1.Early Versus Late Post Cesarean Section Warfarin Initiation and Increased Risk of Maternal Complications in Patients With Mechanical Heart Valves:A Randomized, Open-Label Pilot Study

Background and Objectives:
The timing of the reinstitution of warfarin after cesarean section (CS) delivery was not adequately addressed in the literature. This study aims to evaluate the risks of early versus late initiation of warfarin post-CS in patients with mechanical heart valves.
Methods:
This randomized, open-label cohort study included 114 pregnant women with mechanical heart valves planned to be delivered by CS at or after 28 weeks of gestation.Patients were randomly divided into two groups: Day-2-group, where warfarin was started on day 2, and Day-5-group, where warfarin was started on day 5 after CS. Maternal postoperative bleeding complications, mechanical valve thrombosis, need for blood transfusion or reoperation, and maternal mortality were identified.
Results:
Ten women (8.8%) had 11 bleeding complications, of whom 2 patients (20%) had intraperitoneal hemorrhage (none in Day-2-group and 2 in Day-5-group), 3 patients (30%) had subcutaneous hematoma (none in Day-2-group and 3 in Day-5-group), and 6 patients (60%) had sub-rectus hematoma (3 in Day-2-group and 3 in Day-5-group). No mechanical valve thrombosis, other thromboembolic events, or in-hospital maternal mortality were reported.
Conclusion
Despite the small number of events, the bleeding risk was lower in the group with early post-CS warfarin introduction than in the group with late warfarin introduction in patients with prosthetic heart valves.

This study evaluated the sensitivity, specificity, and accuracy of computed tomographic angiography (CTA) versus handheld Doppler (HHD) in detecting perforators for anterolateral thigh (ALT) flap surgery.
Methods:
This study was conducted on 20 patients randomly assigned to two groups from April 2023 to November 2024: One group received CTA and HHD, while the other group received only HHD. Perforators were evaluated for their number, location, and source and compared with intraoperative findings.
Results:
The sensitivity and specificity of CTA were 86.0% and 98.0%, respectively, while those of HHD were 81.0% and 86.5%. CTA exhibited an accuracy of 92.0% for perforator identification, whereas that of HHD was 83.5%.
Conclusion
CTA offers higher sensitivity, specificity, and accuracy in identifying ALT flap perforators than HHD. Its superior imaging capabilities can enable improved surgical planning, minimizing intraoperative challenges, reducing the risk of complications, and potentially increasing flap survival rates. As such, CTA could be considered a valuable standard tool in preoperative planning for ALT flap surgery, particularly in cases where anatomical variation poses a challenge.

ABSTRACT
Teledentistry has been proven to have the potential in addressing the gap in accessing oral health care
worldwide. Nevertheless, more information is needed to support evidence-based teledentistry practice in
Malaysia. The article aims to review teledentistry in view of implementing it to be a common practice
in Malaysia. The authors describe teledentistry application worldwide, as well as its uses in the field of
service and education. The article summarises the needs of teledentistry in South East Asia, as well as the
ethical perspectives and legal practices. In addition, the article also reviews the opportunities arising from
practicing teledentistry, including during the unprecedented COVID-19 pandemic. Following review,
teledentistry seems to be a promising path as it could offer solutions to shortages in the workforce, high
demand for treatment needs and unequal service distribution due to lack of accessibility in rural areas in
Malaysia. Furthermore, teledentistry can extend care to underserved patients and provide opportunities
to supplement teaching in dental education. The authors provide recommendations to overcome
challenges to ensure a sustainable teledentistry. Incorporating teledentistry into practice in Malaysia is
achievable but proper guidelines are required.

9.The Concept of Lamina–Pedicle Perpendicularity: Part 1. Lumbar Spine

Methods:
This was a retrospective study on normal lumbar spine lateral radiographs of patients who presented with low back pain and were reviewed by two observers. Different inclination angles were constructed to estimate the safety zones of the pedicle screws’ sagittal inclination.
Results:
Radiographs of 30 consecutive patients, 25 females and five males, with a mean age of 39.43±11.18 years, were studied. The mean angle of the interlaminar line and the pedicle axis was almost orthogonal at all the levels, with a range of 89.16°–94.63°, which was not affected by the lumbar sagittal profile. The safety zones of the pedicle screws were measured, and they revealed a safe sagittal range of 19.73°–24.40° if the screw was inserted from the pedicle axis, 21.03°–22.59° if inserted from the most cephalic part, and 13.31°–17.03° if inserted from the most caudal part.
Conclusions
Our results confirmed the perpendicularity of the interlaminar line with the pedicle axis in the lumbar spine at all the levels. The interlaminar line is a useful guide for pedicle screw sagittal inclination.

Methods:
This was a retrospective study on the computed tomography (CT) chest scans of patients with no spinal pathologies. The ILL–PA, superior and inferior safe angles of the pedicle screw trajectories, and the exit zone of the screw perpendicular to the ILL were reviewed by two observers via three-dimensional multiplanar reconstruction mode of the Horos DICOM software (https:// horosproject.org/).
Results:
The CT chest images of 30 consecutive patients (20 males and 10 females) with a mean age of 49.87±15.48 years (range, 24–74 years) were evaluated. The mean ILL–PA angle was almost orthogonal for all levels. This angle ranged between 86.21°±3.01° at D5 and 90.59°±2.72° at D10. The safety zones of the sagittal inclination of the pedicle screws were demonstrated. The results revealed that the least safe angle was when the screw was directed cranially along the middle part of the pedicle between 4.43°±0.75° at D8 and 6.94°±1.19° at D11.
Conclusions
The results of this study confirmed the ILL–PA angle perpendicularity in the thoracic spine at all levels. The ILL is a useful guide for pedicle screw sagittal inclination.
