1.Factors affecting home-based exercise rehabilitation for chronic kidney disease patients: a qualitative analysis

ObjectiveTo investigate the facilitators and barriers of home-based exercise rehabilitation for patients with chronic kidney disease (CKD). MethodsFrom December, 2023 to March, 2024, 15 CKD patients receiving treatment at the Nephrology Department of Beijing Bo'ai Hospital and undergoing home-based rehabilitation were selected, as well as eight healthcare professionals from the same department. Semi-structured interviews were conducted, and the data were coded and analyzed using Nvivo 12.0. ResultsTwo main themes were identified. The facilitators included perceived benefits, good health literacy, potential benefits of online media, supervision by healthcare professionals and adequate social support. The barriers included severe negative emotions, adverse effects of complications, insufficient emphasis on exercise rehabilitation, lack of motivation for exercise, constraints related to time and location for exercise, and insufficient rehabilitation support. ConclusionThere are vavious and individual factors influencing home-based exercise rehabilitation for CKD patients, which need an individualized exercise rehabilitation guidance to improve the adherence to self-management.
2.Molecular epidemiological investigations of human parvovirus B19 among blood donors in Lanzhou

[Objective] To investigate the molecular prevalence and genotype of human parvovirus B19(B19) among blood donors in Lanzhou, and provide data support for monitoring the positive rate of B19 DNA in local blood donors. [Methods] A total of 7 644 blood donor samples collected from January to September 2022 were randomly screened using real-time fluorescent PCR, resulting in 23 samples testing positive for B19 DNA. The characteristics of the B19 DNA reactive donors including gender, age, blood donation recruitment and promotion mode, and donation frequency were analyzed using SPSS 22.0. Additionally, the VP1 gene fragment of B19 DNA reactive samples was sequenced and an evolutionary tree was constructed by the N-J method. [Results] The results showed that the positive rate of B19 DNA in Lanzhou was 0.30%, and the positive population mainly consisted of female individuals aged 18-30 years old who were first-time blood donors; furthermore, genotype 1a was identified as predominant. [Conclusion] The positive rate of B19 DNA is low among blood donors in Lanzhou, with genotype 1a being predominant. It is recommended to periodically monitor the B19 prevalence in blood donors and enhance prevention and control measures, thus improving blood quality and safety.
3.Research progress on the differential effects of novel anti-tumor drugs on pulmonary hypertension

Pulmonary hypertension (PH) is a severe and rare chronic cardiopulmonary disorder for which existing vasodilator therapies can only alleviate symptoms, rather than target or fundamentally reverse the disease. Additionally, the long-term prognosis remains poor. Recent studies have found that some novel anti-tumor drugs (NADs) can relieve PH, such as imatinib, gefitinib, sorafenib, olaparib, piperacillin, everolimus, rituximab, etc. However, some NADs can induce PH or exacerbate its symptoms, including dasatinib, lorlatinib, carfilzomib, bevacizumab, trastuzumab, nivolumab, etc. The effects of lapatinib, ruxolitinib, and bortezomib on PAH are controversial. Individualized medication should be adopted in clinical practice when using NADs for treatment, with close monitoring being essential.
4.Acupoint selection patterns for epilepsy in ancient texts based on visual network analysis.

OBJECTIVE:
To analyze the disease patterns and acupoint selection characteristics of acupuncture for epilepsy in ancient acupuncture texts, providing references and ideas for clinical acupuncture treatment of epilepsy.
METHODS:
Texts from the Chinese Medical Classics (5th edition) regarding acupuncture for epilepsy are reviewed. The frequency of acupoints, meridian association, distribution, specific points, corresponding epilepsy subtypes, and needling techniques are statistically analyzed. The Apriori algorithm is used for association rule analysis, and a complex network analysis is conducted for high-frequency acupoints and their corresponding subtypes and treatments.
RESULTS:
A total of 205 acupuncture prescriptions are identified. Ancient texts favored differentiation-based treatments for epilepsy, primarily classified into epilepsy, wind epilepsy, and five epilepsy. Commonly used acupoints include Baihui (GV20), Jiuwei (CV15), Shenmen (HT7), Shenting (GV24), and Xinshu (BL15), with a focus on the acupoints of the governor vessel, the bladder meridian, and the conception vessel. The acupoints on the head, face are combined with the acupoints on the limbs, with skillful use of the five-shu points and intersection acupoints. The most frequent combinations are Shenmen (HT7)-Baihui (GV20), Shenting (GV24)-Baihui (GV20), and Xinshu (BL15)-Shenmen (HT7). Visual network analysis revealed that Baihui (GV20)-Shenting (GV24), Baihui (GV20)-Shenmen (HT7), and Baihui (GV20)-Zhaohai (KI6) are core acupoint combinations. Treatment mainly involved moxibustion or combined acupuncture and moxibustion.
CONCLUSION
The acupoint selection for epilepsy treatment in ancient texts is precise, frequently using Baihui (GV20), Jiuwei (CV15), Shenmen (HT7), Shenting (GV24), and Xinshu (BL15), etc., with emphasis on calming epilepsy, awakening the spirit, relaxing tendons, and nourishing the heart.

