1.Ten-year real-world data analysis of clinical characteristics in treatment-naive patients with highly suspected prostate cancer and PSA level ≥20 ng/mL

[Objective] To analyze the clinicopathological characteristics of treatment-naive patients with highly suspected prostate cancer (PCa) with prostate-specific antigen (PSA) level ≥20 ng/mL, to provide reference for promoting early screening of PCa. [Methods] A retrospective analysis was conducted on the clinical data of treatment-naive patients with PSA level ≥20 ng/mL, undergoing prostate biopsy for highly suspected PCa at the Department of Urology, Tianjin Medical University Second Hospital during Jan.2013 and Jun.2023. The correlation between patients' age, body mass index (BMI), PSA, prostate volume (PV), prostate cancer-specific antigen density (PSAD), prostate imaging reporting and data system (PI-RADS) score, and International Society of Urological Pathology (ISUP) grade with highly suspected PCa metastasis and PSA stratification were analyzed. [Results] A total of 1778 suspected patients were enrolled. Pathological findings confirmed PCa in 1465 cases (82.4%), with 487(33.2%) diagnosed as metastatic PCa. Over the past decade, the number of patients undergoing prostate biopsy for highly suspected PCa and being confirmed has been increasing annually, with the proportion of metastatic cases remaining at around 30%. Compared with those with PSA level being 20-50 ng/mL, patients with PSA level >50 ng/mL had older age, lower BMI, higher PSAD, higher PI-RADS, higher ISUP, more diverse pathological types, and a higher incidence of metastasis (P<0.05) with lower proportion of urban residents. Additionally, analysis of metastatic PCa cases showed that 46.8%(228/487) had oligometastasis (≤5 metastatic lesions), including 99.0% bone metastasis, 4.1% extraregional lymph node metastasis, and 4.3% other organ metastasis. [Conclusion] Over the past 10 years, there has been a continuous increase in the number of treatment-naive biopsied cases and newly diagnosed cases of highly suspicious PCa with PSA level ≥20 ng/mL, while the proportion of metastatic cases remains high. Therefore, proactive efforts should be made to promote early screening of high-risk suspected cases.
2.Preparation and osteogenic properties of liquid crystal display light-cured polylactic acid scaffold loaded with nano-tantalum

BACKGROUND:Polylactic acid(PLA)has good biocompatibility and a controllable degradation rate and is currently widely used in biomedical engineering.However,PLA has shortcomings such as low mechanical strength and insufficient biological activity,which limits its further application in bone tissue engineering. OBJECTIVE:To construct polylactic acid/polydopamine/tantalum(PLA/PDA/Ta)bone tissue engineering scaffolds,and explore their biosafety and in vitro osteogenesis. METHODS:A PLA scaffold with a porous structure was prepared through liquid crystal display light-curing technology.PLA/PDA scaffolds and PLA/PDA/Ta scaffolds were prepared by soaking PLA scaffolds in dopamine solution and dopamine-tantalum nanoparticle solution,respectively.The microstructure and water contact angle of scaffolds were characterized.MC3T3-E1 cells were co-cultured with PLA,PLA/PDA,and PLA/PDA/Ta scaffolds,respectively,and CCK-8 assay and live/dead cell staining were performed.After osteogenic differentiation,alkaline phosphatase,alizarin red staining,and osteogenic gene detection were performed. RESULTS AND CONCLUSION:(1)The scanning electron microscope results exhibited that the three kinds of prepared scaffolds had an interconnected porous three-dimensional structure,and the average pore diameter was 200 μm.The water contact angle of PLA/PDA/Ta scaffolds was lower than that of PLA and PLA/PDA scaffolds(P<0.05).(2)CCK-8 assay showed that compared with PLA and PLA/PDA scaffolds,PLA/PDA/Ta scaffolds could promote cell proliferation(P<0.05).Live/dead cell staining showed good cell proliferation in the three groups.(3)Alkaline phosphatase and alizarin red staining showed that compared with PLA and PLA/PDA scaffolds,PLA/PDA/Ta scaffolds could promote the expression of alkaline phosphatase and the formation of mineralized nodules.RT-qPCR showed that compared with PLA and PLA/PDA scaffolds,PLA/PDA/Ta scaffolds could enhance the mRNA expression of cell bone morphogenetic protein,Runx-2,and type I collagen(P<0.05,P<0.01).(4)The results showed that the PLA/PDA/Ta scaffold had excellent osteogenic activity and the ability to promote cell proliferation.
