2.Prognostic value of NLR and LMR in patients with ischemic stroke treated with thrombolysis

Objective To analyze the predictive value of neutrophil to lymphocyte ratio(NLR)and lymphocyte to monocyte ratio(LMR)before thrombolysis in patients with acute ischemic stroke(AIS)who received intravenous thrombolysis.Methods The clinical data of 196 AIS patients who received intravenous thrombolysis in Inner Mongolia Autonomous Region People's Hospital from January to June 2023 were analyzed retrospectively.According to the modified Rankin scale(mRS)score 90 days after thrombolysis,the patients were divided into improvement group(mRS score ≤2 points)and non-improvement group(mRS score>2 points).The clinical data of patients were collected.Binary Logistic regression analysis was used to explore the independent factors affecting the prognosis of patients.Receiver operating characteristic curve was drawn to analyze the predictive value of NLR and LMR.Results The age,proportion of diabetes history and antiplatelet drug history in improvement group were significantly lower than those in non-improvement group(P<0.05).The D-dimer,neutrophil absolute value,C-reactive protein and NLR in improvement group were significantly lower than those in non-improvement group,while lymphocyte absolute value and LMR in improvement group were significantly higher than those in non-improvement group(P<0.05).Binary Logistic regression analysis showed that elevated NLR,diabetes history,and antiplatelet drug history were independent risk factors affecting the prognosis of AIS patients(P<0.05).The area under the curve(AUC)of NLR,diabetes history,and antiplatelet drug history predicted the prognosis of AIS patients was 0.682,0.616,and 0.563,respectively.The AUC of combined prediction of these three indexes was 0.742,with a sensitivity of 66.9%and a specificity of 75.0%.Conclusion NLR,diabetes history,and antiplatelet drug history have predictive value for AIS patients.

新冠病毒感染疫情严重威胁着世界各国人民的生命健康。目前,对病毒感染的防治研究主要集中在抑制病毒与分子受体的结合上。AXL作为新发现的严重急性呼吸综合征冠状病毒2型(SARS-CoV-2)受体,在协助病毒感染人体呼吸系统中发挥着重要作用,是未来临床干预的潜在靶点。本研究对已发表的单细胞测序数据进行整理和分析,发现AXL在年轻人肺细胞中的表达水平明显高于老年人。人胚肺二倍体成纤维细胞(2BS)是衰老研究的公认细胞株。本文采用2BS细胞构建复制性细胞衰老模型,发现年轻细胞中AXL的蛋白水平明显高于衰老细胞,据此推测年轻人感染的风险可能更高,需要注意防护。我们发现一种羊栖菜褐藻多糖硫酸酯组分(SFW-3)可显著下调年轻2BS细胞中AXL的表达水平,表明SFW-3具有一定的抗SARS-CoV-2感染的研究价值,同时表明2BS细胞株也可作为潜在的SARS-CoV-2体外感染模型。

4.CD103 +CD8 +T cells combined with neutrophil-to-lymphocyte ratio predict response to neoadjuvant chemoimmunotherapy in advanced oral squamous cell carcinoma