Olfactory disorders are a common symptom in patients with chronic rhinosinusitis, and their diagnosis and treatment have garnered extensive attention from both patients and doctors. Currently, there are various evaluation and treatment methods for olfactory dysfunction; however, choosing a simpler and more accurate assessment, as well as an effective treatment, remains a clinical challenge. In this article, we review the assessment and treatment methods commonly used in clinical practice in recent years to provide better support for the diagnosis and treatment of olfactory disorders.

8.Analysis of HPV vaccine-related KAP and its influencing factors among resident women aged 16-45 years in Pudong New Area

Objective To analyze the related knowledge-attitude-practice(KAP)and the influencing factors of human papilloma virus(HPV)vaccine among 16 — 45 year old resident women in Pudong New Area.Methods Six of the 36streets(towns)in Pudong New Area were randomly selected as the survey site,the resident women aged 16 — 45 years were randomly selected excluding those with reading or comprehension disabilities and those with mental disorders,and a total of 1 022 valid questionnaires were collected through self-filling questionnaires to understand the KAP status of the HPV vaccine among the resident women in Pudong New Area. The relationship among KAP of HPV vaccine and its influencing factors were analyzed by univariate analysis and structural equation model.Results The overall awareness rate of HPV vaccine among 16-45 year old resident women in Pudong area was higher. Univariate analysis showed that marital status,educational level,employment status and household annual income were related to the awareness level of HPV vaccine(χ~2=12. 832,17. 636,16. 770 and 20. 030,respectively,each P < 0. 05);Age,marital status,employment status and children′s status were correlated with HPV vaccination level(χ~2= 12. 382,25. 777,8. 830 and 20. 138,respectively,each P <0. 05);HPV vaccine health education,HPV and HPV vaccine knowledge scores affected HPV vaccination status(χ~2=97. 561 and 68. 969,respectively,P < 0. 001);Subjects' knowledge of cervical cancer was positively affected by knowledge of HPV infection(γ_(11)= 0. 756,P < 0. 001). Knowledge of cervical cancer not only positively affected subjects′ attitudes towards the efficacy of HPV vaccine(β_(21)= 0. 557,P < 0. 001),also had a direct effect on the HPV vaccination behavior of the subjects,showing a promoting effect(β_(31)= 0. 274,P = 0. 004). Subjects′ approval of the efficacy of HPV vaccine had a positive effect on their actual vaccination behavior(β_(32)= 0. 175,P = 0. 016).Conclusion The willingness of the 16 — 45 year old resident women in Pudong New Area to inoculate HPV vaccine was positive,but the actual vaccination rate was low. It is suggested to strengthen the HPV vaccine publicity while strengthening the education of cervical cancer and HPV infection,and consider the necessity of including the suitable age males in the scope of human vaccination.

10.Research on the current situation and influencing factors of nurses'clinical decision-making ability

Objective:To investigate the current situation and influencing factors of clinical nurses'clinical decision-making ability,and provide a reference and evidence for improving and enhancing the correct clinical decision-making ability of clinical nurses.Methods:A total of 300 clinical nurses in Shaanxi Province were selected as the research subjects by using the convenience sampling method.The General Information Questionnaire,Clinical Decision-Making Scale,and Comprehensive Decision-Making Style Scale were used to conduct the investigation and study.The influencing factors of clinical nurses'clinical decision-making ability were analyzed by using single-factor analysis and multivariate linear regression.Results:The total score of clinical nurses'clinical decision-making ability was(163.85±13.78)points,reaching the high-level standard of clinical decision-making ability(146.68-200.00)points.The results of Spearman rank correlation analysis showed that there was a significant positive correlation between the total score and the score of each dimension of clinical decision-making ability and the score of rational decision-making style(P<0.05).The score of the dimension of searching for information or new information was only negatively correlated with the score of intuitive-impulsive decision-making style(P<0.05).Avoidant decision-making style was negatively correlated with the scores of seeking information or new information,clarifying goals and values,the total score of clinical decision-making ability,and the dimensions of finding alternative solutions(P<0.05).The results of multiple linear regression showed that rational decision-making style can positively affect clinical nurses'clinical decision-making ability,while avoidance decision-making style can negatively affect it.Conclusion:The clinical nurses'clinical decision-making ability has been maintained at a relatively constant high level,and it is greatly influenced by rational and avoidant decision-making styles.Medical managers need to organize targeted training activities and other measures to promote the construction of clinical nurses'rational decision-making style and minimize the influence of avoidant decision-making style,thus improving the clinical nurses'clinical decision-making ability.
