1.Imaging and Surgical Repair of the Left Pulmonary Venous Anomaly to Coronary Sinus With Small Patent Ductus Arteriosus

A 47-year-old female was admitted with progressive exertional dyspnea and diagnosed with anomalous drainage of the left pulmonary veins into the coronary sinus (CS), combined with an intact interatrial septum and a small patent ductus arteriosus (PDA). Multimodal imaging, including echocardiography, computed tomography, and cardiac catheterization, helps diagnosis, optimal management and monitoring. The patient successfully underwent surgical repair, which involved rerouting the left anomalous pulmonary veins into the left atrium through a surgically created atrial septal defect, while the CS continued to drain into the right atrium and the PDA was closed directly via a transpulmonary approach. The postoperative course was uneventful.
2.Pancreaticoduodenectomy with superior mesenteric artery first-approach combined total meso-pancreas excision for periampullary malignancies:A high-volume single-center experience with short-term outcomes

Background:
s/Aims: Pancreaticoduodenectomy (PD) is the only radical treatment for periampullary malignancies. Superior mesenteric artery (SMA) first approach combined with total meso-pancreas (MP) excision was conducted to improve the oncological results.There has not been any previous research of a technique that combines the SMA first approach and total MP excision with a detailed description of the MP macroscopical shape.
Methods:
We prospectively assessed 77 patients with periampullary malignancies between October 2020 and March 2022 (18 months). All patients had undergone PD with SMA first approach combined total MP excision. The perioperative indications, clinical data, intra-operative index, R0 resection rate of postoperative pathological specimens (especially mesopancreatic margin), postoperative complications, and follow-up results were evaluated.
Results:
The median operative time was 289.6 min (178−540 min), the median intraoperative blood loss was 209 mL (30−1,600 mL).Microscopically, there were 19 (24.7%) cases with metastatic MP, and five cases (6.5%) with R1-resection of the MP. The number of lymph nodes (LNs) harvested and metastatic LNs were 27.2 (maximum was 74) and 1.8 (maximum was 16), respectively. Some (46.8%) patients had pancreatic fistula, but mostly in grade A, with 7 patients (9.1%) who required re-operations. Some 18.2% of cases developed postoperative refractory diarrhea. The rate of in-hospital mortality was 1.3%.
Conclusions
The PD with SMA first approach combined TMpE for periampullary malignancies was effective in achieving superior oncological statistics (rate of MP R0-resection and number of total resected LNs) with non-inferior short-term outcomes. It is necessary to evaluate survival outcomes with long-term follow-up.

Gastrointestinal arteriovenous malformations (AVMs) are a rare disease. Sigmoid-anorectal AVM has only been reported in a few cases. The condition is usually detected when patients have gastrointestinal bleeding complications. The diagnosis and treatment of colorectal AVMs are still challenging. This paper presents a case of an Asian 32-year-old female patient admitted to hospital because of lower gastrointestinal bleeding lasting 17 years. The patient was diagnosed with sigmoid-rectal arteriovenous malformation and failed with other medical treatments. The damaged gastrointestinal tract was removed by a laparoscopic low anterior resection. The results were positive after a three-month follow-up; the bleeding was resolved, and the anal sphincter function was intact. Laparoscopic low anterior resection is a safe, less invasive, and effective approach for managing patients with digestive tract bleeding due to extensive colorectal AVM and preservation of the anal sphincter.
4.Mental health and coping methods of medical students during the fourth wave of the Covid-19 pandemic: a descriptive cross-sectional study

