1.Syndrome Element Distribution and Complication Risks in Type 2 Diabetic Patients:A Retrospective Cross-Sectional Study

		                        			ObjectiveTo investigate the distribution of traditional Chinese medicine (TCM) syndrome elements in type 2 diabetes mellitus (T2DM) patients based on maximum body mass index (maxBMI) and explore their association with complication risks. MethodsA retrospective cross-sectional study was used to collect clinical data from hospitalized T2DM patients, extracting age, gender, smoking history, alcohol consumption history, duration of disease, HbA1c level, complications, and TCM syndromes, and extracting the syndrome elements of disease location and disease nature based on their TCM syndromes. MaxBMI was calculated by telephone survey of patients' self-reported maximum body weight; patients with maxBMI ≥24 kg/m2 were classified into spleen-heat syndrome group, and those with maxBMI <24 kg/m2 were classified into consumptive-heat syndrome group. The distribution of TCM syndrome types and syndrome elements of patients in the two groups were analysed. Then the propensity score matching method was used to balance the baseline characteristics between the two groups and compare the differences in the distribution of syndrome types and syndrome elements and the risk of macrovascular and microvascular complications between the two groups. ResultsAmong the 1178 T2DM patients, syndrome elements in spleen-heat patients (1034 cases) were primarily located in the spleen (351 cases, 33.95%), liver (240 cases, 23.21%), and stomach (139 cases, 13.44%), while in consumptive-heat patients (144 cases), they were concentrated in the spleen (57 cases, 39.58%), liver (34 cases, 23.61%), and kidneys (17 cases, 11.81%); regarding syndrome elements of disease nature, spleen-heat patients were predominantly characterized by qi deficiency (481 cases, 46.52%), phlegm (353 cases, 22.73%), and dampness (241 cases, 23.31%), whereas consumptive-heat patients showed more qi deficiency (84 cases, 58.33%) and yin deficiency (44 cases, 30.56%). After propensity score matching, 132 cases were included in each group, and no statistically significant differences were observed in the distribution of syndrome elements of disease location between the two groups (P>0.05), but the phlegm element was significantly more prevalent in spleen-heat patients than in consumptive-heat patients (P = 0.006). Regarding the risk of complications, spleen-heat patients had a significantly higher risk of developing macrovascular complications compared to consumptive-heat patients (OR=2.04, P=0.010), while no significant differences were found between groups in the occurrence of microvascular complications (P>0.05). ConclusionThe spleen-heat T2DM patients show a more frequent syndrome element of disease nature of phlegm, and a higher risk of developing macrovascular complications compared to consumptive-heat patients. 
		                        		
		                        		
		                        		
		                        	
2.Mid-long term follow-up reports on head and neck rhabdomyosarcoma in children

		                        			Objective:To analyze the clinical characteristics of children with head and neck rhabdomyosarcoma (RMS) and to summarize the mid-long term efficacy of Beijing Children′s Hospital Rhabdomyosarcoma 2006 (BCH-RMS-2006) regimen and China Children′s Cancer Group Rhabdomyosarcoma 2016 (CCCG-RMS-2016) regimen.Methods:A retrospective cohort study. Clinical data of 137 children with newly diagnosed head and neck RMS at Beijing Children′s Hospital, Capital Medical University from March 2013 to December 2021 were collected. Clinical characteristic of patients at disease onset and the therapeutic effects of patients treated with the BCH-RMS-2006 and CCCG-RMS-2016 regimens were compared. The treatments and outcomes of patients with recurrence were also summarized. Survival analysis was performed by Kaplan-Meier method, and Log-Rank test was used for comparison of survival rates between groups.Results:Among 137 patients, there were 80 males (58.4%) and 57 females (41.6%), the age of disease onset was 59 (34, 97) months. The primary site in the orbital, non-orbital non-parameningeal, and parameningeal area were 10 (7.3%), 47 (34.3%), and 80 (58.4%), respectively. Of all patients, 32 cases (23.4%) were treated with the BCH-RMS-2006 regimen and 105 (76.6%) cases were treated with the CCCG-RMS-2016 regimen. The follow-up time for the whole patients was 46 (20, 72) months, and the 5-year progression free survival (PFS) and overall survival (OS) rates for the whole children were (60.4±4.4)% and (69.3±4.0)%, respectively. The 5-year OS rate was higher in the CCCG-RMS-2016 group than in BCH-RMS-2006 group ((73.0±4.5)% vs. (56.6±4.4)%, χ2=4.57, P=0.029). For the parameningeal group, the 5-year OS rate was higher in the CCCG-RMS-2016 group (61 cases) than in BCH-RMS-2006 group (19 cases) ((57.3±7.6)% vs. (32.7±11.8)%, χ2=4.64, P=0.031). For the group with meningeal invasion risk factors, the 5-year OS rate was higher in the CCCG-RMS-2016 group (54 cases) than in BCH-RMS-2006 group (15 cases) ((57.7±7.7)% vs. (30.0±12.3)%, χ2=4.76, P=0.029). Among the 10 cases of orbital RMS, there was no recurrence. In the non-orbital non-parameningeal RMS group (47 cases), there were 13 (27.6%) recurrences, after re-treatment, 7 cases survived. In the parameningeal RMS group (80 cases), there were 40 (50.0%) recurrences, with only 7 cases surviving after re-treatment. Conclusions:The overall prognosis for patients with orbital and non-orbital non-parameningeal RMS is good. However, children with parameningeal RMS have a high recurrence rate, and the effectiveness of re-treatment after recurrence is poor. Compared with the BCH-RMS-2006 regimen, the CCCG-RMS-2016 regimen can improve the treatment efficacy of RMS in the meningeal region.
		                        		
