1.Spatio-temporal clustering analysis of influenza in Jiaxing City

Objective:
To investigate the epidemiological and spatio-temporal characteristics of influenza in Jiaxing City, Zhejiang Province, so as to provide insights into perfecting the prevention and control strategies of influenza.
Methods:
Data of influenza in Jiaxing City from 2019 to 2023 were collected from the Chinese Disease Prevention and Control Information System. Population data of the same period were collected from the Zhejiang Health Information Network Reporting System. The epidemiological characteristics of influenza were analyzed using descriptive analysis. Vector map information was collected from the Open Street Map, and the spatio-temporal clustering characteristics of influenza were analyzed using spatial autocorrelation and spatio-temporal scanning.
Results:
A total of 181 501 cases of influenza were reported in Jiaxing City from 2019 to 2023, with an average annual reported incidence of 653.93/105. The majority of cases were aged 5 to <15 years (59 785 cases, 32.94%). The majority of the occupations were students (78 239 cases, 43.11%) and pre-school children (33 715 cases, 18.58%). The county (city, district) with the highest reported incidence was Haining City (1 451.70/105), and the town (street) with the highest reported incidence was Chang'an Town (1 932.78/105). Spatial autocorrelation analysis showed that the incidence of influenza in Jiaxing City from 2019 to 2023 had positive spatial correlations (all Moran's I>0, all P<0.05), with a high-high clustering in the southern region. Spatio-temporal scanning analysis showed that there was a spatio-temporal clustering of influenza in Jiaxing City from 2019 to 2023, with the southern region being the primary-type clustering area and the period between November and January of the following year being the clustering time.
Conclusion
There was a significant spatio-temporal clustering of influenza in Jiaxing City from 2019 to 2023, with winter being the peak season and the southern region being the primary area.
2.Epidemiological characteristics of pertussis in Jiaxing City from 2004 to 2023

Objective:
To investigate the epidemiological characteristics of pertussis in Jiaxing City from 2004 to 2023 and spatio-temporal clustering characteristics from 2022 to 2023, so as to provide insights into formulation of pertussis control measures.
Methods:
Data of pertussis cases in Jiaxing City from 2004 to 2023 were collected through the Infectious Disease Report Information System of Chinese Disease Prevention and Control Information System. The epidemiological characteristics of pertussis cases in Jiaxing City from 2004 to 2023 were descriptively analyzed, and the spatio-temporal clustering characteristics from 2022 to 2023 were analyzed using spatio-temporal scanning.
Results:
A total of 478 pertussis cases were reported in Jiaxing City from 2004 to 2023, with an average annual reported incidence of 0.53/105. The reported incidence showed an upward trend from 2004 to 2023 (P<0.05), with the highest in 2022 (3.17/105). Higher incidence of pertussis was reported in June to August (149 cases, 31.17%) and November to December (112 cases, 23.43%). There was no statistically significant difference in the reported incidence between males and females (0.56/105 vs. 0.50/105, P>0.05). The cases aged under one year accounted for the highest proportion, with 199 cases (41.63%). Haining City (0.68/105), Jiashan County (0.64/105) and Tongxiang City (0.60/105) ranked the top three in the reported incidence of pertussis. Spatio-temporal scanning analysis showed that from 2022 to 2023, the primary clustering area of pertussis was centered in Daqiao Town of Nanhu District, covering 27 towns (streets) in Nanhu District, Jiashan County, Xiuzhou District and Pinghu City, and the clustering time was from November to December, 2023.
Conclusions
The reported incidence of pertussis was at a low level in Jiaxing City, but showed an upward trend from 2004 to 2023. The incidence of pertussis was higher among infants under one year of age, peaked in June to August and November to December, and was concentrated in Nanhu District and its surrounding areas.
3.Key Information Research on Famous Classical Formula Shegan Mahuangtang