3.Study on the mechanism of Xiaoqinglong decoction in intervening in airway inflammation of asthma with syndrome of cold retention accumulation in lung

OBJECTIVE To investigate the effects and potential mechanisms of Xiaoqinglong decoction on airway inflammation in asthma with syndrome of cold retention accumulation in lung based on the metastasis-associated lung adenocarcinoma transcript 1 (MALAT1). METHODS Forty Wistar rats were randomly divided into blank group, model group, dexamethasone group (positive control, 1 mg/kg), and Xiaoqinglong decoction group (2.72 g/kg), with 10 rats in each group. A rat model of asthma with syndrome of cold retention accumulation in lung was established, and the corresponding drugs were administered once daily starting from the second day of modeling for 21 consecutive days. Lung histopathological changes and lung function were evaluated. The levels of superoxide dismutase (SOD), malondialdehyde (MDA), interferron-γ (IFN-γ), tumor necrosis factor α (TNF-α), and interleukin-13 (IL-13) in serum were measured, and the mRNA expression levels of MALAT1, TNF- α, IL-13, IFN- γ, and transient receptor potential melastatin 2 (TRPM2) in lung tissue were determined. Twenty C57BL/6J wild-type mice and twenty C57BL/6J MALAT1(-/-)mice were randomly divided into wild-type model group, wild-type Xiaoqinglong decoction group, MALAT1 model group, and MALAT1(-/-) Xiaoqinglong decoction group, with 10 mice in each group. The same asthma model was E-mail:yan_peizheng@163.com established, and Xiaoqinglong decoction was administered once daily for 21 days starting from the second day of modeling. The serum levels of SOD, MDA, IFN-γ, TNF-α and IL-13 were measured, along with the mRNA and protein expression levels of TRPM2 in lung tissue. RESULTS The results of the rat experiment showed that, compared with model group, the airway resistance, functional residual capacity, the serum levels of IL- 13, TNF-α and MDA as well as inflammatory infiltration and collagen fiber deposition in lung tissue, and the expressions of IL- 13, TNF-α and TRPM2 in lung tissue were all significantly decreased in the treatment group (P<0.05 or P<0.01). The peak expiratory flow, forced expiratory flow at 50% of forced vital capacity, the serum levels of SOD and IFN-γ, and the expression levels of IFN-γ and MALAT1 in lung tissue were significantly increased (P<0.05 or P<0.01). The results of the mice experiment demonstrated that, compared with the wild-type model group, serum levels of IL-13, TNF-α and MDA in wild-type xiaoqinglong decoction group were significantly reduced (P<0.05 or P<0.01), while serum IFN-γ levels and SOD activity were significantly increased (P<0.01). Compared with the wild-type Xiaoqinglong decoction group, the MALAT1(-/-) Xiaoqinglong decoction group showed significantly decreased serum IFN-γ levels and SOD activity (P<0.01), along with significantly increased levels of IL-13, TNF-α and MDA (P<0.05 or P<0.01), as well as significantly elevated TRPM2 mRNA and protein expression in lung tissue (P<0.05). CONCLUSIONS Xiaoqinglong decoction may alleviate airway inflammation by regulating the expression of MALAT1, modulating oxidative stress, inhibiting TRPM2 activation, and reducing the release of pro-inflammatory cytokines.