		                        		
		                        		
		                        	
3.Health literacy prediction models based on machine learning methods: a scoping review

		                        			Objective:
		                        			To conduct a scoping review on the types, construction methods and predictive performance of health literacy prediction models based on machine learning methods, so as to provide the reference for the improvement and application of such models.
		                        		
		                        			Methods:
		                        			Publications on health literacy prediction models conducted using machine learning methods were retrieved from CNKI, Wanfang Data, VIP, PubMed and Web of Science from inception to May 1, 2024. The quality of literature was assessed using the Prediction Model Risk of Bias ASsessment Tool. Basic characteristics, modeling methods, data sources, missing value handling, predictors and predictive performance were reviewed.
		                        		
		                        			Results:
		                        			A total of 524 publications were retrieved, and 22 publications between 2007 and 2024 were finally enrolled. Totally 48 health literacy prediction models were involved, and 25 had a high risk of bias (52.08%), with major issues focusing on missing value handling, predictor selection and model evaluation methods. Modeling methods included regression models, tree-based machine learning methods, support vector machines and neural network models. Predictors primarily encompassed factors at four aspects: individual, interpersonal, organizational and society/policy aspects, with age, educational level, economic status, health status and internet use appearing frequently. Internal validation was conducted in 14 publications, and external validation was conducted in 4 publications. Forty-two models reported the areas under the receiver operating characteristic curve, which ranged from 0.52 to 0.983, indicating good discrimination.
		                        		
		                        			Conclusion

4.Research progress on the action mechanism of monomer components and compound formulations of traditional Chinese medicine in the treatment of vitiligo

		                        			Vitiligo is a chronic and refractory pigmentary loss skin disease with a complex pathogenesis. Traditional Chinese medicine(TCM) offers advantages in treating vitiligo, such as multi-component and multi-target effects, delivering definite clinical efficacy. This article summarizes the action mechanisms of TCM monomer components and compound formulations in the treatment of vitiligo. It is found that pinostrobin, tribuloside and Erzhi pills can activate the cyclic adenosine monophosphate/protein kinase A (cAMP/PKA) signaling pathway; cannabidiol and tanshinone Ⅱ A can activate the p38 mitogen-activated protein kinase(p38 MAPK)signaling pathway; Ginkgo biloba extract EGb761 and astragaloside Ⅳ can activate the aryl hydrocarbon receptor(AhR) signaling pathway; escin and psoralen derivative BSP-1 can activate the Wnt/β-catenin signaling pathway; apigenin and Baiban granules can activate the nuclear factor erythroid 2-related factor 2/heme oxygenase-1(Nrf2/HO-1) signaling pathway; hyperoside and kaempferol can activate the phosphatidylinositol 3-kinase/protein kinase B(PI3K/Akt) signaling pathway; Ruyi heibai powder can activate the programmed death-1/programmed death-ligand 1(PD-1/PD-L1) signaling pathway; Compound honghua buji granules and demethylzeylasteral can inhibit the Janus kinase/signal transducer and activator of transcription (JAK/STAT) signaling pathway. 6.Progress in research on health literate schools

		                        			Abstract
		                        			Health literate schools (HeLit-Schools) play a significant role in fostering students  health literacy. The paper elucidates the background and conceptual connotations of HeLit-Schools, and analyzes how HeLit-Schools effectively integrate and enhance the health literacy of schools in three aspects: philosophy and core drivers, strategy and method implementation, as well as evaluation mechanisms and standard setting. Furthermore, the paper explores the implications of foreign HeLit-Schools research and practice for China under the context of "Healthy China" construction, as well as the key strategies for Chinese schools in the implementation of HeLit-Schools, aiming to provide a new perspective and theoretical support for Chinese schools to practice the  "Healthy  China initiative" and strengthen school construction from the perspective of health literacy.
		                        		