Objective:To investigate the relationship between the expression of CD103 +CD8 +T cells in locally advanced oral squamous cell carcinoma (LA-OSCC), and the response to neoadjuvant chemoimmunotherapy (NACI). Methods:Thirty LA-OSCC patients from the Department of Oral and Maxillofacial Surgery, Sun Yat-sen Memorial Hospital, Sun Yat-sen University, who underwent NACI from June 2020 to December 2022 were analyzed, including 16 responders and 14 non-responders. Using multiple immunofluorescence technique to stain sections of patients to verify the correlation between the expression of CD103 +CD8 +T cells and the efficacy of NACI. CD103 +CD8 +T cell density was counted using Inform and HALO software. The Spearman correlation coefficient in rank correlation is used to describe the correlation between CD103 +CD8 +T cell and neutrophil-to-lymphocyte ratio (NLR), derived neutrophil-to-lymphocyte ratio (dNLR), platelet-lymphocyte ratio (PLR), systemic immune inflammation index (SII) It′s effectiveness as a predictive marker to NACI was analyzed by receiver operator characteristic (ROC) curve analysis and decision curve analysis (DCA). Two-tailed t-test or Mann-Whitney U-test was used to compare data between two groups, and one-way ANOVA was used to compare data between multiple groups. SPSS 22.0 and GraphPad prism 9.0 software were used for statistical analysis and plotting of relevant statistical graphs such as histograms. P<0.05 was considered a statistically significant difference. Results:The density of CD103 +CD8 +T cells has expanded in advanced OSCC patients who are responsive to NACI. The CD103 +CD8 +T cell densities in the responsive and nonresponsive groups were 118.30(41.92, 197.80) pcs/mm 2 and 21.63(4.91, 71.92) pcs/mm 2 respectively, with statistically significant differences( U=52.00, P=0.012). CD103 +CD8 +T cell abundance was negatively correlated with NLR, dNLR, PLR, and SII ( P<0.05). ROC curve analysis showed that the AUC for predicting efficacy of NLR, dNLR, PLR, and SII were 0.781 ( P=0.009, 95% CI: 0.5715-0.9910), 0.671 ( P=0.105, 95% CI: 0.467-0.881), 0.679 ( P=0.020 95% CI: 0.549-0.951), 0.750 ( P=0.096, 95% CI: 0.461-0.896), respectively. The AUC for CD103 +CD8 +T cells alone was 0.861 ( P=0.013, 95% CI: 0.585-0.950), and the AUC of combining CD103 +CD8 +T cells with NLR was 0.896 ( P=0.025, 95% CI: 0.454-0.938). Conclusions:The density of CD103 +CD8 +T cells is expanded in advanced OSCC patients who are responsive to NACI. CD103 +CD8 +T cells positively predict favorable responses as a strong indicator to NACI in advanced OSCC patients. Co-interpretation of CD103 +CD8 +T cells and NLR value enhances the predictive accuracy of NACI in advanced OSCC patients.
5.Abnormalities of efficiency in resting state functional brain network in first-episode paranoid schizophrenia

Objective:To explore the abnormalities of efficiency in resting state functional brain network in patients with paranoid schizophrenia and the correlations between efficiencies and clinical symptoms.Methods:A total of 73 patients with schizophrenia (SZ group) met with the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-Ⅳ) criteria for schizophrenia and 70 healthy controls (HC group) were included .All subjects were checked by using functional magnetic resonance imaging (fMRI), and positive and negative syndrome scale(PANSS) were used to assess the symptoms.Abnormalities of global and local efficiency of brain regions in brain functional network were analyzed by graph theory.Pearson correlation was used to analyze the correlation between the abnormal global efficiency and local efficiency of brain regions of SZ group and PANSS.SPSS 20.0 software was used for dependent-sample t-test, ANOVA test and Pearson correlation analysis. Conclusion:The brain networks of patients with first-episode paranoid schizophrenia may have regional dysfunction in the transmission efficiency and fault-tolerant ability of resting state brain functional network, and the abnormalities of efficiency may be associated with the severity of psychiatric symptoms in several brain regions.
6.Construction and application of three-dimensional evaluation model of single bed efficiency in hospital