4.Effects of Ephedra-Cinnamomum couplet medicinals on respiratory function and airway inflammation in rats with asthma of cold-fluid retention in lung syndrome and its mechanism

OBJECTIVE To investigate the effects of Ephedra-Cinnamomum couplet medicinals on respiratory function and airway inflammation in rats with asthma of cold-fluid retention in lung syndrome and its mechanism. METHODS Sixty Wistar rats were randomly divided into blank group, model group, dexamethasone group [1 mg/(kg·d)], and Ephedra-Cinnamomum low-, medium-, high-dose groups [0.234, 0.936, 1.872 g/(kg·d)], with 10 rats in each group. The model and treatment groups were sensitized by intraperitoneal injection of antigen solution (ovalbumin 100 mg + aluminum hydroxide 100 mg) and challenged with 1% ovalbumin nebulization, along with exposure to a cold environment and ingestion of cold water, to establish the asthma model with cold-fluid retention in lung syndrome. From day 2, rats received corresponding drugs or normal saline intragastrically, once a day, for 21 consecutive days. The general behavioral changes in each group of rats were observed during the experimental process. The lung function parameters [peak expiratory flow (PEF), airway resistance (Raw), functional residual capacity (FRC), expiratory flow at 50% of forced vital capacity (EF50%)] Δ 基金项目 国家自然科学基金青年科学基金项目(No.82004233); were measured before modeling and after the last medication as well as serum contents of interleukin-6 (IL-6), IL-13, tumor necrosis factor- α (TNF- α) and interferon-gamma (IFN- γ) after the last medication were determined; the histopathological morphological changes in the lung tissues of rats were also observed; mRNA expressions of IL-6, IL-3, TNF-α, IFN-γ, thymic stromal lymphopoietin (TSLP), and Toll-like receptor 4 (TLR4), as well as protein expressions of TSLP and TLR4 were determined in lung tissue. RESULTS Compared with model group, the lung tissue damage of rats was relieved significantly; Raw, FRC, the contents and mRNA expression levels of IL-6, IL-13 and TNF-α, as well as the mRNA and protein expressions of TSLP and TLR4 were significantly decreased (P<0.05), while the contents and mRNA expressions of PEF, EF50 % and IFN-γ were significantly increased in the dexamethasone group and Ephedra-Cinnamomum medium- and high-dose groups (P<0.05). Moreover, only a few rats in the two groups exhibited typical symptoms of asthma. CONCLUSIONS Ephedra-Cinnamomum couplet medicinals improve respiratory function and ameliorate airway inflammation in asthma rats with cold-fluid retention in lung syndrome, the mechanism of which may be associated with inhibiting TSLP/TLR4 signaling pathway and modulating Th1/Th2 imbalance.
5.Evaluation of surgical efficacy in patients with hepatic cystic echinococcosis in Gansu Province from 2006 to 2023

Objective To evaluate the therapeutic efficacy for surgical treatments among patients with hepatic cystic echinococcosis in Gansu Province from 2006 to 2023, so as to provide insights into optimization of the diagnosis and treatment strategies against hepatic cystic echinococcosis. Methods The demographic and clinical data of all echinococcosis cases included in central government fiscal transfer payment program for echinococcosis control and undergoing surgical treatments in Gansu Province from 2006 to 2023 were captured. Hepatic cystic echinococcosis patients with complete medical records and follow-up data were included in the study, and patients’ characteristics, including hospital where patients received diagnosis and treatment, methods of case identification, year of surgery, classification of lesions, number of lesions, size of lesions, course of disease, surgical methods, and post-surgical follow-up data. The cure and recurrence of hepatic cystic echinococcosis were evaluated according to the Guidelines for Management of Echinococcosis Patients in the Central Government Fiscal Transfer Payment Program, and the cure and recurrent rates were calculated. Results Data were collected from 1 686 surgical patients with hepatic cystic echinococcosis. According to the inclusion and exclusion criteria, 1 222 hepatic cystic echinococcosis patients undergoing surgical treatments were included during the period from 2006 to 2022, including 1 166 cured patients (95.42%) and 88 patients with postsurgical recurrence (7.20%), and the cure rate of surgical treatments appeared a tendency towards a rise among patients with hepatic cystic echinococcosis from 2008 to 2022 (χ2trend = 19.39, P < 0.05). The cure rates of hepatic cystic echinococcosis were 100% (177/177), 94.81% (128/135) and 94.62% (861/910) among patients detected through regular physical examinations, screened by the central government fiscal transfer payment program for echinococcosis control, and those who passively sought healthcare services, respectively (χ2 = 9.95, P < 0.05). The cure