Background: The Covid-19 pandemic that occurred at the end of 2019, especially the fourth wave in Vietnam, has affected the world not only in terms of economy, society, and physical health but also affected the mental health of population in general and of medical students in particular. The use of different coping methods may be protective or risk factors for mental health problems. Objectives: 1. To investigate the prevalence of anxiety, stress, and depression among medical students during the Covid -19 pandemic 2. To describe several coping behavior among medical students during the pandemic. Subjects and methods: A descriptive cross-sectional study conducted on 2350 students of medicine programme range from the first year to the sixth year in the school year 2021 - 2022 at Hue University of Medicine and Pharmacy. Using the DASS-21 scale to survey anxiety, depression and stress in research subjects and a self-designed questionnaire to describe the coping methods of medical students. Results: out of 2350 sample, the prevelance of depression, anxiety, and stress was 38%, 33.2%, and 17% respectively. 13.8% had both anxiety and stress, 15.4% had both stress and depression, and 12.8% had a combination of anxiety, depression and stress. Coping behavior was commonly selected including: eating and sleeping regularly as usual (68.1%); doing exercise at home at least 5 days/week and 30 minutes/day (23.9%); 36.5% of students performed their favorite activities under appropriate conditions most of the time or often. In addition, 59.7% of students had learn new things on the internet, 4.5% of students had practiced meditation and relaxation, 2.9% of students had chosen to write a diary and things they were grateful for; 32% had still maintain contacted with friends via video calls, text messages and 34.4% had chosen to participate in volunteer activities. Conclusion: This study highlights a high rate of anxiety, stress and depression among medical students during the fouthth wave of the Covid-19 pandemic. Personal lifestyle changes and social activities were amongst the most comon coping behavior reported.
5.Evaluation of Depression Screening and Intervention Programs for the Elderly in Quang Ngai, Vietnam

According to a survey published by the National Center for Health Statistics, depression can affect people of any age, but the prevalence of the condition was shown to be second greatest among people aged 65 and older. The most significant repercussion of late-life depressed syndrome is an increase in the overall death rate, which can be attributed to either suicide or other illnesses. The primary objective of this research is to evaluate the impacts of depression caused by biological factors, interpersonal variables, and other socially linked factors on Vietnamese senior people. The Geriatric Depression Scale (GDS-15) was used to evaluate and screen the elderly for signs of geriatric depression in the final data set, which included 630 males (40.1% of the total) and 942 females (59.9% of the total). The participants ranged in age from 60 to 80 years and were all older than that. The most important findings revealed that aspects of this study, such as gender and the amount of time spent sleeping each day, had the potential to operate as predictors of elderly depression.
6.Pathological brain lesions in girls with central precocious puberty at initial diagnosis in Southern Vietnam

Purpose:
Cranial magnetic resonance imaging (MRI) is recommended to identify intracranial lesions in girls with central precocious puberty (CPP). Yet, the use of routine MRI scans in girls with CPP is still debatable, as pathological findings in girls 6 years of age or older with CPP are limited. Therefore, we aimed to identify the prevalence of brain lessons in CPP patients stratified by age group (0–2, 2–6, and 6–8 years).
Methods:
This retrospective cross-sectional study recruited 257 girls diagnosed with CPP for 6 years (2010–2016). MRI was used to detect brain abnormalities. Levels of luteinizing hormone, follicle-stimulating hormone, and sex hormones in blood samples were measured.
Results:
Most girls had no brain lesions (82.9%, n=213), and of the minor proportion of girls with CPP that exhibited brain lesions (17.1%, n=44), 32 girls had organic CPP. Pathological findings were detected in 33.3% (2 of 6) of girls aged 0–2 years, 15.6% (5 of 32) of girls aged 2–6 years, and 3.6% (8 of 219) of girls aged 6–8 years. Hypothalamic hamartoma and tumors in the pituitary stalk were the most common pathological findings. The likelihood of brain lesions decreased with age. Girls with organic CPP were more likely to be younger (6.1±2.4 vs. 7.3±1.3 years, p<0.01) than girls with idiopathic CPP.
Conclusion
Older girls appeared to have a lower prevalence of organic CPP. Clinicians should cautiously use cranial MRI for girls aged 6–8 years with CPP.
7.Systematic sequencing of imported cases leads to detection of SARS-CoV-2 B.1.1.529 (Omicron) variant in central Viet Nam