Shegan Mahuangtang was a famous classical formula for treating asthma and is included in the the Catalogue of Ancient Famous Classical Formulas(The Second Batch). By means of bibliometrics, this study conducts a textual research and analysis on the key information of its formula origin, composition, drug origins, processing, dosage, decocting methods, efficacy, and clinical application. According to research, Shegan Mahuangtang was first recorded in Synopsis of the Golden Chamber and is the ancestral formula for treating cold asthma, which has been used to this day. Suggestions for the drug origins in Shegan Mahuangtang is as follows:Shegan is selected from the dried rhizomes of Belamcanda chinensis(Iridaceae), Mahuang is selected from the dried herbaceous stems of Ephedra sinica(Ephedraceae), Shengjiang is selected from the fresh rhizomes of Zingiber officinale(Zingiberaceae), Xixin is selected from the dried roots and rhizomes of Asarum heterotropoides var. mandshuricum, A. sieboldii var. seoulense or A. sieboldii(Aristolochiaceae), Ziwan is selected from the dried roots and rhizomes of Aster tataricus(Compositae), Kuandonghua is selected from the dried flower buds of Tussilago farfara(Compositae), Nanwuweizi is selected from the dried mature fruits of Schisandra sphenanthera(Magnoliaceae), Dazao is selected from the dried mature fruit of Ziziphus jujuba(Rhamnaceae), and Banxia, a plant of the Araceae family, is selected as the processed products of dried tubers from Pinellia ternata. The recommended dosage is 41.4 g of Shegan, Xixin, Ziwan and Kuandonghua, 55.2 g of Mahuang and Shengjiang, 37.5 g of Nanwuweizi, 21 g of Dazao, 34.5 g of Banxia. The decoction method is to boil Mahuang first in 2.4 L of water, remove the froth on the top, and add the rest of the herbs and decoct them together, and then boil them to 600 mL, and then take it at warm temperature, 200 mL each time, 3 times a day. In terms of clinical application, Shegan Mahuangtang is most commonly used for respiratory system diseases, especially in the treatment of adult or pediatric bronchial asthma and cough variant asthma. Phlegm sound in the throat is the core symptom of Shegan Mahuangtang in clinical practice, and the core pathogenesis is "cold fluid stagnated in the lungs". By excavating and sorting out the ancient and modern literature of Shegan Mahuangtang, key information is confirmed, which can provide literature reference for the modern clinical application and new drug development of this famous classical formula.
4.Relationship Between Severe Pneumonia and Signaling Pathways and Regulation by Chinese Medicine: A Review

Severe pneumonia is one of the most common and critical respiratory diseases in clinical practice. It is characterized by rapid progression, difficult treatment, high mortality, and many complications, posing a significant threat to the life and health of patients. The pathogenesis of severe pneumonia is highly complex, and studies have shown that its occurrence and development are closely related to multiple signaling pathways. Currently, the treatment of severe pneumonia mainly focuses on anti-infection, mechanical ventilation, and glucocorticoids, but clinical outcomes are often not ideal. Therefore, finding safe and effective alternative therapies is particularly important. In recent years, with the deepening of research into traditional Chinese medicine (TCM), it has gained widespread attention in the treatment of severe pneumonia. This paper reviewed the relationship between severe pneumonia and relevant signaling pathways in recent years and how TCM regulated these pathways in the treatment of severe pneumonia. It was found that TCM could regulate the Toll-like receptor 4 (TLR4)/myeloid differentiation factor 88 (MyD88)/nuclear factor-κB (NF-κB), Janus kinase (JAK)/signal transducer and activator of transcription (STAT), phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt)/mammalian target of rapamycin (mTOR), NOD-like receptor protein 3 (NLRP3), and nuclear factor E2-related factor 2 (Nrf2) signaling pathways, playing a role in reducing the inflammatory response, inhibiting cell apoptosis and pyroptosis, improving oxidative stress, and other effects in the treatment of severe pneumonia. Among these pathways, it was found that all of them regulated inflammation to treat severe pneumonia. Therefore, reducing inflammation is the core mechanism by which Chinese medicine treats severe pneumonia. This review provides direction for the clinical treatment of severe pneumonia and offers a scientific basis for the research and development of new drugs.
5.Relationship between serum miR-501 and miR-195 levels and sensitivity to concurrent chemoradiotherapy in patients with locally advanced cervical cancer

