2.Clinical features and prognosis of pediatric acute megakaryocytic leukemia.

OBJECTIVE:
To study the clinical features and prognosis of children with acute megakaryocytic leukemia (AMKL) and the clinical effect of acute myeloid leukemia 03 (AML03) regimen for the treatment of pediatric AMKL.
METHODS:
The clinical data were collected from 47 children with AMKL who were diagnosed from May 2011 to December 2019. The treatment outcomes and prognostic factors were analyzed. The Kaplan-Meier method and the log-rank test were used for survival analysis.
RESULTS:
Among the 47 children with AMKL, 22 with non-Down syndrome-AMKL were treated by the AML03 regimen, with a median follow-up time of 11.4 months. For the 22 non-Down syndrome-AMKL patients, the remission rate of bone marrow cytology was 85% and the negative rate of minimal residual disease (MRD) was 79% after induction Ⅱ, with a 2-year overall survival (OS) rate of (50±13)% and a 2-year event-free survival (EFS) rate of (40±12)%. The group with positive immunophenotypic marker CD56 had significantly lower 2-year EFS and OS rates than the group with negative CD56 (
CONCLUSIONS
Children with AMKL tend to have a low remission rate and a poor prognosis. Positive immunophenotypic marker CD56, bone marrow cytology during early treatment response, and MRD results are important factors influencing the prognosis. Allogeneic hematopoietic stem cell transplantation has no significant effect on the prognosis of AMKL.

A retrospective surveillance study on enterovirus D68 was performed in Beijing, China, following the largest and most widespread EV-D68 infection, which occurred in the USA. From January 2011 to July 2015, EV-D68 was identified in 12 individuals with respiratory infections in Beijing, China. The phylogenetic relationships based on the genomic sequence alignment showed that there were two lineages circulating in Beijing from 2011 to 2015. Eight EV-D68 strains belonged to group 1 and four belonged to group 3. All EV-D68 strains from Beijing in 2014 were separately clustered into subgroup II of group 1. Based on these results, we concluded that the Beijing EV-D68 strains had little association with the EV-D68 strains circulating in the 2014 USA outbreak.

5.Study on the Fast Testing Strategy for identifying the wild poliovirus Ⅰ

Objective To explore the Fast Testing Sstrategy (FTS) for wild poliovirus Ⅰ (WP1).Methods Epidemiological investigations were carried out on 671 students from WP1 epidemic areas in China.A set of real time RT-PCR assays,including panenterovirus testings (PE) assay,poliovirus serotypings(PS) assay and the assay distinguishing wild strain from vaccine strain of poliovirus Ⅰ (DWV) were introduced into the screening program for WPV1 to replace the conventional RT-PCR,recommended by the China National Polio Laboratory (GNPL).Additionally,sensitivities of all the assays were assessed by poliovirus type Ⅰ to Ⅲ (Sabin stain) and the isolated WPV I.Results ( 1 ) 33 non-poliovirus enterovirus (NPEV) cases were detected,with 16 polio vaccinerelated cases including 5 polio Ⅰ,1 polio Ⅱ,3 polio Ⅲ,1 polio Ⅰ +Ⅱ,4 polio Ⅰ + Ⅲ and 2 polio Ⅰ + Ⅱ + Ⅲ.Three WPV 1 cases were also detected in this study and confirmed by GNPL.(2) For polio virus vaccine strain,sensitivities of the set of real time RT-PCR assays ranged from 1 to 100 times than that of the in-housc RT-PCR assay.The sensitivities of PE and PS assays for the detection of polio Ⅱ were 100 times than that of the RT-PCR assay and the sensitivity of DWV assay used for the detection of polio Ⅰ were 10 times than that of the RT-PCR assay.For WPV1,the sensitivity of three real time RT-PCR was 10 times higher than that of the RT-PCR assay.Conclusion The novel FTS for WPV I suggested by this study would include PE,PS and DWV.It not only could greatly shorten the testing time but also more sensitive than the RT-PCR and suited for emergency detection for WPV1.
6.Analysis on health information extracted from an urban professional population in Beijing.

BACKGROUNDThe assembled data from a population could provide information on health trends within the population. The aim of this research was to extract and know basic health information from an urban professional population in Beijing.
METHODSData analysis was carried out in a population who underwent a routine medical check-up and aged > 20 years, including 30 058 individuals. General information, data from physical examinations and blood samples were collected in the same method. The health status was separated into three groups by the criteria generated in this study, i.e., people with common chronic diseases, people in a sub-clinic situation, and healthy people. The proportion of both common diseases suffered and health risk distribution of different age groups were also analyzed.
RESULTSThe proportion of people with common chronic diseases, in the sub-clinic group and in the healthy group was 28.6%, 67.8% and 3.6% respectively. There were significant differences in the health situation in different age groups. Hypertension was on the top of list of self-reported diseases. The proportion of chronic diseases increased significantly in people after 35 years of age. Meanwhile, the proportion of sub-clinic conditions was decreasing at the same rate. The complex risk factors to health in this population were metabolic disturbances (61.3%), risk for tumor (2.7%), abnormal results of morphological examination (8.2%) and abnormal results of lab tests of serum (27.8%).
CONCLUSIONSHealth information could be extracted from a complex data set from the heath check-ups of the general population. The information should be applied to support prevention and control chronic diseases as well as for directing intervention for patients with risk factors for disease.