As authorities braced for the arrival of the Omicron variant of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), infrastructure investments and government directives prompted action in central Viet Nam to establish capacity for genomic surveillance sequencing. From 17 November 2021 to 7 January 2022, the Pasteur Institute in Nha Trang sequenced 162 specimens from 98 150 confirmed SARS-CoV-2 cases in the region collected from 8 November to 31 December 2021. Of these, all 127 domestic cases were identified as the B.1.617.2 (Delta) variant, whereas 92% (32/35) of imported cases were identified as the B.1.1.529 (Omicron) variant, all among international flight passengers. Patients were successfully isolated, enabling health-care workers to prepare for additional cases. Most (78%) of the 32 Omicron cases were fully vaccinated, suggesting continued importance of public health and social measures to control the spread of new variants.

BACKGROUND:
Currently, it is recognized that water polluted with toxic heavy metal ions may cause serious effects on human health. Therefore, the development of new materials for effective removal of heavy metal ions from water is still a widely important area. Melanin is being considered as a potential material for removal of heavy metal from water.
METHODS:
In this study, we synthesized two melanin-embedded beads from two different melanin powder sources and named IMB (Isolated Melanin Bead originated from squid ink sac) and CMB (Commercial Melanin Bead originated from sesame seeds). These beads were of globular shape and 2-3 mm in diameter. We investigated and compared the sorption abilities of these two bead materials toward hexavalent-chromium (Cr) in water. The isotherm sorption curves were established using Langmuir and Freundlich models in the optimized conditions of pH, sorption time, solid/liquid ratio, and initial concentration of Cr. The FITR analysis was also carried out to show the differences in surface properties of these two beads.
RESULTS:
The optimized conditions for isotherm sorption of Cr on IMB/CMB were set at pH values of 2/2, sorption times of 90/300 min, and solid-liquid ratios of 10/20 mg/mL. The maximum sorption capacities calculated based on the Langmuir model were 19.60 and 6.24 for IMB and CMB, respectively. However, the adsorption kinetic of Cr on the beads fitted the Freundlich model with R values of 0.992 for IMB and 0.989 for CMB. The deduced Freundlich constant, 1/n, in the range of 0.2-0.8 indicated that these beads are good adsorption materials. In addition, structure analysis data revealed great differences in physical and chemical properties between IMB and CMB. Interestingly, FTIR analysis results showed strong signals of -OH (3295.35 cm) and -C=O (1608.63 cm) groups harboring on the IMB but not CMB. Moreover, loading of Cr on the IMB caused a shift of broad peaks from 3295.35 cm and 1608.63 cm to 3354.21 cm and 1597.06 cm, respectively, due to -OH and -C=O stretching.
CONCLUSIONS
Taken together, our study suggests that IMB has great potential as a bead material for the elimination of Cr from aqueous solutions and may be highly useful for water treatment applications.

9. 20-Hydroxyecdysone from Dacrycarpus imbricatus bark inhibits the proliferation of acute myeloid leukemia cells

Objective To investigate the anti-proliferative effects of 20-hydroxyecdysone isolated from the bark of Dacrycarpus imbricatus (Blume) de Laub. Methods Column chromatography was used for isolation of compounds from plant material. The structure of the isolated compound was identified by mass spectrometry and nuclear magnetic resonance techniques, including HSQC, HMBC, NOE-difference experiments. The isolated compound was tested for its anti-proliferative activity in acute myeloid leukemia (AML) and OCI-AML cells. Results Compound 1 was isolated from the ethyl acetate fraction of Dacrycarpus imbricatus barks by column chromatography. Its chemical structure was identified as 20-hydroxyecdysone (20HE), a cholestane-type ecdysteroid, by a combination of mass spectrometry and nuclear magnetic resonance spectrometric analyses. Our goal was to test the anti-proliferative activity of 20HE using the OCI-AML cell line. 20HE significantly decreased OCI cell number at a concentration of 1 mg/mL, whereas lower concentrations were ineffective.