Objective:To investigate the relationship between serum miR-501 and miR-195 levels and sensitivity to concurrent chemoradiotherapy in patients with locally advanced cervical cancer (LACC).Methods:Clinical data of 96 patients with LACC admitted to Nanyang Central Hospital from January 2020 to June 2022 were retrospectively analyzed and compared with those of 96 healthy subjects during physical examination in our hospital during the same period to compare the differences of serum miR-501 and miR-195 levels. Tumor status was reviewed at 6 months after concurrent chemoradiotherapy. Patients were divided into the sensitive and resistant groups according to the evaluation criteria of solid tumor efficacy. The relationship between serum miR-501 and miR-195 levels and the sensitivity to concurrent chemoradiotherapy in LACC patients was analyzed by univariate and multivariate analyses. The receiver operating characteristic (ROC) curve was also drawn to predict differential efficacy of concurrent chemoradiotherapy sensitivity in LACC patients. Multivariate analysis was conducted by binary logistic regression analysis. P<0.05 indicated statistically significant differences. Results:In LACC patients, serum miR-501 level was significantly higher, whereas serum miR-195 level was significantly lower than those in physical examination subjects (both P<0.05). Univariate analysis showed that serum miR-501 level at admission in the resistant group was significantly higher, whereas serum miR-195 level was significantly lower than those in the sensitive group (both P<0.05). Multivariate analysis showed that serum miR-501 and miR-195 levels were significantly correlated with the sensitivity to concurrent chemoradiotherapy in LACC patients. The area under the ROC curve (AUC) was 0.736 and 0.913, respectively. Conclusions:The higher the serum miR-501 level and the lower the serum miR-195 level before treatment, the higher the probability of resistance to concurrent chemoradiotherapy in LACC patients. Serum miR-501 and miR-195 levels of LACC patients before treatment have certain predictive value for the sensitivity to concurrent chemoradiotherapy.
6.Epidemiological characteristics of measles in different immunization program periods in Jiaxing City

Objective:
To investigate the epidemiological characteristics of measles in Jiaxing City of Zhejiang Province in different immunization program periods, so as to provide the evidence for improving prevention and control strategies of measles.
Methods:
Data of measles cases in Jiaxing City from 2005 to 2023 was collected through the Chinese Disease Prevention and Control Information System. Population, temporal, and regional distribution of measles during the periods of pre-expanded immunization program (from 2005 to 2007), expanded immunization program (from 2008 to 2010), and enhanced immunization program (from 2011 to 2023) were descriptively analyzed.
Results:
There were 3 997 measles cases reported in Jiaxing City from 2005 to 2023, with an average annual reported incidence of 4.66/105, showing a decreasing trend (P<0.05). The average annual reported incidence of measles during the periods of pre-expanded immunization program, expanded immunization program, and enhanced immunization program were 22.88/105, 7.24/105 and 0.83/105, respectively. The incidence of measles during different stages of immunization strategies all peaked from March to May. The highest reported incidence was 5.94/105 in Pinghu City and the lowest was 3.92/105 in Tongxiang City. The districts with the highest average annual reported incidence in the periods of pre-expanded immunization program, expanded immunization program, and enhanced immunization program were Nanhu District (31.10/105), Pinghu City (11.10/105) and Jiashan County (1.55/105), respectively. There was spatial clustering of measles in Jiaxing City during the period of pre-expanded immunization program (Moran's I=0.787, P<0.05), with a high-high cluster in Nanhu District.
Conclusions
The reported incidence of measles in Jiaxing City showed a decreasing trend from 2005 to 2023. There was a spatial cluster of measles incidence during the period of pre-expanded immunization program, but no significant clusters during the periods of expanded immunization program and enhanced immunization program.
7.Analysis of Association Between Immune Cells and Breast Cancer Based on Two-sample Mendelian Randomization Method