rates of hepatic cystic echinococcosis were 95.96% (1 046/1 090) among patients with a disease course of 2 years and less and 90.90% (120/132) among patients with a disease course of over 2 years (χ2 = 6.87, P < 0.05), and there were significant differences in the cure rates among patients with hepatic cystic echinococcosis in terms of number of lesions (χ2 = 24.44, P < 0.05) and surgical methods (P < 0.05). The cure rate of hepatic cystic echinococcosis patients was significantly higher following initiation of the central government fiscal transfer payment program for echinococcosis control (96.06%, 1 096/1 141) than before the program (86.42%, 70/81) (χ2 = 16.06, P < 0.05), and the cure rate of hepatic cystic echinococcosis patients was significantly higher in designated hospitals (96.48%, 741/768) than in non-designated hospitals (93.37%, 366/392) (χ2 = 5.78, P < 0.05). The median follow-up period was 4 (interquartile range, 7) years among 1 222 hepatic cystic echinococcosis patients undergoing surgical treatments. The recurrent rate of hepatic cystic echinococcosis appeared a tendency towards a decline from 2008 to 2022 (χ2trend = 36.86, P < 0.05), with a reduction from 23.08% (9/39) in 2008 to 1.85% (1/54) in 2021, and the post-surgical recurrence rate of hepatic cystic echinococcosis was lower following initiation of the central government fiscal transfer payment program for echinococcosis control (5.87%, 67 / 1 141) than before the program (25.93%, 21/81) (χ2 = 45.51, P < 0.05). In addition, the post-surgical recurrence rate of hepatic cystic echinococcosis was higher in non-designated hospitals (10.46%, 41/392) than in designated hospitals (5.60%, 43/768) (χ2 = 9.12, P < 0.05), and there was a significant difference in the post-surgical recurrence rate among patients with hepatic cystic echinococcosis in terms of surgical methods (P < 0.05), with the highest recurrence rate (11.54%) seen among patients undergoing percutaneous fine-needle aspiration of cyst fluids-based surgical procedures (P < 0.05). Conclusion Since the initiation of the central government fiscal transfer payment program for echinococcosis control in Gansu Province in 2006, an increase in the surgical cure rate and a reduction in the recurrence of hepatic cystic echinococcosis had been found among patients with hepatic cystic echinococcosis, indicating a high overall therapeutic efficacy.
6.Spatio-temporal gait characteristics of the elderly and the risk of adverse health outcomes

BACKGROUND:Gait is one of the optimal indicators of functional status in older adults.Gait parameters are associated with adverse health outcomes in older adults. OBJECTIVE:To summarize and compare the practical value of gait parameters,and to review the progress of research on spatio-temporal parameters and the risk of adverse health outcomes in older adults. METHODS:PubMed,Web of Science,and CNKI were searched for relevant literature published from January 2010 to June 2023 using the search terms of"gait speed,walking speed,step length,cadence,step frequency,step time,walking base,aged and elderly"in English and Chinese,respectively.A total of 52 papers were finally included for the systematic review. RESULTS AND CONCLUSION:Currently,researchers have focused on three basic temporal-spatial parameters,namely,step speed,step length and step frequency,to explore their association with adverse health outcomes in older adults.Reduced gait speed in older adults may increase the risk of falls,all-cause mortality,and may be an independent predictor of all-cause mortality.Older adults with slow gait speed are at higher risk of cognitive decline,and may be the preferred gait parameter in assessing cognitive impairment in older adults.Older adults with slow gait speed are more likely to have debilitating or common chronic illnesses,and are at higher risk of hospitalization and public care.Step length is positively correlated with individual height and lower limb length,but age-induced shortening of step length is not related to height or lower limb length,and shortening of step length reflects reduced physical function in older adults,and shortening of step length is a risk factor for falls and cognitive decline in older adults.Stride frequency is often used as a measure of exercise intensity,and a stride frequency of≥100 steps/minute is predictive of all-cause mortality,and slowing of step frequency may lead to a higher risk of falls,hospitalization and all-cause mortality in older adults.Changes in gait parameters are closely related to the level of physical fitness in older adults,and there may be a bidirectional relationship between the two.The results of the study will provide a theoretical basis for further improving risk screening in clinical populations,refining health risk assessment in the elderly,improving health protection and promoting active health in the elderly.