7.Bone density changes in the apical area after rapid orthodontic extrusion of subgingivally fractured tooth.

OBJECTIVETo evaluate the bone density changes in the apical area of subgingivally fractured tooth after rapid orthodontic extrusion.
METHODSTwelve fractured incisors in 11 patients extended 2 - 5 mm below the gingival line were selected. Two weeks after root canal therapy, the subgingival fragment was lifted up and the fracture line was brought 1.5 - 2.0 mm above the level of the gingival line by means of edgewise fixed appliance. After the extrusion completed, the tooth had been stabilized and held for 6 months. The CT Analyser software was used to measure the bone density changes in the apical area on radiographs once a month. Changes of relative value in bone density was quantitatively analyzed.
RESULTSThe average period of extrusion was 11 days. The relative value of bone density in the apical region was -39.6% immediately after extrusion and continuously increased afterwards. In the third month, the value (18.5%) changed most rapidly (P < 0.01).
CONCLUSIONSUnder the continuous and proper tooth axial extrusion force, the tooth moved rapidly and steadily. The bone density in the apical area approached normal value within 3 months after treatment.

8.Functional magnetic resonance imaging and immunohistochemical study of hypothalamic function following oral glucose ingestion in rats.

BACKGROUNDThe hypothalamus plays a central role in the regulation of metabolism by sensing metabolic demands and releasing regulatory neurotransmitters. This study investigated the response of the hypothalamus to glucose ingestion in rats by blood oxygen level-dependent functional magnetic resonance imaging (BOLD-fMRI) and immunohistochemical techniques to determine the role of the hypothalamus in glyco-regulation during disturbances in carbohydrate metabolism.
METHODSThe signal intensity of the hypothalamus was monitored by fMRI for 60 minutes after oral glucose intake in 48 healthy rats (age 14 months), which included 24 normal weight rats (weighing (365 +/- 76.5) g) and 24 overweight rats (weighing (714 +/- 83.5) g). Then, 12 rats (6 normal, 6 overweight) underwent a repeat fMRI scan after consuming an equivalent amount of water without glucose on a separate day. The procedure for fMRI with water intake was the same as for glucose ingestion. fMRI data was processed using time cluster analysis and intensity averaging method. After fMRI, the expression of neuropeptide Y (NPY) and 5-hydroxytryptamine (5-HT) in the hypothalamus of all rats was determined by immunohistochemistry. Positive cells for NPY or 5-HT were counted.
RESULTSThere was a transient, but significant, decrease in fMRI signal intensity in all rats (mean (3.12 +/- 0.78)%) in the hypothalamus within 19.5 - 25.5 minutes of oral glucose ingestion. In overweight rats, the decrease in signal intensity in response to the glucose ingestion was more markedly attenuated than that observed in normal weight rats ((2.2 +/- 1.5)% vs (4.2 +/- 0.7)% inhibition, t = 2.12, P < 0.05). There was no significant response in the hypothalamus after oral water ingestion. The percentage of NPY positive cells in obese rats were slightly lower than those in control group (21% vs 23%, t = 0.71, P > 0.05); but there was no significant difference between the two groups; the percentage of 5-HT positive cells in obese rats were significantly lower than those in the control group (22% vs 31%, t = 3.25, P < 0.01).
CONCLUSIONSThere is a transient, but significant, decrease in BOLD signal intensity in the hypothalamus following glucose ingestion, which is similar to that observed in humans. The response of the hypothalamus to glucose ingestion was different in overweight and normal weight rats. The percentage of NPY positive cells in obese rats were lower than those in the control group, although this difference was not statistically significant. The percentage of 5-HT positive cells in obese rats was significantly lower than those in the control group.

9.Trial study on DENG Tie-tao's coronary heart disease capsules in improving patients' quality of life.

OBJECTIVETo assess the efficacy of the Coronary Heart Disease (CHD) Capsules worked out by Prof. Deng --in improving quality of life of CHD patients of qi deficiency with phlegm and blood stasis syndrome.
METHODAccording to the WHO's diagnosis criteria of CHD, a total of 93 stable angina patients were divided into 3 groups using the single blinded method. The groups were evenly distributed into CHD Capsule treated group (CHDC), isosorbide dinitrate control group (ID), and Compound Prescription Danshen Droplet Pills control group (CPDDP). Two courses of treatment lasting for 6 months were given. During the courses of treatment, the following parameters were observed: clinical symptoms of angina pectoris, ECG change, treadmill exercise test, 36 items in short form of health survey (SF-36) and Seattle Angina Questionnaire (SAQ) scale.
RESULTSAfter 6 months of treatment, all the three groups showed good curative effect in angina pectoris, ECG and treadmill exercise test, differences between them had no statistical significance. The CHDC group showed a better result in nitro-glycerine stopping or alleviation rate and in improving symptoms than the other groups (P < 0.05). The general health, vitality, role-emotional, mental health and reported health transition in the CHDC group were significantly better than those in the control groups (P < 0.05). The scores in physiological functioning role, physiological function and pain alleviation were not different among the three groups.
CONCLUSIONProf. DENG Tie-tao's CHDC is effective in treating CHD with qi deficiency, phlegm and blood stasis and also in improving the quality of life. CHDC is more suitable to be used in long-term treatment than isosorbide dinitrate. The SF-36 and SAQ can be used to appraise the curative effect of traditional Chinese medicine agents for CHD angina pectoris.