Objective To explore the causal relationship among 731 types of immune cells and breast cancer.Methods Genome-wide association data for immune cells and breast cancer were used.Mendelian randomization analysis was performed using the inverse variance weighting(IVW)and weighted median(WM)methods,and sensitivity analysis was conducted to assess heterogeneity and pleiotropy.Results A total of 19 immune cell phenotypes were identified to potentially have a causal association with breast cancer,using IVW as the main analysis method(P>0.05)and correcting P values using the false discovery rate method at a significance level of 0.05,excluding reverse causality.Of these,eight and 11 immune phenotypes may increase and decrease the risk of breast cancer,respectively.Conclusion This study explored the causal relationship between immune cells and breast cancer.Results show that certain immune cell phenotypes could serve as predictive markers for the early diagnosis of breast cancer and the development of new immun-otherapeutic strategies.
8.Establishment and evaluation of a quantitative PCR-based assay for the detection of Mycobacterium marinum in skin biopsy specimens

Objective:To establish a rapid quantitative PCR (qPCR) technique for Mycobacterium marinum skin infections, and to analyze its clinical diagnostic efficiency. Methods:DNA was extracted from Mycobacterium marinum colonies and serially diluted (10 -1 to 10 -8). Twelve pairs of previously reported primers and probes, as well as 6 pairs of newly designed primers and probes in this study, were used for qPCR amplification to identify the most sensitive primers and probes for the detection of Mycobacterium marinum. Skin lesion tissues were collected from 72 patients with confirmed Mycobacterium marinum infections (experimental group) and 68 with other mycobacterial infections (control group) at Shandong Provincial Hospital for Skin Diseases & Shandong Provincial Institute of Dermatology and Venereology, Shandong First Medical University & Shandong Academy of Medical Sciences in 2021. These skin tissues were subjected to qPCR amplification, interferon-gamma release assay (IGRA), acid-fast staining, and tissue culture to evaluate the diagnostic efficacy. Results:The newly designed primers and probes targeting the mycobacterial enhanced infection locus 2 (Mel2) demonstrated the highest sensitivity, with a detection limit of 0.86 copies/μl (cycle threshold value = 37) ; the qPCR amplification with the Mel2 primers/probes did not yield positive results when used for the detection of other mycobacteria (including Mycobacterium leprae and Staphylococcus spp) . Among the 72 patients in the experimental group, 44 were positive for qPCR with a sensitivity of 61.1% (95% CI: 49.6% - 71.5%), and 47 were positive for culture with a sensitivity of 65.2% (95% CI: 53.8% - 75.3%) ; all the 68 controls were negative for both qPCR and culture, with their specificities both being 100%. Among 65 patients subjected to IGRA, 31 were positive with a sensitivity of 47.7% (95% CI: 36.0% - 59.6%), while 16 out of 25 controls were negative for IGRA with a specificity of 64.0% (95% CI: 44.5% - 79.8%). Among 58 patients subjected to acid-fast staining, 37 were positive with a sensitivity of 63.8% (95% CI: 50.9% - 74.9%), and 52 out of 66 controls were negative for acid-fast staining with a specificity of 78.8% (95% CI: 67.5% - 86.9%). The combination of qPCR and culture resulted in a sensitivity of 93% and a specificity of 100% for the detection of Mycobacterium marinum. Conclusion:In this study, a highly sensitive qPCR assay was developed for the detection of Mycobacterium marinum, and its combination with culture could further improve the detection sensitivity.
9.Epidemic characteristics and infection modes of human brucellosis in Jiaxing City from 2021 to 2023