Objective:To build a three-dimensional evaluation model of single bed efficiency in an obstetrics and gynecology hospital and provide reference for bed management in hospital.Methods:The sample ward and key indicators were determined through interviews. A two-level database was built according to patients′ data from hospital information system. K-means cluster analysis was used to get the beds classified by annual average vacancy(x), annual average turnover(y) and annual average case-mix index per capita(z). The authors built the three-dimensional bed efficiency model with x, y, z as boundaries and analyzed the bed efficiency by comparing the within group average point A k( x k, y k, z k) with the overall average point A0( x, y, z). Whereafter the bed efficiency of each medical work team was analyzed. Results:Thirty-six beds were classified into 4 categories according to utilization efficiency. 50% of the beds(18 beds) were well used, 28%(10 beds) had room for improvement, and 19%(7 beds) may have resource waste. Significant differences existed in bed efficiency among medical work teams.Conclusions:The model in our study can realize in-depth exploration by evaluating bed efficiency from two aspects of the whole ward and each medical work team. This model, which is mainly applicable to the situation where beds are under the charge of fixed medical work groups or doctors, can be adjusted and extended to meet different strategic needs of hospitals.
7.Angiographic diagnosis and therapeutic embolization of Dieulafoy disease

Objective To evaluate the clinical efficacy of interventional techniques in the diagnosis and therapy of Dieulafoy disease. Methods A retrospective study was performed, including 17 patients with massive upper gastrointestinal hemorrhage (patients without peptic ulcer and portal hypertension or diagnosed with Dieulafoy disease by endoscopic examination). All patients had both DSA and interventional embolization treatment, and were followed for 12 months to appraise the clinical effectiveness. Results Sixteen patients were diagnosed as Dieulafoy disease by using DSA. Fifteen of the 16 patients were treated with embolization successfully withoutserious complications. One patient received subtotal gastrectomy because of upper gastrointestinal hemorrhage recurrence. Nine patients with irregular upper abdominal pain and burning sensation had complete remission after symptomatic management. Fifteen patients who had embolization showed no serious complications during the follow-up period of 12 months, there was no hematemesis and melena for the 15 cases with successful embolization. Conclusion The angiography and embolization are safe and efficacious in the diagnosis and therapy of Dieulafoy disease.

Traditional Chinese medicine placebo has been put more and more attention to. However, there is no ac-cepted quality evaluation method for TCM placebo. How to evaluate TCM placebo objectively and quantitatively is a common problem in the industry of Chinese medicine. New technologies such as the Intelligent Sensory Technique have been used to establish the placebo evaluation methods which are suitable for TCM characteristics. This article provided the basis for establishing scientific, rational and objective evaluation guiding principles for TCM.

Objective Nanoparticles are widely investigated and applied in clinical diagnosis and treatment as drug carrier,and their transmembrane process is related to their biological effects.The aim of this study was to investigate the interaction of fluorescein-labeled PLGA nanoparticles and HL60 cells via fluorescence tracking.Methods The transmembrane process of nanoparticles was quantitatively analyzed by laser scanning confocal microscopy.Results The analyzing results showed that the interaction of fluorescein-labeled PLGA nanoparticles and HL60 cells was strongly temperature-dependent.The receptor-mediated endocytosis mechanism played an important role in the transmembrane process for cellular uptake of nanopaticles.Conclusions This study provides a theoretical basis for design and application of nano-medicines.
10.Preparation and drug-loading property of phosphatidyl polyethyleneglycol monomethyl ether liposome

Objective To develop liposome loaded with paclitaxel for the treatment of lung cancer,novel phosphatidyl polyethyleneglycol monomethyl ether liposomes were prepared and studied.Methods A series of phosphatidyl polyethyleneglycol monomethyl ether liposomes were synthesized.Liposomes with or without drug were prepared by ultrasound.The entrapment efficiency and size of drug loaded liposomes were evaluated.The influence of liposomes to A549 lung cancer cell was investigated by MTT method.The cell uptake of liposome was observed by laser confocal scan microscope.Results The liposome loaded with paclitaxel had an entrapment efficiency of 83% and size of 100~200 nm.The MTT results showed phosphatidyl polyethyleneglycol monomethyl ether liposomes loaded paclitaxel have inhibition effect to A549 cells with long circulation,while liposomes without paclitaxel have not inhibition effect.Conclusion Drug loaded polyethyleneglycol monomethyl ether liposomes have sustained delivery to paclitaxel.They have potential application in the therapy of cancer.