Purpose/Significance To conduct the real world research on the application and impact of artificial intelligence(AI)technology in medical institutions,and to assist in intelligent governance decision-making in the field of medicine and health.Method/Process Taking Peking University Third Hospital as the research scene,the AI medical social experiment is carried out through literature meta-analysis,knowledge graph construction and mixed methodology research.Result/Conclusion Scholars at home and abroad are cautious and optimistic about the application of AI technology in the field of health.The research,development and application of AI technology in domestic public hospitals are very active,but small-scale field studies suggest that AI technology may lead to problems such as misleading decision-making and weakening of human subjectivity while improving the efficiency of diagnosis and treatment.In-telligent governance in the field of health,represented by AI technology,requires a long period and a wide range of sociological observa-tions.It is recommended to build a research base for medical AI technology R&D and supervision based on public hospitals,and continue to pay attention to issues such as human subjectivity,medical ethics and scientific and technological ethics in AI social governance.
8.The application of mixed reality technology in maxillofacial orthopedics

The maxillofacial orthopedics surgery is aimed to help patients reconstruct three-dimensional structure and restore original function. The anatomical structures involved in this surgery are complicated, which requires higher technical requirements for surgeon. Mixed reality (MR) technology realizes in-depth interaction and feedback between the real world, the virtual world and the users by presenting virtual scene information in the real scene, which has the advantages of virtual-reality fusion, real-time interactivity, three-dimensional registration, rapid alignment, etc. It can be used for pre-operative planning and intra-operative navigation, which can improve the accuracy and safety of surgery, reduce the occurrence of postoperative complications.At the same time, MR technology can reduce the requirements for the surgeon’s surgical experience and skills. As a result, the MR technology has a broader application prospect in the field of maxillofacial orthopedics. The manuscript summarized the characteristics of MR technology, introduced the application of MR technology in the field of maxillofacial orthopedics. It also put forward the urgent problems and future development direction of MR technology in clinical application.