Objective:To analyze the epidemic characteristics and infection modes of human brucellosis in Jiaxing City, and provide a basis for refining brucellosis prevention and control strategies.Methods:Data on brucellosis epidemic reported in Jiaxing City from January 2021 to December 2023 were collected, and statistical analysis was conducted using descriptive epidemiology method.Results:A total of 122 cases of brucellosis were reported in Jiaxing City from 2021 to 2023, with an average annual incidence rate of 0.74/100 000. Specifically, the incidence rates from 2021 to 2023 were 0.43/100 000, 1.03/100 000 and 0.77/100 000, respectively, showing an upward trend(χ 2trend = 4.38, P = 0.036). The onset time was mainly from March to August(88 cases). The cases of brucellosis were mainly distributed in Tongxiang City (102 cases). The incidence rate of male (2.98/100 000) was higher than that of female (1.41/100 000, χ 2 = 15.00, P < 0.001). The age of onset was concentrated in 40 - 69 years old (100 cases), the incidence rate of 60 - 69 years old was higher (5.50/100 000). The main modes of infection among occupational population (72 cases) were slaughtering sheep and handling sheep scraps, accounting for 18.03% (22/122) and 17.21% (21/122), respectively. The main modes of infection for non-occupational population (46 cases) were cleaning and cooking of infected lambs and exposure to contaminated environments from diseased sheep, both accounting for 18.03% (22/122). Among them, the proportion of patients with the disease through cleaning and cooking infected lambs showed an increasing trend year by year (χ 2trend = 9.44, P = 0.002). The average time interval between the onset and diagnosis of brucellosis in patients was 20 (10, 40) d. The positive detection rate of standard tube agglutination test (SAT) in Brucella serological test was 67.88% (93/137), and the identification results of all case strains through pathogenic test were Brucella melitensis. Conclusions:The incidence rate of human brucellosis in Jiaxing City from 2021 to 2023 has shown an upward trend. The main population affected by the disease is occupational, but the proportion of non-occupational patients is increasing. Currently. It is suggested that targeted measures should be formulated for non-occupational population while implementing the prevention and control measures for occupational population.
10.Drug resistance surveillance in Mycobacterium tuberculosis in Jiaxing City

Objective:
To investigate the resistance of Mycobacterium tuberculosis to first-line anti-tuberculosis drugs in Jiaxing City, Zhejiang Province from 2017 to 2019, so as to provide insights into improvements of the therapeutic effect of pulmonary tuberculosis.
Methods:
Data pertaining to pulmonary tuberculosis in Jiaxing City from 2017 to 2019 were collected from the Tuberculosis Surveillance System of Chinese Disease Prevention and Control Information System, including demographics, treatment classification, sputum culture and drug resistance. The spectrum, types and prevalence of drug resistance in M. tuberculosis to four first-line tuberculosis drugs, including isoniazid (INH), rifampicin (RFP), streptomycin (SM) and ethambutol (EMB), was analyzed using a descriptive epidemiological method.
Results:
A total of 1 310 M. tuberculosis isolates were cultured from pulmonary tuberculosis patients in Jiaxing City from 2017 to 2019, and there were 259 M. tuberculosis isolates that were resistant to anti-tuberculosis drugs, with an overall drug resistance rate of 19.77%. The prevalence rates of drug resistance to INH, SM, RFP and EMB were 13.36%, 11.83%, 5.50% and 3.59%, respectively. The prevalence of drug resistance was lower in M. tuberculosis isolates from treatment-naïve patients than from retreated patients (18.45% vs. 34.58%, P<0.05). M. tuberculosis isolates presented high resistance to SM (4.50%) and INH alone (4.35%), the highest resistance to INH-SM combinations (3.28%), and the highest resistance to INH+RFP+SM combinations (1.83%). Sixteen isolates were resistant to all the four drugs, with a drug resistance rate of 1.22%. The proportions of resistance to a single drug, RFP resistance, multidrug resistance and resistance to two and more drugs were 10.31%, 5.50%, 4.73% and 4.73%, respectively. In addition, the prevalence of RFP resistance among all patients and treatment-naïve patients both showed a tendency towards a rise from 2017 to 2019 (P<0.05). The prevalence of RFP resistance (7.01% vs. 3.76%) and resistance to two and more drugs (6.01% vs. 3.25%) was both higher among interprovincial mobile tuberculosis patients than among local non-mobile patients (P<0.05).
Conclusions
The overall prevalence of drug resistance was lower in M. tuberculosis isolates in Jiaxing City from 2017 to 2019 than in Zhejiang Province, with INH and RFP resistance as predominant types.