		                        		
		                        		
		                        	
7.Screening and bioinformatics analysis of key autophagy-related genes in alcoholic hepatitis

		                        			Objective To screen key autophagy-related genes in alcoholic hepatitis (AH) and investigate potential biomarkers and therapeutic targets for AH. Methods Two AH gene chips in Gene Expression Omnibus (GEO) and autophagy-related data sets obtained from MSigDB and GeneCards databases were used, and the key genes were verified and obtained by weighted gene co-expression network analysis (WGCNA). The screened key genes were subject to gene ontology (GO), Kyoto Encyclopedia of Genes and Genomes (KEGG), protein-protein interaction (PPI) and immune infiltration analyses. Messenger RNA (mRNA)- microRNA (miRNA) network was constructed to analyze the expression differences of key autophagy-related genes during different stages of AH, which were further validated by real-time fluorescence quantitative polymerase chain reaction (RT-qPCR) in the liver tissues of AH patients and mice. Results Eleven autophagy-related genes were screened in AH (EEF1A2, CFTR, SOX4, TREM2, CTHRC1, HSPB8, TUBB3, PRKAA2, RNASE1, MTCL1 and HGF), all of which were up-regulated. In the liver tissues of AH patients and mice, the relative expression levels of SOX4, TREM2, HSPB8 and PRKAA2 in the AH group were higher than those in the control group. Conclusions SOX4, TREM2, HSPB8 and PRKAA2 may be potential biomarkers and therapeutic targets for AH.
		                        		
		                        		
		                        		
		                        	
8.A cost-benefit analysis on nursing care in thoracoscopic sublobectomy without indwelling bladder catheter

		                        			Objective To analyze the nursing cost effectiveness of non-indwelling bladder catheter in thora-coscopic sublobectomy,and in order to further determine the feasibility of patients undergoing sublobectomy without indwelling catheter.Methods We prospectively collected the clinical data on a total of 254 patients undergoing thoracoscopic sublobectomy in the department of pulmonary surgery of Guangdong Provincial People's Hospital from May 2021 to January 2023.The patients were randomly divided into a study group(128 patients without catheter)and a control group(126 patients with catheter).The nursing cost-effectiveness indexes and postoperative comfort scores were compared between the two groups.Results Seven patients in the experimental group and sixteen patients in the control group needed repeated placement of urinary catheter There were no significant differences in the general demographic and clinical data between the two groups(P>0.05).The cost of materials related to urinary catheter,nursing cost,and total cost in the control group were higher than those in the study group.The total nursing time in the control group was longer than that in the study group.The per capita material cost,nursing cost and total cost in the control group were higher than those in the study group,and the differences were statistically significant(P<0.01).The total score of the comfort scale and the physiological and environmental dimension of postoperative comfort were significantly higher in the study group than in the control group,with statistical significances(P<0.05).Conclusions Thoracoscopic sublobectomy without indwelling bladder catheter can lower medical expense,reduce nursing workload,and improve postoperative comfort.
		                        		
		                        		
		                        		
		                        	
9.Advances on the long-term prognosis of septic shock in children

10.Prediction of Ki-67 expression status in breast cancer based on ultrasound radiomics combined with clinicopathologic features

		                        			Objective:To investigate the prediction of the tumor proliferation antigen(Ki-67) expression status in breast cancer patients based on ultrasound radiomics combined with clinicopathologic features.Methods:Breast cancer patients who underwent 2D ultrasound and Ki-67 examination from January 2018 to February 2022 in Changzhou Second People′s Hospital, Nanjing Medical University were retrospectively analyzed. Among them, 427 patients from Chengzhong campus were randomly divided into training and validation sets in the ratio of 8∶2, and 229 patients from Yanghu campus were used as an independent external test set. Radiomics features were extracted from the region of interest of 2D ultrasound images, and the Mann-Whitney U test, recursive feature elimination, and minimum absolute shrinkage and selection operators were used to perform feature dimensionality reduction and to establish a radiomics score(Rad-score). Subsequently, single/multifactor logistic regression regression analyses were used to construct a joint prediction model based on Rad-score and clinicopathological features. Model performance and utility were assessed using the subject operating characteristic area under the curve (AUC), calibration curve, and decision curve analyses. Results:The AUCs of the joint model for predicting Ki-67 expression status in breast cancer in the training, validation, and test sets were 0.858, 0.797, and 0.802, respectively, which were superior to those of the radiomics (0.772, 0.731, and 0.713) and clinical models (0.738, 0.750, and 0.707). Calibration curve and decision curve analyses indicated that the joint model had good calibration and clinical value.Conclusions:A joint model based on ultrasound radiomics and clinicopathological features can effectively predict the Ki-67 expression status of breast cancer, which is expected to become a non-invasive tool for Ki-67 detection and provide clinicians with an important auxiliary diagnostic and therapeutic decision-making basis.
