experimental group, we use the new "cigarette core" product. In the second-stage operation, the urethra was anastomosed. The operation time was recorded, and the mucosal survival, complications, urination and patients’ satisfaction with the operative effect were followed up. The measurement data of normal distribution were expressed as Mean±SD and analyzed by t-test. Results:A total of 107 male patients were included, including 53 in the control group and 54 in the experimental group, aged from 1 to 28 years, with an average age of 8.5 years. The first-stage operation time of control group was significantly longer than that of experimental group [(61.5±11.5) min vs. (43.8±15.2) min], and the difference was statistically significant ( P<0.01). The patients were followed up for 3 to 20 months. In the first-stage operation, the survival rate of oral mucosa was 98.11%(52/53) in the control group and 100%(54/54) in the experimental group. There were 4 cases of urinary fistula in the control group after the second-stage operation, and the complication rate was 7.55%. There was 1 case of urinary fistula in the experimental group, and the complication rate was 1.85%. All patients had good urination after operation. In the satisfaction survey one year after surgery, the satisfaction rate of the control group was 94.34% (50/53) and that of the experimental group was 100% (54/54). Conclusion:The patented "cigarette core" product is good in consistency. In hypospadias repair, it has a high survival rate of mucosal transplantation and a low incidence of postoperative complications. It can effectively reduce operation time and anesthesia time.