9.Three-year survival status of 664 older patients with acute exacerbation of chronic obstructive pulmonary disease and the prognostic factors of traditional Chinese and western medicine

Objective Acute exacerbation of chronic obstructive pulmonary disease (AECOPD) is the leading cause of hospitalization and death in patients with chronic obstructive pulmonary disease (COPD). Exploring the prognostic factors of AECOPD patients will assist in optimizing treatment strategies and standardizing disease management. Methods This study retrospectively collected the clinical data of 664 patients with AECOPD admitted to the Respiratory Department of Dongzhimen Hospital of Beijing University of Chinese Medicine from January 2013 to September 2019. The 3-year survival rate and treatment of the patients were investigated. According to whether the patients died,they were divided into a non-survivors group and a survivors group,and clinical data such as basic information,comorbidities,and auxiliary examination results were compared between the two groups. Incorporating clinical experience of researchers and previous research evidence,a secondary screening of variables was conducted to ultimately determine the covariates to be included in the multifactorial Cox proportional hazards regression model,and the factors affecting the 3-year survival rate of the patients were analyzed. Results A total of 664 cases were included in this study,including 362 males and 302 females,with an average age of (77.25±6.89) years old. The 3-year all-cause mortality rate of older hospitalized patients with AECOPD was 20.48%(136 patients). Older age (HR:1.071,95%CI:1.040-1.102,P<0.001);smoking history (HR:1.788,95%CI:1.173-2.723,P=0.007);Charlson comorbidity index (HR:1.209,95%CI:1.029-1.421,P=0.022);lower arterial partial pressure of oxygen (HR:1.014,95%CI:1.006-1.022,P<0.001);higher brain natriuretic peptide(HR:1.001,95%CI:1.000-1.001,P=0.025);cor pulmonale(HR:1.896,95%CI:1.235-2.908,P=0.004);respiratory failure (HR:2.437,95%CI:1.378-4.311,P=0.003);TCM syndrome differentiation elements,including kidney (HR:1.639,95%CI:1.055-2.546,P=0.028) and fluid retention (HR:2.512,95%CI:1.653-3.816,P<0.001),were independent risk factors for 3-year all-cause death of older hospitalized patients with AECOPD. Long-term regular use of bronchiectasis (HR:0.474,95%CI:0.324-0.695,P<0.001) was an independent protective factor for 3-year all-cause death. Conclusion The 3-year survival rate of elderly hospitalized patients with AECOPD is relatively low,with the TCM syndrome elements manifested as lung-kidney qi deficiency,yang deficiency with water retention,and blood stasis obstruction. Patients with moderate to severe impairment of lung function due to COPD have an increased risk of death within 3 years. Therefore,for such patients,nourishing lung-kidney qi,resolving phlegm and water retention,activating blood circulation to remove blood stasis and dredging collaterals,combined with regular use of bronchodilators,may help improve their 3-year survival rate.
10.Clinical analysis of 14 patients aged ≤ 50 years with high-risk multiple myeloma treated with allogeneic hematopoietic stem cell transplantation

Objective:To evaluate the efficacy of allogeneic hematopoietic stem cell transplantation (allo-HSCT) in young patients with high-risk multiple myeloma (HRMM) and analyzed the factors affecting patient prognosis.Methods:In this retrospective study, we analyzed the clinical data of 14 patients with HRMM with cytogenetic abnormalities or high-risk biological factors who underwent allo-HSCT at the Hematopoietic Stem Cell Transplantation Center of the Institute of Hematology & Blood Diseases Hospital between November 2016 and November 2022.Results:There were seven males and seven females included in the study, with a median age of 39.5 (31-50) years at the time of allo-HSCT. The median number of treatment lines before transplantation was 2 (1-6) . Before allo-HSCT, 42.9% (6/14) of the patients did not achieve complete remission, while 35.7% (5/14) of the patients achieved measurable residual disease positivity. After transplantation, all patients were evaluated for their treatment response, and the overall response rate was 100% (14/14) . All 14 patients successfully underwent allo-HSCT, with median engraftment times for neutrophils and platelets of 11 (10-14) days and 13 (9-103) days, respectively. Acute grade Ⅱ-Ⅳ graft-versus-host disease (GVHD) occurred in five patients (35.7%) , and two patients (14.3%) developed moderate-to-severe chronic GVHD. The median follow-up time after allo-HSCT was 18.93 (4.10-72.53) months, with an expected 2-year transplant-related mortality rate of 7.1% (95% CI 0%-21.1%) and an expected 2-year overall survival rate of 92.9% (95% CI 80.3%–100.0%) . Moreover, the expected 1-year and 2-year progression-free survival rates were 92.9% (95% CI 80.3%-100.0%) and 66.0% (95% CI 39.4%-100.0%) , respectively, and the 2-year cumulative incidence of relapse was 28.9% (95% CI 0%-56.7%) . Upfront allo-HSCT following complete remission after induced therapy and the presence of chronic GVHD might be favorable prognostic factors.
